Abstract. In recent years, the attention of scientists to the study of the unique properties of saliva and related diagnostic capabilities has increased significantly. Many data on the function and composition of oral fluid in healthy people and in various somatic pathologies. The ducts of three pairs of large (parotid, mandibular, sublingual) and up to a thousand small salivary glands open into the oral cavity of an adult. The amount of saliva secreted per day depends on the nature of food, age and sex. But on average, an adult secretes 750-1500 ml of saliva per day. Upon entering the oral cavity, saliva mixes with leukocytes, microorganisms and their products, exfoliated epithelium of the oral mucosa, oral detritus, gingival fluid, food debris, toothpaste, bronchial and nasal secretions. The purpose of the study: to investigate changes in the quantitative and qualitative properties of oral fluid in the replacement of dentition defects in patients with coronary heart disease. Object and methods of research. Sources of domestic and foreign literature. Research results. Oral fluid is an integral liquid medium of the human body and the first biological fluid that connects the internal environment of the body with the external. The activity of more than 100 enzymes, which differ in the origin and performance of the function, is determined in human oral fluid [6]. Changes in the activity of oral enzymes due to direct exposure to metal ions, plastics and related pathology can lead to a weakening of protective, mineralizing, digestive and other properties, which harms not only the dental system but also the human body as a whole. It is established that due to changes in permeability in the conditions of stress pathology the dynamic balance of the main internal environment - blood is preserved. It is proved that changes in the biochemical composition of blood and saliva correlate with the severity and features of myocardial infarction. Therefore, due to recent research in biochemistry and dentistry, laboratory methods continue to be improved and introduced into clinical practice, which allow to diagnose somatic and dental diseases by studying the parameters of oral fluid, which significantly expands diagnostic capabilities and allows more effective control of patients with combined pathology. Thus, in recent years it has been proven that an important role in the development of coronary heart disease and vascular pathology belongs to homocysteine, which is an intermediate product of metabolic conversions of methionine to cysteine. In case of insufficient utilization, homocysteine ​​leaves the cells in the liquid environment of the body, performing thrombogenic and pro-inflammatory effects. One of the main indicators of the immunological state of oral fluid is cationic antimicrobial peptides, which are important components of the immune system of a wide range of organisms. They play a key role in providing the first line of defense for the microorganism against infection. NHP1-NHP3 contain only 30 amino acid residues. Defensins show their microbial properties in micromolar conferences. Each representative of NHP has its own characteristic antimicrobial specificity of these peptides to each other. Defensins have the ability to suppress viral infection. They are effective against DNA and RNA viruses. Defensins inhibit the replication of human immunodeficiency virus, have high activity against the herpes virus. Also, these peptides inhibit vesicular stomatitis virus, human cytomegalovirus, influenza virus, adenovirus, papillomavirus and others. Conclusion. With secondary adentia (regardless of size and location), the volume of oral fluid decreases and its microelement composition changes. In addition, the replacement of dentition defects with different types of dentures causes metabolic disorders associated with the activation of free radical oxidation of biomolecules.

To evaluate possibilities of aspirin-induced gastroduodenopathy treatment in the patients with chronic ischemic heart disease by means of applying the internal endogenous prostaglandins stimulant.  Material and methods. 340 patients suffering from chronic coronary heart disease and receiving a long-term acetylsalicylic acid (ASA) therapy were examined on the base of the cardiovascular care unit of The Nizhny Novgorod Regional Clinical Hospital named after N.A. Semaschko. There were evaluated frequency, nature and severity of the aspirin-induced gastroduodenopathy. The patients with coronary heart disease and aspirin-induced gastroduodenopathy were divided in two groups. In the first group of patients there was applied rebamipide therapy (in a single daily dose 300 mg) in combination with the proton pump inhibitor (PPI) — pantoprazole. In the second group there was applied only pantoprazole therapy. For the purpose of specification of AIG pathogenetic mechanisms development, all the examined chronic coronary heart disease cases were tested on the prostaglandin E2 (PGE2) level in blood serum before the therapy beginning and after the treatment. The control group was formed of chronic coronary heart disease patients showing no AIG evidence. Statistical processing of the received data was fulfilled with the program «Statistika 10.0». Results. AIG was registered in 15% out of 340 chronic coronary heart disease patients. According to the endoscopic examination erosive disease of the body and antrum prevailed among the patients. The PGE2 level in the blood serum was significantly lower (р = 0,00087) in these patients in comparison with the control group. In association with PPI and rebamipide mixed therapy, esophagogastroduodenoscopy results showed no pathological findings in gastrointestinal mucosa and statistically significant (р = 0,00067) blood serum PGE2 level growing in all the treated patients. As a result of exclusive PPI therapy there was marked positive dynamics in endoscopic view in 19 out of 25 patients and a tendency to normalization of PGE2 level in the blood serum. However, PGE2 level growing was insignificant. Conclusion. The presented research demonstrates the possibility of AIG treatment with the use of internal endogenous prostaglandins stimulant — rebamipide in complex with proton pump inhibitor PPI therapy.

