Background: Acute pancreatitis sequelae require a multidisciplinary approach and ICU care. Ulinastatin is a serine proteases inhibitor that reduces inflammation by suppressing the infiltration of neutrophils and elastase release and inflammatory mediators that help improve clinical symptoms and reduce mortality. This study aims to evaluate the clinical utility of Ulinastatin. Methods: Fifty-two patients admitted to ICU with acute pancreatitis were divided into; Ulinastatin group who received a 3 to 5 days course of 200,000IU, and Control Group who didn’t receive ulinastatin. Pearson's Chi-square and Fisher's exact test were used and a p-value < 0.05 was considered statistically significant. Results: Mean age was lower among the Ulinastatin group at 43 years (p-Value 0.014) and Hepatic dysfunction was more among this group (p-value 0.04). Among new onset of organ dysfunction, only CVS dysfunction was significant among the Control group ( p-value 0.044) while respiratory function recovery (p-value 0.04) and coagulation profile improvement (p-value 0.017) was statistically significant among the Ulinastatin group. The mean duration of hospital stay was shorter among control group, 9.65 days vs 14 days, a p-value of 0.05and also the average duration of stay in MDICU was lower, 4 days vs 8.5 days, p-value 0.0044 in comparison to Ulinastatin group. Overall mortality incidence was 15.38%, 19% in Ulinastatin group vs 11.5% in Control group. Conclusion: This retrospective study is our experience in the use of Ulinastatin which has shown little efficacy in declining mortality and/or hospital stay duration though it helps prevent new organ dysfunctions.

Background: A suspected peripheral neuropathy outbreak was reported from Dechentsemo Central School, Thinleygang, Punakha, following which the investigation team was immediately dispatched in the field.Objective: The aim of investigation was to ascertain the cause and risk factor for the outbreak in order to implement control measures.Methods: A case control study was devised for the investigation to study about the past exposure or deficiencies in order to find out the suspected cause and risk factors.  A semi-quantitative food frequency questionnaire was administered to both cases and controls to collect information on the type of food they have consumed. The information garnered was analyzed using Chi-Square or Fischer Exact test for categorical variables and Man-Whitney U-test for quantitative variables. Results: All 17 cases were females with mean age of 13 years (SD 2.7 years). The average daily amount of thiamine intake was 0.6 mg/day for case and 0.8 mg/day for controls against the recommended daily allowance (RDA) of 1.2 mg/day. Case and control patients differed significantly with respect to fat intake (p-value = 0.02), more strongly with folate and iron intake (p-value 0.01).Conclusion: The outbreak of peripheral neuropathy in Dechentsemo Central School appears to be linked to reduced dietary intake rich in vitamin B1 coupled with low intake of folate and iron in their diet.

In Indonesia, Yoga is currently one of the most popular sports, not only for the general public but also for pregnant women. Many benefits can be obtained from prenatal yoga including the mother feeling comfortable and relaxed throughout pregnancy and during childbirth, training the pelvic floor muscles and perineum, keeping the body's muscles relaxed, balanced during childbirth, reducing stress, increasing and improving oxygen circulation to the body and fetus. . This study aims to determine the effect of prenatal yoga on optimizing fetal position on third trimester Primigravida maternal delivery output at Puskesmas Cirebon City. This research is a quasi-experimental research pre post control group design and only post control group design with T test and Chi Square test. Paired T Test results of the intervention group (mean difference = 3.111, P value = 0.000, control group (mean difference = 0.833, P value = 0.318). Independent T Test results obtained (average difference = -1.944, P value = 0.024), Chi Square test obtained a value of P = 0.73. There is a difference in the average anxiety before and after the intervention in the intervention group, there is no difference in average anxiety before and after the intervention in the control group. There is a difference in the average anxiety between the intervention and control groups. There is a difference in mean Long I stage between the intervention and control groups There was no effect of prenatal yoga on the type of labor.

