Background. In recent years, the prevalence of allergic diseases of the anterior chamber of the eye has increased. They are often accompanied by the development of dry eye syndrome, which can complicate their course. Therefore, it is important to take a comprehensive approach to their diagnosis and treatment. The purpose was to study the effectiveness of 0.4% sodium hyaluronate (Gilays) in children with dry eye syndrome on the background of allergic diseases of the conjunctiva. Materials and methods. The study involved 42 patients (84 eyes) aged 3 to 18 years, who were examined and treated on the basis of Regional Eye Clinic. Patients were treated with local H-1 histamine blockers twice a day for 14 days, f necessary — with topical glucocorticoids by the scheme for 12 days. In the first group (21 patients, 42 eyes), the main treatment was added with the drugs containing 0.2% sodium hyaluronate, used topically 4 times a day for 14 days. In the second group (21 patients/42 eyes), 0.4% sodium hyaluronate (Gilays) was added to the treatment regimen. H-1 histamine blockers were not used systemically. Before starting therapy and 2 weeks later, the following values were evaluated: the changes in the anterior chamber of the eye using biomicroscopy, determination of lacrimal film rupture time, Schirmer 1 test, questionnaire. Conclusions. The use of sodium hyaluronate in the complex therapy of dry eye syndrome in allergic eye diseases is quite effective. The use of 0.4% sodium hyaluronate was more effective compared to the control group. It is advisable to use a questionnaire to communicate with the child, but it is necessary to refine the form of questions to eliminate as much as possible the subjectivity of the data.

The results of observation of patients who underwent vision eximerlaser correction by the LASEK method regarding medium degree of hyperopia were analyzed.Purpose:to investigate the severity of the «dry eye» syndrome and corneal subepithelial fibroplasia in patients after laser correction of  hyperopia by the LASEK method, and their prevention by reducing  the preservative action on the eye surface.Patients and methods.The study included 40 patients (76 eyes), aged 21–46 years (32.98 ± 1.79) who underwent LASEK surgery for  moderate hyperopia (4.03 ± 0.38 D). Patients were divided into two  groups: 1) control group (20 patients, 37 eyes) — postoperative  therapy included the corneoprotector “Stilavit®” and the preparation  of hyaluronic acid “Oksial®”, containing preservatives;  2) the study group (20 patients, 39 eyes) — postoperative therapy  included the corneoprotector “Hilozar-comod®” and the artificial tear  preparation “Hilo-comod®”, which did not contain reservatives. Ophthalmic examination included, in addition to standard diagnostic  methods, the Norn test, the Schirmer test, the staining of the eye  surface with lissamine green. In addition, a questionnaire was  conducted on a modified OSDI questionnaire; there a section  characterizing the quality of vision (section B) was removed.Results.The analysis of the obtained data showed that in the study group there was less pronounced “dry eye” syndrome at 3,6,12  months after the operation. The maximum difference in the  indicators of the “dry eye” syndrome between the groups was at 3  and 6 months of the postoperative period. When evaluating corneal  subepithelial fibroplasia, maximum attention was paid to the  frequency and the intensity index. It was shown a significant  difference in the intensity of the haze between the study group and the control group by 26.1 % at 3 months, by 62.2 % at 6  months and by 65.3 % 12 months after the operation. Conclusion.Nonconservative therapy with sodium hyaluronate and  dexapanthenol in the early postoperative period and only sodium  hydrolurate allows to reduce the severity of the dry eye syndrome  after laser correction of hypermetropia and, as a result, to reduce  the intensity of haze development by 65.3 %, and its frequency on 11,5 %.

Purpose To verify the presence of dry eye syndrome (DES) in treated patients with glaucoma and to analyze DES's impact on the patients' quality of life (QOL) versus the control group. Methods In this observational cross-sectional study, 61 patients were enrolled at a clinical practice. Patients were divided into three groups by number of glaucoma drops instilled per day (G1=1 drop/day, G2=2 drops/day, G3=3 drops/day). A control group of 20 subjects was also selected (G0). All subjects were submitted to a complete ocular examination (including tear function and ocular surface status) and completed the 25-item National Eye Institute Visual Function Questionnaire (NEI-VFQ), Glaucoma Symptom Scale (GSS) questionnaire, and Ocular Surface Disease Index (OSDI). DES was defined as presence of punctate keratitis and decreased break-up time. Statistical analysis was performed applying the Kruskal-Wallis analysis of variance and Mann-Whitney U tests (to compare median values between groups) as well as the χ2 and Fisher test (to verify significant differences). Results A total of 40% of G3 and 39% of G2 patients presented DES versus 11% of G1 and 5% of G0 (p=0.01). QOL was significantly influenced and altered (NEI-VFQ 25 total mean and GSS total mean and symptoms average: p=0.0085, p=0.006, and p=0.03, respectively). OSDI pointed out differences by group: 26% of G2 and 15% of G3 presented moderate OSDI and 15% of G3 and 8.7% of G2 severe OSDI (p>0.05). Conclusions Patients with topically treated glaucoma present DES more often than a similar control group (p=0.01). The presence of DES negatively influences the patient's QOL. The patients with glaucoma's ocular surface status should be evaluated regularly to ensure the timely detection and treatment of pathologic signs on the ocular surface.

