Visfatin Levels May be an Early Marker of Atherosclerosis in Patients with Acromegaly

AbstractThe purpose of the study was to compare serum visfatin levels between patients with acromegaly and healthy controls and to evaluate the relationships between visfatin levels and epicardial fat thickness (EFT), carotid intima media thickness (cIMT), and ankle brachial index (ABI). We conducted a cross-sectional case-control study of 54 patients with acromegaly (37 females and 17 males) and 34 healthy controls (22 females and 12 males). Serum visfatin was measured by ELISA. Acromegalic and control participants and those with active or controlled acromegaly were compared with respect to their serum visfatin, clinical and metabolic parameters, EFT, cIMT, and ABI. Linear correlation was used to identify associations between these parameters and visfatin in all participants. Serum visfatin and glycated hemoglobin (HbA1c) were higher in the acromegaly group than in the control group (p<0.001 and p=0.007, respectively). There was no difference in visfatin between the active and controlled acromegaly groups, but HbA1c was higher in the active than the controlled acromegaly group (p<0.04). EFT, cIMT, and ABI were similar between the acromegaly and control groups and between the active and controlled acromegaly groups. Serum visfatin positively correlated with HbA1c, growth hormone (GH), and insulin-like growth factor-1 (IGF-1)/upper limit of normal ratio (r=0.245, p=0.024; r=0.259, p=0.017; and r=0.282, p=0.009, respectively). This study has revealed that a high visfatin level is associated with glycemic dysregulation and higher levels of GH and IGF-1 in acromegalic patients.

Background: The current cardiovascular disease (CVD) primary prevention guidelines prioritize risk stratification by using clinical risk scores. However, subclinical atherosclerosis may rest long term undetected. This study aimed to evaluate multiple subclinical atherosclerosis parameters in relation to several CV risk scores in asymptomatic individuals. Methods: A cross-sectional, single-center study included 120 asymptomatic CVD subjects. Four CVD risk scores were computed: SCORE, Framingham, QRISK, and PROCAM. Subclinical atherosclerosis has been determined by carotid intima-media thickness (cIMT), pulse wave velocity (PWV), aortic and brachial augmentation indexes (AIXAo, respectively AIXbr), aortic systolic blood pressure (SBPao), and ankle-brachial index (ABI). Results: The mean age was 52.01 ± 10.73 years. For cIMT—SCORE was more sensitive; for PWV—Framingham score was more sensitive; for AIXbr—QRISK and PROCAM were more sensitive while for AIXao—QRISK presented better results. As for SBPao—SCORE presented more sensitive results. However, ABI did not correlate with any CVD risk score. Conclusions: All four CV risk scores are associated with markers of subclinical atherosclerosis in asymptomatic population, except for ABI, with specific particularities for each CVD risk score. Moreover, we propose specific cut-off values of CV risk scores that may indicate the need for subclinical atherosclerosis assessment.

ABSTRACT Objective: To investigate whether there is an association between suicidal ideation (SI) and allergic diseases in adults. Methods: This was a comparative cross-sectional study involving individuals ranging from 20 to 50 years of age recruited from a university hospital in the city of Guadalajara, Mexico. We included patients with a confirmed diagnosis of allergic asthma, those with a confirmed diagnosis of allergic rhinitis, and healthy controls. All subjects completed the Beck Depression Inventory-II (BDI-II), which includes an item that evaluates the presence of suicidal thoughts or desires within the last two weeks, in order to identify SI. Results: The sample comprised 115 patients with allergic asthma, 111 patients with allergic rhinitis, and 96 healthy controls. The number of individuals identified with SI in the three groups were, respectively, 17 (14.8%), 13 (11.7%), and 8 (8.3%). Regarding the presence of SI, no statistically significant association was found in the allergic asthma group (OR = 1.98; 95% CI: 0.78-4.64; p = 0.154) or in the allergic rhinitis group (OR = 1.46; 95% CI: 0.58-3.68; p = 0.424) when they were compared with the control group. However, the presence of depression was associated with SI in the three groups: allergic asthma (OR = 12.36; 95% CI: 2.67-57.15; p = 0.001); allergic rhinitis (OR = 6.20; 95% CI: 1.66-23.14; p = 0.006); and control (OR = 21.0; 95% CI: 3.75-117.36; p < 0,001). Conclusions: In comparison with the control group, no association was found between SI and the groups with allergic diseases. In contrast, there was association between SI and depression in the three groups.

