AN ALTERNATIVE METHOD FOR OBTAINING A TWO-LAYER IMPRESSION WITH NON-REMOVABLE DENTAL PROSTHETICS

The main type of orthopedic treatment of the patient at the dentist is the manufacture of dentures to restore damaged or lost functions. Understanding and coordinated work of the doctor and dental technician in this case is achieved by transmitting information about the patient through the impression. It is the accuracy of the impression that affects the quality of the plaster model on which dentures are made. The article presents a description of the comparative characteristics of the time spent on taking impressions by various methods and the depth of penetration of the impression material into the "dental groove" when obtaining impressions by various methods on an experimental model.

Актуальность: на сегодняшний день практически в каждодневной практике врача стоматолога-ортопеда и зубного техника есть заказ-наряд на изготовление металлокерамических зубных протезов. С сожалением, приходится констатировать факт, что металлокерамические коронки и мостовидные протезы не являются совершенными конструкциями, несмотря на долголетний опыт применения, использование современных конструкционных и вспомогательных материалов, высоких технических и инструментальных возможностей ортопедической стоматологии и зуботехнического производства. Целью работы является повышения качества ортопедического лечения путем подробного анализа возможных ошибок, осложнений при изготовлении металлокерамических протезов и способов и способов их недопущения. Материалы и методы: систематизация причин, ошибок и осложнений -как предмет данного исследования, на клинико-лабораторных этапах изготовления металлокерамических реставраций и изучение методов устранения. Результаты: после изучения клинико-лабораторных этапов изготовления металлокерамических конструкций выявлены основные виды ошибок, которые были разделены на клинические, лабораторные и ошибки на этапах литья. Предметом настоящего исследования стало подробное изучение технологических ошибок. Нами было установлены причины ошибок на каждом лабораторном этапе изготовления и представлены рекомендации по их недопущению и методы устранения. Выявлено, что наиболее часто встречающие недочеты выявляют на этапах моделирования и отливки каркаса, а также на этапах нанесения и обжига керамических масс. Заключение: возникающие ошибки и осложнения резко сокращают сроки пользования металлокерамическими конструкциями, влияют на неудовлетворенность больных качеством ортопедического лечения, провоцируют конфликты и разбирательства, а главное резко снижают качество стоматологического здоровья и жизни человека Relevance: today, almost in the everyday practice of a dentist-orthopedist and dental technician, there is an order-order for the manufacture of metal-ceramic dentures. Unfortunately, it is necessary to state the fact that metal-ceramic crowns and bridges are not perfect structures, despite the long experience of application, the use of modern structural and auxiliary materials, high technical and instrumental capabilities of orthopedic dentistry and dental production. The aim of the work is to improve the quality of orthopedic treatment by a detailed analysis of possible errors, complications in the manufacture of metal-ceramic prostheses and methods and ways to prevent them. Materials and methods: systematization of causes, errors and complications - as the subject of this study, at the clinical and laboratory stages of the manufacture of metal-ceramic restorations and the study of methods of elimination. Results: after studying the clinical and laboratory stages of manufacturing metal-ceramic structures, the main types of errors were identified, which were divided into clinical, laboratory and errors at the casting stages. The subject of this study was a detailed study of technological errors. We have identified the causes of errors at each laboratory stage of manufacturing and provided recommendations for their prevention and methods of elimination. It is revealed that the most common defects are identified at the stages of modeling and casting the frame, as well as at the stages of applying and firing ceramic masses. Conclusion: the resulting errors and complications dramatically reduce the time of use of metal-ceramic structures, affect the dissatisfaction of patients with the quality of orthopedic treatment, provoke conflicts and proceedings, and most importantly, dramatically reduce the quality of dental health and human life

