Aging is a status often associated with multiple comorbidities which require pharmacologic intervention and complex medication regimens. Aging population results to the increase of chronic diseases and subsequent comorbidities that require concomitant multiple medications. It is reported that about 80 % of elderly have at least one chronic condition, and about half of them have at least two (such as heart disease, hypertension, diabetes mellitus, arthritis, and cancer). According the literature the worldwide polypharmacy prevalence has a wide range (between 5 to 78 %) due to different definitions on the number of medications taken (ranging between 2 and 9) and the different samples studied. In most studies polypharmacy was more common between women and in more elderly people. Many multicenter studies have been performed to define the term of polypharmacy and its prevalence in the elderly population. Polypharmacy is of growing concern for the older adults, because it can be very dangerous for this population due to altered absorption, distribution, metabolism and excretion of the drugs within their body reflecting unexpected pharmacokinetics and pharmacodynamics of various medications. This fact can lead to adverse drug reactions (ADR), drug interactions, noncompliance and reduced adherence, reduced functional status, geriatric syndromes, high risk of hospitalization and possible death. Over the last decade, there are several evaluation tools which can help the General Practitioner prevent the polypharmacy in the elderly. As prescribing for this group of individuals is a very challenging procedure, the General Practitioner should balance between under-treatment, over-treatment and risks and benefits. This review concluded that we need more cross-sectional clinical studies on practical preventive interventions to be applied not only on the population which is already exposed to polypharmacy, but also to the general population.

Background The elderly population is in increasing trend all over the world. Elderly people experience many physiological, biological and psychological changes. The general objective of the study is to assess the utilization of health care services among the elderly population of Butwal sub-metropolitan city.Methods A quantitative descriptive-cross sectional study, with a total number of 212 elderly people residing in Butwal sub-metropolitan city, was selected for the study purpose. The study period was from March to October 2015. The samples were randomly selected from the sampling frame of 3 different wards of Butwal sub-metropolitan city. Information was collected using an interviewer administered semi-structured questionnaire. Chi-square test was used to identify the factors associated with the utilization of health care services among elderly population.Results Our findings reveal that 84.4% of elderly people visited to the health facility during past 1 year. There were more females (50.9%) than males (49.1%). Majority of elderly people were diagnosed with one of the chronic ailment; were under regular medication. More than 4/5th (84.9%) of older adults rated “satisfactory” to their health condition. Monthly family income, chronic disease, elderly on medication and self-rated health status are statistically significant with utilization of health care services at 95% level of confidence.Conclusions The finding suggests that monthly family income, chronic disease, elderly on medication and self rated health are strongly associated with utilization of health care services by elderly people. Each year in the world, especially in Indonesia, many elderly people exposed to higher risk of dementia due to an increased number of the elderly population and the incidence of mild cognitive impairment in the elderly. The aim of this study is to find out what factors are related with cognitive function in the elderly population. The study used analytic method with cross sectional study design. Subjects were 60 elderly people; male or female ≥ 60 years old. Data were analysed with Chi Square Test (α = 0.05). Cognitive function was measured by MMSE test. The result were found a significant relation between age and cognitive function ( p = 0.014), level of education and cognitive function (p value = 0.000), physical activity and cognitive function (p value = 0.003), cognitive activity and cognitive function (p value = 0.002), and social interaction and cognitive function (p value = 0.000). Gender variable did not have relation with cognitive function (p value = 0.343). This study conclude that there was a relation between age, education level, physical activity, cognitive activity and social interaction with cognitive function in the elderly population. There was no relation between gender and cognitive function in the elderly population.Keywords: cognitive function, cognitive impairment, elderly, MMSE

The elderly population is currently increasing. Based on population projection data, it is estimated that in 2017 there are 23.66 million elderly people in Indonesia (9.03%). It is predicted that the number of elderly people in 2020 (27.08 million), 2025 (33.69 million), 2030 (40.95 million) and 2035 (48.19 million). Data obtained from cadres showed that the elderly had low awareness of joining the posyandu for the elderly. one cader said that the majority of the elderly rarely visit the posyandu for the elderly (only 40% are active). This research is a quantitative study with cross sectional design. The research was conducted at the elderly Posyandu in the Puskesmas Paal X Work Area with 36 respondents in December 2019-January 2020. Data were analyzed univariat and bivariat with the Chi Square test. The results showed that there was a relationship between family support and the utilization of the elderly posyandu, there was no relationship between education and distance from the house with the use of the elderly posyandu in the elderly posyandu in the working area of Puskesmas Paal X, Jambi City.

