The COVID-19 pandemic: an analysis of the epidemic situation and the restructuring of medical education (according to the Republic оf Mordovia)

Возникновение нового варианта вируса и эпидемический потенциал, проявленный возбудителем COVID-19, поставили перед специалистами здравоохранения задачи, связанные с быстрой разработкой диагностики и профилактики новой инфекции, а также тактики оказания медицинской помощи больным. Активное распространение коронавирусной инфекции заставило активно вносить изменения в систему медицинского образования. Целью данной работы был анализ эпидемиологической ситуации по COVID-19 в Республике Мордовия для понимания особенностей развития эпидемического процесса и составления его прогноза, а также рассмотрения перестройки образовательного процесса в условиях пандемии. Проведен ретроспективный анализ эпидемиологической ситуации по заболеваемости COVID-19 в Республике Мордовия, проанализирована эффективность реализации профилактических и противоэпидемических мероприятий, приведены данные по динамике заболеваемости, выздоровлению и летальности пациентов. Изучены элементы перестройки образовательного процесса. Проанализирована эпидемиологическая ситуация в сравнении с некоторыми регионами РФ, изучены данные по проведению вакцинации, по динамике коечного фонда за период пандемии. Вероятность смерти от коронавируса COVID-19 в Республике Мордовия аналогична мировым показателям. Можно предположить, что дистанционное обучение займет одну из лидирующих позиций в высшей школе. Благополучная ситуация в Республике Мордовия с заболеваемостью и летальностью от новой коронавирусной инфекции складывалась благодаря реализации научно обоснованной стратегии опережающего реагирования – на основе постоянной оценки масштабов распространения новой коронавирусной инфекции, анализа тенденций эпидемического процесса, моделирования развития эпидемической ситуации и проводимых превентивных мероприятий. Непосредственное участие преподавателей университета в лечебной работе, использование их опыта и знаний в решении практических задач будут способствовать повышению качества медицинской помощи и развитию системы здравоохранения Республики Мордовия не только в условиях пандемии COVID-19, но и в дальнейшем. The emergence of a new variant of the virus and the epidemic potential manifested by the causative agent of COVID-19 have set health professionals tasks related to the rapid development of diagnostics and prevention of a new infection, as well as tactics for providing medical care to patients. The active spread of coronavirus infection has forced active changes to the system of medical education. The goal is to analyze the epidemiological situation of COVID-19 in the Republic of Mordovia in order to understand the development of the epidemic process and make a forecast, consideration of the restructuring of the educational process in the context of a pandemic. In this article, a retrospective analysis of the epidemiological situation on the incidence of COVID-19 in the Republic of Mordovia is carried out, the effectiveness of the implementation of preventive and anti-epidemic measures is analyzed, data on the dynamics of morbidity, recovery and mortality of patients are presented. The elements of the restructuring of the educational process are studied. The epidemiological situation was analyzed in comparison with some regions of the Russian Federation, data on vaccination, on the dynamics of the bed fund during the pandemic period were studied. The probability of death from the COVID-19 coronavirus in the Republic of Mordovia is similar to the indicators around the world. It can be assumed that distance learning will take one of the leading positions in the process of studying in higher education. A favorable situation in the Republic of Mordovia with morbidity and mortality from a new coronavirus infection was formed positive to the implementation of a scientifically grounded strategy of proactive response – based on a constant assessment of the scale of the spread of a new coronavirus infection, analysis of trends in the epidemic process, modeling the development of the epidemic situation and ongoing preventive measures. The direct participation of university teachers in medical work, the use of their experience and knowledge in solving practical problems will contribute to improving the quality of medical care and the development of the healthcare system of the Republic of Mordovia not only in the conditions of the COVID-19 pandemic, but also in the future.

 The pandemic of the novel coronavirus infection (COVID-2019) has brought significant changes to the learning process, in particular to the process of continuing medical education. The use of distance education methods has significantly helped to reduce the growth of the incidence rate, contributed to the implementation of educational programs in full. In connection with the increase in the proportion of the vaccinated population, the process of transition of the educational process to full-time mode is possible. But in order to maintain a favorable epidemiological situation, some part of the educational process can remain in a remote form.