Purpose: Many printers are tempting at low prices, but later their accuracy turns out to be insufficient. The study has included checking the accuracy of printing and reproducing details of 3D printers used in dental technology and dentistry such as MultiJet Printing (ProJet MP3000, 3D Systems) and Fused Deposition Modelling (Inspire S2000, Tiertime). Design/methodology/approach: The 3D prints were created from scans of the maxillary gypsum model with the loss of left premolar. In the test, objects were set to the X and Y-axis. In order to check the dimensional differences after printing, scans of the printed models were superimposed on scans of the plaster model in the GOM Inspect V8 SR1 (Braunschweig, Germany). The focus was on the distance of scans from each other and a deviation map was created for each object. Findings: The average absolute value of deviations for each of models were equalled: FDMfor X-axis 0.06 ± 0.04 mm, for Y-axis 0.07 ± 0.04 mm; MJP- for X-axis- 0.04 ± 0.02 mm, for Y-axis- 0.06 ± 0.02 mm. A chart of arithmetic averages calculated for each tooth for the best printouts in each series show that higher deviation values exist in case of FDM printout. The models printed in the X-axis have smaller values of deviations from those printed in the Y-axis. Practical implications: MultiJet Printing technology can be used to create more precise models than the FDM, but these printouts meet the requirements of dimensional accuracy too. Originality/value: CAD / CAM technology in the future will exist in every dental technology laboratory so it is important to be aware of the way the 3D printers works. By paying attention to the quality of detail reproduction, a Dental Technician is able to choose the best 3D printer for them.

Abstract Spondylolisthesis is defined as a slippage of a vertebral body on the one below. Isthmic spondylolisthesis, caused by stress fracture or a developmental anomaly at the pars interarticularis, is the most common type of lumbar spondylolisthesis with a 6% incidence in adults. Isthmic lumbar spondylolisthesis is a frequent cause of disabling low-back and leg pain. The goal of this study is to improve the quality of treatment of these patients.

Nowadays, the problem of prosthetic with non-removable orthopedic structures is quite common and is widely described in domestic and foreign literature. Prosthetics of non-removable structures of prosthetics has a number of advantages: complete rehabilitation of biting function, chewing efficiency, aesthetic norm of a smile. Invisible prosthesis designs, usually small in size, have a short period of psychological and physiological adaptation, which has a good reflection on their long-term use by the patient. The examination of dental patients in the clinic of orthopedic dentistry with unsatisfactory results of prosthetic dentures after 5 years with orthopedic treatment allowed to highlight the main indicators of inapplicability regarding the further use of dentures in terms of biocompatibility, mechanical strength and aesthetic properties. The main aim of the study was to improve the quality of patients’ treatment by identifying the causes of treatment complications by an orthopedist using the method of comparative analysis during the examination of patients with unsatisfactory results of dentures using permanent dentures. To identify the causes of complications of orthopedic treatment, a clinical examination of patients with unsatisfactory results of treatment with permanent constructions of dentures on a cast base was performed. The criteria for evaluation were the following indicators: bad breath, gums pain and bleeding, pain and mobility of the supporting teeth, pain under the intermediate part, metallic taste, pain in the blood vessel, darkening of the lining, chipping the lining, obscuring gums, excessive abrasion, and cleavage. From anamnesis and records in the history of the disease, the period of difficulty was determined. Our research results indicate that a significant part of ineffective treatment is due to the wrong choice of auxiliary and structural materials. Given that the choice of materials is performed arbitrarily, in almost half of cases, taking into account the wishes of patients and their financial capabilities, including the physical, mechanical and clinical and technological characteristics of dental materials, in our opinion, it is necessary to create a computer model of rational choice of material, therapeutic and prophylactic measures to improve the effectiveness of orthopedic treatment for patients with defects in hard tissues of teeth and dentition with non-removable structures. The complex of the following studies will allow the orthopedic dentist to continue improving the quality of patients’ treatment at the clinical stages of treatment with non-removable dentures.

Relevance. Staged orthopedic treatment was used to improve the quality of life of patients who underwent radical maxillofacial surgeries for cancer.Patients and methods. 197 patients receiving treatment for maxillofacial cancer were observed at the Department of head and neck tumors, National Medical Research Centre for Oncology of the Ministry of Health of Russia, in 1998- 2018. All patients underwent radical surgical treatment resulting in postoperative defects of the upper jaw, soft tissues of the zygomatic- buccal-orbital region, nose, or auricle.Results. Removable obturator prostheses with various supporting and retaining elements were made for 159 (80.7 %) patients. Individual facial prostheses were made for 38 (19.3 %) patients: 17 (44.7 %) – external orbital prostheses, 14 (36.8 %) – external nasal prostheses, 6 (15.8 %) – external zygomatic- buccal-orbital prostheses, 1 (2.7 %) – external auricle prosthesis. Combined prostheses were made for 4 patients– removable upper jaw obturator and nose prosthe[1]sis; removable upper jaw obturator and eye prosthesis. Combined prostheses were fixed to each other using magnets. The results of maxillofacial prosthetics were evaluated according to the aesthetic requirements of the patients and their quality of life. Maxillofacial prostheses allowed a complete restoration of chewing, swallowing, and speaking, restored facial deformation, and improved the appearance of patients.Conclusions. Timely and comprehensive orthopedic treatment of patients with postoperative maxillofacial defects after radical surgeries for malignant tumors takes the main place in the complex of rehabilitation measures. Early elimination of extensive defects is aimed at maximum restoration of oral dysfunctions and appearance preservation. The apparent advantages of maxillofacial prostheses involve improvement of social adaptation and the quality of life of patients, which promotes complete rehabilitation and a return to socially useful activities.

