The article presents modern data on the features of the oral microbiome and its medical significance when prescribing systemic antibacterial therapy for infectious and inflammatory processes in the oral cavity. The main microbial associations in normal conditions, as well as in diseases of periodontal tissues and apical periodontitis are presented. The use of antibacterial drugs in dentistry is aimed at eliminating the etiological factor of the pathogenic microflora of the oral cavity. The effectiveness of the treatment depends on how well the doctor aware of the main pathogens and microbial associations in generalized periodontitis, periodontitis, a choice of optimal antibacterial agent, compliance with indications and contraindications upon treatment.

The concentration of plasmalogen bacterial and endotoxin levels in the saliva of patients with different severity of periodontal disease, injury prosthetic bed and with various degrees of the oral cavity microbiocenosis violations was studied. Determination of the presence of the pathological process was carried out clinically, according to the condition of periodontal tissues. The degree of microbiological disorders was assessed by the quantitative ratio of the types of microorganisms isolated from the smear taken from the gingival groove. It was found that the concentration of plasmalogen for normal microbiocenosis is not less than 0.7 µg/g. For the intermediate type of microbiocenosis, the concentration of 1.82 µg/g was determined; for dysbiosis - 5.64 µg/g, and for the expressed violation of the microbial composition accompanied by inflammatory processes - 6.54 µg/g. An increase in the concentration of bacterial endotoxin (be) more than 6.25 nanomole/g indicates the pronounced inflammatory process, regardless of the determined intensity of contamination of opportunistic gram-negative microflora.

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

This article is a review of the work of domestic and foreign authors on the effect of smoking on periodontal tissues, the mechanism of action of nicotine on the microvasculature, hygiene and soft tissues of the oral cavity. A modern diagnostic method using the Florida Probe computer system is considered. A treatment plan is proposed for patients with inflammatory periodontal diseases, the etiological factor of which is exposure to nicotine.This article is a review of the work of domestic and foreign authors on the effect of smoking on periodontal tissues, the mechanism of action of nicotine on the microvasculature, hygiene and soft tissues of the oral cavity. The diet is related to the diversity of bacteria in the oral cavity, and the less diverse microbiota of the oral cavity may favor the growth of pathogenic bacteria of all bacterial complexes. Literature data indicate that disturbances in the balance of the bacterial flora of the oral cavity seem to contribute to both oral diseases, including periodontitis, and systemic diseases. If left untreated, periodontitis can damage the gums and alveolar bones. Improper modern eating habits have an impact on the oral microbiome and the gut microbiome, which increase the risk of several chronic diseases, including inflammatory bowel disease, obesity, type 2 diabetes, cardiovascular disease and cancer. The subject of our consideration is the influence of the traditional diet on the formation of oxidative stress and inflammation caused by bacterial biofilm in the oral cavity. Through dental, biomedical and laboratory studies, we wanted to investigate the effect of individual nutrients contained in specific diets on the induction of oxidative stress inducing inflammation of the soft tissues in the oral cavity in the presence of residual supra- and subgingival biofilm. In our research we used different types of diets marked as W, T, B, F and noninvasively collected biological material in the form of bacterial inoculum from volunteers. The analyzed material was grown on complete and selective media against specific strains of all bacterial complexes. Additionally, the zones of growth inhibition were analyzed based on the disc diffusion method. The research was supplemented with dental and periodontological indicators. The research was supplemented by the application of molecular biology methods related to bacterial DNA isolation, PCR reactions and sequencing. Such selected methods constitute an ideal screening test for the analysis of oral bacterial microbiota. The obtained results suggest that certain types of diet can be an effective prophylaxis in the treatment of civilization diseases such as inflammation of the oral cavity along with periodontal tissues and gingival pockets.

Introduction. The tissues surrounding the implant, which have common features with periodontal tissues, but still possessing a certain specificity are the actual topic of the study. A considerable amount of textbooks has been written on this topic, but so far it has remained little studied. Purpose ― to summarize all knowledge of the structural features of the tissues surrounding the implant, etiology, pathogenesis, clinical picture, differential diagnosis of the pathologies of these tissues. Results. This review is devoted to the analysis of the modern understanding of the influence of the structural features of the soft tissues surrounding the implant and the nonspecific protection factors of the oral mucosa on the potentiation of inflammatory processes and their relationship with the biodegradation of the material. The work of scientists on the criteria for choosing an implantable material depending on the resistance to aggressive environments of the oral cavity, as well as biocompatibility with the tissues of the oral cavity. The risks of complications are considered depending on the implantable material. The etiopathogenesis of such diseases as implantation mucositis and dental periimplantitis is also considered. The clinical picture of these diseases is described, a correlation is made with the materials used for the manufacture of suprastructures. The prevention of complications arising from prosthetics on dental implants is considered.

Resume The most informative methods of diagnosing dermatological diseases in dogs are a clinical study of the skin and its derivatives, and to identify the main etiological factor, dermatological tests are used to identify signs of ectoparasitosis. In the treatment of pyotraumatic dermatitis in dogs, after eliminating the effects of the main etiological factor, the most effective is the use of janus kinase inhibitors against the background of local and systemic antibacterial therapy.

