Abstract Objectives There is high prevalence of micronutrient inadequacy among preschool-children in the Philippines. In addition, 60% of Filipino pre-school children do not consume any dairy on a given day despite the government recommendation of one serving of milk (180 ml) per day. Milk for pre-school children (milk henceforth), a source of energy, protein, and micronutrients, tailored to the nutritional needs of children in this age group, may improve nutrient adequacy in this population. This study models the nutritional impact of adding one serving of milk in the diets of Filipino children who currently do not consume any dairy products. Methods Dietary intake data of Filipino pre-school children aged 3 to 5 years were analyzed from the 8th National Nutrition Survey 2013. Non-dairy consumers were identified (n = 1313) and daily mean intakes of iron, calcium, folate, zinc, thiamine, riboflavin, vitamins C and D were calculated (pre-simulation). A serving of milk was added to each individual's diet and daily mean intakes of nutrients were recalculated (post-simulation). Mean nutrient intakes and percentage of children with inadequacy (using Estimated Average Requirements (EAR) cut-off method) were compared pre- and post-simulations. Results The mean nutrient intakes post-simulation were higher and closer to the Filipino dietary recommendations: iron (pre-simulation (mean ± SD) = 4.8 ± 3.4 mg/d; post-simulation = 6.5 ± 3.4 mg/d), calcium (191.5 ± 153.5 mg/d; 384.9 ± 153.5 mg/d), zinc (2.9 ± 1.9 mg/d; 4.3 ± 1.9 mg/d), thiamine (0.5 ± 0.3 mg/d; 0.7 ± 0.3 mg/d), riboflavin (0.5 ± 2.1 mg/d; 0.7 ± 2.1 mg/d), folate (120.3 ± 101.4 µg/d; 180.8 ± 101.4 µg/d), vitamin C (17.0 ± 29.2 mg/d; 35.8 ± 29.2 mg/d), and vitamin D (1.5 ± 2.2 µg/d; 3.0 ± 2.2 µg/d) respectively (P < 0.001). The proportions of children with inadequate nutrient intakes post-simulation were reduced by 13.3% for iron, 19.2% for calcium, 23.2% for folate, 35.2% for zinc, 34.8% for thiamine, 43.3% for riboflavin, 73.1% for vitamin C, and 4.9% for vitamin D. Conclusions The modeling study shows that if pre-school children (who do not normally consume any dairy) were to meet their dairy recommendations with a single serving of milk tailored to this age group, the proportion of pre-school children in the Philippines with inadequate micronutrient intakes could be reduced. Malnutrition is a major public health concern in the Philippines. Milk and dairy products are important sources of energy, protein, and micronutrients for normal growth and development in children. This study aims to assess the contribution of different types of milk to nutrient intakes and nutrient adequacy among young and preschool children in the Philippines. Filipino children aged one to four years (n = 2992) were analysed while using dietary intake data from the 8th National Nutrition Survey 2013. Children were stratified by age (one to two years and three to four years) and by milk beverage consumption type: young children milk (YCM) and preschool children milk (PCM), other milks (mostly powdered milk with different degrees of fortification of micronutrients), and non-dairy consumers (no milks or dairy products). The mean nutrient intakes and the odds of meeting nutrient adequacy by consumer groups were compared, percentage of children with inadequate intakes were calculated. Half (51%) of Filipino children (all ages) did not consume any dairy on a given day, 15% consumed YCM or PCM, and 34% consumed other milks. Among children one to two years, those who consumed YCM had higher mean intakes of iron, magnesium, potassium, zinc, B vitamins, folate, and vitamins C, D, and E (all p < 0.001) when compared to other milk consumers. Non-dairy consumers had mean intakes of energy, total fat, fibre, calcium, phosphorus, iron, potassium, zinc, folate, and vitamins D and E that were far below the recommendations. Children who consumed YCM or PCM had the highest odds in meeting adequacy of iron, zinc, thiamin, vitamin B6, folate, and vitamins C, D, and E as compared to other milks or non-dairy consumers, after adjusting for covariates. This study supports the hypothesis that dairy consumers had higher intakes of micronutrients and higher nutrient adequacy than children who consumed no milk or dairy products. Secondly, YCM or PCM have demonstrated to be good dairy options to achieve nutrient adequacy in Filipino children.

