Background. The steady growth of obesity in the children’s age group determines the need for integrated modern approaches to diagnosis and therapy.Objective. To determine the body composition of adolescents with different body mass for the quantitative analysis of the internal environment of the organism using the method bioimpendancemetria.Design and methods. To determine the composition of the body, 121 adolescents aged 10–17 years were examined, the average age was 13.9 years (± 2.1). Evaluation of anthropometric data and determination of BMI at the 1st stage of the study allowed to identify 3 groups of patients: group 1 — adolescents with normal body weight, 40 people (33 %), group 2 — overweight, 48 people (40 %) and group 3 — obese, 33 adolescents (27 %). At stage 2nd, the analysis of the body structure using computer impedance, which allows to determine the composition of the body in a percentage.Results. When assessing the structure of the body, in group with normal body weight, the content of adipose tissue corresponds to the normal value. In adolescents overweight and obesity in 100 % of cases there was an increased content of fat mass. In the group of adolescents with normal body weight, the percentage deviation of AKM is 16–17 %, in the group with excess body weight 12–40 %. Insufficient as well as excessive percentage of AKM causes hunger. The higher AKM in full adolescents, the more difficult the process of weight loss. During the evaluation of the main metabolism it was found that in the group of obese patients the indicators exceeded those in the groups with normal and overweight.Conclusion. The use of bioimpedance analysis allows to create an optimal set of sequential effects aimed at correcting the fat mass, water composition and muscle mass of the patient, which determines a more directed and effective weight loss and the possibility of dynamic control.

Abstract Introduction. It’s known that the issue of polytrauma is one of the most urgent problems of surgery, and among injured patients a special approach is required for patients with overweight and obesity of varying degrees. Purpose of the study. To study prognostic features of traumatic disease course and to improve the results of diagnostics and surgical treatment of patients with polytrauma suffer obesity. Materials and methods. Clinical material was made up of 106 patients with combined body trauma, which were divided into three groups according to body mass index. Results. The results of the research showed a significant difference in the course of traumatic disease in patients with normal body weight and obesity. In particular, in the process of diagnostics of blunt chest and abdominal trauma the frequency of application of interventional methods of diagnostics was directly proportional to the increase of body mass index. The course of traumatic disease in the obese patients had a number of characteristic features that formed the basis for the development of diagnostics and differential program of treatment. Conclusions. The results of the research showed that the course of traumatic disease in combined injury obese patients is directly proportional to the body mass index and has certain features that differentiate them from patients with normal body weight. Purpose : to study the morphofunctional and motor characteristics of the organism of students with overweight and obesity. To compare these data with data of students with normal weight. Participants : it was examined students (females - n=1937; males - n=1302) of 17-21 year-olds. (Baikal region, Russia). It was performed an anthropometrical and physiometric examination of students. It was determined the content of muscle and fat mass in the component structure of a body. It was determined the motor qualities of students. Results : the indicators of anthropometrical and physiometric characteristics, results of motor action tests, the content of muscle and fat mass in a body of students with overweight and obesity are lower in comparison with the indicators of females and males with normal body weight (р<0.05). The integral criterion of physical fitness of students with overweight and obesity is lower in comparison with students with normal body weight (р<0.05). It is defined the direct dependence between the decrease in the level of physical fitness and increase in body weight of males and females. It was obtained data concerning interrelation of anthropometrical and functional characteristics with motor qualities of males and females with different body weight. Conclusions : Results of the research allow correcting physical training classes of youth. Such classes are focused on the differentiated decrease in fat mass content and increase in muscle mass in the component structure of students' bodies.

