The use of certain drugs is known to be an extrinsic factor the risk of falling in the elderly (elderly) because it can cause postural hypotension, sedation, dizziness, weakness, drowsiness, muscle weakness, and other side effects. This study aims to determine the profile of drug use that have the potential to fall in the elderly. This study used a cross-sectional descriptive design with purposive sampling technique. The study was carried out at two regional hospitals in Madiun (RSUD Dr. Soedono Madiun and RSUD Kota Madiun). The data is sourced from the outpatient's medical record of Neurology and Internal Medicine clinics and from the medication record in the Pharmacy Installation from May to October 2018. The results showed that there were 343 patients (64.11%) out of 535 patients involved in the study using fall risk medicines (FRM). Three FRM groups that were often prescribed were antihypertensive 52.71% (282 patients), anticonvulsants 20.19% (108 patients), and benzodiazepines 10.47% (56 patients). A number of 46.54% (249 patients) used FRM included in the low-risk category (Medication Fall Risk Score 1-5), and as many as 17.57% (94 patients) were included in high risk (Medication Fall Risk Score ≥ 6). The elderly that use 1 FRM was 24.86% (133 patients), followed by 2 drugs was 19.07% (102 patients). The results of this study realized that the use of fall risk medicines in the elderly is quite high, so it is necessary to weigh the risks and benefits of its use to prevent falls in the elderly.

In the patient safety standards / International Patient Safety Goal (IPSG), especially the prevention of the risk of falling must be applied by nurses, age is one of the internal factors that affect compliance. This study aims to determine the correlation between age of nurses with adherence in the implementation of prevention of falling patients in Samarinda Government Hospital using a descriptive correlation design and cross sectional approach. The instrument used in this study was a questionnaire and observation sheet with a sample of 51 nurses in the non-intensive inpatient room with a purposive sampling technique. The analysis test in this study uses the Spearman rank test with the results of p value 0.026 (rho = 0.312), so it can be concluded that there is a significant influence between the age of nurses with adherence in the implementation of prevention of falling patients.

Purpose: National Institute for Health and Care Excellence (NICE) has recently published quality standards for assessment of fall risk and preventing further falls. According to the standards, multifactorial fall risk assessments should include: identification of falls history; analysis of gait, balance, mobility and muscle strength, among other factors. Despite being based on subjective analysis or simple timing and not being multifactorial, physiotherapists and physicians quite often use these tests as reference scales to differentiate between lower and higher risk of falling. Instrumented TUG has been recently reported to provide important additional information to the overall score. Objective: To explore a case-based approach of fall risk assessment to identify the most relevant and informative risk factors that in combination could better define a person risk profile. Materials and Methods: A multifactorial assessment of fall risk through questionnaires, standard functional tests, tests instrumented with inertial sensors, and force platforms has been studied within a group aged 55-80 years old. Different fall risk factors and fall risk assessment methods were analyzed in a case-based descriptive study. Results & Discussion: Subjects at higher risk of falling were identified based on their detailed profiles. A set of features were obtained from the instrumented standard tests differing significantly between subjects presenting higher or lower fall risk. Therefore, instrumenting conventional tests with wearables containing inertial sensors and force platforms gives more detailed and quantitative insights. This information can be used to better define and tailor fall prevention exercises and to improve the follow-up of the evolution of the subject.

Falls are the most worrying incidence of patients in the hospital and that has an impact on injury and even death. The incidence was second ranks of adverse event after medication errors. Hospitals are already making efforts to reduce the fall incident but in reality, the incidence of falls still occurs. Data obtained from March to September 2016 have 6 cases of incident patients falling from a total of 43 patient safety incidents. The results of the observations show that most of the fall prevention programs that have not been done are falling risk assessments. Fall risk assessment is the first step to prevent the occurrence of falls in the patient, if not done then the incident will occur. This study aims to explore the implementation of falls risk assessment conducted by nurses in the hospital wards. The research method using qualitative research with phenomenology approach. The population used is inpatient ward nurses as many as 304 nurses. The sample used by purposive sampling technique with 10 informants. Data collection using primary and secondary data. Primary data obtained by in-depth interview with semi-structured interview to all informants. Secondary data was used document review of SOP prevention of fall risk, assessment protocol, and patient's medical record status. Data analysis used Miles and Huberman analysis model by reducing data, making display data, and drawing conclusions. The results of the research are two themes that are the existence of internal training and socialization affects the nurse's understanding of the implementation of falls risk assessment both initial of falls risk assessment and re-assessment of falls risk and implementation of falls risk assessment influenced by the inhibiting and supporting factors, both of these factors affect compliance of falls risk assessment conducted by the nurse. This study suggests that the understanding of fall risk assessment does not guarantee the nurse to always adhere to the implementation of fall risk assessment. It is expected that there will be cooperation between hospital management, working group prevention of falls risk, and head of ward to always supervise and monitor evaluation related to implementation of falls risk assessment along with giving reward and punishment.

