Background: Labour and delivery experience is one of the most significant events in a woman’s life. Support in labour has an impact on the childbirth experience and childbirth outcomes. A positive childbirth experience can help in mastering the major change in life. Therefore, the objective of this study was to find out postnatal mothers' perception on care during labour. Methods: A descriptive cross-sectional design was used to explore the perception on care during labour of postnatal mothers in postnatal ward Tribhuvan University Teaching Hospital. Total 294 postnatal mothers with normal delivery were selected by purposive sampling method after receiving ethical approval from research department of Institute of Medicine and concerned authority. Data was analyzed by using descriptive as well as inferential statistics. Results:  Among 294 women; 208(70.75%) were received care on infection prevention, 160(54.42%) received bladder care, 175(59.66%) received care on hydration maintenance, 149(50.57%) received care on injury prevention, 103(34.88%) care on comfort measures, 180(61.14%) care on information, 207(70.54%) care on emotional support, and 201(68.31%) care immediately after childbirth. There is significant relationship among different types of care during labour (such as emotional support (44.44%), infection prevention care (19.7%), care for bladder empty (14.5%), maintenance of hydration (17.1%), care for prevention of injury (31.9%), care on pain relieve and comfort measures (50.1%), informational care (64.5%), and care immediately after delivery (18.5%)) and overall perception on care during labour.  The perception level was based on mean score and was categorized as; below 3 is negative perception, 3 and above is positive perception. The finding of this study shows that 93(31.6%) postnatal mothers were perceived care negatively and 201(68.4%) perceived care positively during labour. It indicates that care during labour needs to be improved for best care perception. Conclusions: Women expect professional, supportive and respectful care during labour. Continuous care and support during labour has meaningful benefits for women and infants. It indicates that the care during labour need to be improved for best care perception.

Aims:  This study was done to analyze the semen parameters of fertile Nepalese men with a known time to pregnancy of less than or equal to 12 months and to compare the values obtained with the newer reference limit of WHO, 2010.Methods: A hospital based prospective cross- sectional study was conducted at Tribhuvan University Teaching Hospital, Institute of Medicine, from 15th April 2012 – 15th April 2013. Sixty fertile men with a known time to pregnancy (TTP) ≤ 12 months were included in the study and semen collection and analysis done at the laboratory of Department of Pathology. Results: The following one-sided reference limits, the fifth centile (with 95th percent confidence interval), were generated: semen volume, 1.0 ml (0.8-1.2); total sperm number, 25 million per ejaculate (11-39); sperm concentration, 10 million per ml (5-15); progressive motility, 15% (10-20); total (progressive plus non-progressive motility) motility, 30% (25 -35); vitality 50% (46-54); morphologically normal form, 99% (99.3- 99.4). Conclusions: The study concluded that only a single semen parameter cannot be conclusive of fertility, therefore combination of all semen parameters is more predictive. In addition the study also concluded that the semen parameters of Nepalese fertile men were found lower as compared to reference limits of WHO. DOI: http://dx.doi.org/10.3126/njog.v9i2.11772

Introduction: Sexually transmitted infections (STIs) are a major cause of public health problem. In developing countries, human immunodeficiency virus (HIV) infection is spreading rapidly, with sexually transmitted infections acting as a cofactor for the spread of HIV. Yet adequate awareness regarding these infections is lacking.Objective: To explore the knowledge and awareness about STIs and HIV infection in patients visiting Dermatology and Venereology Clinic of Institute of Medicine, Tribhuvan University Teaching Hospital (TUTH) with genital symptoms.Material and Methods: A cross sectional descriptive study was carried out in the department of dermato-venereology, TUTH for a period of one year. Total of 130 consecutive cases were enrolled. After taking consent and assuring confidentiality they were interviewed regarding awareness of STIs and HIV/AIDS.Results: Out of 130 cases, 95% and 98.5% had heard of an STI and HIV/AIDS respectively. Only 43.8% could mention discharge from genitalia as a symptom of STI whereas 38.5% could mention none. Most (91.5%) knew about the sexual mode of transmission of both STIs and HIV. None could mention about mother to child transmission of an STI, whereas 15% knew that an infected mother can transmit HIV to her baby. About 58% were aware that condoms could protect them from acquiring an STI, 63% perceived this as best method of prevention from HIV. When only 26% knew that being faithful to one partner is the best mode of prevention from an STI, 45% mentioned avoiding contact with multiple partners could prevent them from HIV transmission.Conclusions: Awareness programme on STIs as a whole could improve knowledge and awareness on HIV and STIs and hence prevent both.

