AbstractDeath can be caused by a number of diseases such as infectious diseases, disease not causing injury. Thepattern of death and cause of death is one indicator used to assess health programs. Objective: to findout the images of the causes and causes of death in 2011 in Bekasi Regency. Methods: Articles are thesubsequent analysis of the study of the Health Control Model Based on Death Registrations and Causesof Death in 12 Districts / Cities in Indonesia. The method is an analysis of national agriculture in 2011.Population: all death events in Bekasi Regency. Samples: deaths of residents of Bekasi Regency who diedin 2011. Instruments used: 1) AV1 Questionnaire, to collect information on perinatal deaths, namely fetal/ infant mortality with a womb age of more than 22 weeks until the age of 6 days and advanced neonataldeaths, 7 days old infants up to 27 days. 2). The AV2 questionnaire, collected information on death,aged 28 days and five years, 5 years. 3) AV3 Questionnaire, collect the 5-year death report above andthe Death Cause Information Form (FKPK). Results: The number of deaths obtained in 2011 was 5,011deaths. As many as 82.7% of deaths were at home and 15.6% were hospitalized. Most deaths were in men(56.1%) compared to women (43.9%). The cause of death due to adolescents is not higher (61.4%) thanother causes. Space deaths increase sharply in the group 65 years and above. The cause of stroke wasdeath in Bekasi Regency in 2011. Conclusion: The main causes of the age group IUFD + 0-6 days werepremature. Diarrhea is the leading cause of death in the age group 29 months -4 years. TB is the leadingcause of death in women and number two in men aged 15-44 years. Stroke is the leading cause of deathin the age group 55-64 years, 65 years and over and in men aged 45-54 years. Kuesioner AV2, mengumpulkaninformasi kematian bayi berumur 28 hari sampai dibawah 5 tahun. 3) Kuesioner AV3, mengumpulkaninformasi kematian berusia 5 tahun keatas dan Formulir Keterangan Penyebab Kematian (FKPK). Hasil:Jumlah kematian yang didapatkan pada tahun 2011 adalah 5.011 kematian. Sebanyak 82,7% kematianterjadi di rumah dan 15,6% di rumah sakit. Kematian paling banyak pada laki-laki (56,1%) dibandingperempuan (43,9%). Penyebab kematian akibat penyakit tidak menular lebih tinggi (61,4%) dibandingkanpenyebab lain. Angka kematian meningkat tajam pada kelompok 65 tahun ke atas. Penyakit strokepenyebab kematian utamadi Kabupaten Bekasi tahun 2011. Kesimpulan: Penyebab kematian utama padakelompok umur IUFD +0-6 hari adalah karena prematur. Diare merupakan penyebab kematian utamapada kelompok umur 29 bulan-4 tahun. TBC merupakan penyebab kematian utama pada perempuandan nomor dua pada laki-laki umur 15-44 tahun. Stroke adalah penyebab kematian utama pada kelompokumur 55-64 tahun, 65 tahun ke atas dan pada laki-laki kelompok umur 45-54 tahun. Kata kunci: kematian, stroke, penyebab kematian, utama

The structure of the leading causes of death in Serbia has considerably changed in the last half century. Diseases which presented the main threat to the population a few decades ago are now at the level of a statistical error. On the one side are causes which drastically changed their share in total mortality in this time interval, while others have shown stability and persistence among the basic causes of death. Acute infectious diseases "have been replaced" with chronic noninfectious diseases, due to the improvement of general and health conditions. One of the consequences of such changes is increased life expectancy and a larger share of older population which resulted in cardiovascular diseases and tumors to dominate more and more in total mortality. Convergent trends in the structure of the leading causes of death in Serbia from the middle of the 20th century are the reasons why there are considerably fewer diseases and causes with a significant rate in total population mortality at the beginning of the 21st century. During the 1950s, there were five groups of diseases and causes which participated individually with more than 10% of population mortality (infectious diseases, heart and circulatory diseases, respiratory diseases, some perinatal conditions and undefined states) while at the beginning of the new century there were only two such groups (cardiovascular diseases and tumors). Identical trends exist in all European countries, as well as in the rest of the developed world. The leading causes of death in Serbia are cardiovascular diseases. An average of somewhat over 57.000 people died annually in the period from 2007 - 2009, which represents 55.5% of total population mortality. Women are more numerous among the deceased and this difference is increasing due to population feminization. The most frequent cause of death in Serbia, after heart and circulatory diseases, are tumors, which caused 21,415 deaths in 2009. Neoplasms are responsible for one fifth of all deaths. Their number has doubled in three decades, from 9,107 in 1975 to about 20,000 at the beginning of the 21st century, whereby tumors have become the fastest growing cause of death. Least changes in absolute number of deaths in the last half century were marked among violent deaths. Observed by gender, men are in average three times more numerous among violent deaths than women. In the middle of the 20th century in Serbia, one third of the deaths caused by violence were younger than 25 and as many as one half were younger than 35 years old. Only one tenth (11%) of total number of violent deaths were from the age group of 65 or older. At the end of the first decade of the 21st century (2009), the share of population younger than 25 in the total number of violent deaths was decreased four times (and amounted to 8%). At the same time, the rate of those older than 65 or more quadrupled (amounted to 39%).

