Abstract Background Despite growing demand for medicinal plants, there is little data about their use by patients with dyslipidemia. We aimed to determine the prevalence, pattern, and associated factors for the use of medicinal plants among patients with dyslipidemia. Methods A 17-item semi-structured questionnaire was filled out by 195 patients with dyslipidemia in a cross-sectional study carried out in two academic endocrinology clinics in Shiraz, Iran. The questionnaire comprised of three main domains of demographic data (6 questions), clinical data (2 of them), and data related to the use of medicinal plants (totally 9 questions). Results A total of 77.4% of patients took medicinal plants. The most common medicinal herbs used by dyslipidemic patients were Zataria multiflora, Cinnamomum zeylanicum, and Zingiber officinale. Duration of dyslipidemia was significantly longer in herbal users than non-herbal users (p=0.04). Patients believing that concomitant use of conventional drugs and herbal preparations had synergic positive effects in addition to those persuaded that herbal preparations possessed less side effects, were significantly more likely to use medicinal plants (p=0.008 and 0.005, respectively). Additionally, most of the medicinal herb users (87.4%) changed neither the pattern nor the dosage of their medications all during herbal preparations use. Conclusions This study demonstrated a high prevalence of medicinal plants’ use among patients with dyslipidemia, which was associated with the duration of dyslipidemia, patients’ viewpoints about herbal preparations’ synergic positive effects, and their fewer side effects.

Gastroesophageal Reflux Disease (GERD) is a form of gastrointestinal motility disorder, where stomach contents reenter the esophagus and oral cavity, causing symptoms and complications. GERD is a condition that is quite often experienced, where the prevalence estimated at 8 - 33% worldwide. One of the suspected cause of  GERD is Ramadan fasting, which has been routinely carried out by Muslim groups. This study aims to prove whether Ramadan fasting triggers GERD. A cross-sectional study (survey) conducted online via Google form on the last three days of the fasting month (21 May 2020 - 23 May 2020). The variables in this study were respondents who fasted Ramadan and those who did not fast, also the total value of the GERD-Q questionnaire along with the final conclusions. Statistical analysis using Chi square with Yates Correction and Independent T-test with Mann Whitney Alternative Test. 311 respondents met the inclusion criteria. The results of Mann Whitney statistical test found that there was no difference in the mean value of the total GERD-Q questionnaire between the fasting and non-fasting groups (p-value: 0.313). Pearson Chi Square with Yates Correction results found no significant relationship between fasting and incidence of GERD (p-value: 0.552), although clinically there was a possibility of fasting had a risk of 1,228 (95% CI: 0.772 -2,088) times to trigger GERD incident.as Conclusion, Ramadan fasting has not been shown to improve GERD symptoms. Further research needs to be done through longitudinal studies.

Background: Karnataka is one of the six high human immunodeficiency virus (HIV) prevalent states in India. We estimated prevalence among primigravida attending antenatal clinics in Karnataka, assuming this as a proxy for HIV incidence level in the general population. Methods: We tried estimating prevalence among primigravida using cross sectional samples. Data was collected in structured data extraction sheet for the month of September 2011, from all Integrated and Counselling tested Centres (ICTCs) of Karnataka. All the pregnant women were tested as per national protocol. We analysed the basic demographic data, geographical distribution including HIV status of spouse of primigravida. Results: In September 2011, 87580, pregnant women were tested and 238 (0.26%) were found HIV positive of which, 95 (40%) were primigravida. Prevalence among primigravida, was 0.3%. The prevalence among primigravida was highest in Bagalkot (1.6%) district. In Yadgir, Kodagu and Udupi the prevalence was zero. The high prevalent blocks were Jamakhandi, Mudhol, Gokak, Hospet and Muddebihal. 73.7% spouse of positive primigravida were tested for HIV and among those tested, 87.1% were found HIV positive. Conclusions: There is striking difference in the prevalence of HIV among primigravida in different districts of Karnataka probably indicates the difference in effectiveness of preventive interventions in these districts and within blocks. The preventive programs should be reached out to the labourer's and farmers in the general population to prevent the new infections in the general population.

