Subject. The prevalence of inflammatory periodontal diseases among elderly people, regardless of their gender, ranges from 69 to 98 % worldwide. Periodontitis is a chronic bacterial disease that proceeds with intoxication of the body, thereby causing serious health problems. It has been proven that periodontitis can cause myocardial infarction and habitual miscarriage. Local and systemic treatment of diseases of this pathology is primarily aimed at suppressing pathogenic bacteria. It is obvious that, in the conditions of total antibiotic resistance of microorganisms and a considerable list of contraindications and complications of the antibiotic therapy, the search for new ways to combat them becomes vital. The main thing is that antibiotics disrupt the biological balance of the biopsy of the body which the patient has to restore from several months to several years. Under these conditions, the use of bacteriophages is an extremely promising solution. Bacteriophages are natural antimicrobial highly sensitive medications. They consist of viruses that target a bacterial cell. In these circumstances, bacteriophages do not harm the microbiota of the oral cavity. A literature review on the use of bacteriophages in dentistry for the treatment of inflammatory diseases of periodontal tissues has been conducted by us. The mechanism of action of bacteriophages, the strengths and weaknesses of the use of phage therapy have also been considered. Goal ― to study the use of bacteriophages for the treatment of patients with inflammatory periodontal diseases. Methodology. The literature review of more than 40 scientific sources has been conducted. Conclusion. The use of bacteriophages is a promising issue. Their effectiveness has been confirmed in the treatment of infectious and inflammatory periodontal diseases and diseases of the oral mucosa. They demonstrate high efficiency to improve the hygiene of the oral cavity and reduce inflammation. Being highly targeted viral medications, they act upon a specific bacterium without affecting the natural microbiota of a person.

Микрофлора человека - сложная саморегулирующая система, способная восстанавливаться при грамотной коррекции. Исследования последних лет показали, что нарушение нормального микробиоценоза полости рта растет из года в год и у жителей Российской Федерации превышает 90 %, вызывая при этом воспалительные заболевания в тканях пародонта, и, как правило, сопровождается дисбиозом полости рта, выраженность которого соответствует степени поражения пародонта. Одно из ведущих мест в развитии данной патологии занимает резидентная облигатно-анаэробная и микроаэрофильная микрофлора полости рта. Изучение микробиологического состава пародонтальных карманов (ПК) при воспалительных заболеваниях тканей пародонта (ВЗТП) имеет большое значение непосредственно для эффективного лечения и понимания патогенетических этапов этих заболеваний. Достаточно серьезной проблемой современной стоматологии как в нашей стране, так и за рубежом являются заболевания пародонта. Указанная патология снижает принятый ВОЗ критерий качества жизни и приводит к преждевременной потере зубов и представляет серьезную медицинскую, социальную и экономическую проблему. Первичным фактором, вызывающим поражение пародонта, являются бактерии зубного налета. Этиологическая структура инфекционных процессов в последнее десятилетие значительно изменилась, что связано с постоянной эволюцией микробов и вовлечением в патологический процесс условно-патогенных микробов, которые могут выступать в качестве комменсалов в составе нормальной микрофлоры и проявлять свою патогенность при снижении иммунного статуса организма The human microflora is a complex self-regulating system that can be restored with proper correction. Recent studies have shown that the violation of the normal microbiocenosis of the oral cavity increases from year to year and in the residents of the Russian Federation exceeds 90 %, causing inflammatory diseases in the periodontal tissues, and, as a rule, is accompanied by dysbiosis of the oral cavity, the severity of which corresponds to the degree of periodontal damage. One of the leading places in the development of this pathology is occupied by the resident obligate-anaerobic and microaerophilic microflora of the oral cavity. The study of the microbiological composition of periodontal pockets (PC) in inflammatory diseases of periodontal tissues (VSTP) is of great importance directly for the effective treatment and understanding of the pathogenetic stages of these diseases. Quite a serious problem of modern dentistry both in our country and abroad are periodontal diseases. This pathology reduces the WHO-accepted quality of life criterion and leads to premature tooth loss, and is a serious medical, social and economic problem. The primary factor that causes periodontal disease is plaque bacteria. The etiological structure of infectious processes has changed significantly in the last decade, which is due to the constant evolution of microbes and the involvement of conditionally pathogenic microbes in the pathological process, which can act as commensals in the normal microflora and show their pathogenicity with a decrease in the immune status of the body

