Pt X is a company that produces steel pipe of various shapes and size. In the production procces not only using machines but also using humans as operators. So in this case humans play an important role in maintaining the quality of production. The purpose of this study is to analyze the human error probability with  Human Error Assessment and Reduction Technique method (HEART). HEART is a method designed as a fast and simple human reliability assessment in quantifying the risk of human error. From this research, it was found that 3 tasks had a high HEP value which caused the decline in pipe quality during the production process, namely task 3.2, 4.1 and task 3.4. Which has the highest HEP value on task 3.2, namely setting the machine with a value of 0,7680. The cause is due to the lack of operator expertice and the center roll was not carried out when installing it, so that training is needed to increase operator expertice.

Traffic accidents, especially with a large capacity such as bus, can be caused by several factors. According to the Indonesian Directorate General of Land Transportation of the Ministry of Transportation in 2012, the factors causing traffic accidents in Indonesia are a human factor of 93.52%, vehicle factor by 2.76%, road factor 3.23%, and environmental factor by 0.49%. Therefore, research is needed to identify which human error has the greatest probability of accident cause using Systematic Human Error Reduction and Prediction Approach (SHERPA) method to identify job desk using Hierarchical Task Analysis (HTA) and Human Error Assessment Reduction Technique (HEART) method to calculate Human Error Probability (HEP). Based on the calculation of Human Error Probability value known the highest HEP value is not running the vehicle in accordance with the provisions of the speed that has been set with 0.375. Next is not to record or forget to record the damage that occurred during the trip with a value of 0.21. It did not check Bus equipment with a HEP value of 0.19, did not report when there was a problem on the street with a HEP value of 0.18 and did not break for the next preparation for departure with a HEP value of 0.15

Kesuksesan usaha mikro kecil menengah dipengaruhi oleh kegiatan-kegiatan supply chain sehingga harus terjalin hubungan baik di dalamnya. Peran manusia sebagai tenaga kerja pada usaha mikro kecil menengah dominan mempengaruhi kualitas produk yang dihasilkan, karena operator memiliki peluang melakukan kesalahan. Penelitian ini bertujuan menganalisis human error pada rantai pasok UMKM makanan di kota Medan dengan adopsi model SCOR (Supply Chain Operations Reference). Metode HEART (Human Error Assessment and Reduction Technique) digunakan untuk mengetahui human error dan tingkat probabilitas human error pada UMKM makanan. Berdasarkan hasil perhitungan nilai assessed effect (AE) diperoleh total AE terbesar pada proses make dengan nilai sebesar 58,69. Human Error Probability (HEP) terbesar terdapat pada proses pengisian adonan dengan varian rasa yang diinginkan oleh konsumen, dengan nilai Human Error Probability (HEP) sebesar 5,282. Dari hasil penelitian dapat menjadi acuan menyusun rencana perbaikan agar human error dapat tereduksi.   The success of micro, small and medium enterprises is influenced by supply chain activities, so there must be a good relationship within it. The role of humans as workers in micro, small and medium businesses predominantly affects the quality of the products produced, because operators have the opportunity to make mistakes. This study aims to analyze human error in the food MSME supply chain in Medan by adopting the SCOR (Supply Chain Operations Reference) model. The HEART (Human Error Assessment and Reduction Technique) method is used to determine human error and the level of human error probability at food MSMEs. Based on the calculation of the assessed effect (AE) value, the largest total AE was obtained in the make process with a value of 58.69. The biggest Human Error Probability (HEP) is in the process of filling the dough with the flavor variants desired by consumers, with a value of Human Error Probability (HEP) of 5.282. From the results of the study can be a reference to draw up plans for improvement so that human error can be reduced.

Human realibility is a big tricky problem. Human failure rates depend on three main factors, namely<br />intrinsic, work environment, and stress. PT. X is a supplier for PT. LG Indonesia manufacturing products<br />made from plastics. Human Realibility Assesment is conducted using HEART methods (Human Error<br />Assessment and Reduction Technique). In the first phase, task analysis on operators’ activities is done using<br />Hierarchical Task Analysis (HTA). The largest Human Error Probability was found when Operator No. 1<br />did not insert the flash side carefully. The value of this probability is 0.53424. This factor is concluded as the<br />main cause to shroud defectives, which results in customer penalyzing the company.

Occupational health and safety is very important point for a company. CV. XYZ is a company that produces paving and have problem about the occupational accident. There are 4 production procceses that can be couse occupational accidents, which is taking raw materials procces, stirring batter, forming and drying. This case must be analyzed using HEART and JSA method as evaluating effort for occupational accident. The results of HEART method analysing imply that HEP value 0,8632is the most dangerous occupational, that is forpicking gravel from cement mixer machine. After known the value of HEP afterward analyzed using JSA method. The goal is as improvement to occupational accident in the production process in order can be reduced. The result of JSA method analysing are improvement suggestion such us levying mask, hat, design of additional components for machine, additional equipment for pallet and hand pallet, and assigning standart operational procedure.