Background: Micronutrients are those vitamins and minerals required in very small quantities in our bodies which are essential for a number of different functions including growth and development. Micronutrient deficiencies (MD) are the conditions which are prevalent in the society and parents are unaware of the same due to their asymptomatic nature. The aim of this study was to study the prevalence of MD in school children in rural area of Kolhapur district.Methods: This prospective study was carried out among 960 school children of, 8 randomly selected Government schools (120 from each school and from each class 30 children) between the age group of 5-10 years in rural Kolhapur, between July-September 2017. Clinical Features of MD for vitamin A (corneal dryness, Bitot’s spot, skin lesions), for vitamin B (angular chelosis, glossitis, knuckle pigmentation), for vitamin C (bleeding gums, scorbutic rosary), for vitamin D (bow legs, rachitic rosary, pot belly, frontal bossing) and pallor, goitre, dental caries for iron, iodine and fluoride deficiency respectively were examined. The data was analysed statistically.Results: The overall prevalence of MD was 35% with 37.45% in girls with highest (40.8%) in the age group of 8-9 years. The features of vitamin deficiency for vitamin B in 30%, for vitamin A in 15%, for vitamin D in 12%, for vitamin C in 2%. The features of mineral deficiency for iron and fluoride in 38.8% and 36.6% respectively were present. Commonest clinical features of vitamin deficiency were glossitis (15.6%), corneal dryness (9.6%), angular chelosis (7.5%) and knuckle pigmentation (6.9%). Multiple MD were seen in 12.7% of children. Conclusions: Rural school children do suffer from MD significantly. These conditions should be timely evaluated as these are preventable and treatable.

Abstract Background Nutritional factors including vitamin D, magnesium, and fat are known to affect bone mineral accrual. This study aimed to evaluate associations between dietary nutrient intakes (both macronutrients and micronutrients) and bone mineral density (BMD) in children and adolescents. Methods Data for this cross-sectional, population-based study were derived from the National Health and Nutrition Examination Survey (NHANES). Participants aged from 8 to 19 years were included. The primary outcome was femoral neck BMD. Results Multivariate analyses revealed that for participants aged 8 to 11, daily sodium intake was significantly and positively associated with femoral neck BMD (B = 0.9 ×  10− 5, p = 0.031); in particular, subgroup analyses by sex found that in male participants aged 8–11, daily total cholesterol intake (B = 5.3 × 10− 5, p = 0.030) and calcium intake (B = − 2.0 × 10− 5, p < 0.05) were significantly associated with femoral neck BMD in a positive and negative manner, respectively, but neither were observed in female participants of this age group. In contrast, daily intakes of vitamin D and magnesium were significantly and positively associated with femoral neck BMD in female participants aged 8–11 (B = 246.8 × 10− 5 and 16.3 × 10− 5, p = 0.017 and 0.033, respectively). For participants aged 16 to 19, daily total fat intake was significantly and negatively associated with femoral neck BMD (B = − 58 × 10− 5, p = 0.048); further stratification by sex found that magnesium and sodium intakes were significantly and positively associated with femoral neck BMD only in females of this age group (B = 26.9 × 10− 5 and 2.1 × 10− 5, respectively; both p < 0.05). However, no significant associations between daily nutrient intakes and femoral neck BMD were identified in participants aged 12–15 before or after subgroup stratification. Conclusion The study found that associations of specific nutrition-related variables with BMD of the femoral neck is dependent upon age and gender.

AbstractData on the diets of young children in the UK are limited, despite growing evidence of the importance of early diet for long-term health. We used the largest contemporary dietary data set to describe the intake of 21-month-old children in the UK. Parents of 2336 children aged 21 months from the UK Gemini twin cohort completed 3-d diet diaries in 2008/2009. Family background information was obtained from questionnaires completed 8 months after birth. Mean total daily intakes of energy, macronutrients (g and %E) and micronutrients from food and beverages, including and excluding supplements, were derived. Comparisons with UK dietary reference values (DRV) were made usingttests and general linear regression models, respectively. Daily energy intake (kJ), protein (g) and most micronutrients exceeded DRV, except for vitamin D and Fe, where 96 or 84 % and 70 or 6 % of children did not achieve the reference nutrient intake or lower reference nutrient intake (LRNI), respectively, even with supplementation. These findings reflect similar observations in the smaller sample of children aged 18–36 months in the National Diet and Nutrition Survey. At a population level, young children in the UK are exceeding recommended daily intakes of energy and protein, potentially increasing their risk of obesity. The majority of children are not meeting the LRNI for vitamin D, largely reflecting inadequate use of the supplements recommended at this age. Parents may need more guidance on how to achieve healthy energy and nutrient intakes for young children.