Endothelial dysfunction is considered one of the main mechanisms for the further development of arterial hypertension (AH) and its complications, and endothelin-1 (ЕТ-1) plays one of the key roles in this process. Endothelium is drawn into the pathological process at the earliest stages of AH development. ЕТ-1 is currently considered as a marker and predictor of the stage and consequences of AH, chronic heart failure (CHF), coronary heart disease (CHD), in particularly, acute myocardial infarction (MI), heart rhythm disorders, pulmonary hypertension, atherosclerotic vascular injury, target vascular disorders. One factor that may influence the level of ЕТ-1 is the endothelin-1 gene Lys198Asn polymorphism, which is considered by many researchers as a possible genetic marker of AH. However, the effect of this polymorphism on the level of plasma ЕТ-1 level in patients with AH of varying severity in Ukraine is understudied. The basis of this work were the materials of a complete examination of 160 patients with a verified diagnosis of AH I, stage II of 1, 2, 3 degrees (main group) and 110 apparently healthy persons (control group). Patients of the main group were divided into three subgroups: 1st group ­– 75 patients with normal body weight; 2nd group – 48 overweight patients; 3rd group – 37 patients with alimentary obesity. In obese patients, the waist circumference in women was around 103 (96–115) cm, and in men – 108 (105–116) cm, which indicates an abdominal type of obesity in these patients. As a result of the study, it was found that the level of ЕТ-1 is higher in patients with AH who suffer with overweight and obesity as compared with patients with AH and normal body weight. Also, the peptide level depends on the genotype of patients and is higher in AH patients with the genotype Asn198Asn and Lys198Asn as compared to carriers of the genotype Lys198Lys.

The number of obese pregnant women increases annually and reaches 20-30%. The metabolism of hormones and minerals changes in the presence of a large amount of adipose tissue in the body of a pregnant woman, which leads to a number of obstetric and perinatal problems. The aim of the work is to study and compare the influence of the gestational process on the indicators of iron and copper metabolism in the blood serum of women with normal body weight and women with obesity. In the blood serum of 125 women of reproductive age, the content of hemoglobin, iron, transferrin, ferritin, copper and ceruloplasmin was determined. The influence of pregnancy on the indicators of iron and copper metabolism in the blood serum of women was revealed. Pregnancy in women with normal body weight increases the content of transferrin and ceruloplasmin. Correlation of ceruloplasmin and ferritin content with body mass index of obese pregnant women was revealed. In pregnancy with concomitant obesity, hyperferritinemia is formed with a reduced content of hemoglobin and serum iron. Knowledge of the indicators of iron and copper metabolism is necessary to optimize the observation of pregnant women, effective prevention and prediction of obstetric and perinatal complications.

Abstract The purpose of the study is to investigate the course of acute pancreatitis in obese patients, the development of local and systemic complications and mortality rates. Materials and methods We have taken and analyzed 482 histories of acute pancreatitis, who has been treatmented at Kyiv Regional Clinical Hospital from January 1, 2011 to February 2, 2019. The data were statistically processed in the Exel 2010 program using a descriptive method using relative , absolute numbers, mean square deviations and their errors. A correlation relationship between variables was studied using the Pearson criterion (R2). Testing the significance of the difference between the two independent groups was carried out with the help of the t-criterion of the Student. Results In our study we had 482 patients, 260 patients (54%) had obesity (the study group), for comparison, a control group of patients with normal body weight was chosen, the total number of which was 222 (46%) patients. Obesity patients had a higher average age (55.4 ± 9.4 years, p = 0.01), also they had statistically greater percentage of severe cases of acute pancreatitis (85 (32.7%) vs 16 (7.2%); p = 0.01 *). We note the increase in the percentage of the course of severe pancreatitis in patients with weight gain from 10.20% to 53.93% (p = 0.03) *. Obesity patients had the longer bed-days in the hospital than in patients with normal body weight. In addition, they observed an increase of twice the bed in the reanimation and intensive care unit (5.8 ± 0.8 vs 2.7 ± 0.5 days, p = 0.01 *). When investigating the mortality rate, it should be noted that the main cause of death was the progression of organ failure of 30 (6.3%), pulmonary embolism (TB) - 15 (3.1%) and DIC 18 (3.7%) . Conclusions. The presence of obesity in patients involves a high risk of severe acute pancreatitis, this risk increases with an increase in the body mass index. In addition, in obese patients we have a higher bed-day in-patient and in-patient-care settings, which increases the total cost of treatment and requires a cost-effective algorithm in the future. A high mortality rate in obese patients requires improved treatment algorithm. Key words: acute pancreatitis, obesity.

