miR-27b-3p is highly expressed in serum of patients with preeclampsia and has clinical significance

Background: Preeclampsia (PE) is defined as a salient complication of late pregnancy. microRNAs (miRNAs) have emerged as critical biological regulators in PE. This study determined miR-27b-3p expression in serum of PE patients and investigated its clinical significance in PE. Methods: Totally 130 pregnant women including 90 PE patients (51 mild PE and 39 severe PE) and 40 healthy controls were enrolled in this study. miR-27b-3p expression in the serum of PE patients and healthy controls was detected using RT-qPCR. The correlation among miR-27b-3p expression and 24-h urine protein, systolic blood pressure (SBP), diastolic blood pressure (DBP), serum creatinine, and fetal birth weight was analyzed using Pearson's correlation coefficient. The targeting relationship between miR-27b-3p and PPARG was verified. PPARG protein level in PE patients was detected using ELISA kits. The predictive efficiency of miR-27b-3p and PPARG in PE was analyzed using the receiver operating characteristic (ROC) curve. Results: Compared to normal pregnant women, PE pregnant women, especially severe PE patients had higher miR-27b-3p expression. miR-27b-3p was positively correlated with 24-h urine protein, SBP, DBP, and serum creatinine, but negatively correlated with fetal birth weight. PPARG was poorly expressed in PE patients and negatively correlated with miR-27b-3p. ROC curve showed that both miR-27b-3p and PPARG had good predictive efficacy on PE. Conclusion: miR-27b-3p expression in serum of pregnant women with PE was positively correlated with the severity of PE symptoms, suggesting the involvement of miR-27b-3p in PE occurrence.

Urinary and blood biomarkers for diagnosis of acute kidney injury (AKI) in hospitalised dogs were evalueted. This prospective study included 97 dogs, classified according to the International Renal Interest Society classification into no AKI and AKI grade 1 (48-hour increase in serum creatinine≥0.3 mg/dl and/or urinary production <1 ml/kg/hour for at least six hours). A total of 62 of 97 dogs (64 per cent) were classified as AKI 1. A statistically significant difference was found between no AKI and AKI 1 in urine protein to creatinine ratio, urinary γ-glutamyl transferase (uGGT) and uGGT/cu (P<0.0001). Thirteen of 97 dogs (13.4 per cent) that developed increased creatinine and change in AKI grade showed high mortality (n=9/13; 69.2 per cent). The receiver operating characteristic (ROC) curve analysis of uGGT/cu index as a marker for AKI grade 1 had an area under the ROC curve of 0.78; optimal cut-off point was 57.50 u/g, with sensitivity and specificity of 75.4 per cent and 75.6 per cent, respectively. Overall intensive care unit mortality was 23.7 per cent (23/97), 13.4 per cent (13/97) of which died during hospitalisation and 10.3 per cent (10/97) within 28 days after discharge. uGGT is an acceptable marker for distinguishing between AKI 1 and no AKI.

