Objective. To estimate the efficiency of using remote technologies and provision of unique service of functional diagnosis in the framework of corporative system of the net of medical institutions, territorially distant from each other. Nowadays, a wide introduction of IT-technologies in medicine has a perspective and priority significance in conditions of unfavorable epidemiological situation including sickness rate of a new coronavirus infection COVID-19. Materials and methods. The efficiency of remote methods was analyzed and assessed using the results of studies among the workers of RZhD and contractual enterprises. Results. Remote consultations permit to increase the number of applied methods of investigation, give the opportunity to obtain qualified conclusions, consolidate information in general database allowing to fill up medical staff deficiency. Conclusions. Thus, patients obtain operatively made unified medical conclusions, which are transferred via defended channels of Internet.

The purpose of work was to develop and implement an algorithm for organizing the work of the nuclear medi-cine department in time of covid-19 epidemy. As a result of the spread of a new coronavirus infection, special requirements are imposed on medical institutions to ensure the safety of patients and medical staff. In ac-cordance with the recommendations of the European association of nuclear medicine, work was carried out on the introduction of preventive examination of per-sonnel, optimization of documentation flow and minimi-zation of direct contact between medical personnel and patients. Patient routing has been adapted, taking into epidemiological standards. The work carried out made it possible to prevent infection of medical staff and pa-tients. The time of contact with patients was reduced to the necessary minimum without any decrease the quality of treatment. Routing deficiencies were identified and eliminated, which significantly extended the time spent by patients in a medical facility. In addition, the load on the medical staff was optimized, which made it possible to improve the quality of medical care provided.

One of the tasks of the hospital-s clinical pharmacologists service is to continuously monitor the consumption of antimicrobial drugs (DDD analysis) depending on the microbiological and epidemiological situation in the hospital. This is necessary for the implementation of various medical programs and technologies aimed at reducing the selection pressure of antimicrobial drugs and reducing the risk of the emergence, accumulation, and spread of bacteria strains with multiple and/or extreme drug resistance to these drugs in the hospital environment. To date, some medical institutions, especially in the various regions of the Russian Federation, do not have a proper team of clinical pharmacologists and a modern, well-equipped and computerized microbiological laboratory. This does not allow full implementation of the above-mentioned programs in such hospitals, e. g. Antimicrobial therapy monitoring system — SСAT and technologies, e. g. «carbapenem-preserving technologies» and «microbiological monitoring».

This study presents the results of the examination of potential donors of blood and its components for immunoglobulins M and G to patients with coronavirus disease 2019 (COVID-19) living in St. Petersburg. A total of 6782 people aged 1824 years were evaluated, which accounted for 2.07% of the regions population (326 760 people) of this age group. The study was carried out in the spring and autumn of 2020. A negative result (absence of antibodies) was obtained in 93.5% of the participants. The rates of immunoglobulins M and M + G were 0.58% and 4.18%, respectively, in the spring and autumn. Moreover, the number of participants who had immunoglobulins M and G + M in the autumn period was four times higher than the indicators of the spring period, which indicated greater infection activities in the population during this period. This is most likely due to the active movement of the population in the summer. When comparing the rates of COVID-19 infection and the frequency of occurrence in donors of the same age, markers of human immunodeficiency virus 1 and 2 and hepatitis B and C in 2020 (0.024, 0.012 and 0.13%, respectively) indicate the urgency of the problem of donor selection during blood services, especially during a difficult epidemiological situation because of COVID-19. Along with organizational measures for the selection of donors (e.g., attracting individuals from organized groups in which there are no signs of an unfavorable epidemiological situation to donation), mandatory testing of potential donors for immunoglobulins M and G should be considered.