ObjectivesThis study was aimed to assess the willingness of elderly people to seek medical care for tuberculosis (TB) and the associated influencing factors.DesignA cross-sectional study.SettingA multistage random survey was conducted in Bao’an District of Shenzhen in China.ParticipantsA total of 1200 elderly people aged 65 or above were recruited for the study and completed a structured questionnaire between September and October 2019.Main outcome measuresDescriptive and binary logistic stepwise regression analyses were conducted to analyse the characteristics of elderly individuals, their willingness to seek medical care for TB and associated factors.ResultsAmong the final 1123 respondents, 943 (84.0%) were willing to seek medical care if they discovered suspicious TB symptoms. Binary logistic stepwise regression analysis indicated that respondents whose family annual income per capita was 50 000–100 000¥ (OR=2.56, 95% CI: 1.44 to 4.54, p<0.01) and who had positive attitudes (≥3 scores: OR=3.10, 95% CI: 1.90 to 5.05, p<0.01) or practices (≥4 scores: OR=3.13, 95% CI: 1.82 to 5.39, p<0.01) towards TB were more willing to seek medical care for TB.ConclusionsWillingness to seek medical care for TB in the elderly population can be improved according to the determinants.

Usia harapan hidup periode 2015-2020 menjadi 73,2 tahun sehingga mempengaruhi estimasi proporsi penduduk lansia. Jumlah Lansia Indonesia akan mencapai 28,8 juta orang atau 11,34 persen pada tahun 2020. Sebaran penduduk lansia tahun 2017 di Indonesia, pada urutan ketiga tertinggi ditempati oleh Jawa Timur yaitu 2,9 juta (12,25%) lebih dari 10% sehingga Jawa Timur bisa dikategorikan sebagai provinsi dengan penduduk tua (aging population) dengan jumlah lansia di Malang diperkirakan pada tahun 2020 sebesar 371.977 (Badan Pusat Statistik, 2015). Berdasarkan studi pendahuluan di Desa Toyomarto Pebruari 2018 diperoleh 320 lansia potensial. Sesuai hasil wawancara dengan 10 lansia potensial diperoleh data 70% masih aktif bekerja, 40% aktif dalam kegiatan kemasyarakatan dan 100% mandiri dalam pemenuhan kebutuhan sehari-hari, 70% mendapat dukungan keluarga sebagian dari dimensi (fisik, emosional, lingkungan, intelektual, profesional vokasional, sosial kemasyarakatan dan spiritual. Penelitian ini merupakan penelitian cross sectional. Populasi dalam penelitian ini adalah seluruh lansia potensial yang tinggal di Desa Toyomarto Kecamatan Ardimulyo sejumlah 320 orang. Sedangkan sampel dalam penelitian adalah sebagian lansia yang tinggal di Desa Toyomarto Kecamatan Singosari Kabupaten Malang sebesar 30 responden. Tehnik pengambilan sampel yang digunakan adalah simple random sampling. Analisis yang digunakan dalam penelitian ini adalah uji korelasi gamma diperoleh nilai p=0,000 yang menunjukkan bahwa hubungan pelayanan lansia berbasis kekerabatan dengan lansia tangguh bermakna. Dukungan keluarga dalam membina lansia belum optimal sehingga perlu mendapatkan perhatian agar kualitas dukungan menjadi baik. Perlunya peningkatan kualitas pembinaan oleh keluarga/kerabat dalam memberikan pelayanan kepada lansia dapat dilaksanakan dengan cara menjalin kerjasama lintas program maupun sektor agar terwujudnya lansia tangguh akan semakin nyata dan berlanjut hingga pemanfaatan keterlibatan lansia tangguh dalam mengisi pembangunan kesehatan khususnya kesehatan keluarga dan masyarakat. The life expectancy in 2015-2020 was 73.2 years, thus affecting the estimated proportion of the elderly population. The number of elderly in Indonesia will reach 28.8 million people or 11.34 percent by 2020. The distribution of the elderly population in 2017 in Indonesia, East Java  was the third highest by 2.9 million (12.25%), more than 10%, categorized as a province with an aging population (aging population) with the number of elderly in Malang estimated in 371,977 on 2020 (Badan Pusat Statistik, 2015). Based on preliminary studies in Toyomarto Village, February 2018, there were 320 potential elderly people. Based on the interview of 10 potential elderly, 70% were still actively working, 40% were active in community activities, 100% were independent in fulfilling their daily needs, 70% have family support in some part of dimensions (physical, emotional, environmental, intellectual, vocational professional, social and spiritual). This study aimed to analyze the correlation of kinship-based elderly service (emotional, spiritual, social, physical, environmental, intellectual, vocational professional) and resilient elderly in Toyomarto Village, Singosari District, Malang Regency. The study was a cross sectional study. The population was 320 people of all potential elderly living in Toyomarto Village, Ardimulyo Subdistrict. The sample was the majority of elderly people living in Toyomarto Village, Singosari Subdistrict, Malang Regency for 30 respondents. The sampling technique used simple random sampling. The analysis used gamma correlation test which obtained p value = 0,000 indicated that there was a correlation of elderly-based services and resilient elderly. The need to improve quality of guidance by families / relatives in providing services to elderly can be carried out by establishing cross-program and sector collaboration so that the realization of strong elderly people will become more apparent and continue to utilize involvement of strong elderly people in fulfilling health development, especially family and community health.

