The article analyses the Development of a sectoral system of remuneration of medical workers of state healthcare institutions of the constituent entities of the Russian Federation. It was determined, the PM differentiation, as well as the differentiation of the cost of a fixed set of consumer goods and services, is too low for use in the OSS, while the value of the coefficient of variation for the median wage is much higher, and at the same time, if this indicator is used in the OSS, with a significant decrease in the differentiation between the most the richest and poorest budgets will not experience the problems. Thus, the most appropriate indicator, taking into account the existing resources of the federal budget, for use in order to take into account the economic differentiation of a constituent entity of the Russian Federation is the median salary of employees of organizations.

The paper considers the importance of the uninterrupted functioning of the transport network, which is the most important condition for eliminating the transport inaccessibility of Arctic territories of the regions, thus excluding transport discrimination of the population living in the Arctic zone of the Russian Federation. Besides, the paper provides a brief overview of international road repair and maintenance experience on the example of Norway, analysis of the standard for repair and maintenance of roads in the Russian Federation, proposals on the method of calculating the cash expenditures of replacement, repair and maintenance of roads in the Russian Federation. The Decree of the Government of the Russian Federation No. 658 dated May 30, 2017 approved the standards of financial expenditures and the rules for calculating the size of federal budget allocations for replacement, repair and maintenance of federal roads. However, at the moment, not all regions have updated the regulatory framework on the norms of financial expenditures and the rules for calculating budget allocations for replacement, repair and maintenance of regional and inter-municipal roads. The existing methodology for calculating the cost of repair and maintenance of roads in the Arctic territory of Russia does not fully provide the need to bring the existing roads into normal transport and operational condition for their safe operation. The analysis shows that there is no systematic work to ensure sustainable financing of the road economy in the republic and the road industry is financed at a level much lower than the actual needs.

Introduction. The increase in financial resources on the part of the state for the procurement of medicines for treating patients with malignant neoplasms makes the issue of calculation the need in medicinal preparations (MP) actual. The aim of the work is to develop a special tool (oncology calculator) that standardizes the method for determining the need for drugs and can calculate the need for medications for the treatment of malignant neoplasms in a particular constituent entity of the Russian Federation. Material and methods. To develop an oncology calculator, we analyzed the need for the drugs for the treatment of patients with the most common malignant neoplasms was analyzed based on current clinical guidelines approved and published on the Ministry of the Russian Federation website. The number of patients at each stage of a specific cancer was calculated. The frequency of application of a particular treatment regimen was calculated based on the approved standards of medical care for patients with cancer or from a project. The number of patients was calculated on the official statistics presented in the P.A. Hertsen Moscow Oncological Research Institute - branch of the Federal State Budgetary Institution “National Medical Research Center of Radiology” by the Ministry of Health of Russia. Results. We have developed a tool called the “Oncology calculator” that standardizes the methodology for determining the need of a constituent entity of the Russian Federation or a medical institution for drugs based on clinical guidelines, as well as facilitating the planning of the volume of medical care in the conditions of round-the-clock and day hospitals, and in the provision of medical care to patients with malignant neoplasms in outpatient settings. Conclusion. The existing approaches to calculating the need for drugs for the treatment of patients with cancer in the constituent entities of the Russian Federation are scattered, which leads to the ineffective use of funds from the federal budget, compulsory medical insurance funds, budgets of the constituent entities of the Russian Federation. A uniform approach to calculating the need for drugs to treat malignant neoplasms should be based on clinical guidelines containing drug regimens for treating patients.

District coefficients are designed to compensate for differences in the cost of living of the population in the northern and eastern regions compared to the central and southern regions due to the unequal needs of the population. On the basis of the content analysis of the legislation the authors showed that in order to improve the legislation on regional coefficients to the wages of workers in areas with unfavorable natural and climatic conditions for the population, it is necessary to prepare new methodological approaches to determining the size of regional coefficients on the basis of the minimum (recovery) budget or the minimum consumer budget and to introduce the practice of establishing uniform regional coefficients for the wages of all employees of enterprises, organizations and institutions in the constituent entity of the Russian Federation or in municipal districts and urban districts within the constituent entity of the Russian Federation.