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

Periodontal disease is one of the most complex problems of modern dentistry. It is known that in the development of periodontal disease, a prominent role belongs to local factors, but systemic processes, which lead to profound changes in the internal environment of the organism and structural damage to the periodontal tissues, play an important role. It is proved that generalized periodontitis is pathogenetically closely associated with the pathology of internal organs, and inflammatory and dystrophic-inflammatory lesions of parodontium are largely secondary to systemic processes in the body that underlie a number of diseases of the internal organs. Scientists from all over the world believe that periodontal diseases are etiological and pathogenetic factors to heterogeneous diseases, in which the etiology and pathogenesis of a complex of immunological, biochemical, structural and other pathological changes in mechanisms of inflammatory alteration around dental tissues is observed, and metabolic disorders in the body are accompanied by pathochemical , morphological, immune and biological changes in parodontium. In this regard, promising directions for studying the etiology and pathogenesis of periodontal diseases include the identification of factors that affect the entire human body and periodontal tissue in particular. In the body of the patient with generalized periodontitis there are numerous processes, the interaction of which depends on the nature of the course of the disease and the peculiarities of the formation of a local pathological center. The most important is the association of the endocrine and immune system, the violation of the functional state which promotes the development of deep metabolic disorders in the periodontal tissues. As a result of the review of scientific publications and own observations, the most common clinical symptoms and pathogenesis in inflammatory diseases of periodontal tissues have been analyzed. Clinical symptoms of inflammatory diseases are considered as sublimation of their pathogenetic bases, as clinical manifestations of basic for pathology of processes. The conclusion is made on the need for an integrated approach in the treatment of periodontal diseases. The aim of the study – familiarization of broad medical public with the modern views on the etiology, pathogenesis and clinical symptoms in patients with periodontal tissue diseases.

Introduction. More and more researches dedicated to the communication of diseases of the oral cavity of pregnant women with diabetes. It is proved that the intensity of caries and inflammatory periodontal diseases (gingivitis and periodontitis) increase significantly during pregnancy, while the presence of comorbidities, such as diabetes, increase these indexes.Aim. The aim of the work was to study the dental status of pregnant women with diabetes.Materials and methods. The study compared women with gestational diabetes mellitus, type 1 diabetes, and type 2 diabetes, to a control group of pregnant women without diabetes. In addition to clinical research methods, liquid-based cytology of the contents of the gingival sulcus was performed.Results. The results of clinical and laboratory studies have shown that inflammatory diseases of periodontium and teeth within pregnant women with diabetes are more common than within the pregnant women without this disease. It should be noted that the frequency of occurrence and severity of these diseases in the pregnant women with type I diabetes is higher than in the other groups.Conclusion. Pregnant women with diabetes are at risk for dental disease and require more attention from dentists, endocrinologists and obstetricians. The use of liquid-based cytology method helps in the diagnosis of inflammatory periodontal diseases.

The aim: To investigate current trends in the study of oral biofilm and its control. Materials and methods: A research of 32 literature sources has been made and it has been taken into account that some terminological differences in determining objects of study. Conclusions: Detailed analysis of modern domestic and foreign literature argues the necessity of further in-depth study of oral biofilms. Understanding the ethiological factors and mechanisms of the pathogenesis periodontal tissues inflammatory diseases gives the opportunity to treat targetly by destroying complicated sections of the vital activities and oral biofilm microorganisms relationships.

Background. Periodontal tissues inflammatory diseases are widespread among young people. Objective. This study was aimed at elaborating the method to assess risks of periodontal inflammatory diseases and determining its efficacy depending on the state of dental tissues, gum tissues and sex.          Methods. The study included 182 students (93 men, 89 women) aged 19-29: 22 individuals had no lesions of hard dental tissues and no signs of periodontal disease; 51 individuals were found to have DMF index <6; 52 individuals – DMF index ≥6; 57 individuals were diagnosed with chronic catarrhal gingivitis. Primary groups were formed in autumn; re-examination was carried in spring. The research participants were assessed for detection of risks of periodontal inflammatory disease by the method developes by the authors (Patent UA 54041). Results. The study revealed that the risk of development of preiodontitis increases in individuals with high caries and gingivitis intensity. In spring, more individuals suffer from microbial imbalance in in the composition of gingival sulcus fluid and decrease in the mean stability coefficient value that indicates an increased risk of inflammatory periodontal disease development. Women were less likely to experience seasonal dysbiotic changes in the gingival sulcus fluid composition compared with men. Conclusions. The method suggested for assessment of the risk of periodontal inflammatory diseases is of high informativeness. It allows clinicians detecting early pre-nosological signs of oral microbiocenosis imbalance that enhances the effectiveness of early diagnosis of inflammatory periodontal diseases.

