Radioactive Excretion in Human Milk Following Administration of Technetium Tc 99m Macroaggregated Albumin

Albumin-tagged sodium pertechnetate (technetium) is routinely used in nuclear medicine for scanning procedures of the lung. The rate of excretion of this radionuclide into breast milk and the resultant potential radiation hazard to the nursing infant have received little attention. Therefore the milk from a nursing mother who required a lung scan because of suspected pulmonary emboli using an intravenous injection of 4 mCi of technetium Tc 99 macroaggregated human serum albumin was monitored. Albumin tagging severely limited the entrance of technetium into her milk and the radio-activity of the milk returned to base line by 24 hours. A total of 2.02 µCi of technetium was measured in the 24-hour milk collection after technetium injection and 94% of this amount was excreted by 15.5 hours. This amount of technetium administered orally to a newborn would deliver a total body radiation dose of .3 mrad. Therefore, an infant would receive trivial doses of radiation if breast-feeding were resumed 15.5 hours after administration of the radionuclide to the mother and nursing can clearly be resumed safely 24 hours after injection.

In nuclear medicine, radiation absorbed dose estimates calculated by standard models at the whole body or organ are very low. At cellular level, however, the heterogeneity of radionuclide distributions of radiation dose patterns may be significant. We present here absorbed doses at cellular level and evaluate their possible impact on the usually assumed risk/benefit relationships in nuclear medicine studies. The absorbed dose values calculated are surprisingly high, and are difficult to interpret. In the present study, we show calculated doses at the cellular level and discuss possible biological consequences, for two radiopharmaceuticals labelled with technetium-99m: human serum albumin microspheres used for pulmonary scintigrapies and HMPAO used to labelled leukocytes.

Radiation induced cataracts is a disease that is common amongst radiation exposed staff. About 30 percent of retired occupational radiation workers developed eye lens (EL) cataracts. Epidemiological studies on radiation therapy patients, occupational workers, and atomic bomb survivors show that 0.5 Gy of acute or fractioned radiation dose to EL causes one or both lens to cloud. The annual EL dose limit drawn by the International Commission for Radiation Protection (ICRP) was 150 mSv and was changed in 2012 to 20 mSv averaged over 5 years, with no single year exceeding 50 mSv. The limited number of researche done in Nuclear Medicine (NM) clinics with the Hp(3) dosimeters suggest that the annual EL dose from three procedures is measured to be between 4.5 and 9 mSv (i.e. dispensing, preparing and administering). These procedures are performed when the radioactive materials are in closed shielded containers or behind a barrier. Common radioactive material handled by occupational workers in NM clinics are 99mTc and 131I. They pose less radiation hazard to workers EL in the three procedures when they are behind shielded containers. Moreover, once the radioactive material is administered into patients, they become open sources and pose more radiation hazard to workers. The Hp(3) dosimeter is a new uncommon dosimeter. Many radiation facilities use the Hp(0.07) and Hp(10) dosimeters coupled with many conditions and conversion factors to find approximate results. Therefore, simulations are performed to find the EL dose. However, some simulations are performed with little flexibility in simulation geometry, others utilize low-quality phantoms or present the simulation results in terms of fluxes or energy ranges. In the present study, the NM worker EL dose is simulated by utilizing a high-resolution Digital Imagning Communication in Medicine (DICOM) image in GEANT4 Archeticture for Medical Oriented Simulation (GAMOS). A water cylinder homogenously filled by radioactive material, representing the administered patient, was created in the simulation. The worker exposure scenario was simulated by placing the cylinder in three different directions and five different distances with respect to the DICOM image. The results of the simulation reveal that the highest occupational EL radiation dose is received from the anterior-posterior direction, followed by the lateral, and the posterior-anterior directions. The results of the conservative simulated scenario reveal that the worker EL dose is exposed to three tenths of the annual dose limit after 110 131I patients, or 300 99mTc patients.

The radioisotope technetium-99m(99mTc) is widely used in nuclear medicine for a variety of scanning procedures. On occasion, it may be necessary to perform such studies on a nursing mother, and it is important to know what dose of radioactive material a nursing infant might receive and how soon after the isotope administration the mother may resume breast-feeding. Technetium is one of the halide elements and is handled, biologically, much like iodine.1 Like iodide,2,3 the pertechnetate ion (99mTcO4-) is concentrated in the thyroid gland1,4 and stomach5,6 and in the breast milk of nursing mothers4-6 and women with the amenorrhea-galactorrhea syndrome.7 In this report we present studies on 99mTcO4- in breast milk, including its binding to milk protein, biologic half-life, and calculations of radioactivity that might be ingested by a nursing infant.

AbstractChloroquine (CQ) and Hydroxychloroquine (HCQ) have been proposed to be effective at treating COVID-19 patients. We, and others, have previously reported on the capacity of CQ to reduce HIV-1 replication in vitro. We tested CQ administration in post-partum mothers on influencing HIV-1 viral loads in human milk as a means of lowering mother to child transmission. A Phase I/II, randomized, placebo-controlled study to evaluate chloroquine administration to reduce HIV-1 RNA levels in human milk: the CHARGE study. Thirty HIV-1 positive pregnant Rwandese women (CQ n = 20; placebo n = 10) were enrolled in a 16-week study, with the treatment group receiving a 200 mg oral dose of CQ daily. Base-line plasma viral load (pVL) measurements and CD4 counts were determined prior to delivery, and pVL, breast milk VL (bmVL) and CQ levels measured during treatment. For women receiving treatment, CQ concentration was higher in breast milk compared to plasma (over 2.5-fold), with a positive correlation between the levels in the two compartments (P < 0.003). A link between high CQ concentrations in plasma and high CD4 counts (P < 0.001) was observed. Surprisingly, we found a significant increase in pVL after CQ treatment in over half of the mothers (n=11; P < 0.001) and with no alteration to bmVL measurements. No specific amino acid alterations in the gp120 envelope sequences could be associated with CQ administration. CQ usage is associated with a significant increase to pVL in early breastfeeding mothers from Rwanda which cautions against the use of CQ in such individuals. Our results highlight a discrepancy between CQ effects on modulating HIV-1 replication in vitro versus in vivo and indicate caution when prescribing CQ to postpartum HIV-1 untreated mothers. This discrepancy should be taken into consideration when testing CQ or HCQ treatment in COVID-19 clinical trials, especially relating to the post-partum setting.

