Background: Insulin resistance (IR) is a metabolic disorder related to atherosclerosis. Its measurement is of great importance not only as a marker of diabetes but also for cardiovascular disease. The aim of this research study was to evaluate the relationship between various IR indices and coronary risk in an adult population from Maracaibo city, Venezuela. Methods: The Maracaibo City Metabolic Syndrome Prevalence Study is a descriptive, cross-sectional study with random and multi-stage sampling. In this sub study, 1272 individuals of both genders were selected with the measurement of basal insulin and coronary risk according to the Framingham-Wilson formula calibrated for our population. The insulin resistance indices evaluated were HOMA2-IR, triglycerides and glucose index (TyG) and triglycerides/HDL ratio (TG/HDL). The predictive capacity and association between each index and the coronary risk event in 10 years were determined. Results: Of the evaluated population, 55.2% were female, 34.8% had a coronary risk ≥5% in 10 years, with the TG/HDL and TyG indices showing the highest AUC 0.712 (0.681-0.743) and 0.707 (0.675-0.739), respectively; compared to HOMA2-IR. Both were also the indices most associated with increased coronary risk, especially TG/HDL ≥3 with a higher association [OR = 2.83 (1.74-4.61); p<0.01] after multivariable adjustment. Conclusions: TyG (≥4.5) and TG/HDL (≥3) indices showed a great predictive capacity of higher coronary risk, with being TG/HDL more associated even after adjusting for abdominal obesity and hs-CRP. Therefore, these represent useful tools for determining IR.

AbstractGiven the limited research on dietary insulin load (DIL), we examined DIL in relation to cardiovascular risk factors and inflammatory biomarkers in elderly men. For the present cross-sectional study, we recruited 357 elderly men. Dietary intake was assessed using FFQ. DIL was estimated by multiplying the insulin index of each food by its energy content and frequency of consumption and then summing the final value of all food items. After adjustment for covariates, a significant positive association was observed between high DIL with fasting blood sugar (FBS) levels (OR: 7·52; 95 % CI 3·38, 16·75; P=0·0001) and high-sensitive C-reactive protein (hs-CRP) (OR: 3·03; 95 % CI 1·54, 5·94; P=0·001). However, there was no association between high DIL and BMI (OR: 1·43; 95 % CI 0·75, 2·75; P=0·27), serum TAG level (OR: 0·82; 95 % CI 0·26, 2·59; P=0·73), HDL-cholesterol (OR: 2·03; 95 % CI 0·79, 5·23; P=0·13) and fibrinogen (OR: 1·57; 95 % CI 0·80, 3·06; P=0·18). Overall, elderly men with high DIL had higher FBS and hs-CRP levels than those with low DIL. Future studies are needed to clarify the association between DIL and other cardiovascular risk factors in both men and women.

Background: Diabetic retinopathy (DR) is one of the major visual morbidities associated with diabetes. This study determined the levels of serum high-sensitivity C-reactive protein (hs-CRP) in patients with DR and to correlate the estimated levels of serum hs-CRP with the severity of DR and other coexisting factors.Methods: This was a cross-sectional study conducted between March 2009 and August 2010 and included patients with type 2 diabetes mellitus (T2DM) with or without DR. A detailed fundus evaluation was performed using direct and indirect ophthalmoscopy. The retinopathies were observed and documented in accordance with the Kanski's system of classification as background DR (BDR), pre-proliferative DR (PPDR), and proliferative DR (PDR). Laboratory investigations determined the levels of fasting blood glucose (FBG), postprandial blood glucose (PPBG), glycosylated hemoglobin, urine albumin excretion, and serum hs-CRP levels.Results: In total, 80 patients with T2DM were included (DR group, n=40 [BDR, n=22; PDR, n=11; PPDR, n=7]; control group, n=40). Highest serum hs-CRP levels were found in PDR group (6.68 mg/L), followed by PPDR and BDR group (3.2 mg/L and 1.56 mg/L, respectively). The PDR group showed the longest duration of diabetes (16 years), highest FBG (221.8 mg/dL) and HbA1c (6.68 mg/L). The incidence of albuminuria and maculopathy was higher in PDR group (72.7% and 54.54%, respectively). A significant association of hs-CRP levels with DR in patients with T2DM was observed. A significantly (<0.005) positive correlation of hs-CRP was also observed with age, duration of disease, FBG, PPBG, and HbA1c.Conclusions: Patients with severe grades of retinopathy had significantly higher hs-CRP levels than patients with the milder grades.