Abstract: Cardiovascular disease is a huge burden in terms of mortality, disability, and morbidity in this day. Prevention of cardiovascular disease is based on the physical signs. Waist circumference, Ankle Brachial Index (ABI) and Carotid Intima Media Thickness (CIMT) are useful to recognize occult atherosclerosis, so as ear lobe crease. However, medics have less attention about examination of ear lobe crease for detection of coronary artery disease. This study aimed to determine the relation of ear lobe crease and coronary artery disease. This was an analytical observational study with a case control design. The results showed that there were 45 samples for control group and 45 samples for case group. The statistical analysis showed the X2 = 21.78 with a p value <0,001 which indicated that there was a significant correlation between Ear Lobe Crease and Coronary Artery Disease. The OR = 8.9% (95% CI 3.4 -23.3) meant that if a person had ear lobe crease, the possibility of coronary artery disease was 8.9 times higher than a person without ELC. Conclusion: There was a significant correlation between Ear Lobe Crease and Coronary Artery Disease.Keywords: ear lobe crease, coronary artery diseaseAbstrak: Penyakit Kardiovaskular merupakan penyebab utama kematian, kecacatan dan kesakitan saat ini. Deteksi penyakit kardiovaskular sebagai tindakan pencegahan dapat dilihat melalui pemeriksaan fisik. Pengukuran lingkar pinggang, Ankle Brachial Index (ABI), dan Carotid Intima Media Thickness (CIMT) sangat berguna untuk penanda aterosklerosis subklinis, begitu pula dengan Ear Lobe Crease. Namun sampai saat ini, pemeriksaan Ear Lobe Crease untuk menilai penyakit jantung koroner kurang mendapat perhatian petugas medis. Tujuan penelitian ini adalah untuk mengetahui apakah terdapat hubungan antara Ear Lobe Crease (ELC) dengan Penyakit Jantung Koroner. Metode: Penelitian ini dilakukan menggunakan metode penelitian analitik observasional dengan case control. Hasil penelitian: Sampel penelitian terdiri dari 45 orang untuk kelompok kontrol dan 45 orang untuk kelompok kasus. Berdasarkan uji X2 diperoleh X2 = 21,78 dengan p < 0,001. Hasil ini menunjukkan bahwa adanya hubungan yang sangat bermakna antara Ear Lobe Crease (ELC) dengan Penyakit Jantung Koroner (p < 0,001). Dalam uji ini, juga diperoleh OR = 8,9 (95% CI: 3,4 – 23,3). Odd Ratio (OR) ini menyatakan bahwa bila seseorang ditemukan adanya ELC, maka orang tersebut berisiko 8,9 kali mendapat Penyakit Jantung Koroner dibanding dengan orang tanpa ELC. Simpulan: Terdapat hubungan yang sangat bermakna antara Ear Lobe Crease (ELC) dengan Penyakit Jantung Koroner.Kata kunci: ear lobe crease, penyakit jantung koroner.