The aim of the study was to investigate age-related changes in bone mineralization indicators in the lumbar vertebrae in women with normal body weight living in the conditions of the Northern region.Material and methods. A complex somatometric examination and quantitative assessment of the bone tissue mineral density in the lumbar vertebrae were performed according to dual-energy X-ray absorptiometry of a group of Slavic women (n=127) with a normal body weight (BMI values from 18.5 to 24.9 kg/m2) aged 20 to 87 years, permanently residing in the Republic of Karelia. Statistical processing of the material was performed using the program "STATISTICA 6.0".Results. The BMI values in women increased significantly every decade of life. Direct correlations of mineral bone density (MBD) with the body length (r=0.46, p<0.001), with the body surface area (r=0.46, p<0.001), with absolute muscle mass (MM) (r=0.39, p<0.001), and with body mass (r=0.29, p<0.001) were revealed. No significant correlation with the adipose mass was found. The incidence of low MBD of the lumbar vertebrae was 48%: osteopenia was in 29%, osteoporosis was in 19% of women. The analysis of the component composition of the body in women with different levels of bone mass showed significant differences in the absolute content of muscle mass.Conclusion. In women with a normal BMI, body weight and muscle mass play an essential role in maintaining lumbar vertebrae bone mineral density.

BACKGROUND Digital health promotion programs tailored to the individual are a potential cost-effective and scalable solution to enable self-management and provide support to people with excess body weight. However, solutions that are personalised, theory- and evidence- based and widely accessible are still limited. OBJECTIVE This study aimed to develop a digital behaviour change program, Choosing Health, that could identify modifiable predictors of weight loss and maintenance for each individual and utilise these to provide tailored support. METHODS We applied an Intervention Mapping protocol to design the program. This systematic approach to develop theory- and evidence-based health promotion programs consisted of 6 steps: development of (1) a logic model of the problem, (2) model of change, (3) intervention design and (4) production, (5) the implementation plan, and (6) evaluation plan. The decisions made during the Intervention Mapping process were guided by theory, existing evidence, and our own research (including four focus groups, N=40, expert consultations, N=12 and interviews, N=11). The stakeholders included researchers, public representatives (including individuals with overweight and obesity), and experts from the variety of relevant backgrounds (including nutrition, physical activity, and healthcare sector). RESULTS Following a structured process, we developed a tailored intervention that has potential to reduce excess body weight and support behaviour changes in people with overweight and obesity. The Choosing Health intervention consists of tailored personalised text messages and email support that correspond with theoretical domains potentially predictive of weight outcomes for each participant. Intervention content includes behaviour change techniques to support motivation maintenance, self-regulation, habit formation, environmental restructuring, social support and addressing physical and psychological resources. CONCLUSIONS Use of an Intervention Mapping protocol enabled the systematic development of the Choosing Health intervention and guided the implementation and evaluation of the program. Through the involvement of different stakeholders, including representatives of general public, we were able to map out program facilitators and barriers while increasing ecological validity of the program, to ensure that we build an intervention that is useful, user friendly, and informative. We also summarised lessons learnt for the Choosing Health intervention development and for other health promotion programs. CLINICALTRIAL This is an Intervention Mapping study which is currently evaluated through a Randomised Controlled Trial. BACKGROUND: Currently, obesity is one of the most global problem in the world. Redundant accumulation of adipose tissue accompanied by metabolic disorders including high secretion of leptin may lead to an increased risk of gestational complications. AIMS: To assess the level of leptin in pregnant womens blood serum depending on their body weight and the character of content of adipose tissue in different terms of gestation. MATERIALS AND METHODS: We investigated 211 women with single pregnancies, which came in natural cycles and finished with urgent parturitions. All of them got anthropometric study, assessment of adipose tissues weight by bioimpedanceometry, character of fat deposition by ultrasound investigation of adipose tissue, determination of the level of whey leptin at 1014, 2428, 3638 weeks of pregnancy. RESULTS: Established, that the level of leptin increases in proportion to pregestational body mass index from the first trimester. Women with excess body mass and with obesity had higher concentrations of leptin all over gestation, than women with normal body mass (р0.001). The higher term of pregnancy was, the higher the level of whey leptin was in all groups, independently of character of fat deposition. In the end of third trimester, women with normal body mass had the largest value of absolute and relative increase of body mass and fatty mass, what accompanied by enlargement of the concentration of leptin more than in 3 times for the pregestational level. CONCLUSIONS: In this way, high content of leptin in pregnant womens (with excess body mass and obesity) blood serum in the first trimester is explained by redundant accumulation of the adipose tissue. The largest value of gestational increase of body mass and fat mass in group where women had normal initial weight is accompanied by the great increase of the level of leptin on the body weight unit and fat mass. These data indicate, that control and limitation of excessive weight gain while pregnancy can be a measure of prevention of the redundant leptins secretion and different gestational complications related with it.

