Background. The safety effects of individual emission spectra of visible light are currently not well understood. Based on the above works, one can only indirectly judge the presence or absence of an adverse effect of the selected spectrum during mototherapy. Aims: in our work, the goal was to detect the presence of a negative effect of certain wavelengths (410 nm, 580 nm and 630 nm) during mono-exposure to mucous membranes and skin. Methods. At the first stage, the effect of radiation with wavelengths of 410 nm and 630 nm on the mucous membrane of the oral cavity of laboratory animals was determined. At the second stage of the study, 30 women aged 3545 years with signs of photodamage of the skin took part. Results. No undesirable consequences were recorded either immediately after the procedure, or after 20 days. The results of histological examination at all stages of the study indicated the absence of pathological changes. Conclusions. The results showed that mono-exposure with wavelengths of 410 nm, 580 nm and 630 nm is safe and can be recommended for further study and practical application.

Mucous membrane pemphigoid (MMP) is a chronic, autoimmune, subepithelial vesiculobullous disease that very frequently affects the mucous membranes and less often the skin. Oral cavity is the most commonly affected site and desquamative gingivitis (DG) is the most common manifestation. This is the main reason why dentists play a vital role in the diagnosis and managing the oral health of patients. Treatment is usually challenging, however, it only can achieve temporary symptomatic effect. We report a case of desquamative gingivi-tis, manifestation of MMP that was treated successfully with topical corticosteroid. The latter was applied by using of individual made custom trays to improve clinical efficacy.

Taking into account the development of modern surgical stomatology, the problem of optimizing the closure of the wounds of the mucous membrane in the oral cavity remains relevant. In our opinion, it is important to study the morphological properties of the mucous membranes of the oral cavity and optimal tensions and tissue stresses in the area of suturing. It is known that the most susceptible to the action of mechanical factors area is the mucous membrane of the gums. However, taking into account the biomechanical parameters, the mucous membrane of the gum is practically not described. The aim of the study was to carry out a comparative analysis of mucous membranes of varying degrees of tension during fracture operations in the oral cavity. The material is taken to the bureau of forensic examination from corpses and both sexes and the period of mature age for histological studies of conditionally healthy mucous membrane of gum. The fence is carried out in the area of incisors, canines and first premolars from both halves of the upper and lower jaws with a standard dental instrument set were compared: a periodontological kit for curettage. The material is collected both in the area of papilla gum, and from its fixed part. Polarization microscopy is used to study the fibroarchitectonics of connective tissue formations. We used a contact strain gauge device to study the biomechanical parameters of the mucous membrane of the gums in normal and at different degrees of tensile of the mucous membranes. At various degrees of tension of the mucous membrane of the cavity of the mouth connective tissue plate loses the characteristic spatial orientation of the beams, the deep disruption of the entire fibrous frame occurs. Tissue stress recording was performed taking into account the tensile strength: weak (5%), average (7%) and strong (10%). Regardless of the degree of this pathology there is a decrease in the indicator we have chosen. Thus, with a weak degree of tension, the tissue pressure of the gum mucus decreases to 208.08 + 19.2 [6.07] Pa. At an average degree, the selected indicator is 149.43 + \ - 20.25 [6.4] Pa. A strong tension reduces the tissue tension of this area to 102.77 + 12.32 [3.9] Pa. One of the factors that provides high values of tissue gum voltage in the norm (350.4 · 9.69 Pa) is its fibrous components, which form a three-dimensional structure in the collagen beams of the 1st and 2nd order relative to the poorly developed elastic network. Different degrees of tensile of mucous membranes as a model of disorganization with the subsequent drop in tissue tension of the fibrous core were described. At the same time, the index of tissue voltage reduction is directly dependent on the tensile strength and is 208.08 + 19.2 Pa. The data presented indicate the disorganization of the fiber composition of the gums while tightening them. The expressiveness and nature of the changes depend on the degree of tension. Clinical manifestation of pathological processes in fibrous components of ascetic connective tissue is characterized by a decrease in tissue tension indices. Moreover, there is a direct relationship between the degree of tensile of the mucous membrane and the adequate reduction of the tense state of the soft core. Various degrees of tension of the mucous membrane can be considered as a kind of model of the fiber matrix with the dynamics of changes in the biomechanical parameters of tissues. The obtained data on the example of normal anatomy of the gums confirm the general pattern in the structures of the soft core - the quantitative ratio, the way of interaction of collagen and elastic fibers. Their spatial architectonics determine the deformative and strength properties of the connective tissue. For a detailed study of structural and biomechanical correlations we have selected mucous membranes with a different degree of tension. Our research has revealed structural changes in the fibrous gingival cortex against a background of different tensions with adequate tissue tension reduction.