Background: World population is aging, in its truest sense of the term. Therefore socio-economic condition of the elderly population is changing constantly, which is adversely affected by various health problems during their way of life course. This study aimed to describe the relation between different comorbidities and social problems of the elderly people. Methods: A community-based cross-sectional study was carried out by house to house visit in an urban setting in greater Kolkata area, the field practice area of a family physician. A total of 208 elderly patients (60 years old and above) were interviewed using a pre-tested schedule. Study findings were recorded in terms of proportions and percentages and its correlation to social problems was searched. Results: Male older adults were more than their female counterparts, though widows were more in numbers than widowers. Most of them were having multiple morbidities (87.98%). Visual impairment was the commonest problem detected in the studied elderly population with prevalence of 75.96% (158/208). Prevalence of hypertension was 68.75% (143/208). Other significant morbidities seen are auditory, musculoskeletal, urinary, cardiac etc. Conclusions: The result of the study showed that health problem has a significant relation with increasing age in human beings. Awareness generation should be done among the elderly people for regular health check-ups to ensure prevention and early detection of many chronic diseases. So the strategies are required to bring about the quality of life in health, which should be an equal responsibility for individual, family, society and the government.

Background: Ageing is an inevitable process with some specific and distinctive health problems. With increase in life expectancy the elderly population is increasing day by day, so is the concern for the different aspects of their health problems. Objectives: The present study was conducted to find out the proportion of morbidity and morbidity pattern among the elderly population (60 years and above) of a selected rural area of Bangladesh. Methodology: This was a cross-sectional type of descriptive study was conducted from January, 2019 to April, 2019. Two hundred and twenty one elderly people from Kaliakoir Upazilla under the district of Gazipur were interviewed. Data were collected by face to face interview through an interviewer administered semi-structured questionnaire. The study place was selected conveniently and the sampling technique was purposive type of non-probability sampling. Results: Out of 221 respondents 91% (201) had morbidity of any kind related to their health. Common forms of morbidities were musculoskeletal problems (50.3%), hypertension (43.8%), PUD and hyperacidity (27.4%), diabetes mellitus (21.4%) and respiratory problems (19.4%). The mean age of the respondents in this study was 64.97 (± 6.120) years. Maximum proportion (55.2%) belonged to the age group of 60 to 64 years. Among the total 221 respondents 55.7% were female and 44.3% were male. Morbidity among the males was 88.8% and among the females was 92.7%.Most (86.4%) of the respondents were Muslims. Majority (81%) of the respondents was married and only 19% were widow/ers. Fifty five percent of the respondents were literate having different levels of education. The most common occupation among the males was farmer (40%) and among the females was housewife (51.1%).The mean monthly family income was Tk.18,800(±Tk.19210.336) and majority (53.8%) belonged to middle income group (Tk.10001 to Tk.30000). Most (53.8%) of the respondents lived in joint family. Fifty two percent lived in semi pucca house, 78.7% used tube wells as their source of drinking water but unexpectedly 18.6% still used non-sanitary latrines. Out of 201 morbid respondents majority (60.2%) were diagnosed by MBBS doctors and 60.7% were