BACKGROUND: Thyroid disorder is one of commonest endocrine disorder in women and hence constitutes a common endocrine disorder complicating pregnancy. Diagnosing and treating hypothyroidism preconceptionally and during early pregnancy appears to be a useful strategy to improve maternal and fetal outcome. OBJECTIVE: To compare the maternal and perinatal outcome in pregnant women with hypothyroidism diagnosed preconceptionally with hypothyroidism diagnosed during pregnancy. METHODS: A prospective study. One hundred pregnant women with hypothyroidism at less than 20 weeks Period of Gestation (POG) were recruited for the study and grouped into two groups: Group A-hypothyroidism diagnosed and on treatment before conception, Group B-hypothyroidism diagnosed and started on treatment during pregnancy. Both groups were intensively monitored during pregnancy with serial Thyroid Function Test (TFT) and thyroxine replacement doses were adjusted accordingly. Various maternal, perinatal and fetal outcome measures were studied in both groups prospectively till delivery. RESULTS: The maternal and fetal complications were comparable in the two groups, however the fetal birth weight was significantly higher in Group A versus Group B (2.89±0.485kg vs. 2.70±0.453kg; p=0.039). All the new born babies had normal thyroid function. CONCLUSION: Hypothyroidism diagnosed preconceptionally or during early pregnancy and treated appropriately has beneficial effect on fetal birth weight, and hence the total pregnancy outcome. Screening for thyroid dysfunction should be judiciously performed in all high risk women prior to a planned conception or during their first antenatal visit. AbstrakTinggi rendahnya tekanan darah sistolik dan diastolik dalam kehamilan mempunyai pengaruh terhadap berat badan lahir. Penelitian ini bertujuan untuk mengidentifikasi hubungan tekanan darah ibu hamil aterm dengan berat badan lahir di RSUP dr M. Djamil Padang. Metode penelitian : Penelitian yang dilakukan merupakan survei analitik dengan menggunakan studi komparatif dengan design penelitian Cross Sectional Study. Penelitian menggunakan data sekunder yang diambil dari sub bagian Rekam Medik (Medical Record) RSUP dr. M. Djamil Padang bulan Januari 2010 – Desember 2012 dengan jumlah sampel 34 orang ibu hamil yang tidak hipertensi dan 34 orang ibu hamil yang mengalami hipertensi dalam kehamilannya. Hasil penelitian : Ditemukan rata-rata berat badan lahir bayi yang dilahirkan oleh kelompok ibu yang tidak mengalami hipertensi pada kehamilannya adalah 3.408 (SD 307) gram dan rata-rata berat badan lahir bayi yang dilahirkan oleh kelompok ibu yang mengalami hipertensi pada kehamilannya adalah 2.799 (SD 413) gram. Dari hasil uji analisis Independent Sample T-test diperoleh hasil ditemukan adanya hubungan tekanan darah ibu hamil dengan berat badan lahir di RSUP dr. M. Djamil Padang p = 0,00 (p < 0,05). Kesimpulan : Pemeriksaan kehamilan secara teratur penting untuk mengantisipasi adanya peningkatan tekanan darah ibu hamil yang dapat berpengaruh terhadap berat bayi lahir.Kata kunci: tekanan darah, berat badan lahir, ibu hamilAbstractHigh and low level of systolic and diastolic blood pressure in pregnancy has impact to birth weight. Djamil Padang since Januari 2010 until December 2012 with number of samples are 34 pregnant women who have not hypertension and 34 pregnant women who have hypertension in pregnancy.The results: Research found that average of birth weight from non-hypertension pregnant woman is about 3,408 (SD 307) gr and average of birth weight from hypertension pregnant woman is about 2,799 (SD 413) gr. The result from Independent sample T-test analysis found that there is a relation between blood pressure in pregnancy with birth weight at RSUP dr. Objective: Fetal growth is an important indicator of fetal health. Low birth weight (LBW) is also associated with increased perinatal morbidity and mortality. Numerous factors that affect fetal weight have been identified. In this study, we aimed to investigate the effect of body mass index, smoking, and anemia on fetal birth weight on term pregnant women who had vaginal delivery in our clinic. Methods: This study is a retrospective cross-sectional study. This study included patients who had a spontaneous vaginal delivery at our hospital between January 1st, 2018, and June 15th, 2020. Measurements of hemoglobin (Hb) and hematocrit (Hct) levels during birth supported the diagnosis of anemia. Hb levels were compared in three categories in this study: (1) Hb<10 g/dL; (2) ≥10<11 g/dL; and (3) Hb ≥11 g/dL. Anemia was defined as having a Hb level <11 mg/dL. Birth weight was categorized as LBW <2500 g; normal birth weight (NBW) as ≥2500–3999 g, and macrosomia as ≥4000 g. Results: Analysis was performed on 1428 pregnant women. There was a statistically significant difference for the Hct and Hb subgroups between the groups (p<0.05). Fetal birth weight was found higher in the non-smoking group than in the smoking group (3302.1± 381.5 g vs. 2839.7±491.5 g; p<0.001). Body mass index (BMI) and Hb levels positively predicted fetal birth weight (β=0.134; p<0.001 and β=0.051; p=0.046), smoking was negatively predicted fetal birth weight (β=-0.245; p<0.001). Conclusion: BMI, Hb levels, and smoking status during mothers’ delivery are effective on fetal weight. Smoking was the strongest predictor of fetal birth weight compared with the other variables.

Background: Adipocytokines have been recently implicated in the pathogenesis of preeclampsia. Visfatin is one of such adipokines. Objective: To determine the association between serum visfatin levels and preeclampsia. Methods: A prospective, case-control study was carried out in 160 pregnant women consisting of 80 pre-eclamptics and 80 normotensive controls, matched for age and parity during the third trimester. Maternal serum visfatin levels were determined in both groups using a visfatin (Human) enzyme- linked immunosorbent assay. Serum Visfatin levels were compared between the groups and correlated to the blood pressure, proteinuria, fetal birth weight and Apgar scores. Results: The mean serum visfatin level was significantly higher (10.3±6.9 ng/ml) in preeclampsia than (7.4±4.4 ng/ml) in the control group (p=0.001). The mean serum visfatin level was higher in severe pre-eclamptics (10.8±8.9 ng/ml) compared to (9.6±5.8ng/ml) in mild preeclamptics and this was statistically significant (p=0.021). Visfatin levels showed a negative and non-significant correlation with both systolic (r= -0.011 and p=0.924), diastolic blood pressure (r= -0.012, p=0.913) and body mass index (r= -0.142, p=0.209) in both study and control groups. Mean birth weight was significantly lower in the preeclampsia (2.8±0.25 kg) compared to the control group (3.2±0.31 kg) P=0.000. the mean birth weight was lower in severe preeclampsia (2.7±0.25 kg compared to 2.9±0.39 in mild preeclamsia. There was no significant correlation between the visfatin levels and Apgar score at 5minutes and birth weights in both groups (P=>0.05). Conclusion: This study showed a significant increase in the level of visfatin in preeclampsia compared to their normo-tensive controls. However, this increased level was not consistent with the severity of the disease.

