Background: Periodontitis is a major reason of tooth loss in people of middle and older age groups that requires more attention of researchers to the problems of etiology, pathogenesis, diagnosis, and therapy of the disease. Currently, the interaction of the microbial content of dental plaque and local tissue response is considered to be the main reason for the development of inflammatory periodontal diseases. At the same time, Russian and foreign researchers mark a high frequency of so-called torpid or resistant to antimicrobial therapy forms of periodontitis.Aim: To study the specific and quantitative composition of a fungi of the Candida genus in the contents of periodontal pockets of patients with varying severity of chronic generalized periodontitis.Materials and methods: A non-randomized non-controlled retrospective research was conducted. We studied the main clinical and radiological indicators of periodontal status and its contamination with fungi of the Candida genus in people with mild, moderate, or severe chronic periodontitis.Results: Research participants enrolled in four groups: 1–3 groups included patients with chronic generalized light, medium and heavy periodontitis; group 4 included twenty-five subjects with no signs of periodontal disease. Among the examined patients from the control group, the frequency allocation of Candida from the contents of the gingival sulcus was 9.09%, patients with periodontal pathology of periodontal pockets ― 64.91%. High contamination with fungal flora (more than 6.0 CFU / mL) was observed in all patients with chronic periodontitis. Total allocated 40 strains of Candida with a predominance of Candida albicans.Conclusions: The results of microbiological research confirmed an increase of contamination of periodontal with fungi of the Candida genus in patients with periodontitis if compared with healthy people. The degree of the contamination with fungi of the Candida genus decreased with increasing severity of periodontitis. It can be assumed that the presence of Candida fungi in the periodontal pocket can be considered as a possible component of the etiology of chronic periodontitis and as a factor aggravating it.

The susceptibility to the development and progression of inflammatory periodontal diseases, which depends on genetic and external factors (smoking, stress, oral hygiene), varies widely. In the development of these diseases, an important role is played not only by the presence of periodontal pathogenic microorganisms, but also by the presence of congenital or acquired immunodeficiency, immunoregulatory defects. The immune system plays a key role in the physiological and pathological processes of periodontal tissues. In this regard, IL17, produced by CD4+ Th cells, which has both Pro-inflammatory and protective activity, is of particular interest in the pathogenesis of periodontitis. The aim of study was to identify the relationship between polymorphic loci of the IL-17A (rs2275913) and MMP-1 (rs1799750) genes and clinical manifestations of chronic periodontitis in petrochemical workers. Dental examination was performed in 92 ethylene oxide production workers with chronic periodontitis and 74 patients with chronic periodontitis who did not come into contact with chemical factors (control group). Genotyping of polymorphisms rs2275913 of the IL17A gene and rs1799750 of the MMP1 gene was performed by allele-specific real-time polymerase chain reaction (PCR). Hygienic assessment of the degree of air pollution of the working area with harmful substances was carried out by gas chromatography according to the guidelines for the determination of harmful substances in the air № 5098-89, № 3119-84. When comparing the results of studies of both groups, there were no statistically significant differences in the frequency distributions of allelic variants and genotypes of the IL-17A and MMP-1 genes. The AA/AG genotypes of the IL-17A gene were associated with an increased risk of severe disease compared to the GG genotype in workers in the main group (OR=6.1; 95% CI 1.33-28.5; p=0.021) and in the control group (OR=7.26; 95% CI 1.34-39.25; p=0.016). Carriers of the A allele in the control group increased the risk of severe chronic periodontitis by 2.4 times compared to carriers of the G allele (OR=2.41; 95% CI 1.19-4.87; p=0.014). During the dental examination of employees of the ethylene oxide plant, the clinical course of periodontal diseases was more severe in comparison with the control group, and the number of patients with severe periodontitis was twice as high. It was found that the AA/AG genotypes of the IL-17A gene and the carrier of the A allele are associated with increased susceptibility to the development of severe chronic periodontitis. The association between the MMP-1 gene polymorphism and the risk of severe forms of chronic periodontitis has not been established. A risk factor for the development of inflammatory periodontal diseases in employees of the petrochemical complex is a complex of harmful production factors.