diagnosed in government hospitals. Most (86.6%) of the respondents took some sort of treatment and 10% did not take any treatment. Remaining 3.5% took partial treatment. Most common form of treatment taken by the respondents was medicine (87.4%). Majority (60%) of 20 respondents who did not take any kind of treatment was due to poverty and rest 40% was due to negligence. Conclusion: To build up awareness and to explore the situation in detail, in-depth community based epidemiological study may be carried out. This paper discusses the contribution and effectiveness of old age allowance among the elderly people and also comes up with the factors associated with the satisfaction level of old age allowance. A cross sectional descriptive study was carried out among 136 elderly people receiving old age allowance in Kushma Municipality, Parbat. The information was collected from people aged 60 years and above through interviews using semi-structured questionnaires. The blend of qualitative and quantitative data was employed for the study. The majority of the elderly population was in the age group of 70-75 years with 68 percent elderly people illiterate. The result shows a significant association of the level of satisfaction of the allowances received with the physical infrastructure and socio-economic development regardless of age, gender and caste/ethnicity. This gives an impetus for making policies and programmes for the elderly in order for them to live with happiness and satisfaction.

Minimum Standards Service is basic service quality regulation that is entitled to be obtained by every citizen. Elderly people must obtain 100% health services. Various factors can lead to low visits to the elderly posyandu in the Gading Puskesmas area in 2017. Objective: to find out the relationship between family support, knowledge, and elderly activities in the elderly posyandu in Gading Surabaya puskesmas. Method: The type of research is observational analytic with a cross-sectional design. Data processing and analysis is done by univariate and bivariate. Data collection techniques by conducting simple random sampling with a sample of 94 elderly people. Location and time of research from December 2018 at the posyandu in the Gading Surabaya health center area. Results: There is a significant value of 0,000 <0,05, so it can be concluded is have a relationship with family support, knowledge of elderly with activity following the elderly posyandu at Gading Surabaya health center

Insomnia is one of the major and unsolved problems in older people. Most of the sleep studies report that the different forms of insomnia like Difficulty Initiating Sleep (DIS), Difficulty Maintaining Sleep (DMS) and Non-Restorative Sleep (NRS) are common among the elderly that are associated to many factors. The objective of the study was to measure the prevalence of insomnia and the factors associated to it among the elderly people. A cross-sectional study was conducted among 142 elderly people of and above 60 years of age in Sarangdanda VDC of Panchthar District of Eastern Nepal. The presence or absence of insomnia and the associated factors were assessed on them by the help of interview schedule. The results were analyzed using chi-square test in SPSS (version 11.5). DMS was the most common reported form of insomnia among the elderly followed by DIS and NRS. Association between insomnia and each of factors like use of tobacco before sleeping hours, eating too close to bedtime, use of tea/coffee before sleeping hours and use of alcohol before sleeping hours were statistically significant at 95% level of confidence. Insomnia affects a large proportion of elderly and is triggered by many factors like use of tobacco before sleeping hours, use of tea/coffee before sleeping hours, eating too close to bedtime and use of alcohol before sleeping hours.
