Omentin-1 is an anti-inflammatory adipokine secreted by stromal vascular cells. Inflammation and apoptosis of adipocyte tissue in obesity lead to decreased production of omentin-1. This study aims to find the difference in omentin-1 levels in the obese and non-obese groups and the correlation between levels of omentin-1 with BMI and waist circumference. This study was a cross-sectional study involving 70 subjects with 37 people in the obese group and 33 people in the non-obese group. Anthropometric data including weight, height, waist circumference, and BMI were measured. Serum omentin-1 levels were measured by ELISA. The statistical tests used were Chi-square, T-test, Mann-Whitney test, and Spearman test. Test results were significant if p-value < 0.05. Significant difference was observed in serum omentin-1 levels between obese and non-obese group (median 140,31 range (88.08 – 382.76) vs. 210,97 range (124,44–577,96) ng/mL), respectively; p < 0.001). Serum omentin-1 correlated negatively with BMI (p=0.001, r = - 0.398) and waist circumference (p=0.017, r = - 0.286). Obesity causes inflammation and increased death of adipocyte tissues due to apoptosis, autophagy, and fibrosis resulting in decreased production of omentin-1 by stromal vascular cells. Anthropometric parameters of waist circumference and BMI describe the distribution of adipocyte tissue and affect the secretion of omentin-1. Omentin-1 levels in the obese group were lower than in the non-obese group. The higher the BMI and waist circumference, the lower the omentin-1 level.

BACKGROUND: Safety climate is a common insight of staff that indicates individuals’ attitudes toward safety and priority of safety at work. OBJECTIVES: Nursing is a risky job where paying attention to safety is crucial. The assessment of the safety climate is one of the methods to measure the safety conditions in this occupation. The aim of this study was to assess the safety climate of rehabilitation nurses working in hospitals in Tehran. METHODS: This is a cross-sectional study which was carried out on 140 rehabilitation nurses selected from all hospitals and clinics in Tehran in 2019. To collect the required data, a two-section questionnaire was used. The first section was related to demographic factors and the second part (22 statements) was to measure the safety climate using nurses’ safety climate assessment questionnaire. The collected data were analyzed by SPSS V16 using independent t-test, ANOVA, Kruskal-Wallis and Mann-Whitney U test at the 5% level. RESULTS: Findings showed that the total mean of safety climate was 3.06±0.56. According to the results, a significant difference was found between the positive and negative satisfaction of nurses with safety climates (P-value = 0.03), communication with nurses (P-value = 0.01) and supervisors’ attitude (P-value = 0.02). Furthermore, a significant difference in safety climate between the individual with the second job and the individual without second could be observed (P-value = 0.01). CONCLUSIONS: The results indicated that the safety climate was not at an acceptable level. Thus, it is essential to introduce safety training courses (e.g. safety, work-rest balance, and so on) and to improve the safety performance at work.