Relevance. Prognostic value of PD-L1 expression in oral cavity squamous cell carcinoma (OCSCC) and its effect on survival is still controversial. It should be to determine the prognostic role of PD-L1 expression on tumor and immune cells of OCSCC and assess their effect on overall survival (OS) and progression-free survival (PFS).Materials and methods. A prospective study included 145 patients, first diagnosed with OCSCC. PD-L1 expression on tumor and immune cells, infiltrating tumor and its microenvironment, was assessed in all tumor samples by IHC, CPS was calculated. Cut-off values were determined by ROC analysis for identification of PD-L1 expression effect on OS and PFS.Results. Most patients with oral mucosa squamous cell carcinoma showed positive expression of PD-L1 on tumor (77.2%) and immune cells (92.4%). The median PD-L1 expression on tumor cells was 13.5% [1.0-40.0], the median PD-L1 expression on immune cells was 5.0% [1.0-11.0], and the median CPS – 18.0 [3.0-7.8]. Univariate and multivariate analyses revealed a significant negative effect of PD-L1 expression on immune cells ≤ 7% on OS (HR 0.66; 95% CI 0.45-0.93; p = 0.0498); PD-L1 expression in tumor cells ≤ 15% (HR 0.65; 95% CI 0.43-0.98; p = 0.0416) and CPS ≤ 21 (HR 0.62; 95% CI 0.44-0.92; p = 0.0183) for PFS. PD-L1 expression in tumor cells ≤ 6% (HR 0.71; 95% CI 0.47-1.08; p = 0.1096) and CPS ≤ 7 (RR 0.67; 95% CI 0.44-1.01; p = 0.0575) had a confident tendency to negative impact on OS.Conclusion. Positive PD-L1 expression in tumor and immune cells as well as CPS are effective additional factors in the prognosis of the disease course, OS and PFS in patients with OCSCC.

Modern data of the etiology of squamous cell carcinoma (SCC) of the mucous membrane of the oral cavity (MMOC) show an increasing prevalence of the disease, a high mortality rate despite the development of modern methods of diagnosis and treatment. SCC is a group of polyetiological diseases. Among the most often etiological factors the use of tobacco products is found as one of the main cause of the disease with the division into active or passive methods of their use, prevalence and potential pathogenicity. Also the risk of the development of SCC MMOC depends on the consumption of alcoholic product as the long-acting traumatic factor. There were analyzed and considered the most common precancerous diseases of the mucous membrane of the mouth, their private and common significance in the structure of precancerous diseases. There was revealed the high correlation between periodontal disease and the development of SCC MMOC, which corresponded to the general assumption about the risk of oncology development against the background of chronic infection in the oral cavity. There were identified new data of the relation between the carriers of some types of the viruses of human papillomavirus and the development of SCC MMOC. Particular understanding of the causal relation of presented on-copathology allowed judging the lack of preventive measures, the necessity of required qualification of specialists of dental and related profile.

AbstractAccording to some studies, the Entamoeba gingivalis colonizing the gingival tissue is an important agent in bringing about periodontitis. Other studies, however, deem it an opportunist that is able to survive in the medium induced by periodontal disease. The aim of this study was to investigate the prevalence of Entamoeba gingivalis infection in patients from the Department of Periodontology, and compare this population with that of healthy people, so as to analyze the relationships between infection and patient sex and age. The result of this work is that in both groups, a correlation (p = 0,19) has been noted between the occurrence of amoebae and other diseases in the oral cavity. Indeed, 81,4% of all patients with some periodontal disease showed the presence of amoeba. Among those who are not afflicted with oral diseases, the presence of amoeba was indicated in 62,5% of the total. In addition, a correlation between the person's age and the presence of protozoa (p = 0,15) was strongly marked among women (p = 0,19). In the three age groups of women in this study (40-49, 60-69, and above 80 years), we observed a 100% presence of protozoa. Our study leads us to the conclusion that infections with Entamoeba gingivalis should be regarded as an factor that is associated with the pathological changes occurring in patients with periodontal diseases.

In many countries of the world, rural areas are characterized by a defective spatial structure of agricultural land. The most frequent defects are large fragmentation and distribution of farmland. The fragmentation of land has been an issue widely described by many authors throughout the world. The problem of the distribution of land owned by individual farmers is slightly different, since due to the complexity of the problem this issue was not widely explored in Poland (plot patchwork) or in other countries of Europe and the world. Land fragmentation and distribution of plots in rural areas has a negative effect on the profitability and efficiency of agricultural production. Land consolidation and exchange is an operation facilitating spatial structure improvement. The authors attempted to develop a universal land exchange algorithm for eliminating the external plot patchwork. As it turns out, so far no land exchange algorithm has been developed. Specific analyses were carried out in Puchaczów commune, county of Łęczna, Lublin voivodeship in the eastern part of Poland, covering an area of 6907.80 ha, split into 15,211 plots. The chequerboard arrays method was used. The publication presents the algorithm and its practical application using a test sample. A result of the studies is a proposal concerning the exchange of land between landowners in the villages of the commune of Puchaczów. Using the algorithm, the area of individual lands in the commune, after the exchange, will increase by 172.09 ha, which is 2.5% for the area of individual lands, and 1.9% for the commune.