Background Uremic pruritus (UP) is a common and frustrating symptom in patients receiving hemodialysis (HD). The majority of patients have mild to moderate itching of the skin, and a small percentage have severe itching, which seriously affects their quality of life and survival rate. However, little is known about factors that influence the intensity of itching in patients. Methods A cross-sectional study on uremic pruritus in male and female patients receiving HD was conducted in September 2019. This study included 148 eligible patients who received HD at the Blood Purification Center of Xinchang County People’s Hospital, Zhejiang Province, China from March 2019 to June 2019. We collected general data consisted of age, sex, body mass index (BMI), place of residence, educational level, diabetes mellitus status and duration of HD; as well as clinical, biochemical indicators, including serum calcium (Ca), serum phosphorus (P), serum albumin (ALB), haemoglobin (Hb), serum intact parathyroid hormone (iPTH), pre-dialysis serum urea nitrogen (BUN), normalized protein catabolic rate (nPCR), urea nitrogen clearance index (KT/V), ferritin (FER) and pre-dialysis serum creatinine (sCR). We also assayed the inflammatory cytokine serum high sensitivity C-reactive protein (hs-CRP). The Five-Dimensional Itching Scale (5DIS) was used to evaluate the degree of skin itching (none, mild, moderate, or severe). We used multiple logistic regression to analyze influencing factors on the degree of skin itching in patients with UP. Results Of the 148 patients, 60 had uremic pruritus (incidence rate, 40.54%). These included 22 cases of mild skin itching (14.86%), 30 of moderate skin itching (20.27%), and 8 of severe skin itching (5.41%). Compared with uremia patients without skin pruritus, patients with UP had higher levels of iPTH, Hb, BUN, nPCR, and hs-CRP. The composition ratio showed significant differences between urban and rural patients with different degrees of skin itching (P = 0.017); moreover, the difference of iPTH and hs-CRP levels were statistically significant (P = 0.009 and < 0.001, respectively). Using no itching as a reference, multiple logistic regression analysis showed that as hs-CRP level increased, the patient’s risks of mild skin itching (odds ratio [OR] = 1.740; 95% confidence interval [CI], 1.061–2.854; P = 0.028), moderate skin itching (OR = 2.8838 95% CI, 1.744–4.718; P < 0.001), and severe skin itching (OR = 9.440; 95% CI, 3.547–25.124; P < 0.001) all increased as well. Compared with urban residents, rural residents have a higher risk of moderate itching (OR = 3.869; 95% CI, 1.099–13.622; P = 0.035). Conclusion Levels of hs-CRP were associated with the intensity of skin itching in patients with UP. Higher hs-CRP levels were closely related to severe skin itching. The relationship between the intensity of skin itching and the environment in maintenance hemodialysis patients needs further clarification.

Asymmetric dimethylarginine (ADMA) is an endogenous inhibitor of nitric oxideproduction and involved in various pathological processes, especially processes involvingcardiovascular risk. The purpose of this study was to analyze the correlation betweenADMA and anthropometric, glucose, lipid, and inflammatory parameters. The study wasanalyzed by a cross-sectional study of 45 obese subjects at H. Adam Malik Hospital. Bloodtests were carried out after 8-10 hours of fasting against cardiovascular risk: anthropometry(body weight, BMI, and WC), glucose (FPS, PPS, HbA1C, Fasting Insulin, and HOMA-IR), lipid (LDL-C, HDL-C, TG, and sd-LDL), and inflammation (ApoB and hs-CRP)parameters. The results showed of the 45 subjects, the average age was 41.69 ± 5.69 yearsold, and the average BMI was 33.09 ± 5.05 (Obesity I). ADMA was also found to becorrelated significantly with FPG, HBA1c, and TG parameters [r=-0.506, p=0.001; r=-0.334, p=0.013, dan r = -0.315. p=0.017, respectively]. In obesity, ADMA correlatedsignificantly with cardiovascular risk parameters: FPG, HbA1C, and TG.

Abstract Background: To evaluate the association between type 2 diabetes and periodontal disease severity using the rate of alveolar bone loss (ABL) and high-sensitivity C-reactive protein (hs-CRP) value as indices. Methods: In this cross-sectional study of 372 subjects (mean age ± SD, 53.2 ± 11.8 years) from a Japanese hospital, we measured ABL and number of teeth on either panoramic radiographs or intraoral dental radiographs of all teeth. A trial classification of periodontal disease was defined as follows: I (ABL < 25%, clinically mild); II (ABL ≥ 25%, <35%, moderate); III (ABL ≥ 35%, severe); and A (hs-CRP < 440 ng/ml, mild inflammation); B (hs-CRP ≥440 ng/ml, <1,020 ng/ml, moderate inflammation); and C (hs-CRP ≥ 1,020 ng/ml, severe inflammation). Periodontal disease severity was classified into nine groups by combining ABL and hs-CRP. Results: Forty-eight subjects had type 2 diabetes; 324 did not. Univariate analysis showed that type 2 diabetes was significantly associated with age, sex, body mass index, number of teeth, ABL, hs-CRP, and periodontal disease severity. Multivariate analysis showed significant associations between type 2 diabetes and the groups with high severity of periodontal disease. In receiver operating characteristic (ROC) curve analysis predicting the presence of diabetes, area under the ROC curve was 0.762 (95%CI = 0.688 - 0.835) for ABL, and 0.709 (95%CI = 0.635 - 0.784) for hs-CRP, which was significant. Conclusions: This study showed that diabetes could be associated with a periodontal disease severity classification using the combination of ABL and hs-CRP.

