Psoriasis is an inflammatory and autoimmune disease with unknown etiology. This is a chronic, recurrent, distressing and costly disease, which has a great impact on the quality of life of individuals. Its treatment varies from topical to systemic medication and sometimes with a great deal of side effects. Probably, changing nutritional habits, life style modification and applying preventive measures may reduce the high amount use of chemical drugs and the costs of the disease. This study investigates etiology, clinical manifestation and natural treatments of psoriasis from the perspective of Persian Medicine by which prevention and nutritional recommendations and some treatments can be introduced. Searching selected sources of Persian Medicine including the Canon of Medicine, Zakhirah -E- Kharazm Shahi, Kamel alsanaat, Sharh al-asbab va al-alamat, Tebbe-Akbari, Moalejate Aghili, Exir-e-Azam, three diseases named “Ghooba”, “Barase Asvad” and “Saafe-Yabes” were found to be similar to psoriasis in their manifestations. Therefore, study of their specific treatments in the levels of lifestyle management more importantly nutrition and herbal therapy could be noteworthy for the future studies.

Presently, no clear picture is available about the facilities providing clinical services for persons who stutter (PWS) in Kuwait. This information is crucial for any awareness program to be established in the future. The purpose of this study was to identify clinical facilities and speech-language personnel that provide services for PWS in Kuwait. Participants consisted of 21 clinical directors of governmental medical centers, non-profit clinics, and private clinics as well as department heads of governmental school clinics where speech-language services were provided. Participants were interviewed regarding the availability of speech-language services within their centers and whether or not PWS receive services. The results revealed that four out of five governmental medical centers with a total of 32 speech-language pathologists (SLPs) provided services for PWS. Additionally, 12 schools of special education were found to have 62 SLPs on their staff providing fluency services for students. Finally, two stand-alone private clinics and one non-profit clinic provided services for PWS. Results indicated an overall shortage of SLPs in the country, especially in medical settings. This study sets the foundation for a series of future studies investigating the type and quality of stuttering services provided by the identified facilities in Kuwait.

In two parts of the article, more than 70 errors and violations of the Biotechnics of artificial breeding of Pacific salmon in salmon hatcheries are collected and summarized, which lead to an immediate or delayed deterioration of the quality of reared fry and a decrease in their survival rate. Deviations from the biological basis of fish farming are shown and possible consequences of such errors or violations are predicted. In the first part of the article (#7,2020) there were General errors (throughout the entire fish-breeding process), as well as violations of Biotechnics during the work with producers, collecting eggs and preparing them for incubation, in the second part-violations of Biotechnics during the incubation of eggs, holding pre-larvae, rearing and release of young. Their correction, prevention or minimization of consequences will significantly improve the quality of sexual products and offspring from producers, increase the linear and weight gain of juveniles during rearing, improve the coefficient of feed payment, reduce the time of rearing, improve the epizootic situation by developing immunity in fry and mobilizing other protective reactions of their body, significantly reduce waste at all stages of the production process, increasing survival by 10–15%. In conclusion, it is noted that the first and most important condition for an efficient salmon hatchery is the creation of optimal environmental conditions at each stage of the production process. The second condition is strict compliance with the Biotechnics of artificial salmon breeding and the complex of veterinary and sanitary, fish-breeding and meliorative and therapeutic and preventive measures. The third is the availability of competent specialists who regularly improve their skills in accredited Universities to implement, control and manage the first two conditions.

This article presents the results of determining the level of perinatal infection of calves with the bovine leukemia virus at the age from birth to colostrum intake and 1-1.5 months after birth when drinking milk from healthy individuals, excluding the alimentary transmission route, with an infection frequency of 5.4% in the surveyed farm. Improvement of molecular diagnostics based on genomic analysis in the scheme of complex antiepizootic/recreational measures for bovine leukemia has been carried out in order to improve the quality of dairy products. As part of the implementation and improvement of the scheme of preventive measures, a positive trend was found, expressed in a tendency to a 3.6-fold decrease in the frequency of detection of BLV infection of young animals. In the course of work, the efficiency of the use of a gene diagnostic test - multiplex PCR-RT – as a tool for the specific prevention of bovine leukemia of calves in the early stages of the disease was shown: the relative diagnostic sensitivity of the PCR-RT technique is at a level not lower than that of the serological method and differs in the side of overestimation by 3.8 times.