Background: IMT assessment as a non-invasive imaging test is quite widely used especially among RA patients, the clinical applications of using such knowledge is scarce, hence study was conducted to compare the carotid artery intima-media thickness (CIMT) in patients with rheumatoid arthritis (RA) with healthy controls also to study the correlation between duration of rheumatoid arthritis, the activity of rheumatoid arthritis and other factors influencing (CIMT).Methods: In analytical cross-sectional study, of 80 participants of RA and 40 healthy controls, “DAS28” was used to assess disease activity.  Carotid intima-media thickness assessed using carotid ultrasonography.Results: Mean age of the cases and controls was 43.9 and 44.38 years. Subjects with duration of disease <2 years, to 5 years and >5 years were 35%, 45% and 20%. The mean carotid intima-media thickness was 5.61mm in controls, and CIMT was 6.11mm in people below 2 years and 7.08 mm in people between 2 to 5 years and 8.00mm in people above 5 years which was statistically significant. The mean carotid intima-media thickness was 5.61mm controls and 6.86mm in people with low, 7.00mm in people with moderate and 6.95mm in people with high disease activity, which was statistically significant.Conclusions: Study findings revealed risk of increase in carotid intima-media thickness higher among RA patients in the later stages and can increase the patients’ susceptibility to cardiovascular events. The factors showing strong association with intimal medial thickness were the age and symptoms duration.

Background Small for gestational age (SGA) has been associatedwith adult cardiovascular disease. Small for gestational agenewborns may undergo early aortic wall intima-media thickening(aIMT) in utero.Objective To determine the relationship between SGA as a riskfactor for increased aIMT, as a sign of atherosclerosis onset.Methods We conducted a case-control study in the Neonatal Wardand Rooming-in Nursery at Dr. Mohammad Hoesin Hospital,Palembang, between April to June 2012. Subjects were allocatedto either the case group (aIMT 2: 0.9 mm) or to the controlgroup (aIMT <0.9 mm). Newborns were classified as SGA iftheir birthweight (BW) was < l O'h percentile, and appropriatefor gestational age (AGA) if their BW was between lQth - 90'hpercentile, according to the Lubchenco curve. Abdominal aorticintima-media thickness was measured by echocardiographyexamination.Results The case and control groups consisted of 30 n ewbornseach. The proportion of SGA newborns was higher in the casegroup than the control group. The likelihood of infants in the casegroup being SGA was significantly higher compared to the controlgroup, with odds ratio of 10.8 (95%CI 3,26 to 35, 72) . The meanaIMT was significantly higher in SGA than in AGA infants, 0.9(SD 0.16) mm vs. 0.8 (SD 0.13) mm, respectively, with a meandifference of 0, 13 (9 5% CI 0, 050 to 0,209 mm; P"" 0,02).Conclusion Increased aIMT is more likely found in SGA newborns.

ObjectivesComorbid hypertension and hyperhomocysteinemia is an important risk factor for carotid atherosclerotic plaque formation. We put forward the hypothesis that the subjects with comorbid prehypertension and hyperhomocysteinemia also had an increased risk of subclinical atherosclerosis, using carotid intima–media thickness (CIMT) as the marker of the atherosclerotic process.MethodsA total of 4102 asymptomatic Chinese subjects aged 18–60 years were divided into four groups according to blood pressure (BP) and homocysteine (HCY) level: the control group without prehypertension or hyperhomocysteinemia, isolated prehypertension group, simple hyperhomocysteinemia group and prehypertension with hyperhomocysteinemia group. Serum lipids, fasting blood glucose (FBG), HCY and CIMT were measured.ResultsThere was significant difference in the positive rates of increased CIMT among four groups. Compared with the controls, the subjects in the other three groups had a higher risk of increased CIMT (isolated prehypertension group, OR 2.049, 95% CI 1.525 to 2.754; simple hyperhomocysteinemia group, OR 2.145, 95% CI 1.472 to 3.125; prehypertension and hyperhomocysteinemia group, OR 3.199, 95% CI 2.362 to 4.332). However, by multiple logistic regression analysis, only comorbid prehypertension and hyperhomocysteinemia was independently associated with increased CIMT (OR 1.485, 95% CI 1.047 to 2.108, P<0.05).ConclusionsComorbid prehypertension and hyperhomocysteinemia was an independent risk factor of subclinical atherosclerosis in asymptomatic Chinese, but isolated prehypertension or hyperhomocysteinemia was not. Therefore, combined intervention for prehypertension and hyperhomocysteinemia may contribute to decrease the incident of cardiovascular disease.