Purpose: to analyze the epidemiological situation for a new coronavirus infection (COVID-19), to identify some regional features of the Rostov region that contribute to spread of infection.Materials and methods: when assessing the epidemiological situation for a new coronavirus infection in the Rostov region, we used information provided by the Department of the Federal service for supervision of consumer protection and human welfare in the Rostov region. Processing of statistical data was performed by means of generally accepted method.Results: the spreading of a new coronavirus infection in the Rostov region is uneven in nature. When differentiating the territories of the region we identified groups of municipalities with a very high, medium and low number of patients. The administrative territories division of the Rostov region into the “Rostov urban agglomeration” and cluster of municipalities in which pronounced factors and conditions determining the “pendulum” migration of the population are absent, allow analyzing the specific features of the region and identification of territory with the highest risk of epidemic process intensification of a new coronavirus infection.Conclusions: the carried out differentiation of municipalities made it possible to identify and analyze some territorial features of the Rostov region, contributing to the spread of a new coronavirus infection. The obtained results could be used for development of measures aimed at reducing intensification of the epidemic process COVID-19 in condition infection.

Introduction. The topic of telemedicine in Russia is now extremely relevant in connection with the current coronavirus infection in the pandemic. Diagnostics and prevention of the spread of epidemics, as well as remote treatment of patients, is currently the main task of the telemedicine section of the provision of cancer care in the Republic of Bashkortostan. The spread of the coronavirus infection COVID-19 has become a new challenge and a powerful impetus for the rapid development of telemedicine technologies (TMT) in the Republic. Materials and methods. The article describes in detail the algorithm for conducting telemedicine consultations (TMC) at the level  doctor-doctor  in the profile  oncology  upon requests from medical organizations of levels 1, 2, 3 received through the Republican Medical Information and Analytical System of the Republic of Bashkortostan. Results. The results of the work of the Republican clinical oncological dispensary (RСOD) telemedicine service for 2020 are presented. RСOD consultants conducted 32,295 telemedicine consultations on the  oncology  profile directed by medical organizations of the 2nd and 3rd levels of the Republic of Bashkortostan. With the introduction of TMK into the oncological service of the Republic of Bashkortostan, there was a natural shortening of the examination time for cancer patients. The remote appointment of the necessary examinations one day prior to a full-time visit to a specialized RKOD specialist made it possible to shorten the time frame from the time of establishing an oncological diagnosis to the oncological consultation and the beginning of specialized treatment. Conclusions. The widespread introduction of telemedicine in the work of the oncological service of the Republic of Bashkortostan may turn out to be a powerful tool for providing quality care while maintaining patient safety in a pandemic. The impact of the pandemic could turn teleoncology into the main practice of providing outpatient medical care for cancer patients.

The paper deals with the problem of the fundamental training of doctors on the basis of solving various groups of educational and professional tasks in continuing medical education. Analysis of various scientists views allowed the authors to determine that the continuity and integrity of education is ensured by continuity in solving educational and professional tasks, formed from professional tasks. Due to the fact that in health care there is a fairly rapid introduction of the basic sciences (molecular biology, biophysics, biochemistry) achievements results into the processes of medical care, the knowledge-intensiveness of labor functions increases, their diversity increases, which leads to an increase in professional tasks. The interrelation of types and classes of educational and professional tasks ensures the continuity of the content of the fundamental training of doctors. Intra-subject relationships are formed between groups of educational and professional tasks (invariant and variable), ensuring the integrity of the content of the biological training of doctors. Division of educational and professional tasks into classes and types allows to systematize the content of fundamental training of doctors in accordance with the actual educational needs of the trained doctors and the requirements of practical public health, which in turn ensures continuity, expressed in the sequence and consistency of the content, forms and methods of the educational process; in the consistency and coherence of training at various stages of medical education.

The aim of the study is to characterize the trends in the development of the epidemic process of HIV infection in connection with the epidemiological situation of drug addiction in the Republic of Tatarstan.Materials and methods. The official statistical data on the incidence of HIV infection and drug addiction in the Republic of Tatarstan for the period 1987–2016, the data on the prevalence for 2001 and 2016 are analyzed.Results. Direct correlation of the prevalence of HIV infection and prevalence of drug addiction in the different territories of the Republic of Tatarstan were identified for 2001 data (r=0,81, p<0,001) and for 2016 (r=0,82, p<0,001). A statistically significant increase in the incidence of HIV infection in the population against the background of a decrease in the incidence of drug addiction was demonstrated. In the early stages of the epidemic in the region HIV infection spread mainly through injecting drug use and, in recent years, HIV has been sexually transmitted mainly through heterosexual contact.Conclusion. The output of infection outside the population of injecting drug users and the prevalence of sexual transmission of HIV in recent yearsare a reflection of adverse trends in the direction of generalization of the epidemic of HIV infection.

