Abstract Background Demand for high-quality surveillance data for malaria, and other diseases, is greater than ever before. In Uganda, the primary source of malaria surveillance data is the Health Management Information System (HMIS). However, HMIS data may be incomplete, inaccurate or delayed. Collaborative improvement (CI) is a quality improvement intervention developed in high-income countries, which has been advocated for low-resource settings. In Kayunga, Uganda, a pilot study of CI was conducted in five public health centres, documenting a positive effect on the quality of HMIS and malaria surveillance data. A qualitative evaluation was conducted concurrently to investigate the mechanisms of effect and unintended consequences of the intervention, aiming to inform future implementation of CI. Methods The study intervention targeted health workers, including brief in-service training, plus CI with ‘plan-do-study-act’ (PDSA) cycles emphasizing self-reflection and group action, periodic learning sessions, and coaching from a CI mentor. Health workers collected data on standard HMIS out-patient registers. The qualitative evaluation (July 2015 to September 2016) included ethnographic observations at each health centre (over 12–14 weeks), in-depth interviews with health workers and stakeholders (n = 20), and focus group discussions with health workers (n = 6). Results The results suggest that the intervention did facilitate improvement in data quality, but through unexpected mechanisms. The CI intervention was implemented as planned, but the PDSA cycles were driven largely by the CI mentor, not the health workers. In this context, characterized by a rigid hierarchy within the health system of limited culture of self-reflection and inadequate training and supervision, CI became an effective form of high-quality training with frequent supervisory visits. Health workers appeared motivated to improve data collection habits by their loyalty to the CI mentor and the potential for economic benefits, rather than a desire for self-improvement. Conclusions CI is a promising method of quality improvement and could have a positive impact on malaria surveillance data. However, successful scale-up of CI in similar settings may require deployment of highly skilled mentors. Further research, focusing on the effectiveness of ‘real world’ mentors using robust study designs, will be required to determine whether CI can be translated effectively and sustainably to low-resource settings.

Abstract Background In 2018, an estimated 228 million cases of malaria occurred worldwide. Countries are far from having achieved reasonable levels of national protocol compliance among health workers. Lack of awareness of treatment protocols and treatment resistance by prescribers threatens to undermine progress when it comes to reducing the prevalence of this disease. This study sought to evaluate the degree of knowledge and practices regarding malaria diagnosis and treatment amongst prescribers working at the public health facilities of Bata, Equatorial Guinea. Methods A cross-sectional survey was conducted in October-December 2017 amongst all public health professionals who attended patients under the age of 15 years, with suspected malaria in the Bata District of Equatorial Guinea. Practitioners were asked about their practices and knowledge of malaria and the National Malaria Treatment Guidelines. A bivariate analysis and a logistic regression model were used to determine factors associated with their knowledge. Results Among the 44 practitioners interviewed, 59.1% worked at a Health Centre and 40.9% at the District Hospital of Bata. Important differences in knowledge and practices between hospital and health centre workers were found. Clinical diagnosis was more frequently by practitioners at the health centres (p = 0.059), while microscopy confirmation was more frequent at regional hospital (100%). Intramuscular artemether was the anti-malarial most administrated at the health centres (50.0%), while artemether-lumefantrine was the treatment most used at the regional hospital (66.7%). Most practitioners working at public health facilities (63.6%) have a low level of knowledge regarding the National Malaria Treatment Guidelines. While knowledge regarding malaria, the National Malaria Treatment Guidelines and treatment resistances is low, it was higher amongst hospital workers than amongst practitioners at health centres. Conclusions It is essential to reinforce practitioners’ knowledge, treatment and diagnosis practices and use of the National Malaria Treatment Guidelines in order to improve malaria case management and disease control in the region. A specific malaria training programme ensuring ongoing updates training is necessary in order to ensure that greater experience does not entail obsolete knowledge and, consequently, inadequate diagnosis and treatment practices.

AbstractObjectiveThis study evaluated the quality of YouTube content focusing on common paediatric otolaryngology procedures, as this content can influence the opinions and medical decisions of patients.MethodsA total of 120 YouTube videos were compiled to review using the terms ‘adenoid removal’, ‘adenoidectomy’, ‘ear tubes’, ‘tympanostomy’, ‘tonsil removal’ and ‘tonsillectomy’. The Discern criteria was used to rate the quality of health information presented in each video.ResultsThe mean bias Discern score was 3.18 and the mean overall Discern score was 2.39. Videos including US board certified physicians were rated significantly higher (p < 0.001) than videos without (bias Discern score = 3.00 vs 2.38; overall Discern score = 3.79 vs 1.55). The videos had been viewed a total of 176 769 549 times.ConclusionUnbiased, high quality videos on YouTube are lacking. As patients may rely on this information when making medical decisions, it is important that practitioners continually evaluate and improve this video content. Otolaryngologists should be prepared to discuss YouTube content with patients.

