Background: Tuberculosis (TB) incidence remains low in health departments of Castellon and La Plana-Vila-real, but TB elimination is challenging. The objective of this study was to estimate associated factors of pulmonary tuberculosis (PTB) compared with extrapulmonary tuberculosis (ETB) and investigate epidemiological characteristics of these pathologies to orient control and prevention actions. Materials and Methods: A prospective case-case study was implemented by comparing PTB and ETB incidences during 2013-2016 from notification reports, epidemiological surveillance, and microbiological results of hospitals’ laboratories Hospital General Castellon and La Plana-Vila-Real in the province of Castellon of Valencia region in Spain. In this design, cases were patients with PTB and controls were patients with ETB. Directed acyclic graph approach was used for selection of potential risk and confounding factors. Adjusted odds ratios (AORs) were estimated by logistic regression models. Results: The study included 136 patients with PTB and 57 patients with ETB, with microbiological confirmation of 93.4% and 52.6%, and the annual median of incidence rates were 7.5 and 3.1 per 100 000 inhabitants, respectively. In general, patients with PTB were younger with higher male proportion than patients with ETB. Risk factors of PTB were smoking tobacco (AOR = 3.98; 95% confidence interval [CI] = 1.66-9.56), social problems (social marginalization, homeless, residence in shelters for the poor, or stay in prison) (AOR = 3.39; 95% CI = 1.05-10.94), and contact with patients with TB (AOR = 2.51; 95% CI = 1.06-5.95). No-smoking tobacco and no-drug abuse interaction decrease PTB risk (AOR = 0.27; 95% CI = 0.12-0.64). From these results, specific measures of health promotion and prevention can be addressed. Conclusions: The estimated associated factors of PTB may be prevented, and it was demonstrated that the case-case design is useful in the study of TB.

OBJECTIVE: To describe the epidemiological trends and characteristics of extrapulmonary tuberculosis (EPTB) in Galicia, Spain, from 2000 to 2019.METHODS: This was a retrospective cohort study based on data from the Galician TB information system.RESULTS: Of the total number of TB cases (n = 15,871), 5,428 (34.2%) had EPTB. The absolute number of cases and incidence of EPTB decreased dramatically (from 480 cases and 17.8 cases/100,000 in 2000, to 172 and 6.4 cases/100,000 in 2019, respectively), with a mean annual decrease of respectively 64% and 4.7% for absolute cases and incidence rates. The risk for EPTB was higher in men than in women (RR 3.86, 95% CI 3.66–4.07). The most frequent age group was 15–44 years (2,234 patients, 41.2%); overall reductions per age group were 82% (0–14 years), 75% (15–44 years), 44% (45–64 years) and 63% (≥65 years), with statistically significant differences. The most frequently locations were the pleura (1,916 cases; 35.3%) and the lymph nodes (1,504; 27.7%).CONCLUSION: The incidence of EPTB in Galicia has decreased significantly in the last 20 years. The epidemiological characteristics have not changed, except for the number of patients with risk factors. This improvement of EPTB epidemiological trends coincides with the implementation of the programme for the prevention and control of TB, which suggests that it has been very effective in the control of the EPTB.

Introduction. The diseases caused by Leishmania are spread worldwide and represent a significant public health problem. Objective. The aim of this study was to present the results of epidemiological surveillance of leishmaniasis in humans in Montenegro in the period from 1992 to 2013. Methods. The study was planned and realized as a descriptive epidemiological study. The sample included patients of leishmaniasis in Montenegro in the period from 1992 to 2013. The health and demographic data were collected from medical records. The disease was microbiologically proven in the patients. For statistical analysis the ?2-test was used, which examined the significance of the incidence rate. Results. During this period, 66 cases of leishmaniasis were identified (40 men and 26 women) aged 0 to 62 (mean 15.61?16.76 years). A visceral form of the disease was diagnosed in 65 (98%) patients, and one patient was diagnosed with cutaneous leishmaniasis. The average incidence rate for the abovementioned period is 0.48 per 100,000 inhabitants. The highest average incidence rate was identified in patients up to seven years of age (3.50 per 100,000 inhabitants). The highest average incidence rates of leishmaniasis were identified in the coastal region of Montenegro, while seasonal distribution indicates that the disease occurs throughout the year with predominance in late spring and summer. Conclusion. The research has shown that Montenegro is among the countries with low incidence of leishmaniasis. Nevertheless, because of leishmaniasis re-emergence in the entire Mediterranean Basin, a comprehensive research of ecological and epidemiological characteristics of leishmaniasis, including better monitoring and notification system, is required.

