COVID-19 Infodemiology at Planetary Scale: Charting the Information and Misinformation Landscape to Characterize Misinfodemics Spread on Social Media (Preprint)

BACKGROUND The novel coronavirus, also known as COVID-19 or SARS-COV-2, has come to define much of our lives since the beginning of 2020. During this time, countries around the world imposed lockdowns and social distancing measures; our physical movements ground to a halt, while our online interactions increased as we turned to engaging with each other virtually. As our means of communication shifted online, so too did information consumption. While there has been an intentional shift and focus by governing authorities and health agencies on using social media and online platforms to spread factual and timely information, this has also opened the gate for misinformation, contributing to the phenomenon of misinfodemics. OBJECTIVE In this paper, we carry out an over a year-long analysis of Twitter discourse on over a billion tweets related to COVID-19 to identify and investigate prevalent misinformation narratives and trends. We also aim to describe the Twitter audience that is more susceptible to health-related misinformation and the network mechanisms driving misinfodemics. METHODS We leverage a dataset that we collected, and made public, containing over one billion tweets related to COVID-19 spanning between January 2020 and April 2021. We create a subset of this larger dataset by isolating tweets that include URLs with domains that have been identified by Media Bias/Fact Check as being prone to questionable and misinformation content. By leveraging clustering and topic modeling techniques, we identify the major narratives, including health misinformation and conspiracies, that are present within this subset of tweets. RESULTS Our focus is on a subset of 12,689,165 tweets that we determined are representative of COVID-19 misinformation narratives in our full dataset. When analyzing tweets that share content from domains known to be questionable or that promote misinformation, we find that a few key misinformation narratives emerge about Hydroxychloroquine and alternative medicines, United States officials and governing agencies directives, and COVID-19 prevention measures. We further analyze the misinformation retweet network and find that users who share both questionable and conspiracy-related content are clustered more closely in the network than others, supporting the hypothesis that echo chambers can contribute to the spread of health misinfodemics. CONCLUSIONS Our paper presents a summary and analysis of the major misinformation discourse surrounding COVID-19 and those who promoted and engaged with it. While misinformation is not limited to social media platforms, we hope that our insights will shed light on how best to combat misinformation, particularly pertaining to health-related emergencies, and pave the way for computational infodemiology to inform health surveillance and interventions.

With people moving out of physical public spaces due to containment measures to tackle the novel coronavirus (COVID-19) pandemic, online platforms become even more prominent tools to understand social discussion. Studying social media can be informative to assess how we are collectively coping with this unprecedented global crisis. However, social media platforms are also populated by bots, automated accounts that can amplify certain topics of discussion at the expense of others. In this paper, we study 43.3M English tweets about COVID-19 and provide early evidence of the use of bots to promote political conspiracies in the United States, in stark contrast with humans who focus on public health concerns.

This review aims to (a) investigate the characteristics of both the research community and the published research on health-related fake news on social media platforms, and (b) identify the challenges and provide recommendations for future research on the subject. We reviewed 69 journal articles found in the main academic databases up to April 2021. The studies extracted data mainly from Twitter, YouTube, and Facebook. Most articles aimed to investigate the public’s reaction to fake health information, concluding that health agencies and professionals should increase their online presence. The articles also suggest that future work should aim to improve the quality of health information on social media platforms, develop new tools and strategies to combat fake news sharing, and study the credibility of health information. Nonetheless, those in control of the platforms are the only ones which can take effective measures to ensure that their users receive reliable information.

UNSTRUCTURED The ongoing outbreak of SARS-CoV-2 infection was first identified in Wuhan, China at the late of 2019. Following the acceleration of the novel coronavirus spreading, person-person transmissions in family residences, hospitals and other public environments have led to a major public hazard in China. Currently, the SARS-CoV-2 outbreak has been further developed into a public health emergency of international concern. In response to an occurring pandemic, hospitals need an emergency strategy and plan to manage their space, staff, and other essential resources, therefore, to provide optimum care to patients involved. In addition, infection prevention measures urgently need to be implemented to reduce in-hospital transmission and avoid the occurrence of virus super-spreading. For hospitals without capacity to manage severe patients, a referral network is often needed. We present our successful field experience regarding hospital emergency management and local hospitals network model in response to SARS-CoV-2 emerging epidemic.

