Abstract Balance and gait are modifiable targets for falls prevention and may play an important role in preventing falls in older visually impaired individuals. Balance and gait were objectively evaluated in the 239 Falls in Glaucoma Study participants (average age=70.5, 22% with moderate-severe visual field (VF) damage). Greater sway, more time in double support and greater swing time variability were associated with higher fall rates, while higher gait velocity and faster cadence were associated with lower rates (p<0.05 for all). Neither gait nor balance mediated the relationship between VF damage and fall rates, with VF damage remaining an independent predictor of fall in models including gait and/or balance features (RR =1.36 to 1.48, p-value= <0.001 to 0.005). While balance and gait measures are associated with fall rates, they do not explain why persons with greater VF damage fall more frequently, suggesting the importance of other factors such as hazard perception.

Introduction The aim of this study was to determine the rate of readmission to hospital after hip fracture. The relationship between readmission to hospital and a range of social and functional variables, including receiving a home visit by an occupational therapist prior to discharge from hospital, was explored. Method A retrospective cohort study was conducted of 154 patients returning to community living following hip fracture. Multivariate logistic regression identified variables associated with risk of readmission to hospital. Results One in three patients was readmitted to hospital within 12 months after discharge, with 7% readmitted within 30 days. The most common reason for readmission was another fall. A low level of mobility prior to hip fracture was the strongest independent predictor of risk of readmission to hospital. There was no association between receiving a pre-discharge home visit by an occupational therapist and risk of readmission to hospital. Conclusion Rates of readmission to hospital are high after hip fracture, and falls are the single most common reason for readmission. Interventions provided by occupational therapists, including home visits, should emphasise and incorporate evidence-based falls prevention strategies.

Abstract Study question What are the basic elements of the infertility specialist - patient relationship? Summary answer For study participants, elements such as empathy, communication, collaboration and trust are important factors in increasing compliance and satisfaction with treatment. What is known already In the literature, the doctor-patient relationship is seen as fundamental in the treatment of infertility, due to the emotional implications of fertilization procedures. However, not much data is available specifically for this relationship. Communication and interpersonal skills, empathy, trust, collaboration, compliance and satisfaction are essential elements in this medical context. Study design, size, duration A cross-sectional study was conducted between May and June 2020 and follows the methods of a quantitative analysis, the data being collected using questionnaires. The research plan is one specific to the path analysis with mediation effect, in which the hypotheses were tested. Participants/materials, setting, methods The research group consists of 151 women diagnosed with infertility, voluntarily recruited through online support communities. Self-reported questionnaires provided socio-demographic information, information related to infertility, as well as information on the relationship of patients with specialists. In order to test the proposed hypotheses, an analysis of structural equations was performed, using the AMOS 20 program. Main results and the role of chance According to statistical indicators, CMIN / DF = 2.124, RMSEA = .087 and the p value of the Chi-square is less than 5% significance level, the model proposed is accepted. Based on the reported NFI and CFI values (.777 and .867), as well as the values of RFI = .763, IFI = .868 and TLI = .859, it can be concluded that the model is appropriate for the sample data. The p value of all constructors is less than .05, except for trust in doctor - cognitive empathy and patient satisfaction. All the others paths are significant: the effect of communication on treatment compliance, mediated by trust in specialist and collaboration; the effect on communication on treatment compliance, mediated by affective empathy, trust in specialist and collaboration; the effect on communication on patient satisfaction, mediated by affective empathy and trust in doctor; the direct effect of communication on trust in doctor, as well as the mediated effect by affective empathy. Regarding the relationship with doctor during the COVID–19 pandemic, 33% of the study participants stated that the pandemic affected the relationship with doctor, and 44% discontinued contact with specialist and medical procedures between March and June 2020. Limitations, reasons for caution Only infertile, voluntary women participated in this study, which limits the generalizability to other populations. Because the data are cross-sectional and correlational, the direction of causation is not proven. Additional experimental research on larger populations is needed to examine the effects of communication on patient satisfaction and compliance. Wider implications of the findings: The study draws attention to the importance of basic concepts in the relationship of specialists with infertile patients. Thus, it is necessary for health care providers in assisted human reproduction to participate in programs for the continuous training of empathic communication skills, given the sensitivity of this diagnosis. Purpose This exploratory study aimed to investigate the potential impact of sentence-level comprehension and sentence-level fluency on passage comprehension of deaf students in elementary school. Method A total of 159 deaf students, 65 students ( M age = 13.46 years) in Grades 3 and 4 and 94 students ( M age = 14.95 years) in Grades 5 and 6, were assessed for nonverbal intelligence, vocabulary knowledge, sentence-level comprehension, sentence-level fluency, and passage comprehension. Group differences were examined using t tests, whereas the predictive and mediating mechanisms were examined using regression modeling. Results The regression analyses showed that the effect of sentence-level comprehension on passage comprehension was not significant, whereas sentence-level fluency was an independent predictor in Grades 3–4. Sentence-level comprehension and fluency contributed significant variance to passage comprehension in Grades 5–6. Sentence-level fluency fully mediated the influence of sentence-level comprehension on passage comprehension in Grades 3–4, playing a partial mediating role in Grades 5–6. Conclusions The relative contributions of sentence-level comprehension and fluency to deaf students' passage comprehension varied, and sentence-level fluency mediated the relationship between sentence-level comprehension and passage comprehension.

Previous research suggests a relationship between neuroticism (N) and the speed-accuracy tradeoff in speeded performance: High-N individuals were observed performing less efficiently than low-N individuals and compensatorily overemphasizing response speed at the expense of accuracy. This study examined N-related performance differences in the serial mental addition and comparison task (SMACT) in 99 individuals, comparing several performance measures (i.e., response speed, accuracy, and variability), retest reliability, and practice effects. N was negatively correlated with mean reaction time but positively correlated with error percentage, indicating that high-N individuals tended to be faster but less accurate in their performance than low-N individuals. The strengthening of the relationship after practice demonstrated the reliability of the findings. There was, however, no relationship between N and distractibility (assessed via measures of reaction time variability). Our main findings are in line with the processing efficiency theory, extending the relationship between N and working style to sustained self-paced speeded mental addition.

Background: The relationship between gestational diabetes and postpartum depression (PPD) is poorly understood and seldom studied. Objective: In an effort to explore this issue, the present study investigated the relationship between gestational diabetes and PPD. Methods: The present cross-sectional study was performed with 342 women who were referred to four urban health centers of Hamadan city, west of Iran. We used convenience sampling as a method to recruit women in each health center. We used a researcher-made checklist for gathering data on socio-demographic characteristics and potential risk factors of PPD. The Persian validated version of the Edinburgh Postnatal Depression Scale (EPDS) was used to assess PPD. Univariate and multivariable binary logistic regression was applied to estimate the odds ratio (OR) (95% confidence interval [CI]). Results: Gestational diabetes was identified as the most important risk factor for PPD with OR (95% CI) of 2.19 (1.11, 4.31); P-value=0.02 after adjusting for other variables. Moreover, the adjusted odds ratio showed that PPD among lesseducated women (primary school) was 3.5 times higher compared to women with a university education (OR=3.54, 95% CI: 1.27, 9.84; P-value=0.01). Conclusion: Our findings suggested that PPD is more likely among women with gestational diabetes and those who were less educated. Interventional and educational activities for reducing the risk of PPD can be targeted for use with this population.
