Introduction. Issues of etiopathogenesis for various cardiovascular disorders in the structure of extragenital diseases during pregnancy pose an urgent problem and represent a global topic for further scientific research. Due to the high frequency of heart rhythm disorders in pregnant women, it is necessary to improve their management.Aim: to analyze data from the current literature on the etiopathogenesis of arrhythmias during gestation and effective methods of their correction.Materials and Methods. Literature sources of electronic databases PubMed/MEDLINE, Medscape, Google Scholar, Embase, Ovid Healthstar, Cochrane, eLibrary, CyberLeninka and scientific articles in peer-reviewed open access journals published across the last 25 years, including basic research on this pathology have been analyzed. The search in databases used keywords and their combinations in Russian and English: "arrhythmias", "arrhythmias in pregnancy", "pathogenesis of gestational arrhythmias".Results. There have been analyzed major aspects of the etiology and pathogenesis of gestational arrhythmias as well as potential causes for developing first-time rhythm disorders in pregnant women and features of the gestational period as a predictor of emerging arrhythmias in patients with pre-existing pathology of the cardiovascular system are determined. The article describes the most common rhythm disorders and acceptable ways to correct them, taking into account the latest recommendations and research in this area.Conclusion. The authors concluded that the etiopathogenesis of rhythm disorders during pregnancy and their treatment methods require further examination.

Introduction: Cardiac arrhythmia often occurs in the gestational period of pregnant women, contributing to the development of complications of pregnancy, childbirth and perinatal pathology, which requires a more thorough examination of pregnant women and antiarrhythmic treatment, which in turn increases the risk of complications pregnancy and childbirth. Many types of arrhythmias occur in women without structural damage to the cardiovascular system. The aim is to study the occurrence of cardiac rhythm disturbances in healthy pregnant women, depending on the gestational age, the number of previous pregnancies, infectious diseases during pregnancy, and arrhythmia analysis, which required antiarrhythmic treatment. Materials and methods: Retrospectively 60 individual cards of pregnant women were studied. An ECG monitoring was performed to identify the arrhythmia. Results: Among the arrhythmia were: supraventricular and ventricular extrasystoles, unstable paroxysmal tachycardia. All cases of arrhythmia were without lengthening QT interval. Sinus tachycardia was significantly more common in combination with anemia. Heart rhythm disorders are associated with emotional excitement. Conclusions: Most violations of the heart rate occurred in the second trimester of pregnancy. With concomitant anemia, sinus tachycardia is significantly more common, and sinus bradycardia is associated with an enlarged uterus in compression of the inferior vena cava. With the increase in the number of pregnancies, the risk of heart rhythm disturbances increases. However, the past infectious diseases of the bronchopulmonary system during pregnancy did not significantly affect the occurrence of rhythm disturbances. The appointment of antiarrhythmic drugs was observed in all pregnant women whose cards were included in the study.

In a review article based on my own clinical experience of managing patients with acute myocardial injury and fulminant myocarditis, taking into account expert recommendations on the clinical treatment of myocardial damage associated with novel coronavirus infection a National clinical geriatric medical research center, division of cardiovascular diseases, the Chinese geriatrics society, Department of cardiology, Beijing Medical Association and European clinics discusses the pathogenesis, diagnosis and treatment of myocardial damage and FM patients, infected with SARS-CoV-2 in the context of the COVID-19 pandemic. Clinical features and diagnostic criteria are presented, including screening tests of markers of myocardial damage in the form of a highly sensitive troponin test, a natriuretic peptide. The article discusses in detail the pathogenesis and mechanisms of myocardial damage, including immune mechanisms, cytokine storm, systemic inflammation with macro- and microvascular dysfunction and the development of myocardial dysfunction with acute heart failure, hypotension, cardiogenic shock and/or life-threatening heart rhythm disorders caused by hypoxia and metabolic disorders at the cellular level. Features of the clinical course of fulminant myocarditis in infected patients (SARS-CoV-2) in the conditions of the COVID-19 pandemic are presented. For the first time, a detailed histo-morphological analysis of pathological myocardial injuries and complications is presented on the basis of unique autopsy material on post-mortem diagnostics of various pathoanatomic autopsies of those who died from COVID-19 in Moscow. Based on the clinical, functional and morphological material, the Protocol of etiopathogenetic treatment is presented. The basis of standard therapy is considered antiviral drugs, immunoglobulin G, the use of monoclonal antibodies to interleukin-6, anticoagulants, glucocorticoids, depending on the clinical situation, cardioprotectors and symptomatic treatment are recommended to maintain the heart, which in combination can achieve a certain clinical effectiveness. As adjuvant cardioprotective targeted therapy, the sodium salt of phosphocreatine is considered in order to preserve the myocardium, maintain its contractility and vital activity.