Purpose of the study. Healthcare budget impact analysis in the Russian Federation of providing adult patients with chronic obstructive pulmonary disease (COPD) with basic therapy for outpatient treatment. Material and methods. The hypothesis of reducing costs for the treatment of exacerbations of COPD was tested while providing all patients with basic therapy on an outpatient basis. The model calculates the direct medical costs of drug provision and treatment of exacerbations in patients with COPD in the current situation and when all patients with COPD are covered with basic therapy as part of a program of preferential drug provision. The simulation period is 1 year. The analysis does not take into account social losses associated with disability, payment of disability certificates, reduced labor productivity and mortality. Results. The cost of providing all patients with COPD with the recommended basic drug therapy on an outpatient basis is substantially offset by a decrease in the cost of treating exacerbations: drug costs should increase by 14.1 billion rubles compared with the current situation, while the cost of providing medical care to patients with exacerbations is reduced by 9.7 billion rubles. As a result, the difference in direct medical costs, or the necessary additional investments, is 4.4 billion rubles. The price of drugs used for basic therapy has the greatest impact on the size of the required additional investments. Conclusion. Providing all patients with COPD with the recommended basic drug therapy on an outpatient basis, we can expect a decrease in the number of moderate exacerbations per year by 14.7% and severe exacerbations by 31.2%. Taking into account not only medical, but also socio-economic costs can not only offset additional investments, but also demonstrate significant savings. When implementing a pilot project of preferential drug provision for patients with COPD in a specific region of the Russian Federation, it is possible to reduce the cost of purchasing medicines when forming a municipal order.

The article is meant to analyze current problems and prospects for the development of effective tax policy as part of digital transformation of Russian economy. Introduction of a digital tax and the consequences of the digital tax reforms in the EU, the USA and OECD countries are highlighted. The necessity of qualitative transformation of the tax system of the Russian Federation in response to modern challenges is substantiated, taking into account the changes of the Tax Code of the Russian Federation adopted at the end of July 2020. The authors suggested their own concept of a digital tax and the prospects for its adoption in Russia, and consider it inappropriate to impose taxes on Internet users who do not use the Internet for business. Today, the main focus should be made on creating and testing effective technologies that allow on-line monitoring the tax basis of digital economy entities, taking into account the cross-border movement or use of digital products (goods and services). In addition, it would be extremely important to provide for a potential tax exemption for part of the profits of international ICT companies that are received on the territory of the Russian Federation and reinvested in joint with Russian companies projects in the high-tech for civil purposes area.

The author touched upon topical issues of inter-budget alignment, budget provision of regions with financial resources, focusing on factors that directly affect the stability of the budget system, as well as highlighting problems in the budget sphere that require urgent attention from the authorities. It is concluded that the level of interregional differentiation continues to increase, and the modern mechanism of budget equalization requires urgent amendments and adjustments. It is emphasized that the main goals of socio-economic development of the Russian Federation related to the transition to an innovative type of development, changes in the structure of the national economy, set out in strategic documents and messages of the President of the Russian Federation and declared since 2009, are not being implemented, and the effectiveness of state economic policy and Federal budget expenditures for its implementation remains low. This indicates the need to review the current system.

Subject. The article investigates a Russian approach to ensuring economic growth. Objectives. The aim is to identify the basis of the Russian approach to ensuring economic growth. Methods. The study rests on the systems approach, using the methods of statistical, neural network, and cluster analysis. Results. The paper defines the structural importance of exports of goods and services, and changes in inventories. It highlights significant links with public consumption expenditures. The Russian approach to ensuring economic growth comes down to devaluation of the national currencyб resulting from the loss of the real inflationary potential of economic growth to maintain the volume of exports of goods and services in the context of maintaining public consumption expenditures. The increase in M2 forces the Central Bank of the Russian Federation to increase its foreign exchange reserves. This has a restraining effect on the growth of Russian Ruble’s monetary exchange rate. This phenomenon causes a drop in the Russian federal loan bond (OFZ) index and a replacement of strategic priorities with tactical ones. Conclusions. The basis of the Russian approach to ensuring economic growth is a reflection of the limits of the Russian economy development and the effectiveness of the Government of the Russian Federation.

he article presents an analytical review of the All-Russian Library Congress, held in Vladimir — the Library Capital of Russia 2018 — on 12—18 May 2018. The Congress was devoted to the theme “Ba-sing on the Past, We Build the Future. The Role of Cultural Heritage in Transforming the Librarianship in Russia”. The Congress held an open Discussion on the concept of librarianship development in the Russian Federation; it adopted the Guidelines on local lore activities for public libraries of the Russian Federation and approved the Regulations on the Council for Professional Qualifications in the Field of Library and Information Activities, planned for creation under the Russian Library Association. The Congress worked in various formats: pre-session events, plenary sessions, section meetings by library types and activity areas, and special events. The 19th Exhibition of Publishing Products, New Information Technologies, Goods and Services complemented the Congress. The Congress determined Tula to be the Library Capital of Russia 2019.
