Introduction Skull has cranial vault and base. The cranial vault is called as calvarium which roofs the cranial cavity (brain box).Cranial vault consists of frontal, parietal and occipital bones. Total thickness of calvarial bones includes outer table, diploe and inner table. Diploe is made up of spongy bone whereas outer and inner table made up of compact bone. Objective To measure the thickness of Nepalese calvarial bones i.e. Frontal, Parietal and Occipital. Methods It is a hospital base cross-sectional study. CT records of one hundred and fifty adult people were studied. Nine various points on frontal bone, seven on parietal and six on occipital were located and their thickness were measured bilaterally with help of CT scan. Results The present study showed that Mean thickness+_SD of frontal bone were 6.1+_1.8mm; parietal 4.6+_2.2 and occipital 9.5+_3.4. The study also showed that frontal bone had 2.4+_0.8 mm thick outer table; 1.4+_0.6mm inner table and 3.5+_1.3mm diploe. Similarly parietal bone had 1.9+_0.6 mm thick outer table; 1.1+_0.4mm inner table and 0.8+_0.5mm diploe. The occipital bone had 3.5+_1.5 mm thick outer table; 2.2+_0.8mm inner table and 4.3+_1.8mm diploe. Similarly this present study also calculated the mean thickness+_SD of outer table in the calvarium as 2.7+_1.3mm and that of inner table 1.5+_0.9 mm. Thus it concluded that outer table was thicker than inner table. Journal of Nobel Medical College Vol. 2, No.1 Issue 3 Nov.-April 2013 Page 31-35 DOI: http://dx.doi.org/10.3126/jonmc.v2i1.7670

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

This study was done to assess the hypercoagulability and the risk of thromboembolism in women taking oral contraceptive pill for prolonged period of time. This cross sectional study was done in the department of Physiology, Dhaka Medical College, Dhaka from Jan 2012 to Dec 2012. Ninety female subjects with the age range from 25-45 years, were taken as a study population. Among them, 60 women taking oral contraceptives for prolonged period of time ( > 1 years) were included for the study group and age matched 30 women of OCP nonusers were taken as a control. Study subjects were divided into two groups according to their duration of oral pill use: group BI ( 1 to 5 years users) were 30 women and group B2 ( >5 to 10 years users) were 30 women. Prothrombin time and activated partial thromboplastin time were estimated in all groups. Statistical analysis was done by unpaired Student's ? t' test and Pearson's Correlation Coefficient test. In this study, the mean (*SD) PT levels in group B1 & B2 were shortened than that of group A which were statistically highly significant (P < 0.001). Within the study groups, PT levels were positively correlated (r=+0.027) with the group B1 and negatively correlated (r= -0.163) with the group B2. But both the relationships were statistically non significant. The mean (+SD) AM' level in group B1 was shortened than that of group A but the result was not statistically significant. The mean (*SD) AM' level in group B2 was shortened than that of group A but the result was statistically highly significant (P<0.001). Within the study groups, APPT levels were negatively correlated with the group B1 (r= -0.268) and also group B2 (r= -0.122). But both the relationships were statistically non significant. My present study revealed that prolonged duration of OCP use ( at least for 5 years) increases the risk of hypercoagulable state and thromboembolism in women.

Introduction: The exact position of the Parotid papilla and commissure of the lip plays a crucial role in determining the maxillary occlusal plane which directly facilitates rehabilitating edentulous patients with a removable complete denture. Since the beginning, many authors have conducted different studies regarding its position but multiple variations in its location have been noted. Also, this type of study has not been conducted in the Nepalese population which drags the urge to determine the position of these soft-tissue landmarks in context to the Nepalese population. Objective: To determine the position of parotid papilla and commissure of lip in relation to the maxillary occlusal plane. Materials and Methods: A descriptive cross-sectional study was conducted in the 45 dentate subjects visiting Nobel medical college from November 2019 to April 2020. A vestibular impression was made, stone index fabricated and vernier caliper was used to locate position of parotid papilla & commissure of lip. All the data were collected and statistically analyzed. Results: The mean measurement of distances between maxillary occlusal plane and parotid papilla was 3.69 ± 1.19 mm and between maxillary occlusal plane and commissure of lip was 0.41 ± 0.83 mm respectively. Conclusion: This study showed that the position of the parotid papilla was superior to the position of the maxillary occlusal plane with a mean distance of 3.69mm. The commissure of lip was found to be at the level of maxillary occlusion plane in highest number of cases followed by a superior position with least at the inferior position with respect to the maxillary occlusal plane.

Background: Ultrasonographic assessment of gestational age by using different foetal parameter such as BPD, FL, AC are highly reliable in first and second trimester in pregnancy. In third trimester reliability of any single parameter has limitations. Methods: This was a hospital based comparative cross-sectional study done in the Department of Obstetrics and Gynaecology, S.M.S. Medical College and attached hospitals, Jaipur, Rajasthan. The period of study was from June 2018 to October 2019. Results: The mean TCD at 15 weeks and 40 weeks was 15.00 ± 0.00mm and 53.33±1.155mm respectively. The mean BPD at 16 weeks and 40 weeks was 33.50±0.70mm and 93±1.528mm respectively. The mean HC at 15 weeks and 40 weeks was 111.00±1,41mm and 340±2.00mm respectively. The mean FL at 15 weeks and 40 weeks was 18.50±0.70mm and 77.67±1.528mm respectively. The mean AC at 15 weeks and 40 weeks was 99.00±1.41mm and 365.33±8.32mm respectively. Conclusion:  We conclude that foetal transcerebellar diameter can thus be used as an alternative foetal parameter to assess gestational age and can be used in cases of wrong dates or when other routine parameters are not conclusive or did not accurately predict gestational age for e.g. in cases of hydrocephalus, brachycephaly, dolicocephaly, intrauterine growth restriction, achondroplasia or short limb dwarfism.
