AbstractThis paper identifies some of the more important diseases at the wildlife–livestock interface and the role wildlife plays in disease transmission. Domestic livestock, wildlife and humans share many similar pathogens. Pathogens of wild or domestic animal origin that can cause infections in humans are known as zoonotic organisms and the converse are termed as anthroponotic organisms. Seventy-seven percent of livestock pathogens and 91% of domestic carnivore pathogens are known to infect multiple hosts, including wildlife. Understanding this group of pathogens is critical to public health safety, because they infect a wide range of hosts and are most likely to emerge as novel causes of infection in humans and domestic animals. Diseases at the wildlife–livestock interface, particularly those that are zoonotic, must be an area of focus for public health programs and surveillance for emerging infectious diseases. Additionally, understanding wildlife and their role is a vital part of understanding the epidemiology and ecology of diseases. To do this, a multi-faceted approach combining capacity building and training, wildlife disease surveillance, wildlife–livestock interface and disease ecology studies, data and information sharing and outbreak investigation are needed.

We initiated the Vietnam Initiative on Zoonotic Infections (VIZIONS) research program to better understand the epidemiology of disease transmission at the human–animal interface in Vietnamese rural communities and to integrate One Health approaches to disease surveillance. We established a longitudinal community cohort study of individuals with occupational exposure to animals, which involves concurrent targeted sampling from domestic livestock species and follow-up monitoring of human clinical cases. The project was implemented by government agencies at the provincial and district levels of the public health and animal health sectors in Vietnam. Engaging with rural communities builds response capacity at the grassroots level, and has improved dialogue between local stakeholders responsible for monitoring human and animal health. Here we describe the process of initiating participatory research and cohort field implementation over the first year in two study sites in Vietnam.Keywords: Vietnam, participatory research, zoonoses, field epidemiology

Emerging and re-emerging infectious diseases are a significant public and animal health threat. In some zoonosis, the early detection of virus spread in animals is a crucial early warning for humans. The analyses of animal surveillance data are therefore of paramount importance for public health authorities to identify the appropriate control measure and intervention strategies in case of epidemics. The interaction among host, vectors, pathogen and environment require the analysis of more complex and diverse data coming from different sources. There is a wide range of spatiotemporal methods that can be applied as a surveillance tool for cluster detection, identification of risk areas and risk factors and disease transmission pattern evaluation. However, despite the growing effort, most of the recent integrated applications still lack of managing simultaneously different datasets and at the same time making available an analytical tool for a complete epidemiological assessment. In this paper, we present EpiExploreR, a user-friendly, flexible, R-Shiny web application. EpiExploreR provides tools integrating common approaches to analyze spatiotemporal data on animal diseases in Italy, including notified outbreaks, surveillance of vectors, animal movements data and remotely sensed data. Data exploration and analysis results are displayed through an interactive map, tables and graphs. EpiExploreR is addressed to scientists and researchers, including public and animal health professionals wishing to test hypotheses and explore data on surveillance activities.

For most emerging infectious diseases, including SARS-Coronavirus-2 (SARS-CoV-2), pharmaceutical intervensions such as drugs and vaccines are not available, and disease surveillance followed by isolating, contact-tracing and quarantining infectious individuals is critical for controlling outbreaks. These interventions often begin by identifying symptomatic individuals. However, by actively removing pathogen strains likely to be symptomatic, such interventions may inadvertently select for strains less likely to result in symptomatic infections. Additionally, the pathogen's fitness landscape is structured around a heterogeneous host pool. In particular, uneven surveillance efforts and distinct transmission risks across host classes can drastically alter selection pressures. Here we explore this interplay between evolution caused by disease control efforts, on the one hand, and host heterogeneity in the efficacy of public health interventions on the other, on the potential for a less symptomatic, but widespread, pathogen to evolve. We use an evolutionary epidemiology model parameterized for SARS-CoV-2, as the widespread potential for silent transmission by asymptomatic hosts has been hypothesized to account, in part, for its rapid global spread. We show that relying on symptoms-driven reporting for disease control ultimately shifts the pathogen's fitness landscape and can cause pandemics. We find such outcomes result when isolation and quarantine efforts are intense, but insufficient for suppression. We further show that when host removal depends on the prevalence of symptomatic infections, intense isolation efforts can select for the emergence and extensive spread of more asymptomatic strains. The severity of selection pressure on pathogens caused by these interventions likely lies somewhere between the extremes of no intervention and thoroughly successful eradication. Identifying the levels of public health responses that facilitate selection for asymptomatic pathogen strains is therefore critical for calibrating disease suppression and surveillance efforts and for sustainably managing emerging infectious diseases.