We examined the impact of a group-based self-empowerment intervention among diabetes patients, which uses multidisciplinary education, collaborative learning, peer support, and development of diabetes-specific social capital to improve glycemic control and weight management. Thirty-five patients who had primary care established at the Prisma Health Upstate, Internal Medicine Resident clinic and held the diagnosis of diabetes for longer than one year were recruited for our single-arm pilot intervention. Each group intervention session involved one to two internal medicine resident physician facilitators, a clinical diabetic educator, and 5–10 patients. Each session had a framework facilitated by the resident, with most of the discussion being patient-led, aiming to provide a collaborative learning environment and create a support group atmosphere to encourage self-empowerment. Patients’ hemoglobin A1c level and body mass index (BMI) before the intervention and 3 to 6 months after completion were collected from the laboratory results obtained in the participants’ routine clinic visits. All graduates from this three-week intervention were invited to attend monthly maintenance sessions, and we tracked the HgbA1c measures of 29 JUMP graduates one year after the intervention, even though 13 of the 29 chose not to participate in the monthly maintenance sessions. The pre-intervention HgbA1c level averaged 8.84%, whereas the post-intervention HgbA1c level averaged 7.81%. A paired t test showed that this pre–post difference of 1.03 percentage points was statistically significant (p = 0.0007). For BMI, there was an average decline of 0.78 from the pre-intervention mean value of 40.56 to the post-intervention mean value of 39.78 (p = 0.03). Among the 29 participants who agreed to participate in our follow-up measure of their HgbA1c status one year after the intervention, a paired t test showed that there was no significant difference between the post-JUMP measure and the follow-up measure (p = 0.808). There was no statistically significant difference between the HgbA1c level of those participating in the maintenance program and that of those not participating (post-intervention t test of between-group difference: p = 0.271; follow-up t test of between-group difference: p = 0.457). Our single-arm, pilot study of the three-week group intervention of self-empowerment shows promising results in glycemic control and weight loss. The short duration and small number of sessions expected could make it more feasible for implementation and dissemination as compared with popular intervention protocols that require much longer periods of attendance, if the effectiveness of this patient group-based self-empowerment approach can be further established by randomized controlled studies in the future.

Introduction & aimsEstimate the prevalence of cigarette smoking and some of the related factors among schizophrenic and other hospitalized psychiatric patients.MethodsThis was a cross-sectional study on 120 patients hospitalized in Shahid Beheshti hospital in Kerman in 2005. Patients were equally devided in two groups of schizophrenia and other psychiatric disorders. Sampling was based on statistical census and data were collected using a questionnaire including 27 questions on demographic data, psychiatric disorder, smoking cigarettes and other substances, and Fagerstrom test. Data were analyzed by Chi-square and ANOVA tests using SPSS software.ResultsPrevalence and severity of cigarette smoking was 71.6% and 6.47% among schizophrenic and 51.6% and 6.40% among other psychiatric patients, respectively and the difference was not significant. History of withdrawal was 25.6% and 58.1% in the schizophrenia and other disorders respectively and the difference was significant (P < 0.05). Addiction to other substances was 51.6% in schizophrenic and 45% in the other patients and the most prevalent substances in both groups were opium and alcohol. The severity of smoking cigarettes was 6.9 along with other drug abuses and 5.1 in cases with just smoking based on Fagerstrom test and the difference was significant (P < 0.05).ConclusionsThe prevalence of cigarette smoking in both schizophrenia and other psychiatric patients is higher than normal population, but there is no significant difference between these two groups. Schizophrenic patients need persistent supportive and supervising programs for cigarette smoking abuse treatment because of their cognitive, motivate and social problems.