BACKGROUND: Superoxide dismutase (SOD), an antioxidant acting against superoxide (oxygen radical, O2.-), it is released in inflammatory pathways and causes connective tissue breakdown. Increased SOD activity in inflamed gingiva may indicate increased O2.-radical generation by neutrophils and other inflammatory cells at the diseased site. The aim of the study was to evaluate the effects of non-surgical periodontal therapy (NSPT) on SOD levels in gingival tissues of chronic periodontitis patients.METHODS: Forty subjects: 20 periodontally healthy (Control) and 20 chronic periodontitis (Test); age range 24–55 years were recruited. Gingival tissue samples were collected by excising the inner lining of the periodontal pocket at baseline (prior to non-surgical periodontal therapy) and 2 months post therapy. In controls, tissue samples were obtained immediately after tooth extraction scheduled for orthodontic reasons. Clinical parameters included probing depth, clinical attachment level, gingival index, bleeding index, plaque index. SOD activities were assessed spectrophotometrically at baseline and 2 months post NSPT, results were analysed statistically.RESULTS: At baseline, patients with chronic periodontitis had higher mean SOD activity (2.73 ± 1.36) than the control subjects (1.12 ± 1.13) withp= 0.00003 (p< 0.05). At 2 months post NSPT median SOD level (1.00) had come close to median SOD value of control group (0.85);p= 0.99 (p> 0.05). The resolution of inflammation with successful NSPT resulted in decreased SOD levels as in control group. Clinical parameters in patients with chronic periodontitis showed a significant improvement 2 months post NSPT (p< 0.05).CONCLUSION: Non-surgical periodontal therapy significantly improves the clinical parameters and restores previously increased SOD levels to normal in chronic periodontitis patients.

Background. lncRNA and microRNA affect the occurrence and development of many diseases, so they are expected to become diagnostic or predictive indicators. But the relationship between lncRNA FGD5-AS1 and miR-130a and the prognosis of chronic periodontitis is still unclear. The purpose of this study is to explore the prognostic value of the two in chronic periodontitis. Objective. This study set out to investigate the prognostic value of lncRNA FGD5-AS1 and miR-130a in chronic periodontitis. Methods. Eighty-seven patients with chronic periodontitis who visited our hospital from March 2016 to August 2017 were collected as an observation group (OG), and 72 subjects with periodontal health who underwent physical examination at the same time were collected as a control group (CG). The FGD5-AS1 and miR-130a expression levels of subjects in the two groups were compared, and prognosis of 87 patients who were reviewed one year later was counted. The expression levels of patients with different prognoses were compared when they were admitted to our hospital. We drew the ROC curve and explored the prognostic value of FGD5-AS1 and miR-130a. The risk factors for adverse prognosis were analyzed through logistic regression. Results. FGD5-AS1 was lowly expressed in patients, while miR-130a was highly expressed. FGD5-AS1 and miR-130a had certain diagnostic and predictive value in chronic periodontitis and patient prognosis. The higher the periodontal pocket, the higher the attachment loss. Lower FGD5-AS1 and higher miR-130a levels were independent prognostic risk factors. Conclusion. lncRNA FGD5-AS1 is lowly expressed in patients with chronic periodontitis, while miR-130a is highly expressed. Both of them have certain diagnostic and prognostic value in chronic periodontitis and may be potential diagnostic and prognostic indicators.

ABSTRACT Introduction The clinical manifestation of periodontal diseases (such as gingivitis and chronic periodontitis) results from a complex interplay between the etiologic agents such as bacteria that present in the dental plaque, genetic factors, systemic diseases, smoking and exposure of some heavy metals, such as mercury. In this study, we aimed to evaluate hair mercury levels in healthy subjects in comparison with periodontal patients. Materials and methods One hundred twenty subjects were enrolled in this study. The included persons were divided into 3 groups: healthy subjects (n = 40), gingivitis (n = 40) and chronic periodontitis patients (n = 40). Hair samples were collected from occipital area of head. Total mercury levels were determined by atomic absorption spectrophotometry. Results The difference between mercury levels in three groups were statistically significant (p-value < 0.001). Mercury level in periodontitis patients was greater than the gingivitis group (p-value < 0.001). In addition the differences between mercury levels in periodontitis patients vs healthy individuals was significant (p-value = 0.048). The gingivitis patients had lower levels of mercury than the control group, but the difference was not significant (p-value = 0.170). Conclusion The results showed that the levels of mercury are to some extent differed in periodontal diseases in comparison with the healthy individuals. Periodontitis is a chronic inflammatory disease of the vascularized supporting tissues of the teeth. Angiogenesis (neovascularization) is the budding of new capillaries and is thought to be an essential process in the development of chronic inflammatory diseases. Inflamed tissues (such as gingiva coincident in periodontal disease) have evidence of enhanced expression of inflammatory mediators, many of which can promote angiogenesis. Of the various cytokines and growth factors that are involved in angiogenesis, the most potent agent that acts specifically on vascular epithelium is Vascular Endothelial Growth Factor. Even though angiogenesis is a prominent feature of both inflammation and healing, information about its role in periodontal lesions is limited. Hence the aim of the present study was the immunohistochemical evaluation of the expression of VEGF in the gingival tissues of chronic and aggressive periodontitis patients compared to the healthy controls. The present study was carried out in a total of 45 subjects with age range of 18-55 years, reporting to the Department of Periodontology, Rajarajeswari Dental College and Hospital, Bangalore. Gingival tissue samples were collected from all the 45 subjects and categorized into three groups based on their clinical findings as follows: Group 1 (Healthy), Group II (Chronic Periodontitis), Group III (Aggressive Periodontitis). Following sample collection, immunohistochemical staining of tissues was carried out and evaluation was done to compare the grades of expression of VEGF in the three groups. The expression of VEGF in blood vessels was also quantitatively evaluated. The results were statistically analyzed using Kruskal Wallis ANOVA and Mann Whitney test. There was a statistically significant higher expression of VEGF in both chronic periodontitis and aggressive periodontitis group as compared to the control group. Aggressive periodontitis cases showed higher grades of expression of VEGF compared to the chronic periodontitis cases and healthy controls. However, the difference in expression of VEGF was not statistically significant between the two forms of periodontitis. The presence of VEGF in both chronic and aggressive periodontitis clearly indicates the potent role played by VEGF as an inflammatory agent in the initiation and progression of periodontal diseases. Thus, VEGF might be used as a potential vascular marker for the assessment of severity and inflammatory status in periodontal disease.

