For a woman’s dental health, pregnancy is a critical period, characterized by an exacerbation of inflammatory periodontal diseases before pregnancy, as well as a significant increase in dental caries. Using a significant group of drugs during pregnancy is not safe for a woman's body. In this regard, in the treatment of periodontal diseases, the inclusion of phytopreparations into the complex of treatment and prophylactic measures that can restore local immunity, reduce inflammation and accelerate the restoration of the mucous membrane, which have minimal side effects on the body as a whole, is of great importance. The aim of the work was to assess the clinical effectiveness of the local use of a new chewing phytosubstrate containing beeswax and dry aqueous boron extract, consisting of oak bark, alder fruit, birch buds, chamomile flowers and thyme grass in women with gingivitis with gestosis complicated pregnancy. Methods. A survey was conducted of 127 pregnant women, of which 79 pregnant with gestosis and 48 patients with a physiological course of pregnancy at the age of 18-34 years in the II and III trimesters. Results. The dental status and physicochemical properties of the oral fluid in women with gestational complications of pregnancy were studied before and after the application of the new phytosubstrate. Conclusions. The chewing substrate containing beeswax and dry aqueous extract consisting of oak bark, alder fruit, birch buds, chamomile flowers and thyme herbs has been found to be highly effective in the prevention and treatment of gingivitis in women with gestosis complicated pregnancy, which was confirmed by improved biochemical and physico-chemical indicators of oral fluid, as well as clinical indicators of dental status. in women with gestational complications.

Aim: to study the state of local immunity and oral hygiene in patients with GP suffering from AN.Materials and methods: 35 patients with AN were examined at the age of 19-35 years. In the study we used immunological methods, an index assessment of the state of oral hygiene (PMA, API, GI).Results and discussion: in patients of I group poor oral hygiene was established. The amount of lysozyme in the oral fluid in I group was 0.014 ± 0.07 g/l. A decrease SIgA (11SIgA) was established in patients of I group: I, II and control (135.31 ± 23.17, 130.26 ± 24.21 and 300.34 ± 27.38 mg/l, respectively). A tendency to an increase in the level of IgG in the oral fluid in patients of group II compared with I and the control group (5.35 ± 0.36, vs. 4.57 ± 0.04 and 4.98 ± 0.23 g/l, respectively) was established.Conclusions: poor oral hygiene is an objective indicator which reflects low motivation to maintain oral health. The observed immunodeficiency (11SIgA) in patients with GP is a mirror image of changes occurring in the body of patients with AN.

Oral diseases occupy the leading position among common disorders. The etiological factors of the inflammatory processes of the oral cavity can be some general disturbances of the body. Today, the topical medications are important component of the treatment and prophylactic measures of range of pathological conditions in dental practice. Among this type of medicines widely used herbal preparations, which are known for their broad spectrum of activity. The aim of the research was to scrutiny the treatment methods for OD, to study the topical medications assortment of the pharmaceutical market of used in pharmacotherapy of OD with a view to further developing the dosage form and determining the feasibility of its use in the apitherapy of OD and the possibility of occupying it with certain market niche. The data from official information sources, in particular, the List of registered drugs approved for use in Ukraine as of 01. 01. 2019 were used as the objects of study. Market analysis was carried out using the ATC classification system (Anatomical Therapeutic Chemical) taking into consideration international non-patented and trade names, dosage forms, country of manufacture. Statistical, graphic, logical and marketing studies were conducted during the work. Information was used to determine the assortment of the modern pharmaceutical market of drugs that can be used in pharmacotherapy of OD, was the order of the Ministry of Health of Ukraine No. 566 dated 11. 23. 2004. «On approval of protocols for the provision of medical care in the fields of "orthopedic dentistry", "therapeutic dentistry", "surgical dentistry", "orthodontics", "pediatric therapeutic dentistry" and "pediatric surgical dentistry"». According to the official website of the SEC of the Ministry of Health of Ukraine as of 01. 01. 2019, 76 drugs were registered as the A01A group and 59 drugs under trade names. According to the АТС classification system major topical medications belong to group A01A. The share of domestic drugs prevails over foreign ones and amounts to 60.5%. Attention was drawn to the limited range of drugs from beekeeping products, whose share is only 6.6%. It has been found that the production of original propolis-based drugs at pharmaceutical enterprises of Ukraine with a sufficient raw material base is topical issue of pharmacy.

