AbstractThe state of a patient is an important concept in biomedical sciences. While analytical methods for predicting and exploring treatment strategies of disease dynamics have proven to have useful applications in public health policy and planning, the state of a patient has attracted less attention, at least mathematically. As a result, models constructed in relation to treatment strategies may not be very informative. We derive a patient-dependent parameter from an age-physiology dependent population model, and show that a single treatment strategy is not always optimal. Also, we derive a function which increases with the patient dependence parameter and describes the effort expended to be in good health.

It is the professional responsibility of pediatric surgeons to follow the principles of maintaining life and alleviating suffering, often by questioning whether they have acted correctly. Apart from the moral dilemmas of choosing the best treatment strategies, they are often in dilemmas with the parents, who also involve their own “strategy” in the whole story, which they think is the most optimal treatment for their child, despite the contrary recommendations of the profession. Children, and especially adolescents, may be somewhat involved in medical decision making. Mostly the parent-physician-child / adolescent triangle agrees, but this is not always the case, which is why pediatric surgeons encounter problems. Ethical committees, composed of competent people, supported by the legal system of the state, who are able by consensus of team members to advocate and ensure the best interests of patients, must be activated for the full scope of the solution.

For the structural systems with both epistemic and aleatory uncertainties, in order to analyze the effects of different regions of epistemic parameters on failure probability, two regional importance measures (RIMs) are firstly proposed, i.e. contribution to mean of failure probability (CMFP) and contribution to variance of failure probability (CVFP), and their properties are analyzed and verified. Then, to analyze the effect of different regions of the epistemic parameters on their corresponding first-order variance (i.e. main effect) in the Sobol’s variance decomposition, another RIM is proposed which is named as contribution to variance of conditional mean of failure probability (CVCFP). The proposed CVCFP is then extended to define another RIM named as contribution to mean of conditional mean of failure probability, i.e. CMCFP, to measure the contribution of regions of epistemic parameters to mean of conditional mean of failure probability. For the problem that the computational cost for calculating the conditional mean of failure probability may be too large to be accepted, the state dependent parameter (SDP) method is introduced to estimate CVCFP and CMCFP. Several examples are used to demonstrate the effectiveness of the proposed RIMs and the efficiency and accuracy of the SDP-based method are also demonstrated by the examples.

A quality arborization in cities and, mainly, in public places destined for entertainment, such as squares, is indispensable for the well-being of the population, besides, to contribute to the aesthetic enhance of the environment. Thus, this work aimed to analyze the phytosanitary nature of tree vegetation in two squares, Brazil square and square of Estrada 8, located in the municipality of Belterra, in the west part of the state of Pará. The number of individuals found at Brazil square was 56, while at square of Estrada 8, it was 21. According to the results, 61% of crowns of the first square, and around 70% in the second one, were in good health. The tree stems in the two squares did not show any apparent problem, the values of sanity for them were 67%, and 60%, respectively. Concerning the root situation, about 83% of the individuals analyzed at Brazil square had no problems, only one individual from Ipê pointed to rot in their roots. However, at the square of Estrada 8, about 79% had no damaged roots. In Brazil square, 67% of the species are regularly pruned, although, the ficus trees showed a need for removal, indicating 4% of the individuals with severe phytosanitary problems with termite attack. At square of Estrada 8, it is necessary to remove 38% of individuals, followed by 35% of pruning, replacement by 6% and only 21% of arboreal individuals are in an adequate situation for urban squares.

Human capital, as characterized by good education and good health is an important determinant of economic growth. Health finds a predominant place in three of the eight goals, eight of the sixteen targets and eighteen of the forty-eight indicators of the “Millennium Development Goals of the United Nation”. Health is the most important social service sector having direct correlation with the welfare of the human being. This sector assumes focus for reaping the demographic dividend having healthy productive workforce and general welfare of a State. The State of Jammu and Kashmir has performed relatively well in providing health and medical facilities to the people, but the level is still beneath the satisfaction. The progress of health infrastructure in the State can be judged by the health infrastructure i.e., the availability of hospitals, dispensaries and doctors. There are 4433 Govt. Health institutions at primary, secondary and tertiary levels with 6674 doctors in-position. The doctor patient ratio in our State is 1:1880 as against the recommendations of World Health Organization (WHO) of 1:1000. The doctor patient ratio at the National level is 1:2000. Besides, 826 sub-centers, 371 New Type Primary Health Centers (NTPHCs) and 87 ISM dispensaries have been sanctioned recently. Further, 1443 Specialists, MBBS/AYUSH doctors and 5494 paramedics have been hired on contract basis under National Health Mission (NHM) respectively. The Bed Capacity of different hospital under GMC Srinagar /Jammu and its associate hospitals in the State is 12965 comprising of 6967 in Kashmir and 5998 in Jammu regions. Most of the health indices of the Jammu and Kashmir are far better as compared to all India level. This is especially a case with life expectancy, Crude Birth Rate, Crude Death Rate, Infant Mortality Rate and Institutional Births.

Background: Unani System of Medicine (USM) is one of the traditional systems of medicine practiced in India and the Indian subcontinent. It is a holistic system of health care based on inceptive principles applied by Hippocrates, Galen, and Ibn-e-Sina, the founders of present-day medicine. USM encompasses a distinct way of looking at the human body and it considers health to be the result of natural dynamic harmonious balance within the body. USM also recognizes the presence of inherent wisdom which is responsible for controlling all physiological functions, maintenance of health, restoration of health, and self-healing. This intrinsic ability of the body to restore health and the mechanism that activates the body’s healing process is called Tabi’at/ physis. The ultimate goal of prevention of disease can be obtained by optimization of tabi’at/physis, increasing the host defense. It can be achieved through the implication of asbab-e-sitta zarooriyah which includes some Unani Regimenal Therapies also, fostering change in lifestyle habits resulting in good health. Aims: In this raging pandemic all the therapeutic strategies are based on destroying the agent, breaking the means of transmission and immunization. Although we are leaving behind a huge gap in maintaining the host defense even though it has a crucial role in disease dynamics. In this paper, we have attempted to highlight the concept of tabi’at/Physis and its optimization to prevent this disease and its transmission. Methods: The authors had gone through classical Unani medical textbooks books related to the concept of tabi’at, asbab-e-sitta zarooriyah, and various modalities that help in health promotion and host defense. Important Unani medicine textbooks like Al Qanoon, Kamilus Sana’at, Kitab-al- Hawi, Al-asbab wa Alamat, Moalijat Buqratiya, Firdausul Hikmat and Kitabul Mansuri in addition to other published literature on the concerned subject was also reviewed for further clarification on the topic. Keywords: Unani System of Medicine, Tabi’at (physis), Host defense & Asbab-e- Sitta zarooriyah