Background: COVID-19 is a major Public Health challenge that has affected the world’s economy. Assessment of the knowledge, attitude and practices of Healthcare workers (HCWs) towards COVID-19 can improve or sustain the successes recorded by relevant agencies in the fight against COVID-19.Methods: a cross-sectional survey was done using semi-structured questionnaires and simple sampling technique. Data collected were analyzed using SPSS version 22.Results: Out of the 368 respondents, 205 were doctors while 150 were nurses. Others were clerical workers, administrative officers and maintenance workers. Knowledge of COVID-19 was generally high amongst respondents recording a correct response to questions about knowledge of COVID-19 in more than 90% of the case. However, the attitude and practice of COVID-19 infection, prevention and control protocols were poor. Out of the respondents, 41.8% would not stay at home if they had minor COVID-19 like symptoms; only 39.4% would take responsibility to ensure people around them follow good respiratory hygiene. Respondents who would keep social distancing while talking with co-workers, ensure safe disposal of personal protective equipment (PPEs) and safe waste management of PPEs were 56%, 39.9% and 53.8% respectively.Conclusions: Knowledge of COVID-19 prevention protocols is high but attitude/practice of these laid out protocols still desires a lot of effort from HCWs. There is need for training/retraining of HCWs on COVID-19 infection, prevention and control strategies to ensure the success being recorded against the disease is sustained while preventing the possibility of a second wave of infection.

Introduction: Self-harm forms a significant proportion of patient attending in emergency departments, typically 10-30 per 100000 people commit suicide annually. The objective of this study was to find out the factors responsible for self harm. Methods: This is a cross sectional study of 100 patients of self harm coming to Emergency Department of Tribhuvan University Teaching Hospital from 1stOctober 2007 to 31stMarch, 2008. Questionnaire about demographic information of patient along with details about cause of self harm, and any other illnesses associated was filled. Information about duration of arrival in emergency department, place of self harm was gathered. Patients were clinically examined and hematological and biochemical tests including liver function test and the type of poison used and its level in blood were examined. Results: Total number of patients with self harm was 100 (female - 71%; male- 29%).Among female, 16-30 years age group to be the largest (76%). Mean duration of arrival in emergency department was 3.08 hour. Metacid ingestion (29%) was found to be the most common cause for self harm. There is no difference of self harm for suicide and threat cause. Only 5% of patient with self harm has psychiatry illness in the past. Conclusions: Self harm forms a significant proportion of patient attending the Emergency department and the most common form of self harm used was metacid ingestion. There is compelling evidence for the need to improve delivery of care for patients who inflict self harm. Key words: Cause; emergency department; self harm. Childbirthis a processLabor of opening and depleting the cervix and the fetus down into the birth canal. Birth is a process in which the fetus and amniotic are pushed out through the birth canal. (Sarwono, 2008). According to the World Health Organization (WHO) estimates more than 585,000 mothers annually die during pregnancy or childbirth. Indonesia Health Demographic Survey (SDKI) Survey in 2012, Maternal Mortality Rate in Indonesia is still high at 359 per 100,000 live births. The purpose of this study is the knowledge of maternal knowledge, maternal age, and maternal parity associated with normal birth events at Palembang Bari Hospital 2017. This study used analytical survey method with cross sectional approach. The population in this study were all maternal mothers at the Palembang Bari Hospital in 2017. Sampling in the study was conducted non-randomly with the technique of "Accidental Sampling". Data analysis was done univariat and bivariate with Chi-Square statistical test with significance level α = 0,05. The result of this research shows that there is correlation between mother's knowledge with normal delivery incidence with p value 0,001, there is correlation between mother age with normal delivery incidence with p value 0,009, there is relation between mother parity with normal delivery incidence with p value 0,001. From result of this research hopes healthcare workers can improve normal delivery care services and more often to carry out maternal safety counseling.

