Abstract BackgroundMistreatment of medical professionals by patients is an ongoing problem. We aimed to construct and evaluate a curriculum that would prepare medical professionals for mistreatment by patients. MethodsLessons learned from 15 interviews and 2 focus groups with medical professionals were distilled into a multi-modal curriculum including didactics, simulation videos and role-play scenarios aimed to improve confidence in addressing mistreatment. This curriculum was disseminated at five educational workshops to medical professionals of various training groups and experience levels. Pre- and post-surveys were distributed to assess changes in participant’s perspectives on readiness to address mistreatment. The signed-rank test was implemented to compare pre- and post- data. ResultsParticipants were more likely to agree post-workshop that they had the right words to say, had a plan for what to do, and were more willing to speak up when they themselves or someone else was mistreated (p < .001). They were also more likely to agree post-workshop that there was something they could do to address patient mistreatment (p < .001).ConclusionsParticipant familiarity and confidence in responding to patient mistreatment increased. Our curriculum may serve as a foundation for institutions seeking to equip their educators, providers, and trainees with strategies for addressing this important issue.

Background: Infant tongue-tie can cause breastfeeding problems, which may be improved by frenotomy. However, controversy exists among the medical community. Research aim: To examine the influence of frenotomy on infants with posterior ankyloglossia, by quantifying the changes in breastfeeding and maternal nipple pain using standardized tools. Methods: Newborns ( N = 30) below 12 weeks of age were recruited from the Herzl–Goldfarb Breastfeeding Clinic between April 2014 and April 2015. Diagnosis of posterior ankyloglossia was made clinically. Frenotomy was performed. Mothers received breastfeeding counseling before and after the procedure. Pain and breastfeeding were assessed before the procedure, immediately after the procedure, and after 2 days, 7 days and 14 days. Breastfeeding was assessed using the LATCH Tool and by subjective questioning. Maternal nipple pain was assessed using the Numeric Rating System. Results: No complications were reported with frenotomy. There was a significant improvement in LATCH score immediately post-frenotomy, with an increase in median scores from 7.5 to 8.5 ( p < .0001, Wilcoxon signed rank test). There was a significant decrease in median pain score immediately post-frenotomy, from 3.0 on the left nipple and 3.25 on the right nipple, to 0 bilaterally ( p < .0001, Wilcoxon signed rank test). Subjective improvement in breastfeeding was reported by 90% of mothers immediately after frenotomy and 83% of mothers at Day 14. Conclusion: Frenotomy for posterior ankyloglossia may improve breastfeeding and nipple pain.

Abstract Introduction: The Trunk-Pelvis-Hip Angle (TPHA) test is used for assessing the mobility of lumbo-pelvic-hip complex. The aim of the research was to assess the intraobserver reliability of the TPHA test in girls with and without idiopathic scoliosis and to compare the test values obtained by the girls in both groups. Material and methods: The research included girls without scoliosis and girls with double idiopathic scoliosis aged 8-16. The trunk-pelvis-hip angle was measured three times on each body side within one session. Statistical analysis was performed with the use of ANOVA intraclass correlation coefficient for dependent groups, Kolmogorov-Smirnov test as well as non-parametric Wilcoxon signed-rank test for dependent groups and Mann-Whitney U test for independent groups. Results: The research included 49 healthy girls (aged 11.8 ± SD 2.5 years) and 49 girls with double idiopathic scoliosis (aged 12.7 ± SD 2.6 years; Cobb angle Th 27.7° ± SD 13.4° and L Cobb 25.8° ± SD 10.5°). The intraobserver reliability for the TPHA measurements was very high (ICC>0.85). In both groups the left-side TPHA value was significantly higher than the right-side value (p=0.001). The TPHA range of motion on the right side of the body was more limited in the scoliotic girls than in the healthy ones (p=0.001). Conclusions: The TPHA test is a reliable means of assessing mobility in the lumbo-pelvic-hip complex in girls by one observer. The asymmetry of movements in the lumbo-pelvic-hip complex in healthy girls needs to be observed since it may constitute one of the factors predisposing to scoliosis which limits spine rotation range of motion. The TPHA test needs further research.