The objective of this cross-sectional study was to investigate energy and nutrient intakes in pre-school children in Flanders-Belgium, using multistage clustered sampling. Diets of 661 children (338 boys; 323 girls) between 2·5 and 6·5 years old were assessed, with parentally reported 3 d estimated diet records. Usual energy and nutrient intakes were compared with national and international recommendations. Statistical modelling was used to account for within-individual variation. Mean daily energy intakes (boys, 6543 kJ; girls, 5757 kJ) approached the estimated energy requirements (EER) (boys, 6040 kJ; girls, 5798 kJ) for children < 4 years old. For children at least 4 years old, mean energy intakes (boys, 6408 kJ; girls, 5914 kJ) were below the EER of 6995 and 6740 kJ/d, respectively. Mean energy percentage (en%) derived from saturated fatty acids (SFA) (13–14 en%) was above the acceptable macronutrient distribution range (AMDR) upper level of 12 en%. Mean percentages derived from MUFA (10–11 en%) and PUFA (4–5 en%) were below the AMDR lower levels of 12 and 8 en%, respectively. For fibre, iron and vitamin D intakes, < 15 % of the children reached the recommended dietary allowances. Everybody exceeded the tolerable upper intake levels for sodium. Although diets in Belgian children were adequate in most nutrients, the implications of low iron, vitamin D and fibre intakes should be investigated. Furthermore, this affluent diet, characterised by SFA, MUFA and PUFA intakes differing from the recommendations and excessive sodium intakes, might increase the risk for CVD in later life.

Concern has been expressed that high dietary concentrations of non-milk extrinsic sugars (NMES) may potentially compromise nutrient intakes in population groups with low energy intakes (Department of Health, 1991). The objective of the present study was to examine data from the National Diet and Nutrition Survey of Children Aged 1.5 to 4.5 years (Gregory et al. 1995) for evidence of an inverse association between energy from NMES and micronutrient intakes, and if possible to quantify a level of NMES-energy at which micronutrient intakes may, theoretically, be compromised. Energy and nutrient intakes were compared across quintiles of NMES-energy for boys (n 848) and girls (n 827). As the concentration of NMES increased, energy intake rose (in boys only) while percentage energy from fat fell from 40 to 32 % across quintiles 1 to 5. Intakes of most micronutrients also fell, while intakes of vitamin C rose. Mean intakes of most micronutrients (Ca, thiamin, riboflavin, niacin, folate and vitamin C) were adequate in comparison with dietary reference values. However, intakes of Fe, Zn and vitamin D were low at all levels of NMES-energy and fell below the estimated average requirement for Fe and Zn for NMES concentrations exceeding 24 % of energy. Lower intakes of milk, meat, bread and vegetables, and higher intakes of fruit juice largely explain the observed trends in micronutrient intake. It is concluded that the inverse association of NMES with micronutrient intakes is of most significance for the 20 % of children with diets highest in NMES. However, further work is required to establish whether the associations observed have biological significance with regard to micronutrient status.

Vitamin D has pleiotropic effects, including maintaining calcium and phosphate homeostasis, and affecting the immune and endocrine systems. The article summarizes data on the expected biological effects of vitamin D on children’s health. The results of randomized clinical trials evaluating the effect of vitamin D supplementation on the incidence of acute respiratory tract infections are reviewed in detail. It has been shown that daily intake of vitamin D at a dose of 10 to 25 mcg/day (400–1000 IU/day) compared with placebo leads to a significant decrease in the proportion of children and adolescents who have had at least one episode of acute respiratory infection. The criteria for the provision of calciferol and the gradation of the status of vitamin D deficiency and deficiency in different countries are discussed. It has been shown that in the interpretation of the 25 (OH) D results there is a consensus on two points: calcidiol levels below 12 ng/ml (30 nmol/L) are considered clearly insufficient, and levels above 30 ng/ml (75 nmol/L) are considered sufficient in all regions. The incidence of hypovitaminosis D has been reported in children aged 3 to 7 years, where the overall combined assessment, regardless of age group, ethnic composition and breadth of the studied populations, showed that 13% of European children had a serum calcidiol concentration of less than 12 ng/ml (<30 nmol/L), and about 40% had a level of less than 30 ng/ml (<75 nmol/L). The data on the physiological need and the recommended daily dose of vitamin D for preschool children have been analyzed. In each country, the recommended intake of vitamin D is set (from 400 to 4000 IU), depending on the target concentration of serum calcidiol, the level of insolation, and nutritional characteristics. The necessity of conducting clinical studies in the age group from 3 to 7 years to determine an adequate, effective and safe preventive dose of vitamin D for preschool children in the Russian Federation is emphasized.