Abstract Objectives Herbal medicines, as a treatment method, have received a great deal of attention. The effects of two herbal medicines namely Zingiber officinale and Echinacea on alleviation of clinical symptoms and hospitalization rate of suspected COVID-19 outpatients were examined. Methods A clinical trial with 100 suspected COVID-19 outpatients as participants was conducted. The participants were allocated randomly to two groups of 50 members. The intervention group received concurrent Zingiber officinale (Tablet Vomigone 500 mg II tds) and Echinacea (Tablet Rucoldup I tds) for seven days in addition to the standard treatment. The control group only received the standard treatment (Hydroxychloroquine). After seven days, alleviation of clinical symptoms and hospitalization rate were examined. In addition, 14 days after treatment, the hospitalization was assessed again by telephone follow up. Results The two groups were identical in terms of basic characteristics. Improvement level as to coughing, dyspnea, and muscle pain was higher in the intervention group (p value <0.05). There was no significant difference between the two groups in terms of the other symptoms. In addition, the hospitalization rate in the intervention and control groups were 2 and 6% respectively, which are not significantly different (p value >0.05). Conclusions Taking into account the efficiency and trivial side-effects of Zingiber officinale and Echinacea, using them for alleviation and control of the clinical symptoms in COVID-19 outpatients is recommended.

Diarrhea is one of the most common infectious diseases. It has related three factors such ass environment, food and contact with an infected person. One of the environmental factor of diarrhea is the domestic water use behavior. The main objective of this study was to analyse the relation between knowledge, atitude  and habit to  domestic water use behavior in  Martapura 2 Public Health Center, work area Pasayangan Selatan Village, sub-district of Martapura, Banjar, South Kalimantan Province. Case control study design was used in this study. The population of the study were all diarrhea patients in Martapura 2 Public Health Center work area  from february 2017 until february 2018. The Lemeshow was used to determine research sample size. The study sample consisted of 45 case diarrhea patients and 45 control non diarrhea patient. Samples were taken from all eligible cases and controls on specified inclusion and exclusion criteria. Data will analysed use Chi Square. Based on the results of this study comparing between case group and control group, most of the respondents in the case group have knowledge in poor knowledege was  25 people (55.6%) and the control group in good knowledge that was 35 people (77.8) with Chi Square, all p < 0,001. Attitudes of respondents in the case group were in the not good category is 26 people (58.0%) and control group in good category that was 32 people (71,1%) with (p value 0,000 in case and control 0,004). Utilitation habit respondents in the case group were in the non-existent category of 26 persons (57.8%) and the control group in the category of no 37 people (82.2%) with (p value in case 0.006 and control 0.000). Conclusively, the variable of knowledge, attitude  and habit have very significant relation on the group of case and control.

Objectives: Treatment of asymptomatic necrotic teeth is a common type of occurrence in endodontic practice. Root canal treatment of these teeth sometimes results in pain and /or swelling of varying degree. These severe symptoms upset both the patient and dentist. This study was to compare the effect of prophylactic amoxicillin and placebo in endodontic flare-up in asymptomatic necrotic teeth requiring root canal treatment. This was evaluated by using flare-up index. The influence of age, sex, was compared between flare-up and non-flare-up groups. Using a standardized technique and the same materials eliminated the variability of operator. Study Design: Quasi experimental study. Setting: Department of Operative Dentistry, Lahore Medical and Dental College, Lahore. Period: From January 2018 to December 2018. Material & Methods: 102 patients with asymptomatic necrotic teeth with periapical radiolucency were selected from the Operative Dentistry Department of Lahore Medical and Dental College. Patients were divided into two groups experimental and control, by convenient sampling. One half of the patients were given prophylactic amoxicillin and other half was given placebo one hour before treatment. Results: Flare-up occurred in four of the experimental and seven of the control group patients. Statistical analysis revealed no significant differences between flare-up and non- flare- up patients for age and sex. The data outcome was of qualitative in nature i.e. pain and swelling, they were subjected to Chi-Square test of significance with P value of £0.05. The results demonstrated that 10.8% of the 102 patients had a flare-up. No side effects were observed in the patients who were prescribed amoxicillin. Conclusion: The occurrence of flare-up did not significantly influence by Prophylactic amoxicillin (P= .33).

