SARS-CoV-2 Infektionen während Reisen mit Bahn und Bus. Ein systematisches Review epidemiologischer Studien

Zusammenfassung Ziel Das Ziel dieses Reviews ist es, epidemiologische Studien zum Ansteckungsrisiko mit SARS-CoV-2 bei Reisen mit Bahn und Bus zu identifizieren und kritisch auch im Hinblick auf die Übertragbarkeit für Deutschland zu bewerten. Methodik Systematisches Review basierend auf der Suche in zwei elektronischen Datenbanken (PubMed, Web of Science) nach dem Prinzip der „Preferred Reporting Items for Systematic Review and Meta-analysis“ (PRISMA) nach epidemiologischen Studien zu SARS-CoV-2 bzw. COVID-19 und Reisen mit der Bahn oder im Bus. Ergebnisse Die Suche in den beiden elektronischen Datenbanken lieferte 746 Publikationen. Davon erfüllten 55 die Auswahlkriterien und wurden in die Volltextrecherche einbezogen. Schließlich konnten 5 Originalpublikationen zur Beantwortung der Frage nach SARS-CoV-2-Infektionen im Zusammenhang mit Fernreisen per Bahn und 4 mit Bezug auf Busreisen herangezogen werden. Die Studien sind sehr heterogen und beziehen sich fast ausschließlich auf Fernreisen in China. Sie zeigen konsistent ein Ansteckungsrisiko, wenn infizierte Personen im gleichen Waggon oder Bus ohne Mund-Nasen Bedeckung (MNB) mitreisen. Das Risiko ist dabei nicht beschränkt auf jene Reisenden, die in unmittelbarer Nähe zu dem infizierten Mitreisenden sitzen. Trotz aller Unterschiede zwischen Reisen mit Bahn und Bus in China und Deutschland besteht kein grundsätzlicher Zweifel daran, dass die berichteten Ergebnisse aus China in qualitativer Hinsicht auch auf Deutschland zu übertragen sind. Allerdings muss berücksichtigt werden, dass die Ergebnisse der drei Schlüsselpublikationen überwiegend die Zeit vor dem Lockdown in China ohne die strikte Verwendung von MNB einschlossen. Somit bleibt die Frage, ob die Ergebnisse unter den gegenwärtigen Bedingungen mit MNB und virulenteren Virusmutationen ähnlich wären. Es wurde keine einzige Studie im Zusammenhang mit der Infektion bei Nutzung des öffentlichen Personennahverkehrs gefunden. Schlussfolgerungen Es gibt verschiedene Hinweise dafür, dass Reisen mit der Bahn mit einem deutlich niedrigeren Infektionsrisiko verbunden ist im Vergleich zum Ansteckungsrisiko im häuslichen Umfeld. Wegen fehlender Beobachtungsdaten wird man das Infektionsrisiko bei Fernreisen mit Bus und bei Nutzung des öffentlichen Personennahverkehrs auf der Grundlage des Luftaustausches im Fahrgastinnenraum, der Reisedauer, des Abstandes zu anderen Fahrgästen und letztendlich der Fahrgastdichte modellhaft abschätzen müssen.

Cognitive functions are essential in any form of exercise. Recently, interest has mounted in addressing the relationship between caffeine intake and cognitive performance during sports practice. This review examines this relationship through a structured search of the databases Medline/PubMed and Web of Science for relevant articles published in English from August 1999 to March 2020. The study followed PRISMA guidelines. Inclusion criteria were defined according to the PICOS model. The identified records reported on randomized cross-over studies in which caffeine intake (as drinks, capsules, energy bars, or gum) was compared to an identical placebo situation. There were no filters on participants’ training level, gender, or age. For the systematic review, 13 studies examining the impacts of caffeine on objective measures of cognitive performance or self-reported cognitive performance were selected. Five of these studies were also subjected to meta-analysis. After pooling data in the meta-analysis, the significant impacts of caffeine only emerged on attention, accuracy, and speed. The results of the 13 studies, nevertheless, suggest that the intake of a low/moderate dose of caffeine before and/or during exercise can improve self-reported energy, mood, and cognitive functions, such as attention; it may also improve simple reaction time, choice reaction time, memory, or fatigue, however, this may depend on the research protocols.