Infectious diseases remain the leading cause of death in adolescents despite the improvements in public health that have occurred in the past decades. While mortality rates from infections are slowly declining in this age group, an exception is HIV, with HIV-related deaths having tripled in the last decade. As with other infections, the risk of acquiring HIV is partly explained by the biological and physical environment. However, the biological changes and the social and behavioural context of adolescence play an important role in determining risk. Notably, infections can result in long-term complications and consequent disability. While effective methods to prevent and treat many common infections do exist, the major challenges are to make these accessible to adolescents, an age-group that is often neglected by health programmes. In addition, adherence to treatment for chronic infections such as HIV, remains a major barrier to ensuring successful outcomes.

Introduction: Emergency services are the gateway between the community and hospital that provides 24-hour access for most needy patients in critical and emergency conditions. Mortality rate varies in emergency department across the world and even in different emergency units of the same hospital. This retrospective study was done in adult emergency services of a tertiary hospital to determine mortality rate and analyze causes of death. Methods: A retrospective observational study of mortality cases to analyze mortality rate and causes of death of patients for a period of 6 months between October 2017 to March 2018 was carried out in the adult emergency services of Tribhuvan University Teaching Hospital, Kathmandu. Data required were collected from copies of death certificates. Results: During the study period, a total of 128 patients died in emergency, accounting 0.5% of total patient. Male deaths (52.3%) were slightly higher compared to female deaths (47.7%). Age group 66-75 years had the highest (24.2%) of total mortalities in the emergency. The most common immediate cause of death was sepsis/septic shock (21.9%) followed by cardiopulmonary arrest, aspiration, respiratory failure, other causes of shock and poisoning. The commonest antecedent cause of death was attributed to respiratory causes. Similarly, the most common contributory cause of death was chronic obstructive pulmonary disease. Conclusion: Older age group is prone to the mortality risk. Sepsis/septic shock was the most common immediate cause of death. Pneumonia was the most common antecedent causes of death. Chronic obstructive pulmonary disease was the commonest contributory cause.

Introduction: The New Brunswick Trauma Registry is a database of injury admissions from eight hospitals throughout the province. Data tracks individuals in-hospital. By linking this information with vital statistics, we are able to observe outcomes post-discharge and can model health outcomes for participants. We want to know how outcomes for trauma patients compare with the general population post discharge. Methods: Using data from 2014-15, we followed over 2100 trauma registry observations for one year and tracked mortality rate per 1,000 people by age-group. We also compared the outcomes of this group to all Discharge Abstract Database (DAD) entries in the province (circa. 7500 total). We tracked mortality in-hospital, at six months, and one year after discharge. We truncated age into groups aged 40-64, 65-84, and 85 or older. Results: In-hospital mortality among those in the trauma registry is approximately 20 per 1,000 people for those age 40-64, 50 per 1,000 people for those aged 65-84, and 150 per 1,000 people aged 85 or older. For the oldest age group this is in line with the expected population mortality rate, for the younger two groups these estimates are approximately 2-4 times higher than expected mortality. The mortality at six-month follow-up for both of the younger groups remains higher than expected. At one-year follow-up, the mortality for the 65-84 age group returns to the expected population baseline, but is higher for those age 40-64. Causes of death for those who die in hospital are injury for nearly 50% of observations. After discharge, neoplasms and heart disease are the most common causes of death. Trends from the DAD are similar, with lower mortality overall. Of note, cardiac causes of death account for nearly as many deaths in the 6 months after the injury in the 40 -64 age group as the injury itself. Conclusion: Mortality rates remain high upon discharge for up to a year later for some age groups. Causes of death are not injury-related. Some evidence suggests that the injury could have been related to the eventual cause of death (e.g., dementia), but questions remain about the possibility for trauma-mitigating care increasing the risk of mortality from comorbidities. For example, cardiac death, which is largely preventable, is a significant cause of death in the 40-64 age group after discharge. Including an assessment of Framingham risk factors as part of the patients rehabilitation prescription may reduce mortality.

