Bowens' disease (BD) is a precancerous condition of skin and/or mucosa with a predilection towards sun-exposed areas. Extensive literature research failed to reveal any case of multiple pigmented BD in type V Fitzpatrick skin. Multiple BD is a therapeutic challenge with a tendency to recur. Here we present an otherwise healthy Indian male having multiple pigmented lesions of BD on sun-protected sites of the body mimicking malignant melanoma. These lesions were refractory to treatment with different modalities. This paper is an attempt to review the available literature regarding the pigmented variant of multiple BD. Rationale for a therapeutic trial of UVB therapy for multiple arsenic induced BD is also discussed.

Background: A variety of side effects following the tattooing of the skin were reported over the years. Analytical studies showed that some tattoo inks contain harmful compounds. Methods: We presented six patient cases with cutaneous malignancies in tattooed skin and performed an extensive literature research. Results: Two patients with black ink tattoos that were diagnosed with malignant melanoma raises the number of described cases to 36 patients. One of the patients developed an immunologic reaction limited to the tattoo area after treatment with a targeted immune therapy. In the other patient, the malignancy (malignant melanoma) was fatal. Basal cell carcinoma was seen in four patients with tattoos containing varying ink colors (black, green, red). This increased the number of described patient cases to 18. Although some ink components and their cleavage products have carcinogenic properties, epidemiological evidence for a causative correlation fails. Further epidemiologic studies on tattoos and malignancies, as well as on the appearance of naevi in tattoos, are necessary. Determining the type of mutation might be helpful to separate sun-induced tumors from skin cancers due to other pathogenic mechanisms.

Abstract Background Aplasia cutis congenita is regarded as congenital focal absence of skin in the newborn, and occurrence of more than three similar skin defects is rare. The etiology is thought to be multifactorial, and precise etiopathogenesis is unknown. Case presentation A 13-day-old newborn Sri Lankan Tamil girl was referred to the dermatologic clinic with multiple skin defects at birth. There were six lesions on the body, and two of them had healed during intrauterine period, leaving scars. This was a second twin of her pregnancy. Her first twin fetus had demised before 19 weeks of pregnancy and was confirmed to be fetus papyraceous based on ultrasound-guided fetal assessment. The said child was thoroughly investigated and found to have no other congenital abnormalities. Chromosomal studies yielded normal findings. She was treated with tropical antibacterial ointment, and all lesions resolved spontaneously within 4 weeks, leaving scars. Physiotherapy was commenced to prevent contracture formation, and follow-up was arranged in collaboration with the plastic surgical team. Conclusions Aplasia cutis congenita is a rare condition of uncertain etiology, but consanguinity may play a role. This report described a newborn with type V cutis aplasia congenita in whom the diagnosis was confirmed based on clinical features and revision of antenatal history. The management depends on the pattern, extent, location, severity, underlying causes, and associated anomalies.

ObjectiveWe report a case of a 58-year-old gentleman who was hospitalised intermittently for one year due to treatment resistant schizophrenia. Prior to hospitalisation he had been prescribed standard antipsychotics for decades without full resolution of positive psychotic symptoms. During his final admission lasting six months he was guarded, suspicious, irritable, constantly paced the corridor and displayed thought block and paranoid persecutory delusions. He would not enter the assessment room or allow any blood or ECG monitoring, however, he was compliant with oral medication. He was successfully treated with high dose olanzapine (40mg/day) and was discharged to the community. The aim of this study is to bring awareness and add to the body of evidence for the use of high-dose olanzapine in patients with treatment resistant schizophrenia in whom a trial of clozapine is not possible.Case reportThe patient gave written consent for this case report to be written and presented. An extensive literature review was performed and key papers were identified. Discussion focuses on the key areas in the literature.DiscussionThis case demonstrates that high-dose olanzapine can be used effectively as an alternative to clozapine in treatment resistant schizophrenia.ConclusionThis case highlights the need for further evaluation of high-dose olanzapine as an alternative to clozapine in patients with treatment-resistant schizophrenia.

Abstract Background: The traditional medical practices of the Daur region are an important element in Chinese ethnomedicinal knowledge. However, relatively little recording of traditional Daur medicine (TDM) has been carried out. As it is gradually being replaced by traditional Chinese medicine (TCM) and modern medicine, further research is urgently needed. Methods: We collected ethnobotany and ethnozoology data about their local name, parts used and diseases to be treated, dosage, and route for administration through semi-structured interviews with 114 informants (18 sessions), focus group discussions (6 sessions), and extensive literature reviews. Medicinal samples and specimens were collected during field investigations from June 2015 to October 2018. The diseases and ailments reported as being treated were classified according to the International Classification of Diseases 11 th (ICD-11). The expression correlation of medicine classification, medicine, and disease was assessed using Cytoscape 3.6.1 software. Quantitative indices such as informant consensus factor (ICF) and use value (UV) were used to evaluate the importance of ethnomedicinal data. Results: We documented 30 plant species (from 19 families) and 21 animal species (from 11 families). In the past, the majority of plant species were collected from natural habitats, but now they are mainly cultivated. The most widely utilized plants were herbs (21 species), followed by shrubs (4), trees (3), and fungus (2). The most utilized groups of animals were mammals (14 species), followed by birds (5), amphibians (1), and reptiles (1). Medicines were mostly administered orally (43) but were also applied externally (11) or delivered via both routes (9). Informants indicated that medicines were prepared via decoction (21), grinding (11), boiling (10), extraction (8), and burning (7). Medicines were taken as a drink (37), eaten (16), or made into pills and powders (7), but could also be administered as an ointment/wash (6), wrap/dressing (5), in the nose, eyes, and mouth (4), or as a fumigate (2). The TDMs investigated in this study treated 76 human diseases or ailments classified under 13 disease categories, based on the ICD-11. The highest ICF (1.00) was reported for neoplasms and visual system diseases and childbirth or the puerperium, followed by injury, poisoning, or other diseases resulting from external causes (0.92), and symptoms, signs or clinical findings, not elsewhere classified (0.82). The most used medicinal species were Cervus elaphus , Cervus nippon , Capreolus , Gallus , Canis lupus familiaris , Betula platyphylla , and Artemisia integrifolia . Conclusions: A substantial body of ethnobotanical and ethnozoological knowledge could lead to the development of new medicines. Therefore, research into Daur ethnic medicines is urgently required The current research can make an important contribution to the body of knowledge about the national medicine of the Daur people. This is particularly important because the younger generation is not interested in learning and preserving their traditional medical practices. The information documented in the present study will provide a useful basis for future investigations into the ethnopharmacological and traditional knowledge of the Daur region.