<p><em>Every woman wants her labor to run smoothly and can give birth to a baby perfectly. Childbirth can run normally, but it is not uncommon for labor to experience obstacles and must be done through surgery. This means the fetus and mother are in an emergency and can only be saved if labor is performed by surgery. The purpose of this study is to find out the related to the health status of newborn babies at delivery in the City of Bukittinggi.</em></p><p><em>Type of analytic survey research with design cross-sectional</em><em>.</em><em>The object of the study was a newborn baby with Caesarean section and normal delivery at Bukittinggi City Hospital. The test used is thetest Chi-Square with a 95% confidence level.</em></p><p><em>The results showed that as many as 65 people (54.2%) had the incidence of asphyxia. 65 people (54.2%) had normal respiratory conditions, 62 people (51.7%) had meconium aspiration</em><em>.</em><em>63 people (52.5%) had trauma to an abnormal baby</em><em>.</em><em>64 people (53.3%) had infectious diseases. 70 people (58.3%) had IMD.</em><em>72 people (60%) have joined care. 60 people (50%) with type of labor SC. Statistical tests showed that there was a relationship between apgar score (p = 0.003), respiratory conditions (p = 0.010), meconium aspiration (p = 0.0005), trauma in infants (p = 0.0005), joint care (p = 0.002 ) and IMD (p = 0.0005) for the type of labor. While infectious diseases do not have a relationship to the type of labor (p = 0.583).</em></p><p><em>It was concluded that there was a relationship between apgar score, respiratory condition, meconium aspiration, admission and IMD with different types of delivery. Expected to health workers especially midwives can be used as input in order to improve health status in newborns.</em></p>

Infeksi HPV dan pre kanker serviks (sel-sel abnormal pada leher rahim yang dapat menyebabkan kanker) telah menurun secara signifikan sejak vaksin HPV digunakan.Cakupan vaksinasi HPV masih rendah di Indonesia.Orang tua adalah pemegang tanggung jawab dan kewajiban utama di dalam penjaminan pemenuhan hak dasar anak untuk mendapatkan vaksinasi. Tujuan penelitian adalah untuk mengetahui faktor yang berhubungan (sikap, kepercayaan, dukungan orang tua) dengan kesediaan vaksinasi pada remaja putri. Jenis penelitian adalah survey analitik dengan pendekatan cross sectional. Penelitian dilaksanakan di SD Muhammadiyah Macanan, Ngemplak dan SMPN 1 Berbah, Sleman pada Mei-Juli 2019. Jumlah sampel 127 remaja putri beserta orang tuanya dipilih dengan accidental sampling. Instrumen kuesioner dan telah dilakukan uji validitas. Analisis bivariat dengan uji chi square (dukungan instrumen, emosional, kepercayaan orang tua) dan uji fisher exact (dukungan informasi dan sikap). Hasil penelitian menunjukkan 92,9% orang tua tidak memberikan dukungan informasi, 85% tidak memberikan dukungan instrumen, 75,6% memberikan dukungan emosional kepada putrinya terkait vaksinasi. Faktor yang berhubungan dengan kesediaan vaksinasi adalah dukungan instrumen (p-value = 0,048). Faktor yang menjadi pertimbangan terbanyak orang tua untuk mengijinkan anaknya di vaksinasi adalah keamanan vaksin (81,1%). Kesimpulan: Ada hubungan dukungan instrumen orang tua dengan kesediaan vaksinasi HPV pada remaja putri. HPV infections and cervical precancers (abnormal cells on the cervix that can lead to cancer) have dropped significantly since HPV vaccine has been in use. HPV vaccination coverage is still low in Indonesia. Parents are the main responsibility and obligation in guaranteeing the fulfillment of the child's basic rights to get a vaccination. The purpose of this study was to determine the related factors (attitudes, beliefs, parental support) with the willingness to participate in HPV vaccination among adolescent girls. This type of the study was analytic survey with cross sectional approach. The study was conducted at SD Muhammadiyah Macanan, Ngemplak and SMPN 1 Berbah, Sleman in May-July 2019. The sample was 127 adolescent girl and their parents selected by accidental sampling. The instrument used questionnaire and already undergone validity test. The bivariate analysis used chi square test (instrument and emotional support, parental trust) and fisher exact test (information support and attitude). The results showed 92.9% of parents did not provide information support, 85% did not provide instrument support, 75.6% provided emotional support to their daughters related to vaccination. Factors related to the willingness of vaccinations were instrument support (p-value = 0.048). The factor that was considered by most parents to allow their children to be vaccinated was vaccine safety (81.1%). Conclusion: There was a correlation between parental instrument support and HPV vaccination willingness of adolescent girls.