<p class="AbstractContent"><strong>Objective:</strong> Pathogenesis of rheumatoid arthritis (RA) related temporomandibular joint disorder remains unclear. Some studies investigated the change of condyle after Complete Freund's Adjuvant (CFA) injection which is similar to osteoarthritis. However, there were few studies to demonstrate effect of TMJ in CIA which mimics RA. The aim of this study was to investigate the TM-joint response of CFA and CIA group in rat model.</p><p class="AbstractContent"><strong>Methods</strong>: 24 Rats were divided into 4 groups. (1) Control group. (2) 50μl type I collagen injection group. (3) 50μl CFA injection group. (4) 25μl type I collagen + 25μl CFA injection group. Drug injection was performed on day 0 and day 14 and sacrificed on day 7, and day 35. After sacrificed, TMJ tissue was collected to do the H&amp;E stain and investigated the inflammatory (IL-1β and MMP3) gene expression.</p><p class="AbstractContent"><strong>Results:</strong> TMJ tissue of CFA group demonstrated adaptive bone growth. On the other hand, the subchondral bone of CIA group appeared degenerative process. The trend of IL-1β and MMP3 gene increased in CFA and CIA group compared to control group in synovial tissue and disc (p&lt;0.05).</p><p class="AbstractContent"><strong>Conclusion</strong>: Within limitation of this study, it’s concluded that the CIA method can be used in RA related TMD mechanism research.</p>

Periodontal bacterial bioflm causes an innate and adaptive immune response of the host mucosa, leading to inflammation and destruction of the tissues supporting the periodontal. The progression of periodontitis depends not only on bacteria, since an inadequate immune response to microorganisms can accelerate the development of periodontitis. However, the exact mechanisms of the development of immune reactions remain unclear. Recent studies emphasize the existence of a typical innate response of resident and extravasated immune cells.Objective. To investigate the quantitative composition of non-resident subpopulations of lymphocytes in salivary fluid and to study the mechanisms of interaction of the cellular link of the innate and adaptive immune system in chronic generalized periodontitis of varying severity.Materials and methods. 49 people aged 26–67 years of both sexes were examined with a diagnosis of chronic periodontitis. The comparison group consisted of 17 people aged 26–44 years with no periodontal diseases. The state of the cellular link of the adaptive and local immune system of the oral cavity was assessed by the following phenotypes: CD3–CD16+56+; CD3+CD16+56+; CD3+; CD3+HLA-DR+; CD19+, CD19+HLA-DR+; CD19+CD5+CD27–; CD19+CD5–СD 27–; CD19+СD5–CD27+.Results. The number of T-NK cells decreased with a mild degree of periodontitis and increased with a severe degree. Similarly, CD3+HLA-DR+ decreased with mild periodontitis [Me = 0.148 cells/µl] and increased with moderate [Me = 0.247 cells/µl] and severe [Me = 0.448 cells/µl]. The number of B-lymphocytes with the CD19+, CD19+CD5+, CD19+CD5–CD27+ phenotype decreased to single cells per microliter during the development of the disease.Conclusion. The imbalance of the immune system caused by pathogenic colonization of the periodontium, at different degrees of severity, is an important factor in the occurrence and development of periodontitis, in which various subsets of B cells of the adaptive immune system play a certain role, closely interacting with the cellular link of the innate mucosal immune system