Intra Uterine Device post placenta is the installation in the first 10 minutes to 48 hours after the birth placenta plays a role in reducing maternal mortality through prevention of pregnancy, delaying pregnancy, and spacing pregnancies, the effectiveness of use up to 99.4% can prevent 5-10 years of pregnancy. This study aims to determine the factors associated with maternal willingness to post Post Placenta IUD. The study used a questionnaire with a population of third trimester pregnant women who examined their pregnancies and a sample of 98 people by purposive sampling. Analyze data with Chi Square. The results showed that the majority of pregnant women were not willing to do post placenta IUD installation of 58.2% which was influenced by age, parity, knowledge and support of the husband.

Abstract The inner ear of the two higher clades of modern cetaceans (Neoceti) is highly adapted for hearing infrasonic (mysticetes) or ultrasonic (odontocetes) frequencies. Within odontocetes, Platanistoidea comprises a single extant riverine representative, Platanista gangetica, and a diversity of mainly extinct marine species from the late Oligocene onward. Recent studies drawing on features including the disparate tympanoperiotic have not yet provided a consensus phylogenetic hypothesis for platanistoids. Further, cochlear morphology and evolutionary patterns have never been reported. Here, we describe for the first time the inner ear morphology of late Oligocene–early Miocene extinct marine platanistoids and their evolutionary patterns. We initially hypothesized that extinct marine platanistoids lacked a specialized inner ear like P. gangetica and thus, their morphology and inferred hearing abilities were more similar to those of pelagic odontocetes. Our results reveal there is no “typical” platanistoid cochlear type, as the group displays a disparate range of cochlear anatomies, but all are consistent with high-frequency hearing. Stem odontocete Prosqualodon australis and platanistoid Otekaikea huata present a tympanal recess in their cochlea, of yet uncertain function in the hearing mechanism in cetaceans. The more basal morphology of Aondelphis talen indicates it had lower high-frequency hearing than other platanistoids. Finally, Platanista has the most derived cochlear morphology, adding to evidence that it is an outlier within the group and consistent with a >9-Myr-long separation from its sister genus Zarhachis. The evolution of a singular sound production morphology within Platanistidae may have facilitated the survival of Platanista to the present day.

The objective: to study features of obstetric and perinitalny pathology at women with undifferentiated dysplasia of a connecting tissue. Patients and methods. 100 patients with clinical-laboratory signs of an undifferentiated dysplasia of connecting tissue are surveyed. On the basis of the received results of the patient were divided into two groups: the main – 50 women (expression of і6 points) whom regarded as patients with the expressed undifferentiated dysplasia of connecting tissue, and group of comparison – 50 women (expression <6 points) whom regarded as patients without the expressed undifferentiated dysplasia of connecting tissue. Results. Results of the conducted researches testify to the high frequency of obstetric and perinatal pathology at pregnant women with clinical-laboratory and functional signs of an undifferentiated dysplasia of connecting tissue. Conclusion. The received results are the basis for algorithm improvement the diagnostical and treatment-and-prophylactic actions at women with an undifferentiated dysplasia of connecting tissue. In a report to the International Congress of Ophthalmologists in America, in 1922, Finlay reported for the first time that when examining the eyes of one woman who was in the last month of pregnancy, he accidentally stumbled upon a sharp bilateral temporal narrowing of the visual field, and since there were no he could not detect the phenomenon of moments, he suggested that this narrowing of the field of vision depends on the hypertrophy of the cerebral appendage occurring during pregnancy.

Introduction. Adhesive process of the pelvic organs is a pressing issue for operative gynecology that does not allow to consider results of therapeutic and preventive measures as sufficient.Aim: to analyze published data regarding contemporary aspects of etiopathogenesis of the adhesive process in pelvic organs as well as methods of its prevention.Materials and Methods. The literature sources retrieved from electronic databases PubMed, Embase, Medline, Ovid HealthSTAR, Cochrane, Google Scholar, eLibrary, CyberLeninka as well as scientific articles published in peer-reviewed open access journals over the last 30 years, including basic research in the field have been analyzed. While searching, the following keywords and their combinations in Russian and English were used: "adhesive process of the pelvic organs", "pathogenesis of the adhesive process", "prevention of the adhesive process", "gynecology", "pelvic adhesions", "pathogenesis of adhesions", "аdhesion prophylaxis", "gynecology".Results. The current aspects of the etiology and pathogenesis for adhesive process have been summarized. Existing adhesion classifications are presented. The proposed methods for preventing formation of peritoneal adhesions are described exerting most prominent effectiveness as well as describing the properties and characteristics according to the application method, the composition of contained substances and drug form. A phenotype profile of peripheral blood and peritoneal fluid lymphocytes from patients with adhesive process remains debated.Conclusion. A need to further examine formation of peritoneal adhesions at molecular and cellular levels for developing a comprehensive pathogenetically substantiated method to prevent and treat adhesions of the pelvic organs is in demand.