Background:: Rational drug use comprises aspects of prescribing, dispensing and patient use of medicines for different health problems. This study is aimed to assess drug prescribing practice based on the world health organization prescribing indicators in Mizan-Tepi University teaching hospital. Methods:: An institutional based retrospective cross sectional study was conducted to evaluate prescribing practices in Mizan-Tepi University teaching hospital. Data were collected based on World health organization drug use indicators using prescription papers. 600 prescriptions dispensed through the general outpatient pharmacy of the hospital were collected by systematic random sampling method from prescriptions written for a 1-year time in Mizan-Tepi University teaching hospital. Results:: The present study found that the average number of drugs per prescription was 2.04 ± 0.87 in Mizan-Tepi University teaching hospital with a range between 1 and 5. Prescribing by generic name was 97.6 % and 47.8% of prescriptions contained antibiotics in the hospital. 27.7% of prescriptions contained at least one injectable medication in Mizan-Tepi University teaching hospital. From prescribed drugs, 96.7% of them were prescribed from Ethiopian essential drug list. Conclusion:: Present study indicated that the average number of drugs prescribed per encounter, the percentage of generic prescribing and prescribing from the EDL were close to optimal value. However, the percentage of encounters with antibiotics and injections prescribed were found be very high. Thus, the study highlights some improvements in prescribing habits, particularly by focusing on the inappropriate consumption of antibiotics and injections.

Background: Value of fine needle aspiration cytology in preoperative diagnosis of salivary gland lumps has been established in various studies. This study aims to calculate sensitivity, specificity accuracy of FNAC, correlate FNAC findings with histopathology and find out reasons for common diagnostic pitfalls in FNAC.Materials and methods: This study was performed in 67 patients that presented with salivary gland lumps at Department of pathology, Tribhuvan University Teaching Hospital, Institute of medicine, Kathmandu, Nepal from January 2009 to June 2010. FNAC was performed and was correlated with histopathological diagnosis to judge the sensitivity, specificity and accuracy of FNAC.Results: Out of 36 cases there was cyto-histological correlation in 28 cases. Pleomorphic adenoma was the most frequent lesion in this study. Parotid gland was the most common site affected by salivary gland lesions. The sensitivity, specificity and accuracy for non-neoplastic lesions was 100%, 89.28%, 91.67%, for benign neoplasms was 100%, 86.95%, 91.67% and for malignant neoplasms was 57.14%, 100%, 83.33% respectively.Conclusion: FNAC is a useful tool in preoperative diagnosis of salivary gland lesions and can be crucial for the management of the patient.