Job promotion makes motivated employees work well and enthusiasm in self-development to improve the quality of performance. Performance evaluation as material for employee evaluation for promotion in Senayan Apartment Jakarta by utilizing the Technique for Order Preference method by Similarity of Ideal Solution (Topsis). This method can show evaluation ranking from highest to lowest by ranking alternatives in the form of employee data and dynamic criteria. The purpose of this study is the selection of quality employee performance for promotion based on the specified criteria. Based on the data sample used as many as 14 employees, the results of the assessment are based on rank, ie employees on behalf of Ridan get the first rank with a value of 0.50.

Occupational Health and Safety (OHS) is important to maintain and improve so that the quality of human resources in the company is always in prime condition. Every company has a different level or level of Occupational Health and Safety (OHS). PT. XYZ is a company that uses corrosive materials, therefore Occupational Health and Safety (OHS) analyst at PT XYZ is needed. The Occupational Health and Safety (OHS) analysis carried out in this study used the FMEA method to determine which part of the production process had Occupational Health and Safety (OHS) that needed repair the most. From the research results, it was found that the highest Risk Priority Number (RPN) value was the pickling and degreasing production process.

XYZ is a company engaged in fabrication. Quality is very important for the company because it can affect customer satisfaction. In the production process there are still defect problems found on the top side of the structural construction and it affects the quality of the part. The failure rate on the topside product has a 5:28% percentage exceeds the standards set by the company which is 2%. Based on the percentage of defects that have been obtained, the problem can be solved by fishbone analysis and FMEA. In the first stage using CTQ which aims to identify failures that occur and determine quality characteristics. The next step after identifying the failure that occurred can be calculated DPMO and Sigma Level. After getting the DPMO value and the Sigma level, identification of the root causes of the failure occurred. Small and Medium Industries (IKM) H.Musauwimin is a manufacturing industry in the field of tofu production. In the production process there are still frequent product defects, including coarse tofu, tofu surface torn, soft texture, and faded color. For this reason, we need a way to minimize the possibility of product defects. The objectives of this study are (1) Identifying disability factors using the Statistical Quality Control (SQC) method, (2) Improving employee performance performance using the Human Reliability Assessment (HRA) method, (3) Minimizing product defects in the production process so as to increase productivity. The results of this study are (1) of the four types of rejects for the biggest 3 months in tearing surface type with a percentage of 46% of all rejects (2) By testing the adequacy of the data using the SQC method in the production process for 3 months then sample we tested a total of 284. From the results of the data adequacy test data obtained a value of 7,289, so the number of samples is larger than the sample that should be. (3) From the results of measuring employee performance using the HRA method. The biggest HEP value is the type of Possible Error. The operator only estimates and there is no standard in mixing vinegar acid with the filtrate. With an HEP value of 0.48. So this factor is the most dominant cause of disability and must be corrected immediately.

Human factors is playing an important role in every accident, particularly marine accidents. Hence, a lot of researches were conducted to analyze the human factors involvement in the accidents. Since the development of marine industry shows progressively increasing nowadays, especially in Indonesia, as Indonesia vision to be a global maritime axis of the world for marine industry, the awareness of safety life at sea has to be increase as well. Human reliability analysis (HRA) consist of many methodologies to analyze the accidents, the basic steps of HRA is qualitative method and quantitative method, one of the HRA methodology is Human Error Assessment and Reduction Technique (HEART) methodology which has been established in 1982 to assess nuclear power plant. HEART methodology is applied in this study to analyze the cause of marine accidents by human factors. The aims of this study are to know the main cause of accidents by human factors, to increase the awareness of safety at sea especially, and furthermore to improve the quality of ergonomics at sea. There are 93 EPC which discovered in this study for analyzing marine accidents in Indonesia.

PT. ICG is a company that handles the pipeline welding process at PetroChina Jabung Ltd. The welding process carried out is cutting, rolling, pressing and painting. The problem that occurs is that 243 defects are found from the output of 457 welding points. The types of defects after the welding process included 106 points of Root Cancellation defects, 99 points of Incomplete fusion, 29 points of Porosity, 8 points of Slug Inclusion, and 1 point of Tungsten Inclusion. This study aims to control quality by looking at the sigma value of the pipeline production process and analyzing the improvements that should be made to reduce the defects that occur. The results of the calculation of Defect Per Million Opportunities (DPMO) show that the number of welding defects that arise every one million opportunities is 174,10. The quality of the welding process in the pipeline Sabar # 2 project is at a sigma value of 2,43 with a DPMO value of 174,10. From the results of the sigma value obtained, this value is still far from the expectations of Petrochina's management to achieve a minimum sigma value of 4,0. The potential causes of weld defects include welding machines, work errors, work methods and work environment.
