Medical prophylactic examination of certain adult population groups from Russia as a tool for the early detection of malignant neoplasms (the 2015-2016 results)

Among socially significant diseases that are characterized by high prevalence rates, malignant neoplasms occupy the leading positions. Thyroid cancer is the most common malignant neoplasm of the endocrine system. Purpose of the study. Analysis of recurrent disability due to thyroid cancer in the adult population in Moscow (2015–2019). Materials and research methods. The structure of repeated disability due to thyroid cancer by sex, age and disability group was studied. The dynamics of the number of the PPI contingent was traced and the level of repeated disability due to this pathology was determined. The number of observations was 4660 PPI. Research results. It was found that in the structure of repeated disability due to thyroid cancer, women predominated by gender. The trend towards an increase in the number of PPI took place in all major age groups of the adult population. The prevalence of PPI with group III disability, the proportion of which was 66.0% (PPI with group II – 32.5%, PPI with group I – 1.5%). The visibility indicator in 2019 in relation to 2015 (taken as 100%) was equal in the general contingent of PPI among disabled people of group I – 142.9%, group II – 374.1%, group III – 249.3% ...

Relevance: Cancer control is among the key social policy priorities in the Republic of Kazakhstan. 14,000 people die from cancer every year. As a result, our country faces an epidemiologic crisis caused by the increasing malignancy-related incidence and mortality, plus the increasing number of people living with cancer. In 2019, 186 326 patients with malignant neoplasms were under dynamic supervision in Kazakhstan; 52.5% lived for five years or more [1]. WHO prognosis a 1.5-2 times growth of annual malignancy-related incidence and mortality worldwide by 2022. The Republic of Kazakhstan follows the same trend, taking into account the growing welfare and life expectancy of the population and an increase in the detection of malignant neoplasms due to the introduction of early detection programs [2]. This study aimed to assess the implementation in 2019 of the Comprehensive Cancer Control Plan in the Republic of Kazakhstan for 2018-2022. Results: Intermediate Comprehensive Plan implementation results showed the efficacy of the conducted measures as reflected by major cancer service indicators. Thus, the early detection has improved: stage 0-I detection amounted to 27.1% of all new cases (2019 target – 25.2%). This had a positive impact on cancer survival: in 2019, the proportion of 5-year or more survival was 54.7% for breast cancer (2019 target – 54.5%), 56.8% for cervical cancer (2019 target – 55.8%), 46.6% for colorectal cancer (2019 target – 45.8%) [1]. Conclusion: The Comprehensive Cancer Control Plan implementation results evidence cancer service improvement in the Republic of Kazakhstan as of 2019 (increased 5-year survival and decreased mortality). However, a high share of detection of visually accessible malignant neoplasms at generalized and advanced stages should be addressed in the coming years.

The results of the development of a curd functional product with vegetable filler Melissa officinalis L. are presented. An analysis of the health status of the population carried out in recent years by the World Health Organization shows that the world community is facing acute questions related to the need to prevent the so-called “diseases of civilization”, which include diseases of the cardiovascular system, oncology, diseases associated with impaired lipid metabolism and the gastrointestinal tract, al ergii, decreased immunity, disorders of the nervous system. According to experts, many of these diseases leading to death, disability or disability have a steady upward trend. Up to 44% of the population of our country suffer from cardiovascular diseases (as of December 2019), while more than 82% have functional disorders leading to them. Malignant neoplasms and precancerous conditions are observed in 39% of the adult population, diseases of the gastrointestinal tract are found in more than 48% of the working population of the country. The solution to these problems may be the development of functional food products.

The article presents the results of a study of the dynamics of recurrent disability due to malignant neoplasms (ZNO) of the adult population in the Chechen Republic for the period 2008-2017. in comparison with the figures for the Russian Federation. In the Russian Federation, the dynamics of re-disability of the adult population due to the ZNO in the period 2008-2017 was characterized by an increase in the number of repeatedly recognized disabled (PPI), the level of which increased from 19.9 to 27.0 per 10 thousand people. Compared to 2008 (taken as 100%), the indicator of visibility of PPI was 135%. In the PPI contingent, the elderly prevailed with a negative trend of growth in their numbers. The proportion of young and middle-aged PPI decreased. In the Chechen Republic, the epidemiological picture of the disability of the adult population due to the ZNO was characterized by different dynamics. The number of PPI due to ZNO in the country decreased. Compared to 2008 (taken as 100%), the visibility index of the PPI was 57.0%; In the structure of recurrent disability due to ZNO, middle-aged and old-aged PPI prevailed, however, their proportion declined. There is a negative growth trend in the proportion of young people.

Background. The proportion of malignant neoplasms detected at the IIIIV stages of the disease in Chelyabinsk region is large and 43.9%. Breast cancer in 2018 was detected at stages IIIIV at 24% and cervical cancer at 51.14%. Despite the fact that malignant neoplasms of vulva and vagina are diagnosed annually in isolated cases, the level of neglect reaches 66.7 and 100% respectively, which makes it necessary to develop organizational measures. Aims to increase the level of detection of visual localities OF female reproductive system organs in the early stages with the involvement of oncologists of regional clinics in the primary diagnosis. Materials and methods. Patients with malignant neoplasms of the reproductive system of visual localizations, first identified in the examining cabinet of the polyclinic of the central district hospital, geographically located and functionally subordinate oncologist of the rural municipal district. Comparative assessment of early detection of tumors in an area with high breast cancer and cervical cancer and results following the introduction of a new organizational structure. Results. Patients with malignant neoplasms of the reproductive system of visual localizations, first identified in the examining cabinet of the polyclinic of the central district hospital, geographically located and functionally subordinate oncologist of the rural municipal district. Comparative assessment of early detection of tumors in an area with high breast cancer and cervical cancer and results following the introduction of a new organizational structure. Conclusions. The involvement of oncologist of territorial polyclinics to participate in preventive examinations, which were previously within the competence of the medical assistant of the Examining Cabinet, during the first year of work increased detection and reduced the incidence of breast and cervical cancer by 8.3 and 3.7% respectively. The creation of the Breast Pathology Center in the regional oncology center, at the third level of specialized medical care on the profile of oncology, allowed to reduce the frequency of malignancy of benign and pre-cancer breast diseases.