Abstract Background: There is a critical need for speech therapy services for people born with cleft lip and/or palate in developing countries. Objective: Assess the effectiveness of a speech camp and follow-up session for children with cleft lip and/or palate. Methods: A Community-Based Model for Speech therapy was implemented at Suwanaphum Hospital District, Roiet, Thailand. Thirteen children with cleft lip and/or palate (3; 6-13 years) attended a four-day speech camp and a one-day follow-up session (six months later) for remediation of their articulation disorders. Paraprofessional training was also provided. Pre- and post-tests were administered to the participants, caregivers, and paraprofessionals to determine the effectiveness of the program. A pre- and post-articulation test, as well as an audiological evaluation were administered. Five speech and language pathologists provided speech therapy, both individual and group, for a total of 18 hours during the four-day speech camp and six hours in the one-day follow-up session. The median difference of the number of articulation errors was determined by results of the Wilcoxon Signed-Rank Test. Results: There was a significant decrease in articulation errors following both the main speech camp and the follow-up session (z = 3.11, p < 0.01; z = 2.87, p<0.01, respectively). Caregivers’ and health care providers’ satisfaction ratings for participation in the speech camps ranged from good to excellent. Conclusion: A Community-Based Model of both a speech camp and follow-up session provided an effective speech therapy treatment for children with cleft lip and/or palate.

ABSTRACT Background and Aims: There is no study in the literature that investigates an asymmetric morphological feature of the frontal sinus (FS). Materials and Methods: Four hundred and sixty-nine consecutive direct X-rays of FSs were analyzed for the asymmetry between the right and left sides. When an asymmetry in the height and contour of the FS existed, this difference was quantified. Results: Of the 469 patients, X-rays of 402 patients (85.7%), there was an asymmetry between right and left sides of the FS. Of these 235 (50.1%) were dominant on the left side, whereas 167 (35.6%) were dominant on the right, the sinuses of remaining 67 patients (14.3%) was symmetric. Statistical Analysis: The comparisons between parameters were performed using Wilkinson signed rank test. The relationship between handedness and sinus asymmetry was also examined by two proportions test. There is statistically significant difference between the dominance of left and right FS. Conclusions: Hemispheric dominance may have some effect (s) of on sinus asymmetry of the human cranium. Surgeons sometimes enter the cranium through the FS and knowledge of asymmetric FS is important to minimize surgical complications.

AbstractNumerous studies have postulated the possible benefit of corticosteroids on olfaction in patients with nasal/sinus disease. Twenty-nine patients with bilateral nasal polyps were included in our study using strict selection criteria to reduce other aetiologies of olfactory dysfunction. The University of Pennsylvania Smell Identification Test (UPSIT) was performed pre-operatively on the right and left nostrils separately. Following intranasal polypectomy the patients received a six-week course of beclomethasone nasal spray (Beconase) to one nostril only, with the other acting as a control. The UPSIT scores were again obtained for each nostril separately. Wilcoxon Signed Rank test revealed no statistically significant difference in UPSIT scores between treated and untreated nostrils (p = 0.31; power 70 per cent; ES = 0.47). We conclude that topical beclomethasone does not improve olfaction following nasal polypectomy.