According to the WHO statement dated March 11, 2020, the coronavirus infection SARS-CoV-2 has reached the scale of a pandemic and is currently a world health problem. A special group of the population that requires increased attention is pregnant women due to the fact that pneumonia occupies the third place in the structure of indirect causes of maternal mortality. Pregnant women whose immune system has changed appeared to be at an increased risk of infection. Pregnancy is a risk factor for the development of a more severe course of acute respiratory viral diseases and influenza. To date, the issue of intrauterine transmission of coronavirus disease remains completely unexplored, which causes fear among pregnant women for the future generation. Addition of a new coronavirus infection SARS-CoV-2 has a negative impact on pregnancy, childbirth and the postpartum period, the health of newborns, especially in patients with a burdened obstetric history. Pregnancy itself and childbirth do not affect the course of COVID-19, but the infection addition can complicate the course of gestation, causing respiratory distress syndrome, premature birth and spontaneous miscarriages, congenital pneumonia and antenatal death of newborns born from covid-positive mothers. The literature published from December 1, 2019 to July 30, 2021 was searched in several databases, including PubMed, Web of Science, Google Scholar and the WHO COVID-19 database, of which 52 articles were selected for detailed consideration. The relevant literature was searched to understand the issues of infection transmission to newborns from infected mothers and the clinical manifestations of the disease in the former. As a result, it was found that newborns can become infected with SARS-Cov-2 in close contact with infected patients or asymptomatic virus carriers. However, based on the data of modern literature, it is impossible to unequivocally answer the questions posed, more facts are needed to solve the problem. The practical significance of the work is that the results of the study should be used for further studying the consequences of COVID-19 for the health of newborn children.

There were studied epidemiological features of the prevalence rate of acute intestinal infections (AII) in the Republic of Dagestan (RD). The prevalence rate of acute intestinal infections in RD was shown to correspond taken as a whole, to Russian indices, however, the regional feature is the high prevalence rate of shigellosis, there is remained a high proportion of the AII of unidentified etiology. There was substantiated the necessity of development and implementation of targeted programmes aimed at the decline in the morbidity rate in problematic territories. The solution to the problem of the AII in the Republic is possible only under the coordination of efforts of federal and local authorities, sanitary - epidemiological and medical institutions.

Гепарин используется в клинической практике преимущественно как антикоагулянт. Однако спектр его эффектов значительно шире: противовоспалительный, антипролиферативный, гиполипидемический (как прямой, так и посредством повышения активности липопротеинлипазы), антикоагулянтный, противоотечный и др. Именно на противовоспалительное действие гепарина сделан акцент при терапии COVID-19. Одним из патогенетически обоснованных методов профилактики и терапии микрососудистого тромбоза в тканях легких может быть использование ингаляций нефракционированного гепарина, что позволяет реализовать как местное, так и системное действие этого препарата. Терапия ингаляциями гепарина не будет значительно увеличивать риск контаминации вирусом SARS-CoV-2 медицинского персонала при неукоснительном соблюдении стандартов и рекомендуемых мер индивидуальной защиты. Heparin is considered traditionally as an anticoagulant although it has anti-inflammatory, antiproliferative, hypolipidemic (both direct and by increasing the activity of lipoprotein lipase), anticoagulative, anti-edema effects as well. The anti-inflammatory effect of heparin is in the focus of COVID-19 therapy. Inhaled (nebulized) heparin performs local and systemic effects. This way is pathogenetically justified for the prevention and treatment of microvascular thrombosis in lung tissues. Procedures of heparin inhalation should not significantly increase the risk for medical staff to become infected with SARS-CoV-2 when individual protection is fully observed.

Aim: the authors share their proven experience of participating in the development and scientific support of proposals for improving the medical waste management system.Materials and methods. The study was carried out in the format of research work to substantiate proposals for improving the system of medical waste management in extreme conditions.Results. It was found that the system of medical waste management, which functions stably under the conditions of daily activities in peacetime, in extreme situations fails and creates a risk of spreading infectious diseases among the personnel of medical institutions, the population, as well as environmental pollution. This problem is obvious also for military medical institutions of various power structures. The essence of the proposals is to maximize reduction of the stages of the waste life cycle, which in the applied aspect is implemented by equipping military medical institutions with standard installations for the disposal of any medical waste.Conclusion. The value of the conducted research lies not only in the innovative nature of the proposals made, but also in the fact that they were made long before the events associated with the epidemic of coronavirus infection. The results of this work were neither appreciated at their true worth in a timely manner, nor received wide publicity and, as a result, they have not fully used to carry out preventive measures.