Background: Anal fissure (AF) is a common disease associated with severe pain and reduced quality of life. Factors related to lifestyle, including diet and bowel habits, play a pivotal role in its pathogenesis. Most of the chronic fissures are not responsive to drugs and more likely to recur. Given the significance of diet in Persian medicine (PM), investigation on physiopathology and appropriate foods can be useful for decreases in AF symptoms and consequences. Objective: This study was intended to evaluate the role of diet in the formation and progression of AF from the perspective of PM. Methods: In this study, the most important resources of PM dating back to thousands of years were reviewed. All these textbooks contained a section on AF, its causes, and treatment. Further analysis was performed on these resources in comparison with databank and resources of modern medicine to develop a food-based strategy for AF management. Results: From the view of PM, the warmth and dryness of anus temperament accounted for AF. Both Persian and modern medicine identified constipation as another cause for AF. Therefore, avoidance from some foods and commercial baked goods was recommended. Both Persian and modern medicine forbad the following foods: potato, cabbage, cauliflower, pasta, beef, fish, and so forth. High fiber and oligo-antigen diets with some limitations have garnered more attention. Conclusion: An integrative approach is recommended employing both Persian and modern medicine for AF. There have been some evidence in this regard, however standardized clinical trials are required for future research.

Sthoulya (Obesity) is one such disorder which creates lot of physical as well as mental disorder to the sufferer. Due to changing lifestyle, comforts and dietary habit lots of individuals changed their life totally. Obesity is a growing disease in developed and developing countries. Prevalence is drastically hike in past few years. Ayurveda, the science of life with which we can manage and control lots of lifestyle disorders. Focusing on dietary and lifestyle management along with treatment, we can overcome the hazards of obesity which is growing in a uncontrolled manner. The available data is based on the clinical findings only. Aim and objective: To assess the effect of “Guru Cha Atarpanam Chikitsa” in the management of Sthoulya. Setting: Swastharakshana evam Yoga, OPD and IPD, SDMCAH, Hassan. Method: Udwarthana, Parisheka, Shamana Aushadhis, Ahara, and Vihara was followed within the treatment duration and effect of treatment was assessed before and after treatment, advised for follow up. Results: The treatment adopted is effective in the management of Sthoulya and to improve the quality of life.

To limit the spread of coronavirus SARS-CoV-2, a nationwide lockdown started in Italy in March 2020. In this unpredictable situation, a cross-sectional study using an online questionnaire was carried out by the Observatory on Food Surplus, Recovery and Waste of CREA Food and Nutrition Centre. The aim of this work was to evaluate how Italian habits changed during this period, the determinants of changes, and the effect on food waste prevention. In a sample of 2678 respondents, 62% showed low Adherence to the Mediterranean Diet (AMD). During lockdown many of participants improved the quality of their diet, increasing their consumption of fruit (24.4%), vegetables (28.5%), legumes (22.1%), nuts (12%), and fish or shellfish (14%). Unfavorable changes were observed with the excessive consumption of sweets or pastries (36.9%) and comfort foods (22.7%), and a lack of physical activity (37.2%). The main novelty of this study was the examination of dietary changes identified by a cluster analysis. Respondents with generally high AMD improved their eating habits, while the habits of the respondents with generally low AMD remained unchanged. In addition, nearly 80% of respondents were sensitive to food waste. The study provides a useful contribution to the debate on nutritional recommendations in case of further lockdown.