As a result of the continuing changes in the condition of tissues and organs of the dental cavity and the body as a whole, there is a rise in the intensity of tooth decay and periodontal diseases with the onset of pregnancy. The aim of our research was to look into the prevalence and severity of pathology in the hard tissues of teeth and periodontal tissues during pregnancy, as well as to figure out how to better organize therapeutic and prophylactic steps for pregnant women. A comprehensive epidemiological study of 170 pregnant women aged 18 to 40 living in Tbilisi was carried out according to the WHO method. The findings show a high prevalence of dental cavity (from 61 percent to 98.8%) and a high severity of dental caries among pregnant women (from 2.55 1.03 to 14.31 5.44). The intensity of caries indicators increased as the gestational stage lengthened, but the greatest increase was observed in the second trimester of pregnancy. The results indicate a substantial prevalence of periodontal pathology (according to the PMA index) among pregnant women, with the level of pathology increasing with their age and gestational age (from 41.7 percent to 78.8 percent). By the end of the second trimester of pregnancy, the most noticeable improvements in periodontal tissues had been observed. Oral hygiene played a big role in determining the presence and incidence of periodontal disease. Low dental health indicators and worsening dental status during pregnancy point to the need for a more comprehensive diagnosis of all risk factors for major dental diseases, as well as the implementation of all stages of therapeutic and prophylactic interventions during pregnancy, based on the findings of a complete dental examination.

The inhabitants of industrial areas are influenced by anthropogenic factors. Harmful chemicals enter the body from the air, drinking water, food and are found in the oral fluid, hard tooth tissues, dental deposits, biopsy of tissue structures and contribute to the development of caries and inflammatory periodontal diseases. Toothpastes and mouthwashes are used for prevention of major dental diseases. The toothpastes contain a variety of abrasives: silicon dioxide, calcium carbonate, titanium dioxide, calcium bicarbonate, hydroxyapatite, which can act as adsorbents of heavy metal compounds. Biomonitoring using inductively coupled plasma mass spectrometry (ICP-MS) allows to determine the micro-quantities of metals in blood, urine, human milk, hair and oral fluid. The study of oral hygiene products showed that toothpastes and mouthwashes, used by patients for prevention and treatment of inflammatory periodontal diseases, have an adsorption activity for metal ions. The article evaluates the adsorption efficiency of medical and preventive toothpaste relative to the metal ions (chromium, manganese, cadmium, lead, etc.) in the oral fluid of the inhabitants of industrial areas. The use of adsorption-effective toothpaste twice a day for two weeks showed a significant decrease in the concentration of metal ions in the mixed saliva of the inhabitants which are under the influence of unfavorable environmental factors. When using a toothpaste containing silicon and titanium oxides, the concentration of manganese, chromium, arsenic, lead and cadmium ions in the oral fluid decreases within 2 weeks. The modern method of ISP-MS is advisable to use for high-precision monitoring of the oral fluid ion composition in patients living in the industrial areas.

Objective. Hygienic assessment of the quality of drinking water from two rivers, Kama and Chusovaya, which supply the city of Perm, was carried out. Water in them is different in mineral composition, but similar in terms of anthropogenic pollution. Materials and methods. Dental status was determined in 142 schoolchildren aged 15-17 years old, who use water from different sources. The concentration of total calcium and magnesium in the oral fluid was determined for all schoolchildren. Results. The study found that the degree of mineralization of drinking water affects the mineral composition of the saliva. Schoolchildren, who use drinking water of low mineralization, have a low concentration of calcium and magnesium in the saliva and a high intensity of dental caries. Schoolchildren, who use drinking water of increased mineralization, have a high concentration of calcium and magnesium in the saliva, a low intensity of dental caries and a high intensity of periodontal diseases due to an increased formation of tartar. Treatment and preventive measures, carried out among schoolchildren, included the drug Complivit, plates CM 1, low-intensity laser light. Conclusions. The effectiveness of treatment and preventive measures was determined by the activity of the oral fluid enzymes before and after the course of treatment.

Aim. To study the state of local immunity of the oral cavity in patients with fixed aesthetic dental prostheses and inflammatory periodontal diseases.Methods. 90 patients with fixed aesthetic dental prostheses and inflammatory periodontal diseases (main group) and 21 patients without dental prostheses and inflammatory periodontal disease (control group) were examined. Immunoglobulin A, sA, G, M, E classes, interleukin-4, -6 and -1β, interferon α contents in oral liquid were determined using enzyme immunoassay.Results.. As a result of our study of immunoglobulins level in saliva in patients of the main and control groups an increase in the immunoglobulin A level, compared with the normal value, statistically significant increase in secretory immunoglobulin A were revealed; difference in the immunoglobulin G level was not determined. The increase in immunoglobulin E level in patients of the main group was determined. When studying the interleukins content in the oral fluid a statistically significant decrease in the concentration of IL-4 and increase in interleukin-6 and -1β content in patients of the main group were determined. The tendency to decrease in the interferon α content in the main group compared to the control group was registered.Conclusion. In patients with fixed aesthetic dental prostheses and inflammatory periodontal diseases humoral immunity imbalance was identified, manifesting in changes of the immunoglobulin classes A, sA and E, interleukin-4, -6 and -1β contents in oral fluid.