Abstract Background Since the novel coronavirus disease outbreak, over 179.7 million people have been infected by SARS-CoV-2 worldwide, including the population living in dengue-endemic regions, particularly Latin America and Southeast Asia, raising concern about the impact of possible co-infections. Methods Thirteen SARS-CoV-2/DENV co-infection cases reported in Midwestern Brazil between April and September of 2020 are described. Information was gathered from hospital medical records regarding the most relevant clinical and laboratory findings, diagnostic process, therapeutic interventions, together with clinician-assessed outcomes and follow-up. Results Of the 13 cases, seven patients presented Acute Undifferentiated Febrile Syndrome and six had pre-existing co-morbidities, such as diabetes, hypertension and hypopituitarism. Two patients were pregnant. The most common symptoms and clinical signs reported at first evaluation were myalgia, fever and dyspnea. In six cases, the initial diagnosis was dengue fever, which delayed the diagnosis of concomitant infections. The most frequently applied therapeutic interventions were antibiotics and analgesics. In total, four patients were hospitalized. None of them were transferred to the intensive care unit or died. Clinical improvement was verified in all patients after a maximum of 21 days. Conclusions The cases reported here highlight the challenges in differential diagnosis and the importance of considering concomitant infections, especially to improve clinical management and possible prevention measures. Failure to consider a SARS-CoV-2/DENV co-infection may impact both individual and community levels, especially in endemic areas.

Abstract Background In response to the COVID-19 pandemic and associated adoption of scarce resource allocation (SRA) policies, we sought to rapidly deploy a novel survey to ascertain community values and preferences for SRA and to test the utility of a brief intervention to improve knowledge of and values alignment with a new SRA policy. Given social distancing and precipitous evolution of the pandemic, Internet-enabled recruitment was deemed the best method to engage a community-based sample. We quantify the efficiency and acceptability of this Internet-based recruitment for engaging a trial cohort and describe the approach used for implementing a health-related trial entirely online using off-the-shelf tools. Methods We recruited 1971 adult participants (≥ 18 years) via engagement with community partners and organizations and outreach through direct and social media messaging. We quantified response rate and participant characteristics of our sample, examine sample representativeness, and evaluate potential non-response bias. Results Recruitment was similarly derived from direct referral from partner organizations and broader social media based outreach, with extremely low study entry from organic (non-invited) search activity. Of social media platforms, Facebook was the highest yield recruitment source. Bot activity was present but minimal and identifiable through meta-data and engagement behavior. Recruited participants differed from broader populations in terms of sex, ethnicity, and education, but had similar prevalence of chronic conditions. Retention was satisfactory, with entrance into the first follow-up survey for 61% of those invited. Conclusions We demonstrate that rapid recruitment into a longitudinal intervention trial via social media is feasible, efficient, and acceptable. Recruitment in conjunction with community partners representing target populations, and with outreach across multiple platforms, is recommended to optimize sample size and diversity. Trial implementation, engagement tracking, and retention are feasible with off-the-shelf tools using preexisting platforms. Trial registration ClinicalTrials.gov NCT04373135. President Trump’s communications during the novel coronavirus (COVID-19) pandemic violate principles of public health, such as practicing transparency and deferring to medical experts. Moreover, the president’s communications are dangerous and misleading, and his lack of leadership during the crisis limits the nation’s response to the problem, increases political polarization around public health issues of social distancing, and spreads incorrect information about health-related policies and medical procedures. To correct the dangerous path that the nation is on, the administration needs to adopt a more expert-centered approach to the crisis, and President Trump needs to practice compassion, empathy, and transparency in his communications.

