Background. Albumin is a protein whose serum levels decrease in inflammatory conditions such as periodontal diseases. This study was undertaken to evaluate changes in serum albumin levels in patients with and without periodontal diseases prior and subsequent to non-surgical periodontal treatment and its relationship with clinical parameters of periodontal disease.Methods. Twenty patients diagnosed as having chronic periodontitis and 20 periodontally healthy subjects, referring to Tabriz Faculty of Dentistry, were selected. Serum albumin levels and clinical variables of periodontal disease (probing pocket depth, gingival index, bleeding index, clinical attachment level and plaque index) were determined before treatment and three months subsequent to non-surgical periodontal treatment. Data were subjected to descriptive statistical analyses (mean ± SD). Serum levels of albumin and clinical parameters were compared between the two groups with independent-samples t-test. Paired-samples t-test was applied to compare the variables before and after treatment in the case group. Statistical significance was defined at P<0.05.Results. The mean serum albumin level of chronic periodontitis patients (3.62±0.11 mg/dL) exhibited a significantly lower value compared to subjects who were periodontally healthy (4.17±0.29 mg/dL), with the serum albumin levels increasing significantly three months postoperatively (3.78±0.33 mg/dL), approaching the level in subjects who were periodontally healthy (P<0.05).Conclusion. Decreases and increases in serum albumin levels under the effect of periodontal disease and its treatment indi-cated an inverse relationship between the albumin levels of serum and chronic periodontitis.

Tooth loss among adults is associated with progressive periodontitis. Implant prosthetic treatment has long been utilized in periodontal patients. Even when the implants are applied, ongoing management of periodontal disease and control of inflammation is necessary to maintain a healthy oral cavity. Lack of appropriate periodontal treatment can result in recurrence of periodontal disease during a maintenance period; loss of the supportive capacity of the periodontal tissues will increase the susceptibility of residual teeth to traumatic force. For this reason, it is worthwhile to improve oral function by applying implants as a fixed device. Here, we report that implant treatment in a patient with generalized severe chronic periodontitis helped maintain the periodontal and peri-implant tissue for a long term. We propose that initial periodontal treatment and ongoing supportive therapy can help maintain implants in patients with severe periodontitis. In addition, we reviewed case reports in the English literature so far.

INTRODUCTION: Periodontal disease is an inflammatory response to bacteria that reside in the gum tissue and can have systemic repercussion. OBJECTIVE: The aim of this study was to assess the relationship between periodontitis and markers of cardiovascular risk. MATERIAL AND METHOD: Ninety selected patients were assigned into two groups in accordance with their levels of probing pocket depth (PPD) and Clinical Attachment Level (CAL): control group, n= 45 (< 4 sites with PPD ≥ 4.0 mm and CAL ≥ 3.0 mm) and case group, n= 45 (≥ 30% of sites with PPD ≥ 4.0 mm and CAL ≥3.0 mm). Plasma concentrations of C-reactive protein, high sensitive CRP, high-density lipoproteins (HDL-c) and electronegative low density lipoproteins (LDL) were assessed in all participants. Data from medical history and socioeconomic level were also collected from patients. RESULT: Plasma levels of HDL-c were lower in subjects with periodontal disease (p = 0.016) and were inversely associated with the number of sites with PPD ≥ 3 mm (rho= -0.325) and number of sites with PPD ≥ 3 mm and CAL ≥ 3 mm (rho= -0.216). These associations remained significant after adjustments for dental plaque and smoking using Univariate Analysis of Covariance (p < 0.05). Adjusted odds ratio between periodontal disease and levels of HDL-c was 0.94 (CI95% 0.88-0.99) after adjusting for age, smoking and dental plaque. Other investigated markers of cardiovascular risk were not related to periodontal disease. CONCLUSION: Clinical parameters of periodontitis were inversely associated with plasma concentrations of HDL-c.