OBJECTIVE: Periodontitis is the second most prevalent microbiome associated inflammatory disease posing a threat to oral health. Nigella sativa (Kalonji) has been used since ancient times as a remedy for oral inflammatory conditions. Interleukin1 (IL-1 ) is critical for periodontal inflammation, collagen degradation and bone turn over. The motive of our study was to determine the change in the levels of salivary IL-1 after the use of Nigella sativa oil to determine if it has any correlation with salivary IL-1 . METHODOLOGY: A parallel-arm triple-blind placebo-based randomized control trial was conducted on a total of ninety three patients. Out of these, fifty individuals with chronic periodontitis were included in the study as per the eligibility criteria. Baseline screening of the participants was done via clinical periodontal parameters such as periodontal pocket depth (PPD), clinical attachment loss (CAL), plaque index (PI) and bleeding on probing (BoP). These individuals were categorized into two groups; 1. Control Group (n=25), which was given normal saline as placebo; 2. Treatment Group, which was given Nigella sativa oil (n=25). All participants underwent scaling and root planing before the start of the trial. The intervention was given for two weeks. Salivary samples were collected on day 0 and day 15 and were evaluated for interleukin-1 levels using ELISA. The statistical interpretation was done using IBM SPSS (version 25.0, SPSS Inc.) on forty participants due to loss to follow up. RESULTS: Levels of salivary interleukin-1 came out to be statistically insignificant after two-week use of Nigella sativa oil. CONCLUSION: No correlation was found between the salivary IL-1 and the use of Nigella sativa oil in patients with chronic periodontitis in contrast to the patients using normal saline. Introduction: Apatite carbonate material plays an important role in bone tissue regeneration. The use of this membrane is expected to achieve better treatment success than those without additional therapy. The antimicrobial content in the apatite carbonate membrane can be used to support periodontal treatment of chronic periodontitis after scaling and root planing. The purpose of this research was to analyse the effect of carbonate apatite membrane gelatin application on the IL-1β level of the gingival crevicular fluid in chronic periodontitis patients. Methods: This research was a double-blind, randomised controlled trial method, with purposive sampling, and split-mouth design. The parameter was the IL-1β level in the gingival crevicular fluid before and after scaling and root planing. Carbonate apatite membrane gelatin was applied to the periodontal pocket on the test group after scaling and root planing. Data were analysed by the Wilcoxon test with a p-value < 0.05. Results: The reduction of IL-1β level gingival crevicular fluid on day-0 and day-30 on both groups showed significant value (p < 0.001). However, it showed no significant differences statistically, between the test group and the control group. Conclusion: Carbonate apatite membrane gelatin application might reduce the IL-1β level of the gingival crevicular fluid, but not showing a better reduction from the control group.Keyword: Carbonate apatite membrane gelatin, IL-1β, periodontal therapy

Relevance. The study of the microbiological composition of periodontal pockets in inflammatory diseases of periodontal tissues is of great importance directly for the effective treatment and understanding of the pathogenetic stages of these diseases. Objective: to study the microbiological landscape of periodontal pockets with inflammatory diseases of periodontal tissues. Materials and Methods. 62 patients aged 20 to 45 years were selected and divided into groups depending on the form of periodontitis: with localized aggressive periodontitis - group №1, group №2 - with chronic generalized periodontitis, group № 3 - with exacerbation of chronic generalized periodontitis. The material for the study was taken from the periodontal pockets and examined by cultural and microbiological methods. Results and Discussion. It was found that the proportion and number of microbial associations in the periodontal pockets of different depths differs. In localized aggressive periodontitis, mainly facultative Gr+ (A. actinomycetemcomitans) and Gr- (St. aureus, Str. mitis) anaerobes were found in the periodontal pockets no more than 4 mm. In generalized chronic periodontitis of moderate degree, the same microorganisms were present in the periodontal pockets as in the localized form, but Str. mitis was more common among facultative aerobes. With the exacerbation of generalized chronic periodontitis from facultative anaerobes, St. aureus was more common and the proportion and number of A. actinomycetemcomitans significantly increased. Conclusion. According to the results of the study, the microorganisms that inhabit the periodontal pockets in inflammatory diseases of periodontal tissues mainly belong to facultative Gr+ and Gr - anaerobes, obligate Gr+ and Gr-anaerobes and yeast fungi.
