Modern medical and demographic situation in the Tambov Region

Abstract. We understand the medical and demographic situation as a set of demographic phenomena and processes that are factors that negatively affect the state of health of the population in this territory, and actualize the need for making scientifically sound management decisions in the regional health system. The purpose of the study: on the basis of official statistics, to research the features of the demographic situation in the Tambov Region and to identify fundamental demographically determined patterns of formation, the state of health of the region's population. Materials and methods. As an information base, we used the following: official statistics on the demography of Tambovstat for 2000–2020; data from statistical collections of the Ministry of Health of the Russian Federation for 2007–2019. Results. The demographic situation in the region has been in deep crisis over the past decades and is characterized by a steady downward trend in the total population, averaging 10276 per year; lack of female population of fertile age, which is 41,4 % of all women instead of the necessary 50 %; adverse changes in the structure of women of fertile age, which consisted in a decrease in the proportion of intervals of 20–24 years and 25–29 years, which are the most reproductive; the low level of the total fertility rate, according to which the Tambov Region occupies 84th place among 85 subjects of the Russian Federation; completion of the formation of a low-density family model; 90 % of families in the Tambov Region have 1–2 children and only 10 % – more than two; sustainable narrowing of the population; in 2018, the average number of children a woman gives birth to during the re-productive period was 1.33 (TFR); negative natural growth and negative migration balances, which represent steady trends since 1990. In the Tambov Region, it is necessary to conduct fundamental demographic studies that will make it possible to develop scientifically justified models of the reproduction of the population of a typical agro-industrial region of the Central Federal District of the Russian Federation, calculate the forecast and offer management structures recommendations for the formation of a rational and productive regional demographic policy.

The state of maternal and child health plays a key role in promoting and protecting public health. The article analyzes and summarizes the results of the analysis of legislative support for maternal and child health. The analysis of individual indicators that determine the current state of medical organizations ‘activities is presented. The purpose of the study is to determine the current directions for improving the system of maternal and child health in Russia. Materials and methods. The following methods were used: information and analytical, content analysis, comparative analysis, and statistical analysis. Sources of primary information — scientific publications, Rosstat data, regulatory legal materials. Results and discussion. Proposals for the use of the current legislation of the Russian Federation in protecting the health of mothers and children are identified. There is a need to improve the use of the current regulatory and legislative framework to improve individual indicators of the state of health of mothers and children. The analysis showed that positive results were achieved in 2012–2019 in the Russian Federation in terms of improving maternal and child health. Conclusion. The implementation of regulatory legal provisions in this area should be based on a comprehensive coordination of all activities between various departments and structures, regional characteristics of the state of health of mothers and children and such areas as prevention of neglect, medical and social assistance to orphans and children left without parental care, maintaining a healthy lifestyle and providing the children’s population with sanatorium-resort medical organizations should be taken into account.

The subject of the study is the inter-regional differences in the state of public health and the demographic situation in the Russian Federation regions. The theoretical aspect of the subject is determined by the development of priorities of the Russian healthcare development strategy aimed at alignment of regional differences in the levels of healthcare development in the Russian Federation regions. The empirical aspect of the subject is to identify interregional differences in the state of public health and the demographic situation in the Russian Federation regions by using the EM cluster analysis method (Expectation Maximization). The method was implemented in the integrated development environment RStudio. The official statistics from Rosstat for the period 2014–2018 were used as the initial dataset. The purpose of the study is justifying the regulation of inter-regional differences of the Russian Federation regions. As a result of clustering, nine homogeneous clusters of the Russian Federation regions were identified. The main characteristics of the formed clusters are determined. Among the priorities of the RF healthcare development strategy are as follows: implementation of a unified tariff policy in the system of compulsory medical insurance; ensuring the balance of territorial compulsory medical insurance programs within the framework of the basic programme of compulsory medical insurance through financial security based on a single per capita standard; development of telemedicine, providing prompt remote consultation of leading experts in the provision of medical care, regardless of the territorial location of the patient and the doctor; ensuring the implementation of distance education courses and continuing education programs for medical workers; rationalization of the distribution of resources and capacities of medical organizations based on a three-tier system of medical care; development of regional public health centres. The results of this study can be used to develop federal and territorial programs for socioeconomic development, formulate a strategy for the development of healthcare at macro- and meso- levels, and optimize decisions of regional authorities regarding population policy.

The state takes different steps towards solution of demographic problems, one of which is the implementation of State Program for Assisting Compatriots Residing Abroad in Their Voluntary Resettlement in the Russian Federation approved by the President of the Russian Federation on June 22, 2006. The goal of this program is encourage and arrange the process of voluntary resettlement of compatriots in the Russian Federation, promote socioeconomic development of the regions, and solve demographic problems, namely in the territories of priority settlement. Within the framework of this program in 2012-2013, the regions of the Far Eastern Federal District were included into the list of territories of priority settlement. However, there is yet no unified Far Eastern program of resettlement; the regional programs that have been approved by the government of the Russian Federation are being implemented. The subject of this research is the implementation of the program of resettlement in Zabaykalsky Krai. The relevance of this topic is substantiated by the strategic geopolitical location of the region. The author examines the normative legal framework and measures taken by the government aimed at implementation of State Program for Assisting Compatriots Residing Abroad in Their Voluntary Resettlement in the Russian Federation. The two vectors of the program are considered: assistance to the voluntary resettlement of compatriots, and cooperation with compatriots who do not intend to relocate to Russia. The main trends of resettlement are revealed. The need to intensity information support for this project is underlined. The authors believe that more active involvement of civil society in cooperation with the compatriots living abroad may become an additional motivation for improving effectiveness of the state program.