The durability of concrete can be assessed based on the quality of the surface layer of concrete, for which there are a few standardized assessment test methods. Usually, the quality of the surface layer of concrete is evaluated based on its air and water permeability. The aim of this experiment was to compare the outputs which provide some methods for determining the permeability of the surface layer of concrete (GWT, ISAT, TPT, the depth of penetration of water under pressure). The measured values using the individual methods of assessing the permeability of the surface layer of concrete very closely correspond.

Aim of this systematic review was to assess the effects of orthopedic treatment for Class II malocclusion with Functional Appliances (FAs) on the dimensions of the upper airways. Eight databases were searched up to October 2020 for randomized or nonrandomized clinical studies on FA treatment of Class II patients with untreated control groups. After duplicate study selection, data extraction, and risk of bias assessment according to Cochrane guidelines, random effects meta-analyses of mean differences (MDs) and their 95% confidence intervals (CIs) were performed, followed by subgroup/meta-regression analyses and assessment of the quality of evidence. A total of 20 nonrandomized clinical studies (4 prospective/16 retrospective) including 969 patients (47.9% male; mean age 10.9 years) were identified. Orthopedic treatment with FAs was associated with increased oropharynx volume (MD = 2356.14 mm3; 95% CI = 1276.36 to 3435.92 mm3; p < 0.001) compared to natural growth. Additionally, significant increases in nasopharynx volume, minimal constricted axial area of pharyngeal airway, and airway were seen, while removable FAs showed considerably greater effects than fixed FAs (p = 0.04). Finally, patient age and treatment duration had a significant influence in the effect of FAs on airways, as had baseline matching and sample size adequacy. Clinical evidence on orthopedic Class II treatment with FAs is associated with increased upper airway dimensions. However, the quality of evidence is very low due to methodological issues of existing studies, while the clinical relevance of increases in airway dimensions remains unclear.

There is a persisting trend in automotive industry to increase quality of parts along with productivity while decreasing the cost. All of those three goals are possible to achieve with help of the optical forming analysis system. However, optical forming analysis system is not always capable of calculating an exact result and there are cases when there is no such system available. Therefore there is a need for verification of results provided by the optical forming analysis system. Finding another method for cases where there is no such system available is also desirable. The dependence between grain size and strain is examined in this paper. The grain size was selected as an evaluating parameter due to its connection to the strain and because it is relatively easily measured. Measuring of the grain size was proved to be an alternative method to optical forming analyses in order to determine the strain of press formed parts.

In this literature review details the diversity of methods for determining the chewing efficiency, currently existing in the dental practice. Chewing is one of the main functions of the dental system, determine its condition. Chewing efficiency largely depends on the state of the entire dentition. Therefore, the quality of orthopedic treatment at different stages of permissible assessed using various methods for determining the efficiency of chewing. The article discusses in detail current methods of assessing the function of chewing phases of orthopedic treatment. A brief description of the nature of each method definition chewing efficiency, according to the classification is presented in the literature. A comparative characteristic of the positive and negative aspects of the known methods is carried out. The authors assess their labor intensity, sensitivity, availability and distribution, diversity and quality of the used test material. A special place is the consideration of modern methods, based on an assessment of the effectiveness of chewing area of occlusal contact with the various techniques of computerized data analysis. The authors do attempt to explore the possibilities of various systems of computerized analysis of occlusion for the assessment of chewing efficiency, taking into account the accuracy of individual features and occlusal relationship. They emphasize efficiency, high precision of the studied method and the need for further development of this direction in the dental practice.