AbstractObjective:To examine the relationship between non-milk extrinsic sugars (NMES) concentration and micronutrient intakes and status in free-living older people.Design:The National Diet and Nutrition Survey of people aged 65 years and over; a cross-sectional study in Britain in 1994/5.Subjects and methods:Eight hundred and six men and women living in private households who were not on medication for nutritional disorders. Subjects were classified into four groups (Q1 to Q4), using cut-off points corresponding to quartiles of energy from NMES. Relationships were examined between dietary NMES concentration and intakes of calcium, iron, riboflavin, folate, vitamin C and vitamin D. Status indices were also assessed in each group.Results:Overall, a small proportion of men and women had intakes below the Lower Reference Nutrient Intake (LRNI) for calcium, iron, folate, riboflavin or vitamin C, while vitamin D intakes were universally low. Intakes of micronutrients tended to be highest at moderate levels of NMES in the diet (Q2 and Q3, equivalent to 8–15% of energy from NMES). Micronutrient status was low for riboflavin in 38% of the sample, while 15% had low vitamin C status and more than 10% were anaemic. There was little evidence that poorer nutrient status was associated with higher levels of dietary NMES. Although some indices of folate status were lower in Q4, this is most likely to reflect confounding by smoking. Energy intake accounted for 20–40% of the variance in micronutrient intakes, compared with less than 5% for NMES concentration.Conclusions:A diet moderately high in NMES (up to around 15% of energy) is unlikely to have a detrimental impact on micronutrient intake or status in this age group. Energy intake is the major determinant of micronutrient adequacy.

AbstractAlthough breakfast consumption is widely considered to be an important component of a healthy lifestyle, few UK studies have examined differences in nutrient intakes between breakfast consumers and breakfast skippers among children and adolescents. We investigated associations between breakfast skipping in 4–18-year-olds and their nutrient intakes using data from the UK’s National Diet and Nutrition Survey Rolling Programme. Dietary data were derived from 4-d estimated food diaries of 802 children aged 4–10 years and 884 children aged 11–18 years (1686 in total). Daily nutrient intakes of children with different breakfast habits were compared by one-way ANCOVA adjusting for relevant covariates (sex, age, ethnicity, equivalised household income and BMI). Within-person analysis was carried out on children with an irregular breakfast habit (n879) comparing nutrient intakes on breakfast days with those on non-breakfast days using repeated measures ANCOVA. We observed that the overall nutritional profile of the children in terms of fibre and micronutrient intake was superior in frequent breakfast consumers (micronutrients: folate, Ca, Fe and I (P<0·01)) and, for the 4–10 years age group, on breakfast days (micronutrients: folate, vitamin C, Ca and I (P<0·01)). Also, significantly higher proportions of breakfast-consuming children met their reference nutrient intakes of folate, vitamin C, Ca, Fe and I compared with breakfast skippers (χ2analysis,P<0·001). Our study adds to the body of data linking breakfast consumption with higher quality dietary intake in school-age children, supporting the promotion of breakfast as an important element of a healthy dietary pattern in children.

Dairy foods are under-consumed among Chinese children. We modeled the impact of increased dairy consumption on nutrient inadequacy and assessed whether the consumption of formulated milk powder for children ≥3 years (FMP3+) is useful for reducing nutrient gaps. Data from 3–8-year-old children, with completed socio-demographic and dietary measurements from the China Health and Nutrition Survey 2015, were used (n = 1122). Dietary intakes were modeled in two scenarios: Scenario 1 added FMP3+ or cow’s milk to reported diet to reach recommended dairy intakes; Scenario 2 replaced the currently consumed milk with an equal volume of FMP3+. Reported nutrient intakes were compared with each model. Only 32.5% of children consumed dairy products; the average intake amount in total was 48.6 g/day. Most children (97.6%) did not meet dairy intake recommendation. Inadequate nutrient intakes were observed for calcium, potassium, thiamin, riboflavin, vitamin C and selenium. In Scenario 1, both FMP3+ and cow’s milk improved the intake of all analyzed nutrients. In Scenario 2, FMP3+ substitution increased the intake of most nutrients, and reduced the proportion of children with an inadequate intake of vitamin C, thiamin, vitamin A, iron, zinc and potassium. Thus, increasing dairy consumption would reduce nutrient gaps, and FMP3+ is a good food source to help children meet nutrient requirements.