AbstractBackgroundChina’s capacity to control and prevent emerging and re-emerging infectious diseases is critical to the nation’s population health. This study aimed to explore the capacity of Centers for Disease Control and Prevention (CDCs) in China to deal with infectious diseases now and in the future.MethodsA survey was conducted in 2015 among 973 public health professionals at CDCs in Beijing and four provinces, to assess their capacity to deal with emerging and re-emerging infectious diseases.ResultsAlthough most professionals were confident with the current capacity of CDCs to cope with outbreaks, nearly all indicated more funding was required to meet future challenges. Responses indicated that Yunnan Province faced more challenges than Anhui, Henan and Liaoning Provinces in being completely prepared and able to deal with outbreaks. Participants aged 20–39 years were more likely than those aged 40 and over to believe strategies such as interdisciplinary and international collaborations for disease surveillance and control, would assist capacity building.ConclusionThe capacity of China’s CDCs to deal with infectious diseases was excellent. However, findings suggest it is imperative to increase the number of skilled CDC staff, financial support, and strengthen county level staff training and health education programs.

Bartonella spp. bacteria can be found around the globe and are the causative agents of multiple human diseases. The most well-known infection is called cat-scratch disease, which causes mild lymphadenopathy and fever. As our knowledge of these bacteria grows, new presentations of the disease have been recognized, with serious manifestations. Not only has more severe disease been associated with these bacteria but also Bartonella species have been discovered in a wide range of mammals, and the pathogens’ DNA can be found in multiple vectors. This review will focus on some common mammalian reservoirs as well as the suspected vectors in relation to the disease transmission and prevalence. Understanding the complex interactions between these bacteria, their vectors, and their reservoirs, as well as the breadth of infection by Bartonella around the world will help to assess the impact of Bartonellosis on public health.

<p>A quarter century ago the landmark report from the U. S. National Academies of Sciences, Engineering, and Medicine entitled <em>“</em><em>Emerging Infections, Microbial Threats to Health in the United States” </em>was released. This classic study captured the societal changes then underway in our rapidly growing world: The growth of the world’s population and changing human behavior; the advances and globalization of technology and industry; the changes in economic development and land use; the dramatic increase in speed and frequency of international travel and commerce; the adaptation of microbes and the appearance of never before seen pathogens; and the breakdown of traditional public health measures. This societal evolution has only increased and the growing frequency of outbreaks foretold in the report has come to pass. Each new disaster has precipitated changes and adaptations in our global response to infectious diseases designed to reduce risks and avoid future outbreaks. We discuss these past events and how each led to change in an effort to mitigate future threats. We also look to the future to consider what challenges might lay ahead.</p><p><strong>Conclusion. </strong>Major outbreaks over the past quarter century validated the concept of emerging infectious diseases and led to improvements in global policies and national public health programs; however, there will likely always be new diseases and the threat of reemergence of diseases once thought controlled leading to a constant need for vigilance in public health preparedness.</p>

