Background: Tuberculosis (TBC) treatment regimens require the introduction of PMO (Drugs Swallowing Drug) standardized on International Standards for Tuberculosis Care (ISTC) with DOTS strategy (Directly Observed Treatment Shortcouse). The goal of controlling new challenges such as TB/HIV co-infection, Multi Drug Resistance (MDR), Extensively Drug Resistance (XDR) and Loss to follow-up. Objective: Type of cross sectional study. Population 35 PMO using saturated samples. Methods: The instruments used are questionnaires and in-depth interviews. Data analysis is univariate, bivariate and multivariate. Results: The result of the study was the relationship between the role of PMO from family members in the supervision of taking medicine (p=0,024), there was a relationship between the role of PMO from family members in drug taking assistance in Fasyankes (p=0,002) and there was relationship between the influence of PMO role from family member accompaniment of sputum checks in Fasyankes (p=0,005) to TBC patient compliance. Conclusion: There is a statistical correlation between the roles of PMO from the family towards adherence to tuberculosis patients. Suggestion of research is given to related parties be reformed TBC control policy related to election criteria of PMO coming from family member in Semarang regency.

Latar belakang: Penyakit Filariasis merupakan penyakit yang disebabkan oleh cacing filaria. Kota Pekalonganmerupakan kota dengan endemis filariasis dan telah dilakukan Program Pemberian Obat secara Massal (POPM) sejak tahun 2011 hingga 2015, Namun, hasil Survei Darah Jari (SDJ) menunjukkan nilai Mikrofilaria Rate di Kota  Pekalongan  masih  >  1%.  Penelitian  ini  bertujuan  untuk  menggambarkan  peran  Tenaga  Pelaksana Eliminasi (TPE) filariasis dan hambatan yang ditemui pada pelaksanaan POPM di Kota Pekalongan.Metode: Penelitian ini merupakan penelitian cross sectiona. Jumlah sampel 95 orang petugas TPE dengan menggunakan simple ramdom sampling. Pengumpulan data dengan wawancara menggunakan kuesioner.  Hasil: Hasil penelitian menunjukkan bahwa EP dalam memilih anggota keluarga target yang akan dirawat adalah optimal (63,2%). EP dalam membantu puskesmas menentukan dosis dan pemberian obat-obatan untuk masing-masing keluarga yang dibantu (52,6%). EP dalam merekam keluarga yang dibantu yang minum obat pada kartu sudah optimal (55,8%). EP dalam memantau dan mencatat reaksi perawatan yang mungkin timbul dan melaporkan kepada petugas kesehatan adalah optimal (61,1%).Simpulan: Peran tenaga pelaksana eliminasi TPE di Kota Pekalongan dalam menyeleksi anggota keluarga binaan yang akan diobati, dalam membantu puskesmas menentukan dosis dan pemberian obat pada setiap keluarga binaan, dalam pencatatan keluarga binaan yang meminum obat pada kartu, dan dalam pengawasan dan pencatatan reaksi pengobatan yang mungkin timbul serta pelaporan kepada petugas kesehatan sudah optimalKata kunci: Filariasis, Tenaga Pelaksana EliminasiABSTRACT Title: The role of Elimination Personnel (EP) implementing filariasis MDA in Pekalongan City Background: Filariasis is a disease caused by filarial worms, Pekalongan City has carried out filariasis MDA (Mass Drug Administration) since 2011-2015. However, the results of the Finger Blood Survey (SDJ) showed that the microfilaria rate was > 1%. This study aims to describe the role of Elimination Personnel (EP) and the obstacles faced in implementing filariasis MDA in Pekalongan City.Method: This study used a cross sectional research method. Sampling in this study used simple random side, total 95 respondents. Data obtained by structure interviews using questionnaires.Result: The results showed that EP in selecting the target family members to be treated is optimal (63.2%). EP in helping puskesmas determine the dosage and administration of medicines for each of the assisted families (52.6%). EP in recording the assisted families who drank the medicine on the card was optimal (55.8%). EP in monitoring and recording treatment reactions that may arise and reporting to health workers was optimal (61.1%).Conclusion: The roles of TPE elimination workers of Pekalongan City were optimum, such as selecting family member who will be treated, helping Puskesmas in deciding the dosage and giving the medicine to every treated family, record of treated family who take the medicine in the given card, and in the supervision and record of medical reaction that might happen and reporting it to health workers.Keywords: Filariasis, Elimination Personnel

In northern Ghana, families traditionally function as the main provider of care. The role of family, however, is becoming increasingly challenged with the social shifts in Ghanaian culture moving from extended kinship to nuclear households. This has implications for the care of women post obstetric fistula (OF) repair and their family members who assist them to integrate back into their lives prior to developing the condition. This research is part of a larger critical ethnographic study which explores a culture of reintegration. For this article, we draw attention to the findings related to the experience of family caregivers who care for women post OF repair in northern Ghana. It is suggested that although family caregivers are pleased to have their family member return home, there are many unanticipated physical, emotional, and economic challenges. Findings lead to recommendations for enhancing the reintegration process and the need for adequate caregiving support.

