The Effects of Recombinant Human Granulocyte–Macrophage Colony-Stimulating Factor Gel on Third-Degree Frostbite Wounds in Northeastern China: A Randomized Controlled Trial

Abstract The aim of the study was to investigate the effects of the rhGM-CSF gel on third-degree frostbite wounds. Sixty-two patients who had suffered third-degree frostbite on their hand or foot (91 wounds in total) were selected using a convenience sampling method and randomly allocated to two groups: the rhGM-CSF group(31patients,45 frostbite wounds) received the rhGM-CSF gel when wound dressing change daily; however, the control group (31patients, 46 frostbite wounds) received aloe glue. The wound healing time, the score of inflammation about the wound and the positive bacterial culture of wound secretions were used to measure outcomes, respectively. Data were analyzed using SPSS (25.0), Student’s t test or Mann–Whitney U test and chi-square test or Fisher exact test were selected, as appropriate. The healing time of the rhGM-CSF group was (12.2 ± 5.0) days, which was significantly shorter than that of the control group (15.5 ± 4.7) days (P < .0001). The rhGM-CSF group’s wound inflammation scores on the 7th and 14th day of treatment were (0.96 ± 0.21) and (1.88 ± 0.29), respectively, which were better than those of the control group (1.12 ± 0.24) and (1.38 ± 0.15) (both P < .0001). The positive bacterial culture of wound secretions in the rhGM-CSF group was also better than that in the control group on the 3rd, 7th, and 14th day after treatment (P = .027, .004, .030, respectively). According to the results, using rhGM-CSF gel considerably increases the speed of frostbite wounds healing, and have an effect on protecting third-degree frostbite wounds regarding the positive effects. BACKGROUND: The second leading cause of cancer-related death in women is breast cancer. Xenobiotic Metabolizing Enzymes (XMEs) contribute to the detoxification of numerous cancer therapy-induced products. In the metabolism of xenobiotic, cytochrome P450s or monooxygenases perform an important function by catalysing the hydroxylation reaction. In this study, the susceptibility and genetic polymorphisms of CYP450 isoenzymes was investigated that may have an etiological role in breast cancer. AIM: The main purpose of this study was to evaluate the association of CYP1A1 (rs4646421), CYP1B1 (rs1056836), CYP2C8 (rs1058930), and CYP19A1 (rs749292) polymorphisms with the risk of breast cancer in Mazandaran province. MATERIAL AND METHODS: This cross-sectional case-control study were recruited 72 patients and 51 healthy individuals and was performed between March 2018 to May 2018 in the Oncology Department at Imam Hospital in Sari city, Iran. Peripheral blood samples were collected in EDTA tube, and DNA extraction was performed using the salting-out method and WizPrep extraction kits. Breast cancer patients with known clinicopathological characters and healthy women as control group were genotyped for genes polymorphisms by PCR-RFLP technique, using restriction enzymes. Chi-square, Fisher exact test and Logistic regression model, were applied for statistical analysis. RESULTS: The results of the experiments showed that there was a significant relationship between two groups and the age of the patients is significantly higher than the control group (p = 0.044). According to the chi-square and Fisher exact test, education, pregnancy, menopause status and oppose were significant between the two groups. Based on using a logistic regression model in two normalized and age-adjusted models to finding relationship between the genotypes of each gene and breast cancer risk, it was determined that in the CYP2C8 genotype, those who have the CG allele have a 7.74 degree increased risk of breast cancer (CI = 95% 0.95-62.5) and in the CYP19A1 gene, individuals with GA genotype, increased risk of breast cancer (CI = %95 1.52-27.21), about the CYP1B1 gene, people with two genotypes of CG + GG had higher risk of breast cancer (CI = %95 1.19-5.71) and allele G has decreased risk of breast cancer in this gene (P = 0.0271), also allele G in CYP2C8 gene had the protective effect (P = 0.02). In the age-adjusted model, for the CYP2C8 gene, GG genotype increased risk of breast cancer (CI = %95 1.11-75.84) as well as, the CG + GG genotype in CYP1B1 gene (CI = %95 1.31-6.57). CONCLUSION: Our results confirm the association between CYP2C8 (rs1058930), CYP19A1 (rs749292) and CYP1B1 (rs1056836) gene polymorphisms and increased risk of breast cancer in women in Mazandaran province.