Viral diseases in viticulture lead to annual losses in the quantity and quality of grape production. Since no direct control measures are available in practice, preventive measures are taken to keep the vines healthy. These include, for example, the testing of propagation material for viruses such as Arabis mosaic virus (ArMV), Grapevine fanleaf virus (GFLV) or Grapevine leafroll-associated virus 1 (GLRaV-1) and 3 (GLRaV-3). As long-term investigations have shown, GLRaV-1 (2.1%) occurs most frequently in southwestern German wine-growing regions, whereas GLRaV-3 (<0.1%) is almost never found. However, tests conducted over 12 years indicate that there is no general decline in virus-infected planting material. Thus, it can be assumed that a spread of the viruses via corresponding vectors still takes place unhindered. Beyond the examinations regulated within the German Wine Growing Ordinance, one-time tests were carried out on Grapevine Pinot gris virus (GPGV). This analysis showed that GPGV was found in 17.2% of the samples.

Abstract Levels of Assistance (LoA) is an effective caregiving intervention for maintaining activity of daily living (ADL) independence. It is a structured, almost prescriptive, approach to encourage completing ADLs as independently as an elder’s capabilities permit. With appropriate prompts and assistance during dressing, elders can overcome disability, express retained competencies, and experience success. Simultaneously, caregivers learn to view their functions as maintaining the quality of life of able elders, and they receive reinforcement from elders who are more confident and happier. This study is a continuation of a previous project that created and tested a computer application training program for LoA in nursing homes. We refined the app to include grooming LoA and tested on 10 certified nursing assistant (CNA)/resident dyads at a local nursing home. The pilot results showed, although we did not see consistent improvement in CNA’s dressing LoA, we achieved 10% to 30% improvement in grooming LoA. This indicates that the dressing assistance training is able to transfer to grooming LoA. With only an average of one-hour app training, this improvement is cost effective as compared to training provided by care professionals. Future studies should consider incorporating a culture change strategy to improve CNAs’ intention for assisting elders. In addition, the training program should be offered in the initial hire to achieve maximum effect.

Abstract Background Breast reconstruction (BR) is performed to improve outcomes for patients undergoing mastectomy. A recently developed core outcome set for BR includes six patient-reported outcomes that should be measured and reported in all future studies. It is vital that any instrument used to measure these outcomes as part of a core measurement set be robustly developed and validated so data are reliable and accurate. The aim of this systematic review is to evaluate the development and measurement properties of existing BR patient-reported outcome measures (PROMs) to inform instrument selection for future studies. Methods A PRISMA-compliant systematic review of development and validation studies of BR PROMs was conducted to assess their measurement properties. PROMs with adequate content validity were assessed using three steps: (1) the methodological quality of each identified study was assessed using the COSMIN Risk of Bias checklist; (2) criteria were applied for assessing good measurement properties; and (3) evidence was summarized and the quality of evidence assessed using a modified GRADE approach. Results Fourteen articles reported the development and measurement properties of six PROMs. Of these, only three (BREAST-Q, BRECON-31, and EORTC QLQ-BRECON-23) were considered to have adequate content validity and proceeded to full evaluation. This showed that all three PROMs had been robustly developed and validated and demonstrated adequate quality. Conclusions BREAST-Q, BRECON-31, and EORTC QLQ-BRECON-23 have been well-developed and demonstrate adequate measurement properties. Work with key stakeholders is now needed to generate consensus regarding which PROM should be recommended for inclusion in a core measurement set.

The article analyzes the results of monitoring the availability and quality of the drug supply system among the population of various age groups and financial situation. The purpose of this study is to study the satisfaction of citizens in the drug supply system in Russia and to build a medical and social portrait of the patient, which was developed based on the analysis of the results of a sociological survey conducted in dynamics for 2019-2021. In the course of the research, such methods as: sociological, study and generalization of experience, analytical, methods of descriptive statistics were used. The results of the sociological study indicate the need for additional work to improve the availability and quality of LLR and the level of awareness of patients about the system of drug reimbursement. The obtained data can be used for health management bodies at various hierarchical levels, confirm the special social significance of solving this problem and the need to introduce preventive measures to involve patients in healthy lifestyle, increase public confidence in domestic drugs.