Abstract Background This work summarizes the confirmed causes of death of twenty-five free-ranging Eurasian brown bears ( Ursus arctos arctos ) from the Cantabrian mountain range submitted for necropsy in Asturias and Castilla y León (northwestern Spain) from 1998 to 2018. Results Causes of death were classified based on (i) pathological findings and (ii) caused by “human intervention” or “non human intervention”. In four cases (16%) it was not possible to determine the cause of death due to the bad preservation of found remains or insufficient tissue availability. Based on pathological findings seven out of the 21 (33.3%) brown bears in which the cause of death could be determined died due to infectious diseases (clostridiosis n =4 or infectious canine hepatitis n =3), two (9.5%) due to exertional (degenerative) myopathy (one of those bears also showed clostridiosis) and one due to strychnine poisoning (4.8%), neoplasia (4.8%) or mushroom poisoning (4.8%). The remaining animals died due to traumatic lesions (including fights or infanticide), shooting or wire snare. Mortality data was also classified by deaths caused by “human intervention” or “non human intervention”. The death of one bear showing exertional myopathy after handling was not ascribed to any of the former classifications. Six out of the 20 (30%) brown bears died as a consequence of “human intervention” due to illegal hunting (wire snare hunting n =3 or shooting n =2) and, strychnine poisoning ( n =1). In contrast, fourteen (14/20, 70%) brown bears died by “non-human intervention”; nine of them (9/20, 45%) due to traumatic lesions (fights n =4, traumas n =3 or infanticide n =2), three (3/20, 15%) due to infectious canine hepatitis caused by canine adenovirus type 1 (CAdV-1) infection, one (1/20, 5%) due to cholangiocarcinoma and another one (5%) due to mushroom poisoning. Conclusions This study shows that the main causes of death in Eurasian brown bears are those caused by infectious diseases. In contrast to previous data on free-ranging bears for the first time infectious diseases appear as an important cause of death. These data are valuable and may help in the conservation and management of this recovering population.

Background: Autopsy is a traditional method in pathology for the study of diseases or injuries, being key to elucidate the cause of death. However, the number of autopsies has been decreasing progressively. Design and Context: Retrospective cross-sectional study to analyze the presence of discrepancy between clinical and pathological diagnoses as to the cause of death according to the Goldman criteria, verify the epidemiological profile of the main causes of death, and tabulate the number of procedures conducted annually. Method: Analyzing clinical records and autopsy reports from the Department of Pathology and Legal Medicine of the Universidade Federal de Ciências da Saúde de Porto Alegre (UFCSPA) from 1963 to 2012 and performing statistical analysis on the data collected. Results: The predominant age group was of dead fetuses (30.6% of all cases). The main cause of death was infection (68.4% of diagnoses). After a peak in the early 1980s, there was a progressive drop in the rates of postmortem examination. In the 1990s, the average number of autopsies fell by 58% in relation to the previous decade, and the last decade of the Century registered a decrease of 80% as compared to the average of the 1980s. According to the Goldman criteria, there was discrepancy between ante- and postmortem diagnoses as to the cause of death in 26.2% of the cases. Conclusion: The rates of discrepancy between clinical diagnoses and autopsy findings regarding the cause of death remain high, even though medicine has become more and more advanced in technology.

