Clustered regularly interspaced short palindromic repeats (CRISPR)-Cas system has recently gained growing attention as a diagnostic tool due to its capability of specific gene targeting. It consists of Cas enzymes and a guide RNA (gRNA) that can cleave the target DNA or RNA based on the sequence of the gRNA, making it an attractive genetic engineering technique. In addition to the target-specific binding and cleavage, the trans-cleavage activity was reported for some Cas proteins, including Cas12a and Cas13a, which is to cleave the surrounding single-stranded DNA or RNA upon the target binding of Cas-gRNA complex. All these activities of the CRISPR-Cas system are based on its target-specific binding, making it applied to develop diagnostic methods by detecting the disease-related gene as well as microRNAs and the genetic variations such as single nucleotide polymorphism and DNA methylation. Moreover, it can be applied to detect the non-nucleic acids target such as proteins. In this review, we cover the various CRISPR-based diagnostic methods by focusing on the activity of the CRISPR-Cas system and the form of the target. The CRISPR-based diagnostic methods without target amplification are also introduced briefly.

<p class="1">Meeting teacher expectations for a professional development programme (PDP) is expected to strengthen sustainable applied competence as programme outcome since teachers will be more motivated to apply the programme content in practice. A revised distance learning (DL) programme was augmented by a practical component comprising a work-integrated portfolio and audio-visual material, aimed to support the applied competence of practising teachers in the South African context. An evaluation of the way the programme measured up to teacher expectations was deemed critical for future DL programme design. A qualitative study based on an interpretivist philosophical approach collected data of teacher expectations <em>for </em>and <em>of</em> the practical component through multiple methods. Their contributions were linked with four main themes related to applied competence as identified in the literature. Participant expectations and experiences with regards to each theme were compared by means of electronic coding through ATLASti™. The findings show a strong correlation between expectations <em>for</em> and experiences <em>of </em>the way the practical component supports the elements of applied competence. Since DL is viewed as a viable and cost effective way to improve teacher competence in developing countries, these findings serve as impetus for further investigation and refining ways to support applied competence in a distance learning professional development programme (DL PDP).</p>

Renal cell carcinoma is frequently a matter of urological interest. In recent years there were significant improvements regarding the earlier diagnosis more precise preoperative staging and appropriate therapy. One hundred patients (42-78 years old) with the preoperative diagnosis of renal cell carcinoma were analyzed. Preoperative radiological evaluation included transabdominal ultrasound, intravenous urography, computer-assisted tomography, and angiography. In all patients after radical nephrectomy pathohistological diagnosis was established and patients with the confirmed renal cell carcinoma tumor staging was performed. All histological findings were compared with the preoperative results of radiological examinations. Reliability of all of them is separately determined. Our results confirmed that the most efficient method of preoperative staging was computer-assisted tomography (accuracy 93%). Diagnostic methods that were previously used like intravenous urography and angiography, were not useful for routine diagnostic purposes. Ultrasound is a precise but not an enough informative diagnostic tool (accuracy 87%). Combine used of both ultrasound and contrast computer-assisted tomography is cost-effective, and an enough precise combination for everyday use.

Nadi Pariksha is the most effective diagnostic tool known in the medical field. It is cost effective,  accurate,  safe,  and gives quick results. We can conduct Nadi Pariksha on healthy individuals as well as all patients irrespective of stage of the disease also,  and even pregnant woman,  children,  elderly can undergo without any harm or side effects. However,  this technique is not being widely practised at present,  because of lack of training,  practise and knowledge about it in the present day among Ayurveda vaidyas. An iconic factor for identification of a physician,  irrespective of the time,  Region,  Nadi Pariksha can be highlighted as a common factor or even System of Medicine or Civilization of the known world. Thus,  we can perceive that Nadi Pariksha or the pulse examination remains as an effective diagnostic tool since ages. Nadi Pariksha was not been discussed among the Brihatrayees of Ayurveda. Acharya Sharangdhara was the first to document in the doctrines of Ayurveda. Thus Acharya Sharangdhara is considered as ‘The Founder of Nadi Pariksha’in Ayurveda. Nadi Pariksha was titled under the Pancha-Nidana by Acharya Sharangdhara and Ashta Sthana Pariksha by Acharya Yogaratnakara. It was the Foremost among all the other diagnostic tools mentioned by him. Later Acharyas like Acharya Bhava Mishra,  Acharya Yogaratnakara,  Acharya Basavaraja,  Acharya Kanada Maharishi,  and Acharya Ravana have contributed in giving more descriptions and importance. In the recent days Dr. Vasant lad and Dr. Sarvadeva Upadhaya’s research work interest and scope of Nadi Pariksha.