Patient engagement in self health and wellness management has been identified as an important goal in improving health outcomes. As a result, the use of mobile and social media for health and wellness promotion is gathering considerable momentum. Several early adopting health plans and provider organizations have begun to design and pilot social and mobile media platforms to empower members to enhance self management of health and wellness goals. Based on a member survey of a large health plan in Pennsylvania, the authors identify factors that are significantly associated with member interest in adopting such technology platforms for obtaining health related information and services. Analysis of relevant data from more than 4,000 responses from health plan members indicate significant effects of several factors such as age, gender, general health condition (including presence of chronic conditions like diabetes and high blood pressure), level of computer and social media usage and frequency of engaging in different online activities such as banking, shopping, and emailing. This analysis allows us to identify important consumer segments that are correlated with professed willingness to use applications and programs offered by the health plan. Besides, the authors also develop statistical models to predict people's odds of adopting health-related mobile apps and identify the significant predictors thereof. The authors anticipate that these insights can assist health plans to develop and deploy targeted services and tools through integration of mobile and social media platforms for health and wellness management.

Social media has been used as data resource in a growing number of health-related research. The objectives of this study were to identify content volume and sentiment polarity of social media records relevant to healthcare services in China. A list of the key words of healthcare services were used to extract data from WeChat and Qzone, between June 2017 and September 2017. The data were put into a corpus, where content analyses were performed using Tencent natural language processing (NLP). The final corpus contained approximately 29 million records. Records on patient safety were the most frequently mentioned topic (approximately 8.73 million, 30.1% of the corpus), with the contents on humanistic care having received the least social media references (0.43 Million, 1.5%). Sentiment analyses showed 36.1%, 16.4%, and 47.4% of positive, neutral, and negative emotions, respectively. The doctor-patient relationship category had the highest proportion of negative contents (74.9%), followed by service efficiency (59.5%), and nursing service (53.0%). Neutral disposition was found to be the highest (30.4%) in the contents on appointment-booking services. This study added evidence to the magnitude and direction of public perceptions on healthcare services in China’s hospital and pointed to the possibility of monitoring healthcare service improvement, using readily available data in social media.

Social media platforms are home to large volumes of ambiguous hashtag-based claims about the health, modified-risk, and cessation benefits of electronic cigarette products (e.g., #Vapingsavedmylife). The objective of this study was to qualitatively explore how young adults interpret these hashtags on the popular platform Instagram. Specifically, we sought to identify if they view these hashtags as making health-related claims, and if they find these claims to be credible and valid. We conducted 12 focus groups in 2018 with non–tobacco users, smokers, dual users, and vapers between the ages of 18 and 24 ( n = 69). Using real Instagram posts to guide discussion, participants reflected on the meaning of potentially claims-making hashtags. Participants interpreted the majority of the hashtags as making health-related claims. However, many participants felt that the claims were too exaggerated to be entirely valid. Some participants, including dual users and vapers, argued that smoking and vaping were largely equivalent. Smokers were particularly skeptical of claims. Findings suggest that the U. S. Food and Drug Administration should consider hashtag-based claims in their regulatory efforts. However, further research is needed on how to pragmatically address claims taking the form of hashtags given legal and practical constraints.

Background: The widespread use of social media represents an unprecedented opportunity for health promotion. We have more information and evidence-based health related knowledge, for instance about healthy habits or possible risk behaviors. However, these tools also carry some disadvantages since they also open the door to new social and health risks, in particular during health emergencies. This systematic review aims to study the determinants of infodemics during disease outbreaks, drawing on both quantitative and qualitative methods.Methods: We searched research articles in PubMed, Scopus, Medline, Embase, CINAHL, Sociological abstracts, Cochrane Library, and Web of Science. Additional research works were included by searching bibliographies of electronically retrieved review articles.Results: Finally, 42 studies were included in the review. Five determinants of infodemics were identified: (1) information sources; (2) online communities' structure and consensus; (3) communication channels (i.e., mass media, social media, forums, and websites); (4) messages content (i.e., quality of information, sensationalism, etc.,); and (5) context (e.g., social consensus, health emergencies, public opinion, etc.). Studied selected in this systematic review identified different measures to combat misinformation during outbreaks.Conclusion: The clarity of the health promotion messages has been proven essential to prevent the spread of a particular disease and to avoid potential risks, but it is also fundamental to understand the network structure of social media platforms and the emergency context where misinformation might dynamically evolve. Therefore, in order to prevent future infodemics, special attention will need to be paid both to increase the visibility of evidence-based knowledge generated by health organizations and academia, and to detect the possible sources of mis/disinformation.