The emergence of SARS-CoV-2 and its worldwide spread has become a challenge for healthcare professionals related to rapid diagnosis of infection caused by the new coronaviruses, provision of specialized medical care, rehabilitation and secondary prevention. Currently, there is limited information on primary, secondary prevention and medical rehabilitation of this disease. The most common clinical manifestation of the new variant of coronavirus infection is bilateral pneumonia, with 34% of patients developing acute respiratory distress syndrome (ARDS). Given the unusual pandemic situation and the peculiar COVID-19 pathogenesis, the stereotyped application of generally accepted methods may be unsafe or ineffective. The Union of Rehabilitologists of Russia has prepared Temporary guidelines that are based on the advice on medical rehabilitation of patients with COVID-19 at various stages of special medical care published by WHO specialists, analysis of reports from clinics currently involved in rehabilitation of COVID-19 patients, as well as on the results of clinical studies conducted earlier on the rehabilitation of patients with Post-intensive care syndrome and adult respiratory distress syndrome (ARDS) of non-coronavirus etiology, regulatory documents of the Russian Ministry of Health and Federal Service on Surveillance for Consumer rights protection and human well-being. A syndrome-based approach to the use of means and methods of physical and rehabilitation medicine is also considered. The guidelines are intended for heads of medical organizations and their structural divisions, general practitioners, physicians, infectious disease doctors, pediatricians, emergency physicians of intensive care units in infectious hospitals, physiotherapy and sports medicine doctors, physiotherapists, medical psychologists, physical therapy instructors, nurses and other specialists of multidisciplinary rehabilitation teams working in the field of medical care for patients with COVID-19 at various stages.

This paper considers the «problem of choice» of holding meetings of student scientific circles of the departments of operative surgery with topographic anatomy (OKHITA) and medical informatics and statistics (MIIS) N.N. Burdenko VSMU. This topic is currently very relevant, since the pandemic of a new coronavirus infection continues in Russia and around the world. At the beginning of the article, the information on the use of specialized electronic programs and electronic educational environments in the complex educational process is provided; the issues of their use at various stages of distance learning during an unfavorable epidemiological situation are analysed. However, according to the authors, very little attention is paid to additional aspects of the complex educational process, which are various student comeptitions, student scientific circles and student scientific societies. The aim of the work was an attempt to determine the form of meetings of the Council of People's Commissars of departments of various orientations (theoretical and practical), to determine the attitude of the respondents themselves – representatives of the teaching staff and students to the form of holding meetings of the Council of People's Commissars, depending on the epidemiological situation, and also to choose the most suitable electronic educational environment or computer program. The objects of the study were 50 employees of various departments of N.N. Burdenko VSMU (from among the teaching staff, who made up 1 group of subjects) and 100 students of the medical faculty – 2 and 4 years (who made up the 2 and 3 groups of subjects). The study was based on anonymous questionnaires. It was found that regardless of the direction of the department (practical – OKHITA or theoretical – MIiS), many respondents advocated the traditional (face-to-face) meetings of the Council of People's Commissars, subject to the normalization of the epidemiological situation. However, as it worsened, the majority favored remote, online, meetings of the Council of People's Commissars. An «interesting» result was obtained when processing the results of choosing an electronic platform for holding meetings of the Council of People's Commissars. So, despite the presence in N.N. Burdenko VSMU, specialized electronic educational environments, the «winner» and a kind of choice of respondents was the computer program «Zoom». The obvious choice of university staff from among the teaching staff and students makes it possible to recommend this program as a kind of «platform» for holding meetings of student scientific communities and circles.

This paper examines various aspects of distance learning (DL) for medical students in the spring semester of 2020. Provides general data on the peculiarities of the educational process of medical students, as well as on the experience of introducing an electronic educational environment – «moodle» into it. This environment is constantly being improved and modified. Over the years of use, it has become a kind of assistant for university teachers, complementing the educational process: allowing students to gain knowledge of the subject, remotely monitor the material they have passed, make an appointment for a consultation with a teacher, etc. A real «test» for «moodle» was the transition of students to preschool education, due to the unfavorable epidemiological situation caused by the new coronavirus infection. The aim of the work was an attempt to study some aspects of DL in this new and difficult period for the university. The objects of research were 200 students of VSMU named after N.N. Burdenko: 100 students of the 2nd year of dental and 100 students of the 3rd year of medical faculties, who were divided into 2 subgroups, depending on gender. The study was based on a questionnaire survey of students on a number of aspects of DL. The online activity of students was studied, followed by an assessment of their progress; determined the very attitude of respondents to the moodle system and DL in general; identified the main negative points (according to students) associated with this form of education. Most of the respondents were, on the whole, satisfied with the educational process at the university and the introduction of the electronic educational environment into the complex learning process. However, many recognized the «moodle» as somewhat «not ready» for the full transition to DL. Also, the most frequent reasons for «absence» of classes by students were identified and the most significant negative aspects of DL were identified. Based on the results, the educational process at a number of departments of the university was corrected in a timely manner. The data obtained are of interest to employees of various departments of medical universities.