Abstract To date, there is no quantitative review examining the influence of heart rate variability biofeedback (HRV BFB) on the athlete population. Such an undertaking may provide valuable information on the autonomic and respiration responses of athletes when performing HRV BFB. Thus, purpose of this preliminary systematic review and meta-analysis on the effects of HRV BFB on HRV and respiration of athletes. Searches of Springerlink, SportDiscus, Web of Science, PROQUEST Academic Research Library, Google Scholar, and ScienceDirect were conducted for studies that met the following criteria: (1) experimental studies involving athletes that underwent randomized control trial; (2) availability of HRV BFB as a treatment compared with a control (CON)/placebo (PLA); (3) any pre and post HRV variable and/or breathing frequency as dependent variable/s; and, (4) peer-reviewed articles written in English. Four out of 660 studies involving 115 athletes (25 females and 90 males) ages 16–30 years old were assessed in this review. Preliminary findings suggest the promising ability of HRV BFB to improve respiratory mechanics in athlete population. More work is needed to determine the autonomic modulatory effect of HRV BFB in athletes.

ObjectiveTo systematically review and meta-analyse the rate of SARS-CoV-2 genome identification and the presence of SARS-CoV-2 antibodies in breastmilk of mothers with COVID-19.DesignA systematic review of studies published between January 2019 and October 2020 without study design or language restrictions.SettingData sourced from Ovid Embase Classic+Embase, PubMed, Web of Science, Scopus, relevant bibliographies and the John Hopkins University COVID-19 database.PatientsMothers with confirmed COVID-19 and breastmilk tested for SARS-CoV-2 by RT-PCR or for anti-SARS-CoV-2 antibodies.Main outcome measuresPresence of SARS-CoV-2 genome and antibodies in breastmilk.ResultsWe included 50 articles. Twelve out of 183 women from 48 studies were positive for SARS-CoV-2 genome in their breastmilk (pooled proportion 5% (95% CI 2% to 15%; I2=48%)). Six infants (50%) of these 12 mothers tested positive for SARS-CoV-2, with one requiring respiratory support. Sixty-one out of 89 women from 10 studies had anti-SARS-CoV-2 antibody in their breastmilk (pooled proportion 83% (95% CI 32% to 98%; I2=88%)). The predominant antibody detected was IgA.ConclusionsSARS-CoV-2 genome presence in breastmilk is uncommon and is associated with mild symptoms in infants. Anti-SARS-CoV-2 antibodies may be a more common finding. Considering the low proportion of SARS-CoV-2 genome detected in breastmilk and its lower virulence, mothers with COVID-19 should be supported to breastfeed.

This study aimed to perform a systematic review and meta-analysis of the glycemic index (GI) of gluten-free bread (GFB) and its main ingredients. The systematic review followed PRISMA guidelines, using seven electronic databases (PubMed, EMBASE, Scopus, Science Direct, Web of Science, gray literature research with Google Scholar, and patents with Google Patent tool), from inception to November 2020. Eighteen studies met the inclusion criteria evaluating 132 GFB samples. Five articles tested GI in vivo, eleven in vitro; and two studies tested both methods. The analysis showed that 60.7% (95% CI: 40.2–78.1%) of the samples presented high glycemic indexes, evidencing a high glycemic profile for GFB. Only 18.2% (95% CI: 11.7–27.2%) of the bread samples presented in the studies were classified as a low GI. Meta-analysis presented moderate/low heterogenicity between studies (I2 = 61% and <1% for both high and low GIs) and reinforced the proportion of high GIs. Lower GIs were found in formulations based on Colocasia esculenta flour or enriched with fiber, yogurt and curd cheese, sourdough, psyllium, hydrocolloids, enzymes, fructans, and resistant starch, highlighting the efficacy of these ingredients to lower GFBs’ GI. GFB tends to present high GI, impacting the development of chronic diseases when consumed.