  El embarazo en adolescentes es un problema de salud pública mundial que altera la salud física, emocional, la condición educativa y económica de los futuros padres; asimismo, frecuentemente se afecta también al producto de la gestación. La investigación fue de tipo retrospectiva, correlacional y cuantitativa. La muestra fue 461 embarazadas adolescentes atendidas en el Hospital “Verdi Cevallos Balda” de la ciudad de Portoviejo durante el 2016. Los resultados de este estudio mostraron que la mayor tasa de embarazo en adolescentes se produjo en primigestas (88,72 %) de 18 años. La desproporción céfalo-pélvica fue el factor materno-fetal que ocupó el primer lugar como causa de cesárea, seguida de factores fetales como distocia de presentación y ruptura prematura de membranas.   Palabras clave: Parto espontáneo, cesárea, pretérmino, postérmino, embarazo.   Abstract Teenage pregnancy is a global public health problem that alters the physical, emotional, educational and economic condition of future parents; likewise, the gestation product is frequently affected. The research was retrospective, correlational and quantitative. The sample was 461 adolescent pregnant women treated at the Hospital "Verdi Cevallos Balda" of the city of Portoviejo during 2016. The results of this study showed that the highest rate of adolescent pregnancy occurred in first-time pregnant women (88.72 %) of 18 years. Cephalopelvic disproportion was the maternal-fetal factor that ranked first as a cause of caesarean section, followed by fetal factors such as presenting dystocia and premature rupture of membranes.   Keywords: Spontaneous delivery, cesarean section, preterm, postterm, pregnancy.

Endothelial dysfunction is considered one of the main mechanisms for the further development of arterial hypertension (AH) and its complications, and endothelin-1 (ЕТ-1) plays one of the key roles in this process. Endothelium is drawn into the pathological process at the earliest stages of AH development. ЕТ-1 is currently considered as a marker and predictor of the stage and consequences of AH, chronic heart failure (CHF), coronary heart disease (CHD), in particularly, acute myocardial infarction (MI), heart rhythm disorders, pulmonary hypertension, atherosclerotic vascular injury, target vascular disorders. One factor that may influence the level of ЕТ-1 is the endothelin-1 gene Lys198Asn polymorphism, which is considered by many researchers as a possible genetic marker of AH. However, the effect of this polymorphism on the level of plasma ЕТ-1 level in patients with AH of varying severity in Ukraine is understudied. The basis of this work were the materials of a complete examination of 160 patients with a verified diagnosis of AH I, stage II of 1, 2, 3 degrees (main group) and 110 apparently healthy persons (control group). Patients of the main group were divided into three subgroups: 1st group ­– 75 patients with normal body weight; 2nd group – 48 overweight patients; 3rd group – 37 patients with alimentary obesity. In obese patients, the waist circumference in women was around 103 (96–115) cm, and in men – 108 (105–116) cm, which indicates an abdominal type of obesity in these patients. As a result of the study, it was found that the level of ЕТ-1 is higher in patients with AH who suffer with overweight and obesity as compared with patients with AH and normal body weight. Also, the peptide level depends on the genotype of patients and is higher in AH patients with the genotype Asn198Asn and Lys198Asn as compared to carriers of the genotype Lys198Lys.

Objective: To assess factors associated with the average number of dental caries in pregnant women. Method: Basic research design: This cross-sectional study was performed between January 2017 and December 2018 in Santa Maria, Brazil. Clinical setting and participants: Multistage random sampling resulted in the recruitment of 256 pregnant women from public health centers across the city. Clinical exams and semi-structured questionnaires including demographic, socioeconomic and behavioral questions were performed by trained interviewers. Main outcome measure: The experience of dental caries was evaluated through the mean value of the Decay, Missing, and Filled Surface Index (DMFS) by 4 trained and calibrated examiners. Multilevel Poisson regression models were used to evaluate the influence of different variables on the average number of dental caries. Rate ratio (RR) and respective 95% confidence intervals were calculated (CI 95%). Results: The prevalence of untreated dental caries was 62.7% in the sample, while the mean DMFS index was 10.27 (± 10.92). Women who smoked during pregnancy had a higher mean DMFS (RR 1.41; 95% CI 1.25-1.57). Furthermore, pregnant women who had poor self-ratings of oral health had a higher average DMFS (RR 1.21; 95% CI 1.10-1.32). Conclusions: The results showed that older age, ethnicity, fewer years in education and the presence of dental plaque resulted in higher means of DMFS.