Возникновение нового варианта вируса и эпидемический потенциал, проявленный возбудителем COVID-19, поставили перед специалистами здравоохранения задачи, связанные с быстрой разработкой диагностики и профилактики новой инфекции, а также тактики оказания медицинской помощи больным. Активное распространение коронавирусной инфекции заставило активно вносить изменения в систему медицинского образования. Целью данной работы был анализ эпидемиологической ситуации по COVID-19 в Республике Мордовия для понимания особенностей развития эпидемического процесса и составления его прогноза, а также рассмотрения перестройки образовательного процесса в условиях пандемии. Проведен ретроспективный анализ эпидемиологической ситуации по заболеваемости COVID-19 в Республике Мордовия, проанализирована эффективность реализации профилактических и противоэпидемических мероприятий, приведены данные по динамике заболеваемости, выздоровлению и летальности пациентов. Изучены элементы перестройки образовательного процесса. Проанализирована эпидемиологическая ситуация в сравнении с некоторыми регионами РФ, изучены данные по проведению вакцинации, по динамике коечного фонда за период пандемии. Вероятность смерти от коронавируса COVID-19 в Республике Мордовия аналогична мировым показателям. Можно предположить, что дистанционное обучение займет одну из лидирующих позиций в высшей школе. Благополучная ситуация в Республике Мордовия с заболеваемостью и летальностью от новой коронавирусной инфекции складывалась благодаря реализации научно обоснованной стратегии опережающего реагирования – на основе постоянной оценки масштабов распространения новой коронавирусной инфекции, анализа тенденций эпидемического процесса, моделирования развития эпидемической ситуации и проводимых превентивных мероприятий. Непосредственное участие преподавателей университета в лечебной работе, использование их опыта и знаний в решении практических задач будут способствовать повышению качества медицинской помощи и развитию системы здравоохранения Республики Мордовия не только в условиях пандемии COVID-19, но и в дальнейшем. The emergence of a new variant of the virus and the epidemic potential manifested by the causative agent of COVID-19 have set health professionals tasks related to the rapid development of diagnostics and prevention of a new infection, as well as tactics for providing medical care to patients. The active spread of coronavirus infection has forced active changes to the system of medical education. The goal is to analyze the epidemiological situation of COVID-19 in the Republic of Mordovia in order to understand the development of the epidemic process and make a forecast, consideration of the restructuring of the educational process in the context of a pandemic. In this article, a retrospective analysis of the epidemiological situation on the incidence of COVID-19 in the Republic of Mordovia is carried out, the effectiveness of the implementation of preventive and anti-epidemic measures is analyzed, data on the dynamics of morbidity, recovery and mortality of patients are presented. The elements of the restructuring of the educational process are studied. The epidemiological situation was analyzed in comparison with some regions of the Russian Federation, data on vaccination, on the dynamics of the bed fund during the pandemic period were studied. The probability of death from the COVID-19 coronavirus in the Republic of Mordovia is similar to the indicators around the world. It can be assumed that distance learning will take one of the leading positions in the process of studying in higher education. A favorable situation in the Republic of Mordovia with morbidity and mortality from a new coronavirus infection was formed positive to the implementation of a scientifically grounded strategy of proactive response – based on a constant assessment of the scale of the spread of a new coronavirus infection, analysis of trends in the epidemic process, modeling the development of the epidemic situation and ongoing preventive measures. The direct participation of university teachers in medical work, the use of their experience and knowledge in solving practical problems will contribute to improving the quality of medical care and the development of the healthcare system of the Republic of Mordovia not only in the conditions of the COVID-19 pandemic, but also in the future.

Coronavirus infection caused by a new strain of SARS-CoV-2 virus contributed to an increase in the number of infectious patients. The Ministry of Health of the Russian Federation and Rospotrebnadzor took over the organization of work to combat the pandemic. In Moscow, medical and preventive events were organized by the Government of the capital and the Department of Health of the city. As soon as possible, it was decided to reassign medical institutions to infectious hospitals for the treatment of patients with COVID-19 or with suspicion of it. “Maternity hospital No. 8” – the branch of State Clinical Hospital named after V. P. Demikhov was redesigned as a hospital on March 13, 2020. The decision was due to the presence of isolated boxes and an intensive care unit in the institution. The task was solved in one day: the first patients with coronavirus infection were admitted to the hospital for medical care at 17:00 on March 13, 2020.