AbstractMedulloblastoma (MB) is a high-grade pediatric brain malignancy that originates from neuronal precursors located in the posterior cranial fossa. In this study, we evaluated the role of STAT3 and IL-6 in a tumor microenvironment mediated drug resistance in human MBs. We established that the Group 3 MB cell line, Med8A, is chemosensitive (hence Med8A-S), and this is correlated with a basal low phosphorylated state of STAT3, while treatment with IL-6 induced robust increases in pY705-STAT3. Via incremental selection with vincristine, we derived the stably chemoresistant variant, Med8A-R, that exhibited multi-drug resistance, enhanced IL-6 induced pY705-STAT3 levels, and increased IL6R expression. Consequently, abrogation of STAT3 or IL6R expression in Med8A-R led to restored chemosensitivity to vincristine, highlighting a prominent role for canonical IL-6/STAT3 signaling in acquired drug resistance. Furthermore, Med8A-S subjected to conditioning exposure with IL-6, termed Med8A-IL6+ cells, exhibited enhanced vincristine resistance, increased expression of pY705-STAT3 and IL6R, and increased secretion of IL-6. When cocultured with Med8A-IL6+ cells, Med8A-S cells exhibited increased pY705-STAT3 and increased IL-6 secretion, suggesting a cytokine feedback loop responsible for amplifying STAT3 activity. Similar IL-6 induced phenomena were also observed in the Group 3 MB cell lines, D283 and D341, including increased pY705-STAT3, drug resistance, IL-6 secretion and IL6R expression. Our study unveiled autocrine IL-6 as a promoter of STAT3 signaling in development of drug resistance, and suggests therapeutic benefits for targeting the IL-6/STAT3 signaling axis in Group 3 MBs.

Backgrouds: Multi-drug resistance tuberculosis (MDR-TB) adds the burden of tuberculosis (TB). Depression is a common comorbidity in TB patients. Prevalence of depression among MDR-TB patients was higher, i.e. 11-70%. Prevalence of depression among TB patients in Dr. Kariadi General Hospital was 51.9%. Previous study reported the determinants related to the depression in MDR-TB patients. This study explored the prevalence of depression in MDR-TB patients and its determinants. Methods: This study was a cross-sectional study conducted from July-August 2019. Data consisted of primary and secondary data from Dr. Kariadi General Hospital patients. Sample in this study involved 72 respondents, over 16 years old from 151 registered MDR-TB patients. Depression status was determined using Depression Anxiety Stress Scales obtained from the medical record. Subjects’ characteristics and determinants were collected from primary as well as secondary data. Descriptive data were presented in proportion. Chi-square test continued by Binary logistic regression was performed to determine the association between depression status and its determinants. A P-value

Abstract Background and AimsPseudopodium-enriched atypical kinase 1 (PEAK1) has reported to be upregulated in human malignancies and related with poor prognosis. Enhanced PEAK1 expression facilitates tumor cell survival, invasion, metastasis and chemoresistance. However, the role of PEAK1 in breast cancer is not clear. Here, we investigated the PEAK1 expression in breast cancer and analyzed its relation with clinicopathological status and chemotherapy resistance to the neoadjuvant chemotherapy (NAC). We also investigated the role of PEAK1 on breast cancer cells in vitro and in vivo. MethodsImmunohistochemistry (IHC) was performed in 112 surgical resected breast cancer tissues. The associations between clinicopathological status, multi-drug resistance and PEAK1 expression were determined. Effect of PEAK1 overexpression or down-expression on proliferation, colony formation, invasion, migration, metastasis and Doxorubicin sensitivity in the MCF-7 cells in vitro and in vivo was detected. ResultsPEAK1 was overexpressed in breast cancer tissues and NAC -resistant breast cancer tissues. High PEAK1 expression was related with tumor size, high tumor grade, T stage, LN metastasis, recurrence, Ki-67 expression, Her-2 expression and multi-drug resistance. Targeting PEAK1 inhibited cell growth, invasion, metastasis and reversed chemoresistance to Doxorubicin in breast cancer cells in vitro and in vivo. ConclusionHigh PEAK1 expression was associated with invasion, metastasis and chemoresistance of breast cancers. Furthermore, targeting PEAK1 could inhibit cell growth and metastasis, and reverse chemoresistance in breast cancer cells, which provides an effective treatment strategies for breast cancer.

This case study illustrates how family members can assist in the care of teenagers with diabetes but that there are also serious risks actualized by such involvement. In particular, it highlights ethical complications that arise when the role of a family member is changed from ‘parent’ to ‘care provider’. The ‘counseling, self-care, adherence’ (CSA) approach offers a look at the role that family can play to improve these types of care.