Purpose Decisions by construction contractors to bid (or not to bid) require the thorough assessment and evaluation of factors relevant to the decision, as well as the quantification of their combined impact, to produce successful bid/no-bid decisions. The purpose of this study is to present a fuzzy fault tree model to assist construction contractors to more efficiently bid for future projects. Design/methodology/Approach The proposed model consist of two stages: first, identification of the factors that affect bidding decision using a questionnaire survey after an extensive literature review, and second, usage of the identified factors to build a fuzzy fault tree model to simulate the bidding decision. Findings A list of 15 factors that affect bid/no-bid decisions was identified. Analysis of factors revealed that the highest-ranking factors were related to financial aspects of the project. A case study is presented to demonstrate the capabilities of the model, and a fuzzy important analysis is performed on the basic events to demonstrate the differences between three contractors’ bid/no-bid decisions. The results reveal that there is variation between the decisions of each contractor based on their willingness to participate. Besides, the influence of evaluation factors on the final decision for each contractor is different. Originality/value The study contributes to the body of knowledge on tendering and bidding practices. The proposed model incorporated the fuzzy set theory, which suits human subjectivity. The proposed methodology overcomes the limitations of previous models as it can, using the linear pool opinion principle, combine and weigh the evaluations of multiple experts. In addition, the model is convenient for situations where historical data are not available.

Malignant melanoma (MM) in the female genital tract accounts for less than 2% of all melanomas, and the vast majority associated occur in the vulva and vagina. Primary MM of the uterine cervix is extremely rare and its prognosis is very poor. We report a case of primary MM of the cervix with dissemination throughout the vaginal wall. A 66-year-old woman presented with postmenopausal bleeding. Gynecologic examination demonstrated a 2 cm polypoid blackish-pigmented tumor on the cervix with multiple small blackish-pigmented lesions throughout the vaginal wall. Cervical Pap smear cytology showed malignant melanoma. MRI and PET/CT did not detect any distant or lymph node metastases. She underwent radical hysterectomy, pelvic lymphadenectomy, and total vaginectomy. The pathological diagnosis was FIGO stage IIIA primary cervical MM. She received adjuvant chemotherapy with 6 courses of dacarbazine, but 6 months later, multiple lung metastases were detected. Despite 4 courses of anti-PD-1 antibody (nivolumab) treatment, she died of the disease 13 months after surgery.

Moreover, this energy and fitness are directly related to mental health and the signals being transmitted between the brain and other parts of the body. Various activities like walking, talking, eating, and thinking are performed with the help of these transmission signals. Another critical role played by them is that it helps in examining the mechanisms of cells present at various places in the human body and signaling the nervous system and brain if they are properly functioning or not. This manuscript is divided into two parts where, in the first part, it provides the introduction, background, and extensive literature survey on Kirlian experiments to measure the human's organ energy.

Purpose This paper aims to develop a novel theoretical technology acceptance model, namely, for predicting acceptance of the trending technology of intelligent contracts (iContracts) in construction, which aims to integrate the data from emerging cyber-physical systems being introduced to the sector through the industry 4.0 revolution. This model includes main dimensions and critical contributing factors to assess the readiness for the iContract concept within the construction contract environment. Design/methodology/approach Through an extensive literature review, the structure of a unique theoretical technology acceptance model for iContract implementation, within construction, was developed iContract acceptance model (iCAM). Relevant themes were assessed through the lens of the technology acceptance model framework and the four accepted dimensions of the technology readiness index (TRI) concept. The main components of the model were examined with selected practitioners, with relevant experience and understanding of the iContract concept, with thematic mapping of the discussions correlated back to 12 specific iContract contributing constructs of the four adapted TRI dimensions. Findings The paper contributes to the body of knowledge by proposing a novel iCAM for a trending technology based on the specific requirements of iContract adoption. The interviews show that while the desire to digitalise the contractual environment exists, the readiness of the sector for such a disruptive change is unknown. Practical implications The findings and proposed conceptual iCAM offers a lens for the further development of the iContract concept by assisting practitioners to forecast digital readiness of the contract process in construction. Originality/value This study offers a unique and theoretical framework, in an embryonic field, for predicting the success of iContract implementation within construction organisations through the digital, industry 4.0 and revolution.

Self-injury is a major clinical problem on the women's wards at Ashworth Hospital. Ashworth Hospital is one of three special hospitals within England and Wales that provides assessment, treatment and rehabilitation for those legally detained patients under the Mental Health Act (1983) who suffer from a mental disorder and require conditions of special security because of their dangerousness. There is extensive literature on the psychological aspects of serf-injury. However, since patients with self-injury present with damage to the body, it is surprising that there is little information on the physical health care and associated resource costs of this behaviour.