SummaryThe distribution of 32P-polyphosphates (lineal and cross-linked) and 32Porthophosphate in normal and tumor bearing animals has been studied. Differences between the cross-linked and the lineal form are related to a different degree of susceptibility to the hydrolysis by the phosphatases. In contrast to orthophosphate, the polyphosphates showed a lower accumulation in soft tissues which gives an advantageous reduction of the total body radiation dose.

SummaryTo avoid angiography in patients with clinically suspected pulmonary embolism and non-diagnostic lung scan results, the use of D-dimer has been advocated. We assessed plasma samples of 151 consecutive patients with clinically suspected pulmonary embolism. Lung scan results were: normal (43), high probability (48) and non-diagnostic (60; angiography performed in 43; 12 pulmonary emboli). Reproducibility, cut-off values, specificity, and percentage of patients in whom angiography could be avoided (with sensitivity 100%) were determined for two latex and four ELISA assays.The latex methods (cut-off 500 μg/1) agreed with corresponding ELISA tests in 83% (15% normal latex, abnormal ELISA) and 81% (7% normal latex, abnormal ELISA). ELISA methods showed considerable within- (2–17%) and between-assay Variation (12–26%). Cut-off values were 25 μg/l (Behring), 50 μg/l (Agen), 300 μg/l (Stago) and 550 μg/l (Organon). Specificity was 14–38%; in 4–15% of patients angiography could be avoided.We conclude that latex D-dimer assays appear not useful, whereas ELISA methods may be of limited value in the exclusion of pulmonary embolism.

The objective of this work was to characterize the microbiota of breast milk in healthy Spanish mothers and to investigate the effects of lactation time on its diversity. A total of ninety-nine human milk samples were collected from healthy Spanish women and were assessed by means of next-generation sequencing of 16S rRNA amplicons and by qPCR. Firmicutes was the most abundant phylum, followed by Bacteroidetes, Actinobacteria, and Proteobacteria. Accordingly, Streptococcus was the most abundant genus. Lactation time showed a strong influence in milk microbiota, positively correlating with Actinobacteria and Bacteroidetes, while Firmicutes was relatively constant over lactation. 16S rRNA amplicon sequencing showed that the highest alpha-diversity was found in samples of prolonged lactation, along with wider differences between individuals. As for milk nutrients, calcium, magnesium, and selenium levels were potentially associated with Streptococcus and Staphylococcus abundance. Additionally, Proteobacteria was positively correlated with docosahexaenoic acid (DHA) levels in breast milk, and Staphylococcus with conjugated linoleic acid. Conversely, Streptococcus and trans-palmitoleic acid showed a negative association. Other factors such as maternal body mass index or diet also showed an influence on the structure of these microbial communities. Overall, human milk in Spanish mothers appeared to be a complex niche shaped by host factors and by its own nutrients, increasing in diversity over time.

Objective: The objective of this research is to demonstrate the release of SARS-CoV-2 Spike (S) antibodies in human milk samples obtained by patients who have been vaccinated with mRNABNT162b2 vaccine. Methods: Milk and serum samples were collected in 10 volunteers 20 days after the first dose and 7 seven days after the second dose of the mRNABNT162b2 vaccine. Anti-SARS-CoV-2 S antibodies were measured by the Elecsys® Anti-SARS-CoV-2 S ECLIA assay (Roche Diagnostics AG, Rotkreuz, Switzerland), a quantitative electrochemiluminescence immunometric method. Results: At first sample, anti-SARS-CoV-2 S antibodies were detected in all serum samples (103.9 ± 54.9 U/mL) and only in two (40%) milk samples with a low concentration (1.2 ± 0.3 U/mL). At the second sample, collected 7 days after the second dose, anti-SARS-CoV-2 S antibodies were detected in all serum samples (3875.7 ± 3504.6 UI/mL) and in all milk samples (41.5 ± 47.5 UI/mL). No correlation was found between the level of serum and milk antibodies; the milk antibodies/serum antibodies ratio was on average 2% (range: 0.2–8.4%). Conclusion: We demonstrated a release of anti-SARS-CoV-2 S antibodies in the breast milk of women vaccinated with mRNABNT162b2. Vaccinating breastfeeding women could be a strategy to protect their infants from COVID-19 infection.

The benefits of human milk for both mother and infant are widely acknowledged. Human milk could represent a link between maternal and offspring health. The triad mother-breast milk-infant is an interconnected system in which maternal diet and lifestyle might have effects on infant’s health outcome. This link could be in part explained by epigenetics, even if the underlining mechanisms have not been fully clarified yet. The aim of this paper is to update the association between maternal diet and human milk, pointing out how maternal diet and lifestyle could be associated with breast-milk composition, hence with offspring’s health outcome.