Abstract Background Intermittent pneumatic compression (IPC) is commonly used to prevent deep vein thrombosis (DVT) during hospitalization in patients with acute stroke. However, if DVT exists at admission, IPC of the legs with DVT may cause migration of the thrombi, resulting in pulmonary emboli. Whole-leg ultrasonography (wl-US) is a practical tool to detect DVT; however, wl-US is not always performed at admission in all stroke patients. This retrospective cross-sectional study aimed to investigate DVT frequency and identify significant factors indicating the presence of DVT at admission for acute stroke. Methods We included patients admitted within 24 h of stroke onset between 2017 and 2019. Patients who did not undergo blood tests for D-dimer or wl-US within 72 h of arrival were excluded. We collected patient data on age; sex; anthropometric variables; presence of DVT on wl-US; and biomarkers such as D-dimer, high-sensitivity C-reactive protein (hs-CRP), and lipids. Results Of 1129 acute stroke patients, 917 met our inclusion criteria. DVT was detected in 161 patients (17.6 %). Patients with DVT were older; were more likely to be female; had lower body weight; had higher D-dimer and hs-CRP levels; had lower albumin, hemoglobin, and triglyceride levels; and had higher National Institutes of Health Stroke Scale and pre-stroke modified Rankin scale scores than patients without DVT (n = 756). In addition, multiple logistic regression analysis showed that sex (female) and D-dimer levels (≥ 1.52 µg/mL) were independent significant factors for the presence of DVT. Among 161 patients with DVT, 78 (48.4 %) had both these significant factors. Among 756 patients without DVT, 602 (79.6 %) had no or one significant factor. The odds ratio of the presence of DVT in patients with both significant factors was 6.29, using patients without any significant factors as the group for comparison. Conclusions The frequency of DVT is high in acute stroke patients at admission. Female sex and a high D-dimer level were independent significant factors for the presence of DVT. Therefore, in patients with these two significant factors at admission, IPC should be avoided or wl-US should be performed before IPC.

Background: Suicide has grown in global prevalence as a public health problem. We aimed to evaluate the association of socioeconomic factors, biochemical and hematologic tests, and suicide ideation. Methods: In this cross-sectional study, 8267 Iranian adults aged 35 – 65 years old were enrolled. The assessment of suicide ideation was made by the completion of Beck’s depression inventory (BDI) questionnaire; according to one specific item on the questionnaire: “have you ever decided to suicide in the past week?” Results: According to our results, 6.9 % of subjects had ideation of suicide. The results showed high levels of FBG, RBC, MCHC, and hs-CRP were associated with suicide ideation. Obese, single subjects, and current-smokers had a higher risk of suicide ideation. Conclusion: Increased physical activity, obesity, and smoking are associated with a high risk of suicide ideation; whilst, a high MCHC is related to low risk of suicide ideation in Iranian adults.

Abstract Objectives There is limited evidence about the inflammatory potential of diet and cardiometabolic risk in children. The aim of this study was to evaluate the association between the Children’s Dietary Inflammatory Index (C-DII) with cardiometabolic risk factors in Turkish adolescents from 10 to 17 years. Methods Participants aged 10–17 years, who completed a 24-h dietary recall, from which C-DII scores were calculated, were include in this cross-sectional study. Lipid profile, glycemic parameters, high-sensitivity C-reactive protein (hs-CRP), liver enzymes, thyroid-stimulating hormone (TSH), and uric acid were analyzed in blood samples. Sociodemographic characteristics and sedentary behavior were assessed using a semi-structured questionnaire. We compared the distributions of anthropometric, biochemical, and blood pressure measurement levels associated with cardiometabolic risk factors by the median of C-DII with linear regression. Results The mean sample C-DII was −0.16 ± 2.31 and ranged from −3.22 to +4.09. Higher median C-DII scores, indicating a more pro-inflammatory diet among children, were associated with higher blood pressure and body mass index (BMI). However, the C-DII was modestly directly associated with fasting insulin, fasting blood glucose, and waist circumference. The area under the receiver operating curve of C-DII in predicting hs-CRP was found to be quite high (0.864, 95% CI: 0.795–0.933). Conclusions Consuming a pro-inflammatory diet in adolescence was associated with alterations in cardiometabolic risk factors, especially with systolic blood pressure, diastolic blood pressure, and BMI.