Pulmonary hypertension is a hemodynamic disorder defined by abnormally high pulmonary artery pressure that affects the arteries in your lungs and the right side of your heart. In this study, hepatic venous duplex will be done to diagnose and quantify the PH. So that Patients can avoid unnecessary invasive right heart catheterization. This practical demonstration is the key to enrich our experience and knowledge in the field of PH. Objectives of this study was to assess PH status by Hepatic venous Duplex (HVD) as well by right heart catheterization and to compare them. This study was conducted in the Department of Cardiology, BSMMU, Shahbagh, Dhaka extending from July 2018 to December 2019. Total 100 (One hundred) subjects were enrolled in this study. It was an Observational study and includes the subjects between 18 years to 45 years of age. Results of this study shows very close proximity to that of Right heart catheterization. Hemodynamic changes in Hepatic venous duplex study could be used as an alternative diagnostic tool for evaluating moderate to severe pulmonary hypertension. This method could counteract the weakness of the currently used diagnostic methods and improve the accuracy of assessing pulmonary hypertension when combined with other methods. Ocular diseases are sub-diagnosed in bird species, partly because of limited diagnostic methods and lack of reference data. Placement of Schirmer tear test strips in the bottom of the conjunctival sac of birds for tear flow measurement can prove to be difficult, whereas paper tips commonly used for dental procedures may be easier to use, due to a more adequate size and shape. Hence, the aim of this study was to assess and compare tear flow values, using ABC Dental® # 30 color paper tips and TearFloTM Test strips in Red-lored Parrots (Amazona autumnalis) (n = 26). Captive birds of undetermined sex were housed in groups under similar environmental conditions and fed the same diet. All animals were deemed healthy through remote and physical examinations. Average tear flow values of 0.6365 ± 0.032 cm/min and 0.5942 ± 0.032 cm/min were obtained with the Schirmer strips test and the endodontic tips respectively. Lack of difference in observed values between measurement techniques (p = 0.3629) indicates that endodontic tips may be a good alternative for tear flow assessment in Red-lored parrots. In addition, endodontic tips are more cost effective and may be less invasive. This study stresses the need to establish species specific ophthalmic reference values for clinical practice in birds, since differences have been reported even in specimens within the same genus.Figure 1. AbstractTraditionally, the first step in the development of drugs is the definition of the target, by choice of a biological structure involved in a disease or by recognition of a molecule with some degree of a biological activity that presents itself as druggable and endowed with therapeutic potential. The complexity of the pathophysiological mechanisms of disease and of the structures of the molecules involved creates several challenges in this drug discovery process. These difficulties also come from independent operation of the different parts involved in drug development, with little interaction between clinical practitioners, academic institutions and large pharmaceutical companies. Research in this area is purpose specific, performed by specialized researchers in each field, without major inputs from clinical practitioners on the relevance of such strategy for future therapies. Translational research can shift the way these relationships operate towards a process in which new therapies can be generated by linking experimental discoveries directly to unmet clinical needs. Computational chemistry methods provide valuable insights on experimental findings and pharmacological and pathophysiological mechanisms, allow the virtual construction of new possibilities for the synthesis of new molecular entities, and pave the way for informed cost-effective decisions on expensive research projects. This text focus on the current computational methods used in drug design, how they can be used in a translational research model that starts from clinical practice and research-based theorization by medical practitioners and moves to applied research in a computational chemistry setting, aiming the development of new drugs for clinical use.