Background: Plasma glutathione peroxidase (eGPx) is an important selenium containing antioxidant in human defense against oxidative stress. While crevicular fluid (GCF) eGPx levels and its association with periodontal disease is well documented, there is no data on correlation of GCF and serum eGPx levels in chronic periodontitis. Hence this study was undertaken to further probe into the role of oxidative stress in periodontal diseases and effect of nonsurgical periodontal therapy (NSPT) by correlating GCF and serum levels of eGPx.Materials and methods: Thirty subjects (16-Males and 14-Females; age: 30–38 years) participated in the study. The subjects were divided, based on gingival index, probing pocket depth and clinical attachment level into: Healthy (group-1,n=10), Gingivitis (group-2,n=10) and Periodontitis (group-3,n=10). Chronic periodontitis patients after NSPT constituted group 4. GCF and serum samples collected from each subject were quantified for eGPx levels using Enzyme linked Immunosorbent Assay.Results: The mean eGPx concentrations increased from health (14.01 ng/μl and 78.26 ng/ml) to gingivitis (22.86 ng/μl and 90.44 ng/ml) and then to periodontitis (29.89 ng/μl and 103.43 ng/ml), in GCF and serum respectively. After NSPT, there was statistically significant reduction in eGPx concentration in GCF and serum (19.41 ng/μl and 85.21 ng/ml). Further, all the GCF eGPx values showed a positive correlation to that of serum eGPx level.Conclusion: Thus, increased eGPx concentration in GCF can be considered as an indicator of local increase in oxidative stress. While, increase in serum eGPx levels indicates that periodontal disease can also lead to increased oxidative stress at the systemic level.

Periodontitis is a chronic inflammatory disease of the vascularized supporting tissues of the teeth. Angiogenesis (neovascularization) is the budding of new capillaries and is thought to be an essential process in the development of chronic inflammatory diseases. Inflamed tissues (such as gingiva coincident in periodontal disease) have evidence of enhanced expression of inflammatory mediators, many of which can promote angiogenesis. Of the various cytokines and growth factors that are involved in angiogenesis, the most potent agent that acts specifically on vascular epithelium is Vascular Endothelial Growth Factor. Even though angiogenesis is a prominent feature of both inflammation and healing, information about its role in periodontal lesions is limited. Hence the aim of the present study was the immunohistochemical evaluation of the expression of VEGF in the gingival tissues of chronic and aggressive periodontitis patients compared to the healthy controls. The present study was carried out in a total of 45 subjects with age range of 18-55 years, reporting to the Department of Periodontology, Rajarajeswari Dental College and Hospital, Bangalore. Gingival tissue samples were collected from all the 45 subjects and categorized into three groups based on their clinical findings as follows: Group 1 (Healthy), Group II (Chronic Periodontitis), Group III (Aggressive Periodontitis). Following sample collection, immunohistochemical staining of tissues was carried out and evaluation was done to compare the grades of expression of VEGF in the three groups. The expression of VEGF in blood vessels was also quantitatively evaluated. The results were statistically analyzed using Kruskal Wallis ANOVA and Mann Whitney test. There was a statistically significant higher expression of VEGF in both chronic periodontitis and aggressive periodontitis group as compared to the control group. Aggressive periodontitis cases showed higher grades of expression of VEGF compared to the chronic periodontitis cases and healthy controls. However, the difference in expression of VEGF was not statistically significant between the two forms of periodontitis. The presence of VEGF in both chronic and aggressive periodontitis clearly indicates the potent role played by VEGF as an inflammatory agent in the initiation and progression of periodontal diseases. Thus, VEGF might be used as a potential vascular marker for the assessment of severity and inflammatory status in periodontal disease.

Periodontal disease encompasses gingivitis and periodontitis and is one of the most common chronic infections in the adult population. This study aimed to evaluate the influence of Spanish propolis extract (EEP) on the effect of the clinical and microbiological parameters as an adjuvant to scaling and root planning in patients undergoing supportive periodontal therapy (SPT). Forty chronic periodontitis patients were randomly assigned into two groups for the treatment. In the control group (n = 20), the sites were treated by scaling and root planing followed by gingival irrigation with physiological saline and in the test group (n = 20), the sites were treated by scaling and root planing followed by subgingival placement of EEP. At baseline (BL), bleeding on probing positive (BOP+) sites with probing pocket (PPD) ≥ 4 mm were defined as study sites. Plaque index, PPD, BOP, clinical attachment level (CAL), and subgingival plaque were evaluated at BL and 1 month later. The results showed a significant clinical improvement (p < 0.05) in the PPD, CAL and BOP+ comparing them with BL and one month after the periodontal treatment and a significant reduction (p < 0.05) for Tannerella forsythensis, Porphyromonas gingivalis, Prevotella intermedia and Treponema denticola in both groups. In addition, the improvement of clinical parameters was observed with subgingival use of EEP and also statistically significant differences between groups were observed (p < 0.05) such as reductions of BOP+ % and reduced counts of T. forsythensis and P. gingivalis, considered as the “key pathogens” for the periodontal diseases. Our results suggest prophylactic and therapeutic potential for EEP against periodontal diseases, improving clinical parameters, reducing gingival bleeding and decreasing bacterial counts of T. forsythensis and P. gingivalis. The subgingival use of EEP represents a promising modality as an adjuvant in periodontal therapy to avoid microbial resistance and other adverse effects.