Objetivo: Verificar as alterações estruturais e funcionais, evidenciadas através da imagem por ressonância magnética, relacionadas aos déficits de memória identificados em idosos normais, quando comparados a adultos jovens. Metodologia: Procedeu-se à revisão sistemática, cujo protocolo obedeceu ao fluxograma do Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). Foram investigadas as bases de dados PubMed, Scopus, EBSCO, utilizando o gerenciador de referências JabRef, na versão 2.10, e o Web of Science, pelo website. Foram incluídos artigos de estudos quase experimentais, transversais, em coorte ou tipo caso-controle, publicados entre 2005 e 2014, em periódicos indexados nacionais e internacionais, cuja amostra incluísse idosos a partir de 60 anos, não dementes, submetidos à investigação de alterações estruturais e funcionais do sistema nervoso central, por ressonância magnética e sua associação com déficits de memória avaliados por testes neuropsicológicos. Resultados: Quanto à técnica de imagem empregada, identificaram-se dois estudos com imagem por ressonância magnética estrutural, seis estudos com utilização de imagem por ressonância magnética funcional, e quatro estudos que empregaram ambas as técnicas. Nos 12 estudos foi identificado o emprego de 38 testes neuropsicológicos distintos, com uma média de cinco testes por estudo, com variação de um a 12 testes. Dentre os testes mais usados, estiveram o WAIS Digit Span Backwards (em sete estudos), o Trail Making Test A and B (em quatro estudos) e o Wechsler Memory Scale (em quatro estudos). Conclusão: Os estudos demonstraram que no envelhecimento normal, ocorre redução do volume de substância branca para-hipocampal, do volume do hipocampo e do córtex entorrinal com redução de memória verbal, possivelmente por desmielinização das fibras; redução das vias que ligam o lobo temporal e frontal, contribuindo para a redução da memória episódica, da memória de trabalho e da fluência verbal; redução da supressão de informações irrelevantes, o que contribui para menor registro de informação; alterações das áreas frontal e parietal que comprometem a memória de reconhecimento; modificações na atividade e na conectividade do default mode network; reorganização das funções cognitivas, bem como alentecimento de resposta por provável redução de ativação do córtex pré- frontal

AbstractAlthough a considerable volume of data supporting induction or aggravation of psoriasis because of angiotensin-converting enzyme (ACE) inhibitor use exists, it remains insufficient for definitive conclusions. Therefore, we aimed to evaluate the association between ACE inhibitor use and psoriasis incidence through a systematic literature review and meta-analysis. We searched for qualifying studies across PubMed, Web of Science, and Embase. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated to evaluate the strength of the association between ACE inhibitor use and psoriasis incidence. Eight studies with a total of 54,509 patients with a psoriasis diagnosis were included in this meta-analysis. The pooled OR for psoriasis incidence among ACE inhibitor users was 1.52 (95% CI, 1.16–2.00) compared to that among non-users. From subgroup analysis by continent, the OR for ACE inhibitor users versus non-users was 2.37 (95% CI 1.28–4.37) in Asia. Per the subgroup analysis by climate, the OR for ACE inhibitor users vs non-users in dry climate was 3.45 (95% CI: 2.05–5.79) vs 1.32 (95% CI 1.01–1.73) in temperate climate. Our results reveal a significant association between ACE inhibitor use and psoriasis incidence.

Objective:To perform a systematic review and meta-analysis to determine whether fluid and imaging astrocyte biomarkers are altered in Alzheimer's disease (AD).Methods:PubMed and Web of Science databases were searched for articles reporting fluid or imaging astrocyte biomarkers in AD. Pooled effect sizes were determined with mean differences (SMD) using the Hedge’s G method with random-effects to determine biomarker performance. Adapted questions from QUADAS-2 were applied for quality assessment. A protocol for this study has been previously registered in PROSPERO (registration number: CRD42020192304).Results:The initial search identified 1,425 articles. After exclusion criteria were applied, 33 articles (a total of 3,204 individuals) measuring levels of GFAP, S100B, YKL-40 and AQP4 in the blood and cerebrospinal fluid (CSF), as well as MAO-B, indexed by positron emission tomography 11C-deuterium-L-deprenyl ([11C]-DED), were included. GFAP (SMD = 0.94; 95% CI = 0.71-1.18) and YKL-40 (SMD = 0.76; CI 95% = 0.63-0.89) levels in the CSF, S100B levels in the blood (SMD = 2.91; CI 95% = 1.01-4.8) were found significantly increased in AD patients.Conclusions:Despite significant progress, applications of astrocyte biomarkers in AD remain in their early days. The meta-analysis demonstrated that astrocyte biomarkers are consistently altered in AD and supports further investigation for their inclusion in the AD clinical research framework for observational and interventional studies.