Purpose.To conduct a comparative analysis of the effectiveness of assessing the stability of the tear film using corneotopography and the classic Norn test in patients with myopia and myopic astigmatism before keratorefractive surgery. Material and methods. The study included 26 patients planned for keratorefractive surgery for myopia and myopic astigmatism, aged 18 to 47 years. 7 people there was a mild degree of myopia, in 14 people – medium degree, in 5 people – high degree. The average degree of corneal astigmatism was 2.25 diopters. The stability of the pre-corneal tear film was assessed using computed keratotopography and Norn's test – the tear film rupture time (TFRT) was deter- mined. A standard questionnaire was also used to assess the severity of symptoms of dry eye syndrome. A comparison was made of the indicators of the stability of the tear film obtained by the methods of corneotopography and the Norn's test, with their subsequent comparison with the data of the questionnaire. Results. In 17 patients (65.4%), the TFRT index corresponded to the age norm. In the remaining 9 people (34.6%), indicators of stability of the tear film were reduced both by corneotopography (12±4 sec) and by Norn's test (13.5±4.5 sec), and there were also subjective signs of dry eye syndrome according to data questionnaire. The data turned out to be comparable. Conclusions. 1. Among the total population of patients, 34.6% were persons with impaired tear film stability and subjective manifestations of dry eye syndrome. 2. Assessment of the stability of the tear film, determined using the method of corneal topography, showed a high degree of comparability with the results of Norn's test. A significant advantage of the keratotopography method is its non-invasiveness. 3. It is necessary to pay attention to the condition of the ocular surface and identify risk groups to avoid chronicity or aggravation of existing disorders. The article presents the results of Ocuhyl C using in the treatment of «dry eye syndrome» (DES) in 47 eyes of 26 patients with different stages of compensated primary open-angle glaucoma. Experience of instillation of hypotensive eye drops with preservatives was from 1 year to 35 years. Assessment of subjective signs of DES was performed by questioning. Functional tests were performed (for the stability of the precorneal tear film –Norn test, for the level of total tear production –Shirmer test). Examinations were carried out at the first examination before prescribing the drug, then after 4 and 8 weeks of its use. All the patients noted good tolerability of the drug, no side effects. Indicators characterizing the feeling of a foreign body, dryness and redness of the eyes have significantly decreased. Precorneal tear film rupture time has significantly increased. Purpose. To determine and study the features of the development of dry eye syndrome (DES) in gout, depending on urate-lowering therapy, the duration of the gout disease and the concentration of uric acid in the tear. Material and methods. The study involved 149 male patients (261 eyes) aged 35 to 75 years (mean age 60.63 ± 11.00 years). Among them, 110 people (202 eyes) with an established diagnosis: "Cataract with concomitant gout disease" and 39 people (59 eyes) with cataracts without clinical and laboratory signs of gout. All patients underwent both standard ophthalmological examinations and special ones. The concentration of uric acid in tears was determined by high performance liquid chromatography (HPLC). The diagnosis of gout was confirmed according to the criteria recommended by ACR / EULAR (2016). Results. In patients with gout who do not take a urate-lowering drug, DES occurs more often (82.1%) than in patients without gout (23.1%). With an increase in the concentration of uric acid in the tear, there is a high tendency to a shortening of the tear film rupture time (strong correlation coefficient r=-0.51, p<0.001) and to a decrease in tear production (Schirmer I test) (strong correlation coefficient r=-0.59, p<0.001). Conclusion. The data obtained indicate clear changes in the functional parameters of total tear production (Schirmer I's test) and the stability of the pre-corneal tear film (Norn's test) in patients with gout. This can be considered as a fact of the predisposition of this category of patients to the development of DES. A strong dependence of changes in tear production and HRVP on the concentration of uric acid in the tear was established. There is an obvious need for additional examination of this category of patients and an adequate choice of therapy, as well as the timely appointment of "artificial tear" drugs. Background. Determination of local epithelial cells expressing TLR2 as an indicator of local immunity in allergic rhinitis (AR) patients with opportunistic species of staphylococci nasal mucosa colonization. Methods. Washed epithelium samples obtained from patients with seasonal AR (n=8) aged 19—42 years, and perennial AR (n=15) aged 19 —45 years, as well as a control group (20 patients) aged 19—45 years without allergic diseases were investigated. Epithelial cells expressing TLR2 receptors were determined by flow cytometry. Results. The level of epithelial cells expressing TLR2 receptor in patients with seasonal AR was almost in 1,9 times, in perennial AR group — in 1,7 times lower then in healthy individuals. In patients with perennial AR S. aureus was obtained in 96,0% (CI: 79,5—99,2), in association with Str. pyogenes in 29% (CI: 14,9—49,2), Neisseria spp. — in 63,0% (CI: 42,7—78,8). Seasonal allergic rhinitis was characterized by association of S. aureus and S. hemolyticus (70,0%, 44,4—85,8). Conclusion. Ratio of epithelial cells positive for TLR2 in nasal lavage from patients with AR was lower than in healthy volunteers. Indicators proportion of epithelial cells expressing TLR2 in nasal lavage in patients with seasonal AR during an exacerbation period was significantly reduced (1,7—1,9 times), in comparison with healthy volunteers. In AR patients with increased density of staphylococci strains in nasal mucosa increased local epithelial cells expressing TLR2 in nasal lavage was established.