Background. Reactive oxygen species (ROS) are associated with the pathogenesis of inflammatory diseases and have a direct or indirect role in tissue damage constituting oxidative stress. ROS are also involved in impairment of β-cell function during development of diabetes, which leads to genetic ablation of KATP channels, triggering up-regulation of antioxidant enzymes. Several markers of lipid peroxidation, protein oxidation and DNA damage induced by ROS can be measured. Over the last decade, isoprostanes have been considered as the best markers of lipid peroxidation. The aim of this study was to determine the presence of 8-isoprostane in healthy, chronic periodontitis and chronic periodontitis subjects with type II diabetes and to find the correlation between 8-isoprostane levels among groups and with clinical parameters like gingival index, probing depth and clinical attachment levels. Methods. Ninety subjects were selected and divided into 3 groups: healthy, chronic periodontitis and chronic periodontitis subjects with type II diabetes (n=30 each). Saliva was collected from these subjects after obtaining consent and analyzed for 8-isoprostane levels using ELISA kit. Statistical analysis was performed using Kruskal-Wallis test, Mann-Whitney U test and Spearman’s correlation coefficient (P<0.001). Results. Statistically significant difference was found in the levels of 8-isoprostane between healthy, chronic periodontitis and chronic periodontitis subjects with type II diabetes and with all clinical parameters. Conclusion. 8-isoprostane can be considered as a pathophysiological marker to measure oxidative stress in periodontal diseases.

Chronic periodontitis is a silent infectious disease prevalent worldwide and affects lifestyle-related diseases. Therefore, efficient screening of patients is essential for general health. This study was performed to evaluate prospectively the diagnostic utility of a blood IgG antibody titer test against periodontal pathogens. Oral examination was performed, and IgG titers against periodontal pathogens were measured by ELISA in 1,387 individuals. The cut-off value of the IgG titer was determined in receiver operating characteristic curve analysis, and changes in periodontal clinical parameters and IgG titers by periodontal treatment were evaluated. The relationships between IgG titers and severity of periodontitis were analyzed. The best cut-off value of IgG titer against Porphyromonas gingivalis for screening periodontitis was 1.682. Both clinical parameters and IgG titers decreased significantly under periodontal treatment. IgG titers of periodontitis patients were significantly higher than those of healthy controls, especially in those with sites of probing pocket depth over 4 mm. Multiplied cut-off values were useful to select patients with severe periodontitis. A blood IgG antibody titer test for Porphyromonas gingivalis is useful to screen hitherto chronic periodontitis patients (ClinicalTrials.gov number NCT01658475).

ABSTRACTMaternal periodontal infection has been recognizsed as a risk factor for preterm and low birth weight infants. It is hypothesized that pathogens causing periodontal disease might translocate to the amniotic cavity and contribute to triggering an adverse pregnancy outcome. The growing evidence that an infection remote from the foetal-placental unit might have a role in preterm delivery has led to an increased awareness of the potential role of chronic bacterial infections in the body. The aim of this study was to evaluate whether the presence of chronic periodontitis might influence the incidence of preterm labour and preterm birth.This study was designed as a hospital-based case-control study. Seventy pregnant women aged 18-40 years, with a single live pregnancy were recruited from the Department of Gynaecology and Obstetrics of a general hospital in Sibenik, Croatia, from March 2013 to March 2014.The case group included: 30 pregnant women who were hospitalized with signs of preterm labour. The control group included 40 normal pregnancy patients, who were analysed for up to 48 h after the delivery of a term baby having a birth weight of more than 1500 g. A full-mouth periodontal examination was performed on all the patients. Information was collected on the demographics, health behaviours, and obstetric and systemic diseases that might have an influence in preterm delivery.The presence of chronic periodontitis tended to be higher in women with a preterm delivery (the case group), with 20 cases (66%), than in the women in the, control group, in which chronic periodontitis was found in 14 cases (35%); this difference reached statistical significance (p≤0.01). The PTB cases had a significantly worse periodontal status than the controls (p≤0.001). From the PTL group, 18 patients delivered preterm, and chronic periodontitis, found in 15 cases (83%), was more prevalent than in the control group. The risk of women having periodontitis or attachment loss ≥ 4 mm developing PTB showed an OR of 3.7 (95% CI: 1.91 to 4.86; P< 0.001).The study shows a significant association between periodontal chronic disease and an adverse pregnancy outcome. Periodontal disease represents a strong, independent risk factor for preterm births, and periodontal prevention and therapy should be a part of preventive prenatal care.