Diseases of periodontal tissues occupy one of the leading positions among modern dental problems, namely the multifactorial nature of these diseases. In modern dental science, the issue of the development of periodontal pathology against the background of somatic pathology and risk factors remains relevant. Pathology of periodontal tissues in 68–90 % of cases is accompanied by chronic diseases of the gastrointestinal tract. Today, there is no doubt that Helicobacter pylori infection can be present in the biotopes of the oral cavity and can affect the course of periodontal pathology. As you know, smoking is one of the important risk factors for the development of inflammatory-dystrophic diseases of periodontal tissues, which can aggravate the course of the latter. The purpose of the work is to determine the prevalence of oral Helicobacter pylori infection in tobacco-dependent patients with chronic generalized periodontitis on the background of chronic hyperacid gastritis during treatment. Patients who received the proposed therapeutic and prophylactic complex (ultraphonophoresis procedures with the created gel «Apisan», and probiotic drug BioGaia ProDentis and angioprotective drug of natural origin — Detralex) showed a gradual decrease in the level of total urease activity and, as a consequence, a decrease the prevalence of Helicobacter pylori infection in the oral cavity according to the results of a urease rapid test with material from the oral cavity, both in the presence of a risk factor — smoking, and in its absence. The use of the proposed therapeutic and prophylactic complex proved to be effective in reducing the prevalence of oral Helicobacter pylori infection in smoking patients and patients who do not smoke, with chronic generalized periodontitis against the background of chronic hyperacidal gastritis associated with Helicobacter pylori.

Relevance. A long-term inflammation in the oral cavity, unreasonable treatment of periodontal patients with antibiotics cause imbalance between certain types of fungal-bacterial associations accompanied by oral dysbiosis and change of local immune status. So, development and application of new comprehensive diagnosis and treatment techniques in periodontal patients, use of products for correction of microbiota and local immunity are a current and a long-term task. Purpose is to optimize the comprehensive treatment protocol of inflammatory periodontal diseases by introducing probiotics and evaluation of oral microbiome and cytokine profile.Materials and methods. 140 patients were examined. Of these 60 patients had chronic generalized plaque-induced gingivitis and 60 patients suffered from moderate chronic generalized periodontitis. Bacterial and fungal microbiome was assessed and the host immune response was evaluated in all patients before and after the treatment. Half of the patients were treated conventionally and the other half were treated according to a modified scheme: probiotic “Bifidumbacterin Forte” was added.Results. A large number of yeast-like fungi Candida and commensal bacteria were detected in periodontal pockets of patients with chronic generalized plaque-induced gingivitis and moderate chronic generalized periodontitis. That correlates with a significant increase of pro-inflammatory cytokines (TNF-α, IL—8), decrease of concentration of INF-γ and increase of antiinflammatory cytokine IL-4.Conclusion. Changes in clinical, microbiological and immunological values during a modified combination therapy with a probiotic and during a conventional treatment demonstrated that effectiveness of treatment of chronic gingivitis and chronic periodontitis increased by 40% and 37% respectively.

Relevance. Before starting the therapeutic treatment of various periodontal diseases, it should be clearly understood that a high-quality treatment is possible only with complex therapy, excluding local irritating adverse factors in the oral cavity, elimination of inflammatory processes in the periodontal tissues, as well as violations of its function. One of the main tasks of the treatment of periodontal diseases is the preservation of the dentition with a single functional system.Purpose – to assess the use of the drug Kollapan in tooth extraction and treatment of periodontitis.Materials and methods. The clinical observation group consisted of 40 patients aged 19 to 65 years. The gender ratio was 3 : 2 (60% men, 40% women). The largest percentage of patients (65%) came to the clinic with complex tooth extraction. The remaining 35% – with generalized periodontitis of moderate and mild severity.Results. Clinical observation showed that in the first group of patients, whose therapy included the use of Collapan, after a complex tooth extraction, physiological healing of the hole was observed, the disappearance of inflammation within 24–36 hours after the surgery

Objectives Feline gingivostomatitis (FGS) is a painful chronic inflammatory disease of the oral cavity. The purpose of this study was to examine the frequency of detection of certain common feline bacteria and viruses to determine any potential associations with FGS. Methods A multicentre case-control study design was conducted. In total, 72 control cats and 32 cats with FGS were included in the study. Oral swabs were cultured for bacterial identification and a PCR assay was carried out to examine the infection of feline calicivirus (FCV), feline herpesvirus-1 (FHV-1), Chlamydia felis, Mycoplasma felis and Bordetella bronchiseptica. Results There was a significant difference in age distribution between the control and the FGS group. Based on a PCR assay, the positive rate of FCV was significantly higher in FGS cats than control animals. For other infectious pathogens, including FHV-1, C felis and M felis, there was no significant difference. Bacterial culture of oral swabs revealed that Pasteurella multocida was most frequently detected, but the detection rate was significantly lower in FGS cats. In FGS cats, the incidence of Enterococcus faecalis and anaerobic bacteria were more frequently isolated than in control cats. Conclusions and relevance This study indicates that the positive rate of FCV was significantly higher in cats with FGS, and the microflora of the oral cavity of cats with FGS might be disrupted, although additional studies are required to compare the oral microbiome in cats of a variety of ages.