Introduction: Knowledge and skill of school age children is still lack about first aid of wound. The purpose of this study was to determine the effect of role play methodtowards knowledge and wound first aid skill on student. The aim of experiment is to study the effect of role play method towards knowledge and wound first aid skill on student.Methods: The design used in this study was quasi-experimental quasi-eksperimental pretest-posttest control group design. The samples were gathered using simple random sampling method consisted of 50 students based on the inclusion criteria which were divided into experimental and control groups. The independent variable was  health education using role play methods and the dependent variables were knowledge and first aid wound skill. The data were analyzed by using Wilcoxon Signed Rank Test and Mann Whitney U Test with level significance α<0,05.Results: The Wilcoxon Signed Rank Test results showed that there were enhancement for knowledge (p=0.000) and first aid wound skill(p=0.000) for post intervention toward the experimental group. These data were strengthened by the result of Mann Whitney U Test statistical analysis that showed the significant differences for knowledge (p=0.000) and action application of open wounds care (p=0.000).Conclusion: It can be concluded that health education using role play methods affected knowledge and wound first aid skill on student. The suggestion for the further experiment is to compare role play methods with other health education methods to find out effective methods of health education for student.

Progressive training of horses focusses on the muscular development and flexibility to both the right and the left. However anecdotal evidence suggests that horses are not symmetrically structured through the thoracic region and this may have effects on saddle fitting and performance. The aim of this research was to determine if asymmetry exists in the thoracic musculoskeletal structure of ridden horses by analysing retrospective data of saddle fits (n=490) obtained from a saddle fitting company. A flexible wither tracer tool was used to obtain tracings of wither shape. Each tracing yielded four repeatable points (two on the left; two on the right) measured from the midline at the highest and lowest points of the withers. Descriptive assessments of shoulder, wither, and back shape, and demographic information on both horse and rider were recorded. A Wilcoxon signed-rank test compared the means of the wither shape measurements. A GLIMMIX procedure identified relationships between the wither measurements, descriptive assessments, and horse and rider demographics. The means of measurements from the wither tracer tool showed almost 60% of horses having larger measurements on the left side (P<0.0001). There was no effect of horse breed, age, sex, height or level of training on wither measurements (P>0.05). Rider age, gender, height, weight and level of training did not affect wither measurements (P>0.05). These results show that horses are asymmetric in their thoracic structure with a majority of horses larger on their left side than the right. This asymmetry may be due to genetics, environment or training and should be considered when fitting a saddle to the horse.

Salah satu masalah kesehatan yang sering dialami adalah penyakit gastritis. Gejala yang sering dikeluhkan oleh penderita gastritis adalah mual. Salah satu penatalaksanaan keperawatan yang dapat mengurangi rasa mual adalah tirah baring. Penelitian ini bertujuan untuk mengetahui pengaruh tirah baring terhadap penurunan rasa mual pada klien gastritis di Pelayanan Kesehatan. Metode penelitian menggunakan eksperimental dengan bentuk penelitian one group pretest-posttest design. Populasi dan sampel adalah klien yang mengalami mual di Wilayah Kerja Puskesmas  yang berjumlah 15 orang. Sampel diambil dengan teknik purposive sampling. Alat pengumpul data menggunakan observasi. Analisa data melalui uji Wilcoxon Signed Rank Test. Hasil penelitian menujukkan klien gastritis sebelum tirah baring mengalami mual ringan sebanyak 7 orang (46,7%), sesudah tirah baring mengalami tidak mual sebanyak 7 orang (46,7%). Ada pengaruh tirah baring terhadap penurunan rasa mual pada klien gastritis di Pelayanan Kesehatan (ρ value = 0,001).