Abstract Background: To explore the application of GE-E10 four-dimensional color ultrasound equipment for estimation of fetus weight and compare its accuracy with birth weight. Methods: A total of 160 singleton and full-term pregnant women who delivered successfully in our department of obstetrics and gynecology from March 2017 to October 2019 were randomly selected as the study objects. The fetus weight was estimated by using clinically used two-parameter formula and GE-E10 four-dimensional color ultrasound multi-parameter. The accuracy of the two methods in estimating fetal weight was compared and analyzed. Results: The 160 puerpera enrolled: average age (23.64±6.12) years, average gestational weeks (38.56±2.34), average gravidity (2.35±1.02) times. The accuracy of GE-E10 multi-parameter ultrasound in estimating fetal weight and birth weight was 92.50% (148/160), which was significantly higher than that estimated by two-parameter formula: 72.50% (116/160), (P<0.05). The area under the ROC curve of fetus weight estimated by GE-E10 multi-parameter ultrasound was 0.842 (0.024-0.768), which is higher than that estimated by two-parameter formula was 0.717 (0.103-0.832), (P<0.05). Conclusion: The accuracy of GE-E10 four-dimensional color ultrasound equipment in estimation of fetus weight is high and effective, which can be used for estimating fetal macrosomia effectively and providing guidance and reference for delivery of puerpera.

The associations between gestational Bell’s palsy (GBP) and late obstetric complications (LOCs), i.e., preeclampsia (PE), eclampsia (EC), gestational hypertension (GHT), and gestational diabetes mellitus (GDM) remain unclear. This study aimed to evaluate these associations and the neonatal health of the newborns born from pregnant women with and without GBP. A retrospective 1:5 case-control study matching exact maternal age and gravidity between pregnant women with and without GBP in Songklanagarind Hospital from 2006 to 2016 was conducted. The associations between GBP and PE, EC, GHT, and GDM, as well as comparison of the newborns’ health indices were analyzed by bivariate analysis (p < 0.05). Eight GBP cases out of 8,756 pregnant women were recruited. Six GBP cases were first or second gravid. GBP occurred during the third trimester in five cases. Except for higher median systolic blood pressure (125 (114.2, 127.5) vs. (110 (107.0, 116.0), p = 0.045) and diastolic blood pressures (77 (73.0, 80.8) vs. 70 (65.0, 73.2), p = 0.021) in the GBP cases, associations between GBP and all LOCs could not be concluded due to the lack of power. However, a significantly lower mean birth weight in the newborns of GBP mothers was found (2672.2 (744.0) vs. 3154.8 (464.7), p = 0.016) with statistically significant power. Except for the higher blood pressures and lower birth weights of the newborns of GBP mothers, an association between GBP and LOCs remains inconclusive.

Patients are often supplemented with a sufficient dose of thyroxine after thyroidectomy for thyroid cancer. However, the influence of thyroxine supplementation on fetal growth in pregnant women after thyroidectomy for thyroid cancer remains unclear. The aim of this study was to investigate the effect of thyroxine supplementation on neonatal birth weight. This cohort study included 49,896 pregnant women (278 patients with a history of thyroidectomy for thyroid cancer and 39,363 control cases after exclusion). Thyroid parameters were examined in pregnant women and their newborns. The associations between maternal thyroid function and neonatal birth weight and small for gestational age were studied using regression analyses. In the levothyroxine supplementation group, free thyroxine (FT4) levels were significantly higher in both early pregnancy (P &lt; 0.001) and late pregnancy (P &lt; 0.001) groups than in the control group. Furthermore, levels of neonatal thyroid stimulating hormone (P = 0.032) and birth weight (P = 0.043) were significantly lower than those in the control group. We also observed a significant inverse association between maternal FT4 levels in early pregnancy and neonatal birth weight (P=0.028), especially in male newborns (P=0.036). In summary, after thyroidectomy for thyroid cancer, a sufficient dose of thyroxine supplementation in early pregnancy is significantly associated with reduced birth weight and may need to be monitored.