Postsurgical dry eye syndrome is found in a substantial proportion of patients after phacoemulsification. Its onset could be explained by surgical trauma and by use of preservative-containing eye drops. In some patients, it causes substantial discomfort that continues unabated after 3-4 weeks after surgery. Purpose. To investigate the results of 0.3% sodium hyaluronate instillation therapy used to treat dry eye syndrome after phacoemulsification. Materials and methods. Basal tear secretion (Schirmer II test) and tear breakup time (TBUT) test were investigated in 33 patients with symptoms characteristic of dry eye syndrome in 1 month after phacoemulsification. These patients received 0.3% preservative-free sodium hyaluronate instillations for 30 days, whereupon Schirmer II and TBUT tests were repeated. Symptom dynamics was estimated according to the OSDI questionnaire scale. Results. Mean values of basal tear secretion and TBUT tests were slightly below normal ones. In one month of treatment, the TBUT index associated with “Gilan Ultra Comfort” use became somewhat better: mean TBUT value appeared to be 9.8 ± 2.5 sec. OSDI questionnaire patient symptom score evaluation also revealed in patients a mild degree of xerosis severity. Mean score before treatment was 19.6 ± 10.0. Positive disease dynamics after “Gilan Ultra Comfort” use is confirmed by a significant decrease of OSDI score index up to 12.3 ± 6.3.

Aim. Comparative analysis of the effectiveness of keratotopography methods and classical Norn’s test in assessing the stability of the tear film (TF) in patients with myopia before photorefractive surgery. Methods. The study involved 26 patients. All of them were planned for keratorefractive surgery to correct myopic refraction and astigmatism. Their age averaged 32.5±14.5 years (18-47 years). The stability of pre-corneal tear film was assessed using computed keratotopography and Norn’s test - the tear film rupture time (TFRT) was determined. A standard questionnaire was also used to assess the severity of symptoms of dry eye syndrome (DES). A comparison was made of the indicators of tear film, stability obtained by the methods of keratotopography and Norn’s test, with their subsequent comparison with the data of the questionnaire. Results. In 17 patients (65.4 %), TFRT index corresponded to the age norm. In the remaining 9 people (34.6 %), indicators of tear film stability were reduced both by keratotopography (12±4 sec) and by Norn’s test (13.5± 4.5 sec), and there were also subjective signs of dry eye syndrome according to questionnaire data. The data turned out to be comparable. Conclusion. The incidence of impaired stability of the TF and subjective manifestations of DES among patients planned for keratorefractive surgery for myopia and myopic astigmatism was significant and, according to our data, made 34.6 %. To prevent complicated postoperative course of photorefractive surgery, it is necessary to assess the condition of the ocular surface immediately before refractive surgery, to identify the risk group for DES development.