ABSTRAKReumatoid Artritis (RA) merupakan penyakit muscoloskelektal yang sering terjadi pada usia lanjut. Gangguan pada system muscoloskelektal yang ditandai dengan munculnya nyeri sendi dan kekakuan yang mengakibatkan penurunan kemampuan fisiologis atau kualitas hidup lansia. Dampak dari Reumatoid Artritis dapat menimbulkan beberapa keluhan dan dapat menyebabkan kelumpuhan. Untuk menganalisis efektifitas kompres jahe merah hangat dan kompres serai hangat terhadap penurunan intensitas nyeri artitris remauthoid pada lanjut usia.Metode Penelitian: Jenis penelitian adalah quasy experimental dengan two group pre – post test design. Subjek adalah sebagian lansia yang penderita Arthritis Remathoid di Desa Sumberagung Kecamatan Dander Kabupaten Bojonegoro. Subjek dibagi menjadi dua kelompok yaitu kelompok I (n=15) diberi perlakuan kompres jahe hangat dan II (n=15) diberi perlakuan kompres serai hangat. Analisis yang digunakan uji Mann Whitney U Test dan Wilcoxon Signed Ranks Test dengan ingkat kemaknaan α = 0,05.Hasil uji Wilcoxon Signed Rank Test, didapat keduanya mempunyai nilai kemaknaan yaitu ρ value = 0,000. Nilai ρ = 0,031 pada kelompok kompres serai hangat dan kelompok kompres jahe merah ρ value = 0,165. Hasil uji Mann Withney U Test pada Post perlakuan kedua terapi diperoleh selisih nilai nyeri pada kompres jahe ρ= 0,003 dan selisih nilai nyeri kompres serai ρ value = 0,001.Penggunaan kompres jahe merah lebih efektif dibandingkan dengan kompres serai terhadap penurunan intensitas nyeri arthritis remathoid. Kata Kunci: usia lanjut, Reumatoid Artritis (RA), jahe merah, serai, perbedaan efektifitas.    ABSTRACTReumatoid Artritis (RA) is a musculoskeletal which frequently occurs in the elderly. The disorders in the musculoskeletal system are noted by the occurrence of pain in the joints and stiffness which reduces the physiological abilities or life quality of the elderly. The disease causes many such complaints and  consequences of the disease rheumatoid arthritis may experience paralysis. The aims of this study is to analyze the effect of warm red ginger compress therapy and warm lemongrass compress therapy against of  Decreased pain intensity in  the elderly  with  artitris remauthoid. The study was Queasy experimental with two group pre – post test design. Subjects were some elderly people with Arthritis Remathoid in Sumberagung Village, Dander Sub District, Bojonegoro District. Subjects were divided into two groups: group I (n-15) with warm ginger compress therapy, and II (n=15) with warm lemongrass compress therapy. The analyses used in this study were the Mann Whitney U Test and Wilcoxon Signed Ranks Test with α of 0.05. Results of Wilcoxon Signed Rank Test obtained Both have meaning p value of  0.000. ρ value = 0,031 in a warm lemongrass compress therapy group and obtained of warm ginger compress therapy group ρ value = 0,165. The results of Mann Withney U Test on Post treatment second therapy, obtained difference of warm ginger compress therapy with ρ value= 0,003 and difference of warm lemongrass compress therapy with ρ value = 0,001.The use of warm ginger compresses therapy are more effective than a warm lemongrass compress therapy against decreased pain intensity in  the elderly  with  artitris remauthoid.  Keywords: elderly, artitris remauthoid, red ginger, lemongrass, differences in effectiveness

The working practice of the engineering students is part of the learning process that is irreducible and indispensable. The composition of  lecturing between theoretical and practical one is 40% to 60%. With this condition, the students spend more time at the laboratory. Generally, the students perform in the laboratory work by standing position. The design of research is observational cross-sectional. The method applied is observation, interview and measuring. The subjects of research are practicing students amounting to 21 students. Referring to the analysis of statistical test or Wilcoxon signed ranks test, the difference of effect of work position is significant, namely p < 0.05 towards musculoskeletal disorders (MSDs) before and after working. The quantity of the average complaint after working is score 44.62 ± 9.47. The result of Wilcoxon signed rank test shows that there is significant different effects of standing work position, namely p < 0.05 towards fatigue generally before and after working. The degree of the working pulse is on the average of 110.78  ± 17.80 bpm (beats per minutes) which can be categorized into the medium workload. Using paired t-test, the result is p < 0.05.