The urgency of the problem of herpes zoster (shingles) consists in its wide distribution in the world, high incidence rates in a significant part of the subjects of the Russian Federation, as well as the severity of complications caused by this infection. The aim of the study was to analyze the epidemiological characteristics of the incidence of herpes zoster in Russia.Materials and methods. Epidemiological analysis of the incidence of herpes zoster in the Russian Federation was conducted using official statistics. Results and discussion. In 2019, in a number of administrative divisions of the Russian Federation, there was a poor epidemic situation associated with the incidence of herpes zoster. In some regions, the incidence rates were significantly higher than the national average, and the elderly were the most vulnerable. It turned out that the existing methods of prevention of herpes zoster, aimed mainly at promoting a healthy lifestyle and hygienic education of the population, do not bring the desired results, and the incidence of herpes zoster (secondary VZV infection) remains high.Conclusions. The analysis of the incidence of herpes zoster during the reporting period demonstrates the need for constant statistical accounting of the spread of this infection on the territory of Russia, actualizes the conduct of full-fledged epidemiological surveillance and the organization of a system of preventive measures, which should be based on the development, production and practical application of a domestic vaccine against herpes zoster.

The increasing prevalence of high myopia has been noted. We investigated the epidemiological characteristics and the related factors of high myopia in a Japanese adult population. Japan Public Health Center-Based Prospective Study for the Next Generation (JPHC-NEXT) Eye Study was performed in Chikusei-city, a rural area in mid-east Japan, between 2013 and 2015. A cross-sectional observational analysis was conducted to investigate prevalence and related factors of high myopia. A total of 6101 participants aged ≥40 years without a history of ocular surgeries was included. High myopia was defined as a spherical equivalent refraction of ≤−6.00 diopters according to the American Academy of Ophthalmology. Potential high myopia-related factors included intraocular pressure (IOP), corneal structure, corneal endothelial cell density, age, height, body mass index, heart rate, blood pressure, biochemical profile, and current history of systemic and ocular disorders. The odds ratios of high myopia were estimated using the logistic regression models adjusted for the associated factors. The prevalence of high myopia was 3.8% in males and 5.9% in females with a significant difference. Age was inversely associated, IOP was positively associated, and none of other factors were associated with high myopia in both sexes. In conclusion, only age and IOP were associated with high myopia in this community-based sample.

Background. The study of regional characteristics of the incidence rates of perinatal mortality, early neonatal mortality, and stillbirths is of undoubted importance, which will allow to substantiate the main directions of improving care for newborns, both at the federal and regional levels.Aims. To study the indicators of perinatal mortality, early neonatal mortality and stillbirths in the territory of the Nizhny Novgorod region in the period from 2012–2016.Materials and methods. Statistical reporting forms of the Ministry of Health of the Nizhny Novgorod region «Basic indicators of public health and the activities of state medical organizations of the Nizhny Novgorod region» for the period from 2012 to 2016. A retrospective descriptive-estimated epidemiological study of perinatal and early neonatal mortality rates and stillbirth rates was conducted. The processing of statistical indicators was carried out by generally accepted methods of variation statistics. The reliability of the indicators was estimated taking into account the confidence interval (2 errors with 95% CI).Results and conclusions. On the territory of the Nizhny Novgorod region in the period from 2012 to 2016. A high level of perinatal mortality and stillbirths was observed compared to the national indicators. The average annual rates of perinatal mortality and stillbirth were significantly higher in Diveevsky and Sokolsky districts compared with the rest of the Nizhny Novgorod region. The risk areas for neonatal mortality include Lukoyanovsky and Tonkin. It is necessary to improve the quality of medical care for pregnant women at all stages, including prenatal and, of course, obstetric care. An important role here is played by the improvement of the epidemiological surveillance system, which allows an objective analysis of the current situation and an assessment of the quality of medical care.