Abstract Background Endometriosis is one of the most common causes of infertility. The causes of the disease and its definitive treatments are still unclear. Moreover, Anti-Mullerian Hormone (AMH) is a glycoprotein dimer that is a member of the transient growth factors family. This research work aimed to identify the effect of unilateral and bilateral laparoscopic surgery for endometriosis on AMH levels after 3 months, and 6 months, using meta-analysis. Methods In this study, the articles published in national and international databases of SID, MagIran, IranMedex, IranDoc, Cochrane, Embase, Science Direct, Scopus, PubMed, and Web of Science (ISI) were searched to find electronically published studies between 2010 and 2019. The heterogeneous index between studies was determined using the I2 index. Results In this meta-analysis and systematic review, 19 articles were eligible for inclusion in the study. The standardized mean difference was obtained in examining of unilateral laparoscopic surgery for endometriosis (before intervention 2.8 ± 0.11, and after 3 months 2.05 ± 0.13; and before intervention 3.1 ± 0.46 and after 6 months 2.08 ± 0.31), and in examining bilateral laparoscopic surgery for endometriosis examination (before intervention 2.0 ± 08.08, and after 3 months 1.1 ± 0.1; and before intervention 2.9 ± 0.23 and after 6 months 1.4 ± 0.19). Conclusion The results of this study demonstrate that unilateral and bilateral laparoscopic surgery for endometriosis is effective on AMH levels, and the level decreases in both comparisons.

Abstract Rationale: As the role of neurodegeneration in the pathophysiology of multiple sclerosis (MS) has become more prominent, the formation and evolution of chronic or persistent T1-hypointense lesions (Black Holes) have been used as markers of axonal loss and neuronal destruction to measure disease activity. However, findings regarding this subject are controversial. In this study we aim to clarify the level of importance of T1 hypointense lesions for estimating the prognosis of patients.Objectives: To evaluate the correlation between T1 hypointensities (Black holes) lesion load (lesion mean volume) on brain MRI with disability level of patients with Relapsing-Remitting Multiple Sclerosis (RRMS) or Secondary-Progressive Multiple Sclerosis (SPMS).Data sources: We will search MEDLINE (through PubMed), Embase, CENTRAL, Science Citation Index – Expanded (Web of Science), and Conference Proceedings Citation Index – Science (Web of Science). We won’t consider any timeframe, language, or geographical restrictions.Methods: Standard systematic review protocol methodology is employed. Eligibility criteria is reported in line with PICOTS system. Population is limited to adult patients diagnosed with RRMS or SPMS, based on the McDonald criteria. Index (prognostic factor) of interest will be T1 hypointense (black hole) lesion mean volume (lesion load) on brain Magnetic Resonance Imaging (MRI). There will be no comparators. Outcome of interest will be the disability measure using Expanded Disability Status Scale (EDSS). For the timing domain, we will include studies only if the outcome was measured at the same time MRI was performed (or with a very close time interval between). Inpatient and outpatient settings will both be included. All included studies will be assessed for the risk of bias using a tailored version of the Quality In Prognosis Studies (QUIPS) tool. Extracted correlation coefficients will be converted to the Fisher’s z scale and a meta-analysis will be performed on the results. We will then convert back the results to correlation coefficients again for the sake of presentation. For the purpose of assessing heterogeneity we will use prediction intervals. If feasible, we will also try to perform subgroup and sensitivity analyses. We will also evaluate the publication bias using Funnel plots and assess the confidence in cumulative evidence using an adapted version of the GRADE for prognostic factor research.

Abstract Background: Job stress is the most important factor in reducing organizational productivity. Therefore, this study aimed to identify prevalence of job stress among nurses in Iran.Methods: This study was conducted using a meta-analysis. All published articles on prevalence of job stress among nurses in Iran until July 2018 were searched and collected using PubMed, Web of Science, Science Direct, CINAHL, PsychInfo, Scopus, Magiran, IranMedex and SID and Bibliography section of the retrieved papers and related journals. Finally, 22 empirical studies were reviewed using Comprehensive Meta-Analysis software.Results: The overall prevalence of job stress in Iranian nurses was 49.5% (95% CI 41.9%-59.1%). The highest job stress was recorded in Tehran in 2016, 96.4% (95% Cl: 90.3%-98.7%) and the lowest job stress was found in Kashan in 2011, 2.5% (95% Cl: 6.0%-1.0%). Significant statistical correlation was observed between job stress, year and sample size (p<0/05).Conclusion: job stress affects about half of nursing staff in Iran; thus, managers and policymakers should take serious measures to reduce prevalence of job stress among nurses.
