ABSTRACTIntroduction:A significant proportion of patents under hemodialysis treatment manifest anxiety symptoms, which may affect compliance to treatment and quality of life. The aim of the present study was to investigate anxiety levels of patients under hemodialysis treatment, as well as the relationship between socio-demographic/clinical factors and anxiety.Method:A total of 105 patients with chronic kidney failure under hemodialysis treatment (64 men and 41 women) with a mean age of 72.51 ± 7.55 years were recruited and completed (a) a demographic questionnaire and (b) the State-Trait Anxiety Inventory.Results:58.1% of the participants were male; while the 59.6% of the participants were married and the 60.2% were retired. Our participants exhibited higher state, trait and total anxiety levels compared to general population. There were a statistically significant association between factors, including patients’ functionality, gender, physical exercise, hemodialysis complications, marital status, educational level and anxiety levels. Women exhibited a statistically significant higher state anxiety levels compared to men. Widowed participants, that is, people living alone who had experienced the loss of a beloved one, reported higher state, trait and total anxiety levels. Finally, people with higher education showed lower trait and total anxiety. Individuals who were capable of taking care of themselves reported lower state, trait and total anxiety levels with a statistically significant difference compared to those who needed external help. Additionally, those who physically exercised exhibited statistically significant lower state, trait and total anxiety levels compared to those who didn’t exercise. Conclusions:The conclusions of the present study could be a critical point in determining guidelines that would substantially contribute to the improvement of the psychological state of patients undergoing hemodialysis treatment.

Nature sound music is a combination between classical music with sounds of nature. The use of natural sound music like the sounds of birds, waves, wind, and water flow as a medical therapy has achieved satisfactory results that can improve relaxation, physical condition, and psychological state for individuals of various ages. This study aimed to determine the effect of natural sound music to decreased anxiety on critically ill patients in the ICU. This research is helpful in healing the critical patient, and as the basis for realizing evidence based practice in the management of critically ill patients. This research is using quasi-experimental non-equivalent to the pre-posttest control group design, with consecutive sampling by randomized allocation. Respondents were 20 people the intervention group and 18 people in the control group. Nature sounds music that was used is the sound of birds with a combination of 2 x 30 minutes in the morning (08.00-noon) and in the evening (20:00 to 22:00) for 3 days. Anxiety levels were measured using visual analog scale-anxiety (VAS-A). Data then were analyzed using Wilcoxon and Mann Whitney. The results of this study showed a decrease in anxiety in the intervention group with a value of 62.25 7.304 pretest, posttest 41.65 5.976, whereas the control group pretest 46.55 12.76, 43.00 12.35 posttest. The result of the comparison test on the anxiety before and after intervention in the intervention group p = 0.000 (p <0.05) and in the control group p = 0.007 (p <0.05). While the results of the difference test on the anxiety between the intervention group and the control group were p = 0.000 (p <0.05), which means there is a significant difference in anxiety levels between the intervention group and control group. Conclusion. Music sounds of nature is effective to reduce anxiety in critically ill patients. Keywords: music therapy, natural sounds, anxiety.

Abstract Background: Chronic renal failure causes a number of physical problems in patients. Hemodialysis treatment and the stress brought along by the treatment are high and this circumstance sets the ground for alexithymia. Alexithymic feelings basically emerge as restriction in the world of emotion and thought, and inability to recognize physiological changes. Biomarkers that are indicators of physical change are influential in the stress lives of individuals. They lead to negative changes in the physical and mental lives of patients who have chronic kidney failure and individuals who receive hemodialysis treatment. This research was carried out as a relationship seeker in order to determine the influence of urea, creatinin, sodium, potassium, hemoglobin, hematocrit, albumin, calcium, phosphorus and C-reactive protein biomarkers on stress and alexithymia in individuals, who are diagnosed with chronic renal failure and receive hemodialysis treatment. Methods: The research environment was formed of patients who underwent hemodialysis treatment in a hospital in Turkey. The subject group was completed of 72 individuals. Demographic data form, biochemical data form, Hemodialysis Stressor Scale and Toronto Alexithymia Scale were used in the research. Results: It was found that the levels of perceived stress of individuals who participated in the research were high at all dimensions, and 59.7% were alexithymic. The means of the total scale scores of all patients were calculated as 87.81±13.59 for HSS and 62.46±9.84 for TAS. The relationship between TAS-20 and HSS and selected biomarkers were determined (p<0.05). Conclusion: It was concluded that stress and alexithymic feelings were high in patients who received hemodialysis treatment. It was concluded that there is a relationship between C-reactive protein, creatinin, sodium, hemoglobin, hematocrit, potassium from the biomarkers and the scales and scale sub-dimensions. It is necessary to increase the awareness of nurses on the importance of the skills to communicate with individuals who have to cope with stress, manage emotions, and have high stress and emotional deprivation.