Abstract High quality of care (QoC) for antiretroviral treatment (ART) is essential to prevent treatment failure. Uganda, as many sub-Saharan African countries, increased access to ART by decentralizing provision to districts. However, little is known whether this rapid scale-up maintained high-quality clinical services. We assess the quality of ART in the Acholi and Lango sub-regions of northern Uganda to identify whether the technical quality of critical ART sub-system needs improvement. We conducted a randomized cross-sectional survey among health facilities (HF) in Acholi (n = 11) and Lango (n = 10). Applying lot quality assurance sampling principles with a rapid health facility assessment tool, we assessed ART services vis-à-vis national treatment guidelines using 37 indicators. We interviewed health workers (n = 21) using structured questionnaires, directly observed clinical consultations (n = 126) and assessed HF infrastructure, human resources, medical supplies and patient records in each health facility (n = 21). The district QoC performance standard was 80% of HF had to comply with each guideline. Neither sub-region complied with treatment guidelines. No HF displayed adequate: patient monitoring, physical examination, training, supervision and regular monitoring of patients’ immunology. The full range of first and second line antiretroviral (ARV) medication was not available in Acholi while Lango had sufficient stocks. Clinicians dispensed available ARVs without benefit of physical examination or immunological monitoring. Patients reported compliance with drug use (>80%). Patients’ knowledge of preventing HIV/AIDS transmission concentrated on condom use; otherwise it was poor. The poor ART QoC in northern Uganda raises major questions about ART quality although ARVs were dispensed. Poor clinical care renders patients’ reports of treatment compliance as insufficient evidence that it takes place. Further studies need to test patients’ immunological status and QoC in more regions of Uganda and elsewhere in sub-Saharan Africa to identify topical and geographical areas which are priorities for improving HIV care.

Background: Quality indicators are important tools to measure and ultimately improve the quality of care provided. Performance measurement may be particularly helpful to grow disciplines that are underutilized and cost-effective, such as home dialysis (peritoneal dialysis and home hemodialysis). Objective: To identify and catalog home dialysis quality indicators currently used in Canada, as well as to evaluate these indicators as a starting point for future collaboration and standardization of quality indicators across Canada. Design: An environmental scan of quality indicators from provincial organizations, quality organizations, and stakeholders. Setting: Sixteen-member pan-Canadian panel with expertise in both nephrology and quality improvement. Patients: Our environmental scan included indicators relevant to patients on home dialysis. Measurements: We classified existing indicators based on the Institute of Medicine (IOM) and Donabedian frameworks. Methods: To evaluate the indicators, a 6-person subcommittee conducted a modified version of the Delphi consensus technique based on the American College of Physicians/Agency for Healthcare Research and Quality criteria. We shared these consensus ratings with the entire 16-member panel for further examination. We rated items from 1 to 9 on 6 domains (1-3 does not meet criteria to 7-9 meets criteria) as well as a global final rating (1-3 unnecessary to 7-9 necessary) to distinguish high-quality from low-quality indicators. Results: Overall, we identified 40 quality indicators across 7 provinces, with 22 (55%) rated as “necessary” to distinguish high quality from poor quality care. Ten indicators were measured by more than 1 province, and 5 of these indicators were rated as necessary (home dialysis prevalence, home dialysis incidence, anemia target achievement, rates of peritonitis associated with peritoneal dialysis, and home dialysis attrition). None of these indicators captured the IOM domains of timely, patient-centered, or equitable care. Limitations: The environmental scan is a nonexhaustive list of quality indicators in Canada. The panel also lacked representation from patients, administrators, and allied health professionals. Conclusions: These results provide Canadian home dialysis programs with a starting point on how to measure quality of care along with the current gaps. This work is an initial and necessary step toward future collaboration and standardization of quality indicators across Canada, so that home dialysis programs can access a smaller number of highly rated balanced indicators to motivate and support patient-centered quality improvement initiatives.

The growth of graphene on 3C-SiC/Si heterostructure is a promising approach, which provides low production cost, high scalability and easiness of nanoelectromechanical system fabrication. However, the quality of graphene is still insufficient for device applications due to mediocre morphological and structural quality of the 3C-SiC epilayers compared to bulk SiC crystals and to excessive Si out-diffusion from the Si substrate. Here, we propose a solution of inserting a 4H-AlN layer between 3C-SiC and Si, which allows us to polish the 3C-SiC film without worrying about enhancement of the Si out-diffusion despite the thinning after the polishing. With this insertion, a considerable quality improvement is achieved in our graphene on silicon.

UNSTRUCTURED To guide HIV prevention and treatment activities up to 2020, we need to generate and make better use of high quality HIV surveillance data. To highlight our surveillance needs, a special collection of papers in JMIR Public Health and Surveillance has been released under the title “Improving Global and National Responses to the HIV Epidemic Through High Quality HIV Surveillance Data.” We provide a summary of these papers and highlight methods for developing a new HIV surveillance architecture.