Penggunaan warfarin dan antibiotik bersamaan diketahui dapat meningkatkan risiko perdarahan. Kejadian perdarahan mayor terkait warfarin dapat berupa perdarahan fatal hingga menyebabkan kematian. Penelitian ini bertujuan untuk menganalisis interaksi warfarin dan antibiotik dan efeknya terhadap kejadian perdarahan pada pemeriksaan pertama pasca rawat inap di RSUD Wates. Desain studi berupa kohort retrospektif pada 67 kasus penggunaan warfarin dengan/tanpa antibiotik oleh pasien rawat inap. Data berupa diagnosis, terapi, pemeriksaan pendukung, dan kejadian perdarahan saat rawat inap hingga pemeriksaan pertama diambil dari rekam medik pasien tertanggal Januari 2018 hingga September 2020. Analisis data dilakukan secara statistika menggunakan ­uji Chi-square atau uji Fisher exact test dan uji independent t-test atau uji Mann Whitney U test. Terdapat perbedaan bermakna pada kejadian perdarahan dalam waktu 7 hari pasca rawat inap antara kelompok antibiotik dan kontrol (16%; 0%; p = 0.017). Antibiotik yang digunakan oleh pasien dengan kejadian perdarahan yaitu golongan sefalosporin, kuinolon, azitromisin, dan ampisilin-sulbaktam. Kejadian perdarahan yang terjadi meliputi hematemesis melena (25%), hematuria (50%), tidak diketahui (25%). Studi ini menyimpulkan bahwa penggunaan bersamaan warfarin dan antibiotik saat rawat inap dapat berpengaruh terhadap kejadian perdarahan dalam waktu 7 hari setelah pasien dipulangkan.Penggunaan warfarin dan antibiotik bersamaan diketahui dapat meningkatkan risiko perdarahan. Purpose: Sensitization to metals is a significant problem in dental, occupational exposures. The purpose of this study was to evaluate the prevalence of contact sensitization to selected metals during the course of study among students from dental technician school and the prevalence of co- sensitization. Material and Methods: Skin patch testing with potassium dichromate, cobalt, gold, nickel, copper, palladium, aluminium, tin and sodium tetrachloropalladate(II)hydrate was performed among 150 dental technician students (38 – 1st year of study,40 – 2nd year of study and 38 – 3rd year of study); the control group included 34 students without educational exposure to metals. Chi-square test, Fisher Exact Test and multiple binary logistic regression were used in data analysis. Results: During the educational course of dental technician students, a clear tendency of increase in the prevalence of sensitization to nickel (1st year of study - 10.5%, 3rd year of study - 36.8% cobalt (13.2% vs 39.5%) and palladium(II)chloride (10.5% vs 26.3%) was established. For the whole studied population, the prevalence of positive reactions to sodium tetrachloropalladate (II) hydrate was the highest – 30%; the highest recorded value was among the 2nd year students – 57.5%. The prevalence of sensitization to more than one metal allergen among the studied population was high (66.7%), the highest value was reported among 3rd-year dental technician students (78.9%). Conclusions: Our data indicate a high risk of contact sensitization to metals relevant for the dental technician students, which increases in the course of educational exposure for some of the metal haptens. Sodium tetrachloropalladate(II) hydrate could be outlined as a sensitizer of prominent importance, probably due to the increased sensitivity of this test salt. Additional and studies, including more respondents from the othe Colleges in Bulgaria are recommended. Adequate prevention programs and information should be provided, and the European Union regulations should be properly applied.