Infant mortality is still a major problem in our country as its level has remained relatively high by European standards. This points to the need for better preventive measures particularly as regards infant mortality and other adverse consequences of pregnancy, as key indicators of health and health care for mother and child. Namely, the analysis of movement in infant mortality in low mortality countries shows that it can be decreased relatively easily if certain social and health care measures are undertaken. For that reason, it is necessary to engage in permanent organized research to explain and measure both the relative impact of individual factors or groups of factors in our country which are significant in terms of infant mortality and their mutual relationships. We should also try to gain from the experiences of other countries which had already made progress in this respect. One of the elements of prevention is certainly the analysis of causes of infant morbidity and mortality primarily during the pre-natal period with the aim of specifying the most frequent causes of death to enable their elimination and to induce a subsequent decline in infant mortality. Besides showing the efficiency of health service activities, data on causes of infant death also point to the specific measures that should be undertaken and may be used as a base for planning and programming the development of health services, i.e. implementation of health policy as part of the population policy. With the decline in infant mortality in our country there has also been registered a change in the composition of diseases as the most frequent cause of death. During the initial observation period when the general level of infant mortality was exceptionally high, the share of infectious diseases and those of the respiratory system was very large. These deaths were mainly induced by exogenous factors, that is the diseases which the society in general and health services in particular could most easily have checked both by measures to improve the general living conditions and by preventive and curative health care measures. The period from 1989 to 1996 is characterized by endogenous causes of infant mortality primarily during the neonatal period and have to do with the constitutional features of the live-born children, congenital anomalies, premature birth, respiratory distress, etc. Thus, from the socio-medical point of view, the primary causes of infant mortality in this period are genetically induced or can be attributed to the mother in labour birth which modern men and modern medicine cannot influence to a larger extent. The analysis of infant death frequency by group of causes of death points that there still exist possibilities of eliminating the exogenous causes of death (as the same causes prevail in the socio-economically least developed regions of the country). Besides, some improvement can also be expected in the area of endogenous mortality (improvement in pre-natal diagnostics and other measures of health care for pregnant women and those who have just given birth, better conditions for child delivery and application of modern techniques to care for the prematurely born children. The semanatal mortality is probably the major socio-medical problem in our country both because it accounts for the highest percentage in neo-natal mortality and because it displays an almost negligible downward tendency. This justifies another request - for a more extensive and comprehensive analysis of this problem as well as for participation of other scientific disciplines besides medicine. Among the leading causes of semanatal mortality in the most recent observation period are premature birth, congenital anomalies, respiratory distress syndrome and intrauterine hypoxia and asphyxia at birth.

Background: To assign a cause of death to non-medically certified deaths, verbal autopsies (VAs) are widely used to determine the cause of death. The time difference between the death and the VA interview, also referred to as recall time, varies depending on social and operational factors surrounding the death. We investigated the effect of recall time on the assignment of causes of death by VA. Methods: This is a secondary analysis of 2002-2015 survey data of the Nairobi Urban Health Demographic Surveillance System (NUHDSS). The independent variable recall time was derived from the date of death and the date when the VA was conducted. Univariate and multivariate logistic regression methods were used to calculate odds ratios of assigning a cause of death in defined categories of recall time. Results: There were 6218 deaths followed up between 2002 and 2016, out of which 5495 (88.3%) had VAs done. Recall time varied from 1-3001 days (mean 115.5 days, sd216.8). Majority of the VAs (45.7%) were conducted between 1-3 months after death. The effect of recall time varied for different diseases. Compared to VAs conducted between 1-3 months, there was a 24% higher likelihood of identifying HIV/AIDS as the cause of death for VAs conducted 4-6 months after death (AOR 1.24; 95% CI 1.01-1.54; p-value = 0.043) and a 40% increased chance of identifying other infectious diseases as the cause of death for VAs conducted <1 month after death (AOR 1.4; 95% CI 1.02-1.92, p-value = 0.024). Conclusions: Recall time affected the assignment of VA cause of death for HIV/AIDS, other infectious diseases and maternal/neonatal causes. Our analysis indicates that in the urban informal setting, VAs should be conducted from one month up to 6 months after the death to improve the probability of accurately assigning the cause of death.