Relevance. Recent changes in lifestyle and diet have led to a significant increase of overweight and obesity among children and adolescents. According to the results of our previous studies, it was found that among adolescents with overweight/obesity, essential hypertension is 1,5-7,5 times more common than among peers. It is known that children and adolescents with obesity are inherent in emotional instability and impulsivity. Objective of this work was to study the personality characteristics of adolescents with essential hypertension and overweight/obesity. Materials and methods. In the Kiev Children's Clinical Hospital No. 6, a comprehensive clinical and psychological study of 67 adolescents (18 girls, 49 boys) aged 12-17 years (average of 14,40±1,63 years) was carried out, who were hospitalized for essential hypertension. Overweight was found in 7 girls and 21 boys, obesity - in 2 girls and 8 boys. The remaining patients (9 girls and 20 boys) had normal body weight (comparative group). We evaluated the level of anxiety, alexithymia, depression, the type of attitude to the disease, and also determined the personality characteristics of the patients. Results. Adolescents with essential hypertension and overweight/obesity on average differed significantly higher pulse daily blood pressure (61,98±8,13 mmHg versus 57,02±6,70; P<0,005), lower values of anxiety scales (5,11±4,77 points versus 9,62±7,72 points; P<0,01), hypochondria (5,70±5,20 points versus 9,00±5,20 points; P<0,05), neurasthenia (7,19±7,49 points versus 11,14±7,90 points; P<0,05) and egocentrism (5,70±4,44 points versus 8,57±5,08 points; P<0,05) related to TOBOL test. According to the personality test R.-B. Kettell in adolescents with overweight/obesity was on average higher level Q2 scale (6,72±2,14 points versus 4,91±2,63 points; P<0,05). Conclusion. Adolescents with essential hypertension and overweight, obesity, in contrast to patients with normal body weight, are less neurotic by the disease, but they are characterized by non-conformity.

Abstract The results of a number of studies have shown that in arterial hypertension (AH), G/T and T/T genotypes of the adiponectin gene (ADIPOQ) and Gly/Arg and Arg/Arg genotypes of the insulin receptor substrate 1 gene (IRS-1) are associated with a greater severity of metabolic disorders and hemodynamic parameters compared with G/G and Gly/Gly genotypes of these genes. The aim of the study: to evaluate the severity of cardiovascular remodeling and metabolic disorders in hypertensive obese patients in the simultaneous presence of two unfavorable genotypes of the ADIPOQ and IRS-1 genes. Methods: We examined 300 AH patients: 200 patients with AH and obesity, 50 patients with AH and normal body weight, 50 patients with AH and overweight, 40 patients with AH, obesity and type 2 diabetes mellitus (DM2), 30 healthy individuals. The polymorphisms of the ADIPOQ and IRS-1 was assessed by molecular genetic method. Results: It was found that in all groups of hypertensive patients, regardless of body weight and the presence of DM2, the simultaneous presence of two unfavorable genotypes of the ADIPOQ and IRS-1 genes occurred significantly more often than in healthy individuals: in 41% of AH patients with obesity, 30% of AH patients with normal weight, 40% of AH with overweight, 57.5% of AH with obesity and DM2 vs. 13.3% of healthy individuals. In hypertensive patients, in the presence of overweight and obesity, the frequency of combination of the two unfavorable genotypes of these genes was significantly higher than in AH patients with normal body weight. Conducting comparative evaluation of AH patients with obesity depending on the presence of two unfavorable genotypes or two protective genotypes of the ADIPOQ and IRS-1 genes showed that carriers of the combination of the G/T + T/T genotype of the ADIPOQ and the Gly/Arg + Arg/Arg genotype of the IRS-1 had a higher body mass index, more pronounced insulin resistance, cardiovascular remodeling, adipokine imbalance, impaired carbohydrate and lipid metabolism. Conclusions: In AH patients, the frequency of the simultaneous presence of two unfavorable polymorphisms of ADIPOQ and IRS-1 genes was higher than in healthy individuals. In AH patients with overweight and obesity, the frequency of combination of the two unfavorable genotypes of the ADIPOQ and IRS-1 genes was significantly higher than in normal body weight. The presence of a combination of two unfavorable genotypes of the ADIPOQ and IRS-1 genes in patients with AH and obesity was associated with a greater severity of cardiovascular remodeling and metabolic disorders compared with the combination of two protective genotypes of these genes.