<p><strong>Objective:</strong> Aim of this study was to compare the maternal and neonatal outcomes between adolescent multiparas and adults.</p><p><strong>Study Design:</strong> This retrospective cohort study included over 11 years and singleton multiparous pregnancies from women less than 29 years admitted for delivery after 22nd gestational weeks in tertiary educational hospital. All of patients were divided into two groups as adolescent multipara (under 18 years) and control (19-29 years). Pregnancy outcome and perinatal complications were evaluated. Mann-Whitney U-test, Independent sample t test, Pearson Chi-square test and Fisher Exact test were used for statistical analyses.</p><p><strong>Results:</strong> Gestational week at delivery was significantly lower in adolescent multipara group (38.37 ± 2.17) than control group (38.79 ± 1.684) (p=0.014). Preterm birth rate (12.6%) and postpartum hemorrhage (5.7%) were higher in the adolescent multipara group than in controls as to be statistically significant. The cesarean delivery rate and other pregnancy complications such as diabetes mellitus, preeclampsia, intrauterine growth restriction, oligohydramnios, polyhydramnios, and placental abruption were similar with controls.</p><p><strong>Conclusion:</strong> In conclusion, adolescent multipara pregnancies were associated with higher risks of adverse pregnancy outcomes, in particular preterm birth, lower birthweight and postpartum hemorrhage.</p>

Background: Food service success relates to patient plate waste. Patient plate waste in Indonesia Hospitals could be more than 25%. Cause of patient plate waste in Hospital is lack of food and equipment quality. Almost of 32.8% patients in 10 hospitals model in Indonesia clarify that appearance, cleanliness, and equipment are good.Objective: The aim of theses research was to identify the effect of serving utensils and the other factors to the plate waste.Methods: This was a Quasi Experiment study using pre posttest with control. Study subject consisted of 45 patients in the treatment group and 45 patients in the control group. Data were analyzed descriptively and whereas t-test, Fisher Exact test, Chi-Square were performed to differentiate the proportions and means in two groups. The effect of several variables on plate waste was calculated using logistic regression on analysis.Results: This study showed that no significant difference of taste, appearance, attitude of the staff who serve meals, service timeliness, kind of food, kind of diseases and environment to patient plate waste (p>0.05). There was no significant effected of the serving disposable utensils on patient plate waste (p>0.05).Conclusion: The type of serving disposable utensils was no a significant factor effected the plate waste.

Abstract Objective: The objective of this study was to compare the cognitive performance of elderly who are enrolled in a physical activity (PA) program with those who are not, considering some sociodemographic variables that are related to the practice of cognitive activities. Method: 59 elderly people participated in this study divided into control group (CG) and gymnastics group (GG). The Vienna Test System® was used to estimate the attention and concentration capacities, simple reaction time and peripheral perception. To test the normality of the data it was used the Shapiro Wilk Test. The association between PA and socioeconomic variables was verified by Chi-Square Test and Fisher Exact Test. To compare of differences between groups the Mann Whitney U-test for variables with non-parametric distribution and the Student T-test for independent samples for other variables were used. Result: There were no significant differences in the performance of the groups, except in the capacity of attention and concentration in which CG presented best performance (p=0.01). Factors as schooling (p=0.02), income (p=0.001) and regular practice of handcrafts (p=0.06) can explain the best performance of CG. Conclusion: This study found no evidence that the practice of physical activity can generate some cognitive benefit in the elderly when compared to the elderly who did not practice regular physical activity. However, we perceive the existence of other aspects that influence on the cognition, such as schooling, sociocultural level and the reading habit, that have a significant importance degree in the analysis.

SummaryA retrospective study was done to determine the incidence of and the risk factors predisposing to clinical venous thromboembolism (VTE) in patients treated for high grade gliomas. Medical records of 68 consecutive patients diagnosed and treated at Saint Francis Hospital and Medical Center from January 1986 to June 1991 were reviewed. The follow up was to time of death or at least 6 months (up to December 1991). All clinically suspected episodes of VTE were confirmed by objective tests. Sixteen episodes of VTE were detected in 13 patients for an overall episode rate of 23.5%. Administration of chemotherapy (p = 0.027, two tailed Fisher exact test) and presence of paresis (p = 0.031, two tailed Fisher exact test) were statistically significant risk factors for the development of VTE. Thrombotic events were more likely to occur in the paretic limb and this difference was statistically significant (p = 0.00049, chi square test, with Yates correction). No major bleeding complications were seen in the nine episodes treated with long term anticoagulation.We conclude that venous thromboembolic complications are frequently encountered in patients being treated for high grade gliomas and the presence of paresis and the administration of chemotherapy increases the risk of such complications.