Abstract Background Clinicians in critical care medicine considered dapsone administration to treat SARS-CoV-2 inflammasome. Dapsone is useful in the molecular regulation of Nod-like receptor family pyrin domain-containing 3 (NLRP3). Objective To study the targeting of NLRP3 itself or up-/downstream factors of the NLRP3 inflammasome by dapsone must be responsible for its observed preventive effects, functioning as a competitor. Methods This is case series with or without intervention; a cross-sectional study. We set out to use objective criteria of improvement, such as A. a reduction in the FIO2 requirement and B. a decrease in the progression of hypoxia. We treated the patients with standard COVID-19 ARDS treatment with dapsone 100 mg to target NLRP3 inflammasomes. Results The 22 cases were treated with standard COVID-19 therapy with dapsone (trial group), and the 22 cases were the control group. The comparison was made assuming that only decreased FIO2 was influential in the trial and control groups, which applied to only the ARDS onset stage. The chi-square statistic is 5.1836. The p-value is .02280. Fisher’s exact test statistic value is 0.0433. (The result is significant at p < .05) Furthermore, the ARDS-onset mortality rates were 0% (with dapsone) and 40% (without dapsone). Conclusion There was a significant difference in dapsone treatment results in the ARDS-onset group. We confirmed that dapsone clinically treated the onset of ARDS by targeting SARS-CoV-2-activated inflammasomes. Like chemically reacting substances, inflammasome and dapsone are competing, proving that it is only effective in treating early ARDS.

Penggunaan warfarin dan antibiotik bersamaan diketahui dapat meningkatkan risiko perdarahan. Kejadian perdarahan mayor terkait warfarin dapat berupa perdarahan fatal hingga menyebabkan kematian. Penelitian ini bertujuan untuk menganalisis interaksi warfarin dan antibiotik dan efeknya terhadap kejadian perdarahan pada pemeriksaan pertama pasca rawat inap di RSUD Wates. Desain studi berupa kohort retrospektif pada 67 kasus penggunaan warfarin dengan/tanpa antibiotik oleh pasien rawat inap. Data berupa diagnosis, terapi, pemeriksaan pendukung, dan kejadian perdarahan saat rawat inap hingga pemeriksaan pertama diambil dari rekam medik pasien tertanggal Januari 2018 hingga September 2020. Analisis data dilakukan secara statistika menggunakan ­uji Chi-square atau uji Fisher exact test dan uji independent t-test atau uji Mann Whitney U test. Terdapat perbedaan bermakna pada kejadian perdarahan dalam waktu 7 hari pasca rawat inap antara kelompok antibiotik dan kontrol (16%; 0%; p = 0.017). Antibiotik yang digunakan oleh pasien dengan kejadian perdarahan yaitu golongan sefalosporin, kuinolon, azitromisin, dan ampisilin-sulbaktam. Kejadian perdarahan yang terjadi meliputi hematemesis melena (25%), hematuria (50%), tidak diketahui (25%). Studi ini menyimpulkan bahwa penggunaan bersamaan warfarin dan antibiotik saat rawat inap dapat berpengaruh terhadap kejadian perdarahan dalam waktu 7 hari setelah pasien dipulangkan.Penggunaan warfarin dan antibiotik bersamaan diketahui dapat meningkatkan risiko perdarahan. Objective: The objective of this study was carried out to study the characters, determinants of anemia among atrial fibrillation (AF) patients and to estimate the link between chronic anemia and AF. Methods: This is a case–control study, administrated in Al-Hussein Teaching Hospital among admitted patients in the cardiac care unit from 1st week of January 2018 to the last week of April 2019. A random sample of 100 patients was included in the study based cut of time, where a special form of inquiry sheet was constructed to gather data and it was reviewed and revised by matter experts for testing the validity and enrichment of it. Both cases of AF and control with sinus rhythm were divided into anemic and non-anemic, anemia is defined as hemoglobin <13 and <12 in male and female, respectively. A Statistical Package for the Social Sciences (SPSS) version (25) was used for data analysis. Descriptive statistics Chi-square Fischer exact test, and ANOVA tests of significance, correlation and logistic regression analysis had been estimated where p<0.05 was set as statistically significant. Results: A total of 100 individuals had been included in the study, 50 cases with AF and 50 control with mean of age was 62.74003±13.424 years 58 females with 42 males, total non-anemic was 57 while total anemic was 43, 74.4% of AF cases was anemic while 25.6% of control group was anemic, there was statistically significant difference between cases and control according to the presence and absence of anemia where p<0.05. Conclusion: The first study done in the region to prove the direct relation of anemia on incident AF, thus in conclusion, chronic anemia had a relation with AF.