Background: Several studies have demonstrated an association between obesity and migraine. It’s still unclear whether migraine is the cause or it's the result of obesity. Objective: We investigated the prevalence of migraine among obese and non-obese individuals and analyzed the relationship between migraine prevalence and obesity. Patients and Methods: This is cross-sectional observational study was carried out at the neurology outpatients’ department of the Rizgary Teaching Hospital in Erbil, Iraq, from July 1st , 2018 to September 30, 2019. We interviewed and examined 300 persons, both obese (n=154) and non-obese (n=146), and of both gender. All of them were adults (>18 years old). Each person's weight, height, body mass index (BMI), and waist circumference were measured. The diagnosis of migraine was made according to the International Headache Society’s criteria. Obesity was present if the individual’s BMI is ≥30 and/or waist-to-hip ratio is >0.9 in females and >1.0 in males. Results: Out of the 300 persons, 14 males (8.5%) got migraines while migraine was found in 37 females (27.4%), irrespective of their weight. Out of the 300 persons, the prevalence of migraine among obese individuals was 21.4% (n=33) and while in non-obese individuals was 12.3% (n=18). There was a statistically significant difference between the groups (p-value=0.045). The overall prevalence of migraine was 16.9% of the persons interviewed. Conclusion: The results showed that migraine is more prevalent among obese persons than non-obese persons and the difference was statistically significant. Background and Aim: Thalassemia major is one of the most common chronic genetic diseases in the world and in Iran that can impose many adverse effects on the self and society. Psychologically, patients with thalassemia are under stress. According to the World Health Organization (WHO), suicide is one of the leading causes of death worldwide. It is important to identify the people who have the highest risk factors for suicide. The present research investigates the frequency of thoughts, readiness and suicide attempts in people with thalassemia. Methods: This was a cross-sectional study. Its sample was 150 patients with thalassemia in the age range of 10 to 46 years whom we selected through census by sampling at convenience. We gave to patients a standardized Beck Suicide Thought Assessment Questionnaire. We analyzed the research data using descriptive and inferential statistical tests. Results: From among 150 patients with thalassemia in this study, 57.3% of the respondents were male and 42.7% were female. The results showed that there was a negative and significant relationship between suicide thoughts and attempts and age (-0.234) (P <0.05). The mean prevalence of suicide in men (1.89) was significantly higher compared to women (0.79) (P <0.05). We did not observe significant difference in prevalence of suicide thoughts and attempts between married and single groups with a mean difference (0.69) (P <0.05) and, in different educational groups, no significant difference was reported (P> 0.05). Conclusion: The findings of this research showed that among patients with thalassemia, the prevalence of suicidal ideation has a significant percentage. Since suicidal ideation provides a platform for preparation and attempt to commit suicide, so by providing mental health services and regular screening, we can take an effective step to prevent this problem among patients with thalassemia. The performance of nurses can be seen from several cases that occur in hospitals. The incidence of falls in patients, nosocomial infections, inadequate documentation is the result of nurses' low performance. Factors that affect the performance of nurses in hospitals are job satisfaction. So this study aims to describe the relationship between job satisfaction and nurse performance. This study used a cross-sectional study design with a sample of 85 nurses at Arosuka Hospital using proportional sampling technique. The research instrument used a job satisfaction survey questionnaire and the Individual Work Performance quasi (IWPQ) and statistical tests used frequency distribution and chi-square tests. The results showed that the majority of nurses' job satisfaction was in the satisfied category as much as 56.5% and the majority of nurses' performance in the high category was 51.8%. Then obtained a significant relationship between job satisfaction and nurse performance with a p-value of 0.000. So it is expected that hospitals can pay attention to aspects of nurse job satisfaction in making policies

Background: Calculating BMI in an individual is a standard anthropometric procedure to estimate overweight/obesity. But it has been observed to be a bad predictor of central obesity. On the other hand, waist circumference, in many studies, has been found to be a good predictor of central obesity but not so frequently used because of tendency to vary throughout the day. In the recent years, NC has been found to have a good correlation with both BMI and WC. The aim of this study was to find out whether neck circumference is a good predictor of Overweight/Obesity or not. Methods: This was a cross sectional study carried out among the school children, aged 12 to 15 years, in Pokhara city, Nepal. In total, 408 students, 238 males and 170 females were screened. Anthropometric markers of obesity measured: included body mass index (BMI), waist circumference (WC), and compared with neck circumference (NC) of the same subjects. Pearson’s correlation test was used to see the correlation between NC with BMI and WC, and receiver operating characteristic curve analysis was used to determine the best cut off value of neck circumference in predicting high BMI. Results: Among 408 students, 238 (58.3%) were male and 170 (41.7%) were female. Among them 37 (9.1%) were overweight and 32 (7.8%) were obese. All the anthropometric parameters were significantly higher in cases, except height in male, than in controls. NC was significantly correlated with age, BMI, and waist circumference in both boys and girls. The best cut-off value of neck circumference by ROC to identify boys with a high BMI was 29.5 with sensitivity of (76%), specificity (54%), and for girls was 28.5 with sensitivity of (97%), specificity (48%). Conclusion: Statistically significant positive correlation was found between NC with BMI and WC. The value of NC as a screening tool has been found comparably lower in compare to WC.