Objective: To compare the success rate of different obturation procedures in primary mandibular second molars clinically and also by digital radiovisiography. Methods: A total of 40 children aged between 4-8 years with deeply carious mandibular second primary molars indicated for single session pulpectomy were selected. Canals were obturated with Metapex. The 3 study groups (Endodontic plugger, Handheld lentulospiral, Navi Tip syringe) were compared with the control group (reamer) both clinically and radiovisiographically. The data collected were statistically analyzed using Pearson’s Chi-square and Fisher’s exact test. Results: The use of Navi tip syringe led to the least number of voids followed by Endodontic plugger and Reamer and the highest number of voids was reported with Lentulospiral. Navitip presented maximum number of optimally filled cases followed by Endodontic plugger and Lentulospiral and least number of optimally filled cases with reamer. However, there was no statistically significant difference (p>0.05) in any of the groups with clinical (pain and tenderness to percussion) and radiographic parameters (presence or absence of voids and length of obturation). Conclusion: Within the limitations of the present study, though the clinical outcome was statistically insignificant, Navitip syringe exhibited encouraging results and is a promising option for obturation in primary teeth.

AbstractBackgroundAnxiety and fear of labor pain has led to elevated cesarean section rate in some countries. This study was conducted to investigate the effect of yoga in pregnancy on anxiety, labor pain and length of labor stages.MethodsThis clinical trial study was performed on 84 nulliparous women who were at least 18 years old and were randomly divided into two groups of yoga and control groups. Pregnancy Yoga Program consisting of 6 60-min training sessions was started every 2 weeks from week 26 of pregnancy and continued until 37 weeks of gestation. Anxiety severity at maternal admission to labor was measured by the Spielbergers State-Trait Anxiety Inventory, and labor pain was measured by Visual Analogue Scale (VAS) at dilatation (4–5 cm) and 2 h after the first measurement. Data were analyzed using Chi-Square and t-test.ResultsIntervention group reported less pain at dilatation (4–5 cm) (p=0.001) and 2 h after the first measurement (p=0.001) than the control group. Stat anxiety was also lower in intervention group than the control group (p=0.003) at the entrance to labor room. Subjects in the control group required more induction compared to intervention group (p=0.003). Women in intervention group experienced shorter duration of the first phase of the labor than the control group (p=0.002). Also, the total duration of two stages of labor was shorter in intervention group than the control group (p=0.003).ConclusionsPracticing yoga during pregnancy may reduce women’s anxiety during labor; shorten labor stages, and lower labor pain.

Objectives:Relation of demographics of hepatocellular-carcinoma with the aetiology.Tumour characteristics in relation to anti-viral therapy and presence of viral-DNA/RNATreatment modalities offeredMethods: This cross-sectional study enrolled all the patients aged 18-70 years with diagnosed hepatocellular carcinoma either through Triphasic Contrast-Enhanced-CT scan and/or Magnetic Resonance Imaging or biopsy presenting to the Outpatient-Department or multi-disciplinary-team meetings for the year 2019. Demographic variables, biochemical analysis including liver profile and stage of cirrhosis, viral-status, tumour staging and the treatment modalities offered were all noted. ANOVA (normal) and Kruskal-Wallis (non-normal) tests were used to compare quantitative data whereas chi-square-test and fisher-exact-test were used to compare qualitative-data.Results: Out of 195 patients with hepatocellular carcinoma, 76% were males in their fifth to sixth decades of life, 96% had cirrhosis, 94% corresponded to viral hepatocellular-carcinoma (82% Hepatitis-C-Virus, 9% Hepatitis-B-Virus and 3% coinfection), 60% of the cirrhotics landed in Child-Pugh A category with tumour staging BCLC-B being the predominant one (43.6%) and single and multiple bilateral nodules were the commonest lesions encountered. Platelets and Alanine-Transaminase had a significant relation across aetiological groups. Lymph-nodes were the most common extra-hepatic organs for metastasis and the presence of viral PCR had a significant impact on the tumour aggressiveness. Thirty-two percent of the patients were amenable to curative treatment.Conclusion: Viral infection is the main cause of rising prevalence of this tumour in Pakistan. Treatment modalities are expensive and expertise are lacking. A nationwide cancer registry is required for the exact disease burden and tumour behaviour for our population. Continuous....