Early detection of older adults with an increased risk of falling could enable early onset of preventative measures. Currently used fall risk assessment tools have not proven sufficiently effective in differentiating between high and low fall risk in community-living older adults. There are a number of tests and measures available, but many timed and observation-based tools are performed on a flat floor without interaction with the surrounding. To improve falls prediction, measurements in other areas that challenge mobility in dynamic conditions and that take a persons’ own perception of steadiness into account should be further developed and evaluated as single or combined measures. The tools should be easy to apply in clinical practice or used as a self-assessment by the older adults themselves.

The goal of understanding fall risk in the elderly, prevention and protection is to improve clinical and care satisfaction. Another anticipatory method that can be used to predict falling conditions is the assessment of the risk of falling in the elderly. The Hendrich Fall Scale (HFS) and Morse Fall Scale (MFS) are a form of assessment to anticipate the risk of falling in the elderly in nursing homes for patients. The aim is to determine the effectiveness of the Hendrich Fall Scale and the Morse Fall Scale with an assessment of the Risk of Fall in the Elderly. The research design used in this study is a longitudinal comparative design. The total sample in this study was 40 elderly. This research was conducted at the Nursing Home Foundation Guna Budi Bakti Medan Labuhan. Data collection using the Hendrich Fall Scale and Morse Fall Scale. Data analysis using Chi Square. Fall risk assessment using the Hendrich Fall Scale (HFS), elderly people with a high risk of falling (25.0%), moderate risk of falling (65.0%). Fall risk assessment used the Morse Fall Scale (MFS), the elderly who had a high risk of falling (39.1%), moderate risk of falling (47.8%). It is recommended that seniors at risk of falling should be assessed using the MFS instrument.

Cukang Taneuh located in the village Kertayasa District Cijulang Kudat district is a tourist attraction that environment (ecotourism). Based on data Cukang Taneuh increase the number of tourists during the past four years, but a decline in the percentage of growth in the average number of visits are very significant. 2009 to 2010 the number of percentage increase amounted to only 9.24% of the target achievement of the increase in the number of tourists by 25% in 2010. Not achieving the target of tourist arrivals is presumably due to the lack of development of tourism products so that tourists experience burnout or boredom. One effort to improve and restore the number of tourists is the development of tourism products by using the technique of tourism opportunity spectrum. The purpose of this study was 1) to obtain findings regarding the development of tourism products by using the technique of tourism opportunity spectrum in tourist attractions Cukang Taneuh 2) To obtain findings regarding the decision to visit the tourist attractions Cukang Taneuh 3) To determine the effect of tourism product development by using the technique of tourism opportunity spectrum of the decision to visit the tourist attractions Cukang Taneuh. This research is descriptive and verification, because the method used is explanatory survey method using an ordinal scale. The analysis technique used in this research is the analysis of the path (path analysis), the sampling technique using systematic random sampling method through cross-sectional approach, the samples taken from the overall population of 150 visitors. The study of the hypothesis shows that the development of tourism products by using the technique of tourism opportunity spectrum consisting of accessibility, the characteristics of tourism facilities, social interaction, and the degree of management control has direct and indirect influence on the decision to visit the tourist attractions Cukang Taneuh 93.2% while the remaining 6.8% is influenced by other factors not examined in this study. The dimensions of the development of tourism products by using techniques spectrum of tourism opportunity to get high to low valuation is characteristic of tourism facilities, social interaction, the degree of management control, and accessibility