Relevance. A long-term inflammation in the oral cavity, unreasonable treatment of periodontal patients with antibiotics cause imbalance between certain types of fungal-bacterial associations accompanied by oral dysbiosis and change of local immune status. So, development and application of new comprehensive diagnosis and treatment techniques in periodontal patients, use of products for correction of microbiota and local immunity are a current and a long-term task. Purpose is to optimize the comprehensive treatment protocol of inflammatory periodontal diseases by introducing probiotics and evaluation of oral microbiome and cytokine profile.Materials and methods. 140 patients were examined. Of these 60 patients had chronic generalized plaque-induced gingivitis and 60 patients suffered from moderate chronic generalized periodontitis. Bacterial and fungal microbiome was assessed and the host immune response was evaluated in all patients before and after the treatment. Half of the patients were treated conventionally and the other half were treated according to a modified scheme: probiotic “Bifidumbacterin Forte” was added.Results. A large number of yeast-like fungi Candida and commensal bacteria were detected in periodontal pockets of patients with chronic generalized plaque-induced gingivitis and moderate chronic generalized periodontitis. That correlates with a significant increase of pro-inflammatory cytokines (TNF-α, IL—8), decrease of concentration of INF-γ and increase of antiinflammatory cytokine IL-4.Conclusion. Changes in clinical, microbiological and immunological values during a modified combination therapy with a probiotic and during a conventional treatment demonstrated that effectiveness of treatment of chronic gingivitis and chronic periodontitis increased by 40% and 37% respectively.

Oral fluid is an alternative biological material that confirms correlations with blood parameters in various pathological conditions of the body. In order to find a non-invasive approach to stratification of patients with COVID-19 disease, molecular biomarkers of the oral fluid have been determined in patients with moderate coronavirus infection in comparison with clinically healthy individuals. It has been shown that proteomic, carbohydrate, macro- and microelement profiles of the oral fluid in coronavirus infection can be used for diagnostics. The features of protein metabolism were revealed: an increase in the content of total protein, urea; increased activity of enzymes aspartate aminotransferase, gamma glutamyl transpeptidase, creatine phosphokinase, alkaline phosphatase; changes in carbohydrate metabolism, which is expressed by an increase in glucose and lactate levels, an increase in lactate dehydrogenase activity, sodium, chloride, calcium, magnesium, iron content.

With diabetes, there is a violation of the microvasculature, a violation of the immune status, an increase in destructive processes in the bone tissue, which leads to the development and / or intensification of pathological processes. In the oral cavity, the most frequently diagnosed inflammatory periodontal diseases, chronic injuries of the oral mucosa (COP), recurrent aphthous stomatitis, COP candidiasis. The aim of our study was to study the dental status of patients with type 2 diabetes mellitus. Materials and Methods: A descriptive evaluative epidemiologically controlled, non-randomized study was conducted to determine the effect of type 2 diabetes on dental status. The main group (“case”) consisted of 68 patients with type 2 diabetes in the compensation phase (IA) and 56 patients in the subcompensation phase (IB). The comparison group included 60 patients without somatic pathology. All patients underwent a comprehensive dental examination, bacteriological and microscopic examination to identify fungi of the genus Candida. Results: in patients with subcompensated form of type 2 diabetes, there was an increase in the frequency of complaints of plaque on the tongue, burning sensation in the COP, dryness and changes in taste. Periodontal indices indicate that the patients of the main group have a high prevalence of inflammatory and destructive processes in the periodontal tissues (indices of PMA, KPI, Mullemann indices) with poor oral hygiene (OHI-S = 3.5 ± 0.4), which confirms the Friedman criterion (χ2 = 116.27; p = 0.0000) based on the calculation of Kendall's concordance coefficient (0.743). The diagnosis of oral mucosa candidiasis was confirmed in 74.6% of patients in the main group by bacteriological examination.

The aim of our study was to find out the effect of our proposed treatment and prevention complex, which included oral administration of «Kvertulin» complex drug, «Imupret» drops, «Pikovit» multivitamin drug and «Exodent» local irrigation of the oral cavity with a solution of tooth elixir that influence on the rate of salivation and viscosity of oral fluid in children with chronic catarrhal gingivitis and diabetes mellitus.The treatment and prophylactic measures had positive effect on the homeostasis of the oral cavity, which assisted to reduce the viscosity of the oral fluid, increase the rate of salivation, as evidenced by observation of patients for 6 months. The treatment and prevention complex are recommended to use 2 times a year, as the studied indicators deteriorate over time.