A medical trend which is currently gaining strength includes the complex treatment based on the concept of «fast-track» recovery in oncology. An application of modern high-tech approaches allowed minimizing the terms of treatment, shortening economic costs and getting the highest final results. It provides a high quality treatment. Given method could exclude an interim treatment stages, sufficiently reduces terms of general treatment and rehabilitation of patients and could succeed in high quality of life for the patients in short- and long-term period. The authors present the results of successful clinical surgery of 78 patients using this method in public health.

The performance of health workers and patient satisfaction Background: Patient satisfaction related to the quality of health services by knowing the level of patient satisfaction, puskesmas can improve service quality. The percentage of patients who expressed satisfaction with the service based on the results of the survey with standard instruments. HR performance is a term derived from the word Job Performance or Actual Performance (work performance or actual achievement achieved by someone)Purpose: To determine the effect of the performance of health workers on patient satisfaction in Public health centers.Method: A quantitative with the design correlative. The population were patients who were inpatient  at 7 Public Health Centers in West Lampung District, with a total sample of 130 respondents. Sampling in this study using the Cluster Sampling method (Sample Area)Results: Most respondents rated the performance of health workers in the poor category of 78 respondents (60.0%) and the majority of respondents were dissatisfied numbered 69 respondents (53.1%). Based on the results of statistical tests, p-value 0.034 or p-value <α (0.05) is obtained, which means there is a relationship between health worker performance and patient satisfaction.Conclusion: The results of this study are expected that the Public health centre management can improve the performance of nurses and can develop or update the best quality of service for patients, by providing facilities, facilities and infrastructure to support health services, improve all quality service procedures and improve performance related programs, and by improving health status, so that patient satisfaction can be improved.Keywords: The performance; Health workers; Patient satisfaction Pendahuluan : Kepuasan pasien yang berhubungan dengan mutu pelayanan kesehatan dengan mengetahui tingkat kepuasan pasien, puskesmas dapat meningkatkan mutu pelayanan. Pengambilan sampel dalam penelitian ini menggunakan metode Cluster Sampling (Area Sampel)Hasil : Sebagian besar responden menilai kinerja petugas kesehatan dalam kategori buruk berjumlah 78 responden (60,0%) dan sebagian besar responden merasa tidak puas berjumlah 69 responden (53,1%). Berdasarkan hasil uji statistik, didapatkan p-value 0,034 atau p-value < nilai α (0,05) yang artinya terdapat  hubungan antara kinerja petugas kesehatan dengan kepuasan pasien.Simpulan: Hasil penelitian ini diharapkan agar pihak Puskesmas dapat meningkatkan kinerja perawat dan dapat mengembangkan atau memperbaharui kualitas pelayanan yang terbaik untuk pasien, dengan cara menyediakan fasilitas, sarana dan prasarana dalam menunjang pelayanan kesehatan, memperbaiki semua tatacara pelayanan yang bermutu serta meningkatkan program kinerja yang berhubungan dengan peningkatan status kesehatan, sehingga kepuasan pasien dapat ditingkatkan.

The aim of this work was to show what effect biotechnology can have on the quality of human life, and on the condition of the natural environment. A number of biotechnological processes have been analyzed that can significantly improve the quality of human life, while at the same time caring for the natural environment. The prospects for the development of these processes on the global and Polish market were also analyzed. As it turned out, some of the process analyzed need further refinement to be implemented on a global scale, while others may be successfully implemented in the coming years, contributing to the improvement of the quality of such aspects of human life high-quality food products, health protection and good public health. Biotechnological processes may also have wide application in the protection and remediation of the natural environment.

The purpose of this study is to describe how the quality of service in integrated health in Tangerang City, which is one form of power Sourced Public Health Efforts (UKBM) are managed and organized from, by, for and with the community and in this case carried out by cadres posyandu with technical support from health workers with the object of research in North Village Poris Plawad Cipondoh districts Tangerang. The method used to answer the problem in this study is to use qualitative methods, research instrument using direct observation and interview means to the community, health posts and cadres of health workers, who are in the territory of the village gulls Poris Plawad Cipondoh Northern District of Tangerang. The results showed that the service at Posyandu Camar Kelurahan Poris Plawad Utara Cipondoh Sub-district, Tangerang City, still needs to be improved. In order for the public to get more optimal health services, therefore it is necessary also in complete facilities. There needs to be guidance and coordination of Posyandu cadres role, with community leaders, Government employee in Poris Plawad Utara Village Cipondoh sub-district, Tangerang City, in order to improve the quality of health and public knowledge optimally.Keyword: Service, Public Service, Posyandu