Emerging infectious diseases are of great concern to public health, as highlighted by the ongoing coronavirus disease 2019 (COVID-19) pandemic. Such diseases are of particular danger during mass gathering and mass influx events, as large crowds of people in close proximity to each other creates optimal opportunities for disease transmission. The Hashemite Kingdom of Jordan and the Kingdom of Saudi Arabia are two countries that have witnessed mass gatherings due to the arrival of Syrian refugees and the annual Hajj season. The mass migration of people not only brings exotic diseases to these regions but also brings new diseases back to their own countries, e.g., the outbreak of MERS in South Korea. Many emerging pathogens originate in bats, and more than 30 bat species have been identified in these two countries. Some of those bat species are known to carry viruses that cause deadly diseases in other parts of the world, such as the rabies virus and coronaviruses. However, little is known about bats and the pathogens they carry in Jordan and Saudi Arabia. Here, the importance of enhanced surveillance of bat-borne infections in Jordan and Saudi Arabia is emphasized, promoting the awareness of bat-borne diseases among the general public and building up infrastructure and capability to fill the gaps in public health preparedness to prevent future pandemics.

Background: Kerala is one among the Indian states that has incorporated local self-government (LSG) bodies into its public health system. The focus of this research is to study the role of these bodies in the COVID-19 response strategy taken by the state.Methods: Interviews were conducted with the heads of various local self-government bodies in the district of Kannur to collect information on the COVID-19 management role they undertook. The interviews were then transcribed and analysed by adopting the inductive approach to derive themes and conclusions.Results: The interviews reflect that LSGs have undertaken a wide range of initiatives in their COVID-19 response strategy. Organisational capacity, networking with various agencies, community participation, resource mobilization, inter-sectoral coordination, etc. were significant factors in successfully conducting these activities.Conclusions: Managing the pandemic by planning activities at local level has gone a long way in controlling spread of the viral infection in Kerala, during initial phase. The involvement of the local self-government, primarily constituted for legislative and executive purposes, has had a major role in public health activities and positive impact in the health status of the state. This can serve as a model for effective implementation of public health programs as illustrated in the case of COVID-19 pandemic.

In mountain communities like Sebei, Uganda, which are highly vulnerable to emerging and re-emerging infectious diseases, community-based surveillance plays an important role in the monitoring of public health hazards. In this survey, we explored capacities of village health teams (VHTs) in Sebei communities of Mount Elgon in undertaking surveillance tasks for emerging and re-emerging infectious diseases in the context of a changing climate. We used participatory epidemiology techniques to elucidate VHTs’ perceptions on climate change and public health and assessed their capacities to conduct surveillance for emerging and re-emerging infectious diseases. Overall, VHTs perceived climate change to be occurring with wider impacts on public health. However, they had inadequate capacities in collecting surveillance data. The VHTs lacked transport to navigate through their communities and had insufficient capacities in using mobile phones for sending alerts. They did not engage in reporting other hazards related to the environment, wildlife, and domestic livestock that would accelerate infectious disease outbreaks. Records were not maintained for disease surveillance activities and the abilities of VHTs to analyze data were also limited. However, VHTs had access to platforms that could enable them to disseminate public health information. The VHTs thus need to be retooled to conduct their work effectively and efficiently through equipping them with adequate logistics and knowledge on collecting, storing, analyzing, and relaying data, which will improve infectious disease response and mitigation efforts.

Nowadays the provision of the safety of food products for human health, including of animal origin, is extremely important in the world. Since 2007, the Russian Federation has been monitoring the safety of food and feed in order to protect public health, assess the veterinary safety of imported and domestic food raw materials, products of its processing and feed, as well as to develop measures to prevent the entry of dangerous veterinary products into the market in relation to products of animal origin and feed. One of the main factors in production of high sanitary quality is compliance with the high sanitary level of their production, storage and sale. In this regard, it is necessary to pay attention to the choice of disinfectants. They must meet the following requirement and have such properties as a wide range of disinfecting action; detergency property and minimal corrosiveness. In addition they must effectively destroy bacteria, viruses, fungi and spore, be safe for humans, animals and the environment and be as simple as possible in use and relatively inexpensive. In Russia, the use of more than 400 chemicals of domestic and foreign production is allowed and their number is constantly growing. The agents used for disinfection, such as formaldehyde, QAC, aldehydes, etc., are highly volatile, toxic and environmentally unfriendly. The development of new disinfectants is mainly connected with the creation of new compositions of chemical agents, which increases their cost.