Periodontal disease in the adult population is one of the most pressing problems of dentistry around the world. Thanks to the introduction of modern technologies in practice, it was possible to identify the main mechanisms of the development of this disease at the molecular and genetic level. The interaction of periodontal pathogens with immune factors of antimicrobial protection of the body is the basis of periodontal tissue inflammation and further leads to the destruction of the alveolar bone. Epithelial cells of the gum mucosa play a crucial role against periodontal pathogenic bacteria. Factors of innate immunity play a role not only in antimicrobial protection, but they also support the conditions necessary for the healing and regeneration of periodontal tissues. Therefore, various therapeutic approaches that affect the factors of innate immunity are considered as effective and promising. Clinical examination and determination of factors of innate immunity were performed in 115 patients. Healthy individuals made up a group of 30 people. Patients with chronic generalized periodontitis with varying degrees of severity were 85 people. Each patient underwent orthopantomography on the orthopantomograph Orthophosis XG DS/Ceph (SIRONA Dental System GmbH, Germany) in order to assess the state of the bone tissue of the jaws (the degree of destruction of the cortical layer, the degree of resorption of the interalveolar partitions). Targeted intraoral radiographs were used to assess the state of bone tissue and the quality of surgical treatment. Immunological methods of research were carried out in several stages. The findings of this article make the claim that the use of autologous complex immunopeptides in the surgical treatment of periodontitis reduces the time of achieving a therapeutic effect 2 times, resulting in the rapid relief of the symptoms of inflammation and acceleration of reparative processes.

Background: Stress is playing a major role as an aetiologic factor in the initiation of many systemic diseases. Similarly, it is also thought to be an aetiologic factor in the progression of periodontal diseases. Aim: The production of free radical species is associated with various inflammatory diseases and also during stressful conditions. Hence, an effort in the present study is done to correlate psychosocial stress and periodontitis using serum MDA as an oxidative stress biomarker. Materials and Methods: A current cross-sectional study included 201 individuals between 20 to 60 years of age. A periodontal examination included Probing Pocket Depth, CAL, Plaque Index, and Gingival Index. According to CAL individuals were divided into four groups i.e. healthy, slight (CAL = 1-2 mm), moderate (CAL = 3-4 mm) and severe (CAL = ≥5 mm) periodontitis. Psychosocial stress of the individuals was evaluated using occupational stress index. Serum MDA level was evaluated using spectrochromatometer. Results: Patients with stress demonstrated increased levels of MDA along with a higher loss of attachment. Conclusion: The results revealed statistically significant association between psychosocial stress and periodontitis indicating psychosocial stress as a risk factor for developing periodontitis.

The review of the scientific literature is devoted to the topical issues of studying the etiology of periodontal diseases, which are one of the most common and complex pathologies of the maxillofacial region. Analysis of recent studies proves a stable relationship between the development of periodontal diseases and disorders in the immune system, the neurohumoral system, metabolic disorders, genetic predisposition, and so on. The article presents the data obtained in the course of studying the literature on the role of disorders in the functioning of individual organs (gastrointestinal tract, liver, lungs, heart, and urinary system) in the development of chronic periodontal diseases. The article notes that the anatomical and physiological proximity of the periodontal and digestive tract tissues, the generality of innervation and humoral regulation create prerequisites for the involvement of periodontal disease in the pathological process in diseases of the gastrointestinal tract. One of the main etiological factors in the development of inflammatory diseases of the gastrointestinal tract and periodontium is Helicobacter pylori, which is found in the loci of the oral cavity: in the oral and gingival fluid, on the mucous membrane of the tongue and cheeks, and in the periodontal pockets. It is pointed out that the liver also occupies a special place in the development of periodontal diseases, which is explained by the performance of its significant functions for the human body: regulatory, metabolic, antitoxic and other. There is evidence that the pathology of periodontal disease plays a leading role in the structure of dental diseases in patients with chronic obstructive pulmonary diseases, which is clinically manifested by symptoms of generalized periodontitis of the І-ІІ degrees of development and its complications - partial or complete secondary adentia, and with tooth preservation - defects in dental series and violations of occlusion, function, aesthetics. Scientists suggest a general biological mechanism for the development of generalized periodontitis and cardiovascular diseases, linking the development of periodontal diseases in patients with cardiovascular pathology with microcirculatory disorders. The dependence of the severity of inflammatory changes in the periodontal tissues on the disturbances of salt metabolism in urolithiasis is proved. The data obtained indicate that diseases of the internal organs contribute to the structural damage of periodontal tissues and they are a risk factor for periodontal diseases, which necessitate the presence of not only theoretical knowledge and practical skills in dentistry, but also their awareness of the features and clinical manifestations of somatic pathology. An urgent and justified step in the treatment of periodontal diseases is also the involvement in the process of rendering complex dental care to internist doctors capable of quickly and qualitatively assessment the condition of the internal organs and the basic systems of the patient's body.

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.