Background: Cigarette smoking has remained a popular habit since ages. Most people are well aware of the deleterious effects of cigarette smoking yet continue to give a blind eye which deteriorates overall public health. The purpose of the present study was to assess Peak Expiratory Flow Rate among smoking and nonsmoking staffs who work in Kathmandu Medical College. Methods: In this comparative cross-sectional study Peak Expiratory Flow Rate was obtained using Mini Wright’s Peak Flow Meter of 108 smokers and 108 nonsmokers in the age group ranging from 25-45 years. Those who never smoked or who have quit smoking for the past 2 years were grouped as nonsmokers and the smokers with history of smoking at least five or more cigarettes per day for at least two years were included in the study for measuring their Peak Expiratory Flow Rate. Data was collected, compiled and analyzed by using Statistical Package of Social Science (SPSS) software version16. Student ‘t’ test was used for group comparison. Results: The Peak Expiratory Flow Rate value was significantly reduced in the smokers (p value< 0.05). Mean Peak Expiratory Flow Rate was reduced with increasing age of the smokers. However, no significant difference was observed in Peak Expiratory Flow Rate with increase in the number of cigarettes smoked (p value> 0.05). Conclusions: In the study Peak Expiratory Flow Rate among smokers (367.13 ± 74.182) was lower than nonsmokers (471.39±60.842), which was statistically significant proving that cigarette smoking reduced peak expiratory flow rate.

Abstract Motivation A fundamental interest in chronobiology is to compare patterns between groups of rhythmic data. However, many existing methods are ill-equipped to derive statements concerning the statistical significance of differences between rhythms that may be visually apparent. This is attributed to both the form of data used (longitudinal versus cross-sectional) and the limitations of the statistical tests used to draw conclusions. Results To address this problem, we propose that a cosinusoidal curve with a particular parametrization be used to model and compare data of two sets of observations collected over a 24-h period. The novelty of our test is in the parametrization, which allows the explicit estimation of rhythmic parameters [mesor (the rhythm-adjusted mean level of a response variable around which a wave function oscillates), amplitude and phase], and simultaneously testing for statistical significance in all three parameters between two or more groups of datasets. A statistically significant difference between two groups, regarding each of these rhythmic parameters, is indicated by a P-value. The method is evaluated by applying the model to publicly available datasets, and is further exemplified by comparison to the currently recommended method, DODR. The results suggest that the method proposed may be highly sensitive to detect rhythmic differences between groups in phase, amplitude and mesor. Introduction: Though invasive intracranial devices are gold standard to calculate intracranial pressure (ICP); these are not without any complications. Non-invasive measurement of ICP by Ultrasonography could be a safe and portable technique. Objectives: The objective of the study was to measure and compare values of optic nerve sheath diameter of both eyes in healthy Nepalese adults.Methodology: A prospective cross-sectional study of healthy adult Nepalese volunteers was performed using a 7.5 MHz linear Ultrasound probe on the closed eyelids; optic nerve sheath diameter (ONSD) was measured 3 mm behind the globe in each eye. Results: Optic nerve sheath diameter (ONSD) of both eyes was measured in 100 healthy volunteers of age ranged from 15 to 75 years with a mean of 30.21 ± 14.05 years. There were 18 (18%) male and 82 (82%) female. ONSD for right eye ranged from 3.20 to 4.90 mm with mean of 4.10 ± 0.50 mm and left eye from 3.20 to 4.80 mm with mean of 4.22 ± 0.49 mm. P value for right and left eye ONSD (P = 0.06) and male and female (P = 0.12 and 0.85 for right and left ONSD respectively) were within normal limits. ONSD has no correlation with age (P = 0.27 and 0.27 for right and left ONSD respectively). Conclusion Mean of optic nerve sheath diameter (ONSD) is 4.10mm and 4.22 mm for right and left eye respectively. There is no statistical significant difference in mean of ONSD between right and left eye. BJHS 2018;3(1)5 : 357-360