The article considers the main indicators of demographic development of the Russian Federation in recent years in connection with the level of development of social infrastructure. It is shown that the demographic situation in the country is deteriorating, that is due to both subjective and objective reasons. Among the objective reasons, the main one is the change in the age structure of the population, which is expressed in a sharp decrease in the number of women in fertile age. This is due to the acute demographic crisis of the 1990s, «demographic waves» generated by the Great Patriotic War, and other factors. Among the subjective causes of the decline in birth rate, the article highlights the general patterns of reduction in the number of children in families as the level of socio-economic development increases, which are characteristic of all countries of European culture. In particular, the reduction in the number of children is facilitated by such factors as reduction in the share of officially registered families, increase in child support costs, and the financial difficultiesfaced by families with children during economic crises and stagnation. All these cases are typical for regions of the Russian Federation in recent years. Development of social infrastructure — education, healthcare, culture, sports, and other similar systems — could improve the demographic situation in Russia. Unfortunately, in recent years, such systems have been degrading rather than developing in Russia. This is facilitated by the system of statistical reporting that does not reflect the actual situation. The article proposes approaches that could overcome this shortcoming of the present statistical reporting system, thereby contributing to the demographic development of the modern Russian Federation.

Significance. The demographic crisis in Russia, registered since 2016, has deteriorated again and the forecast remains unfavorable, substantiating the need for reserves to protect the reproductive potential. Assessment of medical and demographic processes and the possibility of managing them is an important scientific and practical task, the solution of which is relevant for the Udmurt Republic against the background of the existing unfavorable trends in the socio-economic situation. Purpose of the study. To evaluate medical and demographic situation in the Udmurt Republic. Material and method. Medical and demographic indicators for 2012-2019 were studied on the basis of the Rosstat official statistics and materials of the Territorial Body of the State Statistics Service of the Udmurt Republic (according to the information support schemes approved by the Ministry of Health of the Udmurt Republic). Data of the federal statistical observation forms No. 13 for the period 2012 - 2019 were used as the research material. To compare the analyzed indicators in the Udmurt Republic, the Volga Federal District and the Russian Federation as a whole, the authors used materials available from the website of the Federal Research Institute for Health Organization and Informatics of the Ministry of Health of the Russian Federation (https://mednet.ru). To assess dynamics in the analyzed indicators, the growth rate index (%) was calculated. Results. The study has analyzed and evaluated major health and demographic indicators of the Udmurt Republic and compared them with similar indicators of the Russian Federation and the Volga Federal District. Results of the study include as follows: a steady decline in the Republic population; the reproduction indicators in the Republic are below the replacement level; low level of health across all age groups; the gynecological morbidity among females is 1.5-2-fold higher than in Russia and the Volga Federal District; high morbidity among females during pregnancy, childbirth and the postpartum period.

The article examines the current demographic situation in Russia and assesses its impact on the long-term formation of the labor market. The relevance of the chosen topic is explained by the fact that in recent decades the state of the demographic situation in Russia is unsatisfactory. There are a number of features that slow down the development of the state. Firstly, it is a decrease in the population, secondly, the aging of the economically active population, and thirdly, the high mortality rate of men in working age.

With the development of technologies for collecting, processing and storing data, many countries started to upgrade the systems of official statistics, complementing it with big and administrative data. One of the most detailed and operative sources of data on the economy is banking statistics. The article presents the results of a comparative analysis of the data of the SME Growth Index, formed on the basis of the data of Sberbank, and the data of the Federal Tax Service of the Russian Federation. The analysis showed that the data of banking statistics correspond to the data of the Federal Tax Service of Russia both in the distribution of observations in various sections (regions of the Russian Federation, size of the business), and make it possible to more quickly obtain data on the state of the sector, and can be used both for analyzing the SME sector and forecast parameters of its development. Based on the analysis of banking statistics for the 3rd quarter of 2020, the effect of “the sector whitewashing” and an increase in deductions on insurance premiums and personal income tax due to a decrease in the rate on insurance charges for SMEs from 30% to 15% was revealed. Further analysis based on data from the Federal Tax Service of Russia also confirmed this conclusion.

Introduction. This study is relevant in connection with the search for new points of development of the Sakhalin Region in new sectors of the economy that are not related to the mineral wealth exports model of the economy. The purpose of the paper is to assess the state of the industrial sector in the Sakhalin Region and consider the directions of its development. Materials and Methods. Federal Acts and other regulations of the Russian Federation, data from Rosstat; papers in periodicals on economics, management, and regional studies were used. Results. According to a number of indicators characterizing the state of the economy, the Sakhalin Region occupies a leading position not only in the Far Eastern Federal District but also in the entire country. In 2019, the index of industrial production in the Sakhalin Region decreased by 4%, in particular, the index of production in mining in 2019 decreased by almost 5%. At the same time, about 90% of this type of activity falls on the oil and gas sector. The growth rates of mineral extraction are declining against the background of the growth of other industries: manufacturing (112.7%), electric power, etc. (104.5%), water supply (165%). Discussion and Conclusion. A high level of depreciation of fixed assets of enterprises requires the activation of investment processes for their renewal and modernization. High investment activity is required, which will have a positive impact on the efficiency of production processes. It is necessary to increase the level of graduates training at the institutions of higher, secondary, and primary vocational education due to their insufficient practical training using modern equipment, and bad command of advanced technologies.