Introduction. The aim of this study was to determine the epidemiological characteristics of the most common zoonoses in the Central Banat District of Vojvodina. Material and Methods. A descriptive epidemiological study was conducted using data of the Centers for Disease Control and Prevention of the Institute of Public Health of Vojvodina and Public Health Institute Zrenjanin. The data for this study were collected from January 1, 2002 to December 31, 2016. Results. Apart from Q fever, which showed a stable incidence, the trend of incidence rates of other two zoonoses decreased between 2002 and 2016. Q fever was three times more common in males than females, while the incidence of males and females was similar among patients with Trichinellosis and Salmonellosis. Regarding the age distribution, the highest average incidence rate (12/100,000) of Trichinellosis was reported in patients aged 20-39 years; Salmonellosis predominated among patients aged 0-19 years (64.6/100,000), and Q fever in the 20-39 age group (9.2/100,000). Pork products were the most common source of outbreaks of Trichinellosis, while cakes and cookies were the most common source of outbreaks of Salmonelloses. Out of 92 interviewed patients with Q fever, 50 (54.3%) had a direct daily contact with their domestic animals during the maximum incubation period. Conclusion. In order to improve evaluation of epidemiological characteristics of the most common zoonoses and consequently their control in a timely manner, efficient exchange of information between health sectors for humans and animals is necessary, along with continuous education of food handlers and the general population of the Central Banat District of Vojvodina.

Background: Expatriates represent >80% of Qatar’s population, mostly arriving from countries in Africa and Asia that are endemic with many diseases. This increases the risk for introducing new pathogens into the country and provides a platform for maintenance of endemic pathogen circulation. Here, we report on the incidence and epidemiological characteristics of hepatitis B in Qatar between 2010 and 2014. Methods: We performed a retrospective epidemiological data analysis using the data available at the surveillance system of the Ministry of Public Health (MOPH) in Qatar. Data were collected from distinctive public and private incorporates around the nation. Reported cases of hepatitis B patients represent those who met the stringent case definition as per World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) guidelines and eventually reported to MOPH. Results: The annual incidence rates of hepatitis B cases were 30.0, 34.2, 30.5, 39.4, and 19.8 per 100,000 population in 2010, 2011, 2012, 2013, and 2014, respectively. There was no specific trend or seasonality for the reported cases. The incidence rates were higher in females compared to males between 2010 and 2012, but similar in 2013 and 2014. The highest incidence rates were reported among individuals between 25 and 34 years of age. No cases were reported in children younger than five years in 2013 and 2014. Rates of hepatitis B cases declined dramatically in 2014, in both Qataris and non-Qataris, as compared to the previous years. Conclusion: Our results indicate a dramatic decline of hepatitis B cases in Qatar but mandate improved surveillance and vaccination efforts in expatriates in the nation.

Summary The aim of this paper was to determine the trend of diseases and epidemiological characteristics of viral antigen carrying of hepatitis B for better implementation of prevention and control of the disease activity. The annual reports, reports of diseases - deaths from infectious diseases, epidemiological survey of the Public Health Institute (IPH) Niš were used as the material. The period from 2002 to 2011 in the Nišava District was considered. A descriptive method was used. HBsAg carrying shows an upward trend (y=15+3.27 x). Most carriers are males (57.27%), live in urban areas (98.16/ 100.000 population), average age 41.92 years old ±SD 18.59, pensioners (22.42%). 54.05% are nephrology patients (almost all retirees under the age of 60 years old). Only 15.76% were hospitalized. The data on the vaccination status are insufficient. In 5.45%, co-infection with hepatitis C virus was found. 63.33% belong to the group of patients for whom there were no data on the mode of transmission. Hemodialysis patients make 16.67%, blood donors 9.39%, 6.36% pregnant women and injecting drug users 1.21%. The upward trend of carrying, the presence of all known risk groups in the population of carrying in the Nišava District points to the need for improved epidemiological surveillance, strict application of protective measures, conducting of statutory vaccination of all categories of people exposed to particular risk of infection as well as continuing education on preventive measures of both population and health care providers.