AbstrakSasaran Making Pregnancy Safer (MPS) menurunkan Angka Kematian Bayi (AKB) hingga 28 per 1000 kelahiran hidup dengan salah satu upaya yang dapat dilakukan melalui pemberian kolostrum. Tujuan penelitian ini adalah untuk mengetahui pengaruh mobilisasi ibu post partum terhadap pengeluaran kolostrum. Penelitian ini merupakan quasi eksperimen dengan post test only group design. Data dianalisis menggunakan uji Chi-Square dan Fisher’s Exact Test. Hasil analisis data menunjukkan persentase pengeluaran kolostrum early pada kelompok intervensi 72,2% dan kelompok kontrol 50,0%. Persentase pengeluaran kolostrum late pada kelompok intervensi 27,8% dan kelompok kontrol 50,0%. Tidak terdapat pengaruh bermakna mobilisasi ibu post partum terhadap pengeluaran kolostrum dengan nilai p value 0,305 (>0,05). Tidak terdapat pengaruh bermakna tingkat stres dan IMT ibu terhadap pengeluaran kolostrum dengan nilai p value 1,000 (>0,05). Terdapat pengaruh bermakna daya hisap bayi terhadap pengeluaran kolostrum dengan nilai p value 0,047 (<0,05).Dari hasil penelitian disimpulkan persentase pengeluaran kolostrum early kelompok intervensi lebih besar dibandingkan kelompok kontrol, namun secara statistik tidak terdapat pengaruh bermakna mobilisasi ibu post partum terhadap pengeluaran kolostrum pada kedua kelompok tersebut. This study was a quasi experiment with post-test only group design. Data were analyzed using Chi-square and Fisher’s Exact test. Result of this study showed that early colostrums in the intervention group was 72.2% while in control group only 50%. Late colostrums in intervention group was 27.8% compared 50% in the control group. There was no significant effect between maternal postpartum mobilization against eject of colostrum with p value 0.305 (> 0.05). No significant effect on stress level and Body Mass Index (BMI) and eject of colostrum with p value was 1.000 (> 0.05) but significant effect occurred between infant suction power against colostrums spending with p value was 0.047 (< 0.05). It can be concluded that percentage of early eject of colostrum was greater in intervention group than in control group, but statistically there was no significant effect among maternal postpartum mobilization, stress level and BMI against colostrums spending in both group. Significant effect only showed between infant suction power against colostrums spending.Keywords: postpartum, mobilization, colostrums

Background: The most important psychological problems of dialysis patients are anxiety, depression, and stress. These psychological problems cause different consequences for the patients. ‎Therefore, it seems necessary to use strategies to reduce the psychological problems in hemodialysis ‎patients. Objectives: This study was conducted to evaluate the effectiveness of spiritual therapy on depression, anxiety, and stress in hemodialysis patients referred to the dialysis ward of special diseases center of Birjand in 2019 - 2020. Methods: In this semi-experimental study, 70 patients with chronic kidney disease (CKD) under hemodialysis referred to the dialysis ward of special diseases center of Birjand in 2019 - 2020 were selected by available sampling method and randomized into two experimental groups (35 patients) and control (35 patients). The patients in the experimental group received eight sessions of 60-minute (one session per week) of spiritual intervention therapy, and the control group received no spiritual intervention. Data collection tools were a form of demographic profile and Lovibond SH and Lovibond PF (1995) questionnaire. Data were analyzed using SPSS 26 statistical software, and Fisher’s exact test, chi-square test, independent t-test, repeated measures ANOVA, Bonferroni post hoc test at a significance level of P < 0.05. Results: In this study, 68 patients were ‎examined in two experimental (33 patients) and control (35 patients) groups.‎ The mean score of depression, anxiety, and stress before the intervention in patients in the ‎experimental and control groups was not significantly different ‎(‎P‎ =‎ 0.61‎)‎, but immediately and three ‎months after the intervention in patients in the experiment group was significantly lower than the ‎control group (P < 0.001).‎ Conclusions: Based on the results of the present study, it can be said that spiritual therapy is considered an effective solution for reducing depression, anxiety, and stress in hemodialysis patients.