At a workshop to update the science linking saturated fatty acid (SAFA) consumption with the risk of coronary heart disease (CHD) and ischemic stroke, invited participants presented data on the consumption and bioavailability of SAFA and their functions in the body and food technology. Epidemiological methods and outcomes were related to the association between SAFA consumption and disease events and mortality. Participants reviewed the effects of SAFA on CHD, causal risk factors, and surrogate risk markers. Higher intakes of SAFA were not associated with higher risks of CHD or stroke apparently, but studies did not take macronutrient replacement into account. Replacing SAFA by cis-polyunsaturated fatty acids was associated with significant CHD risk reduction, which was confirmed by randomized controlled trials. SAFA reduction had little direct effect on stroke risk. Cohort studies suggest that the food matrix and source of SAFA have important health effects.

Subject. The scientific works of recent years show an increase in the degree of negative impact of vitamin deficiency and vitamin-like substances on the state of health of the population. With the deficiency of most vitamins, synthetic processes and regeneration of oral tissues are reduced, so often the initial signs of hypovitaminosis are stomatitis, gingivitis, glossitis, and therefore, it is dentists who are the first to diagnose deviations in the body associated with vitamin deficiency. This justifies the relevance and practical value of studying and describing clinical cases of manifestations of deficient vitamin conditions in the oral cavity. The object – is to study the effect of B vitamins on the oral mucosa in order to increase the effectiveness of diagnosis of vitamin-deficient conditions of the human body. Methodology. These clinical examples illustrate the management experience of patients with manifestations of deficient vitamin conditions on the oral mucosa. Clinical and laboratory methods of diagnosing the analysed conditions of the organism were applied. Statistical processing of materials was carried out using the STATISTICA 10 software (StatSoft, Inc., USA). Results. The deficient condition of the organism in relation to vitamins B2, B6, B12 in patients 18-75 years old has been studied. Characteristic clinical changes on the oral mucosa of the examined patients were established. So, in most patients with vitamin B2 deficiency, the classic Sebrel triad was found: dermatitis, glossitis, cheilitis. In those examined with a lack of vitamin B6, language desquamations (smoothed, polished language) were determined in the 83.6%, often combined with glossodinia. Patients with vitamin B12 deficiency were characterized by a lesion in the form of Meller-Gunter glossitis in 74.9% of cases, moreover, 67.6% of patients showed paresthesia in the area of tongue and oral mucosa. Conclusions. The study found that the first clinical symptoms of deficient conditions of the presented vitamins of group B were found from the oral cavity. Thus, it is the dentist who is the first to diagnose the pathological states of lack of group B vitamins in the human body, which emphasizes the importance and relevance of continuing to study these states of the body.

Introduction: Low Density Lipoprotein (LDL) is a type of low-density lipoprotein and the most widely transported cholesterol in the body. Increased levels of LDL in the body can be affected by genetics, age, gender, obesity, physical activity, lifestyle, drug consumption and smoking. Substances in a cigarette can cause an increase of LDL levels. Increased of LDL cholesterol levels can cause Coronary Heart Disease (CHD). The purpose of this research is to know the description of Low Density Lipoprotein (LDL) levels on smoker and non-smoker adolescent in Buyan Hamlet, Pancasari Village, Sukasada District, Buleleng Bali. Method: The type of this research is descriptive. This research was conducted in April-May 2017, which used fasting blood samples of 42 respondents. Result: From the average result of LDL level in smoker adolescent that is 134,91 mg/dL higher than the average of LDL level in non-smoker adolescent that is 74,90 mg/dL. The result of LDL cholesterol levels was determined by 21 smoker adolescent respondents with the close to optimal category (100-129 mg/dL) as many as 9 people (42,8%), and 12 people (57,3%) with worry category (130-159 mg/dL). Whereas in 21 non-smoker adolescent respondents obtained  result of LDL cholesterol level test with optimal category (<100 mg/dL) counted 18 people (87,71%) and 3 person (14,30%) with close to optimal category (100-129 mg/dL). Discussion: Based on the results of this research can be concluded that in smoker adolescent obtained LDL levels with close to optimal category and worrying whereas in non-smoker adolescents obtained LDL levels in the optimal category and close to optimal.

Background. L-arginine takes part in the ornithine cycle, in which ammonia is neutralized with urea formation. Accordingly, in the absence of L-arginine, toxic ammonia accumulates in the organism. Objective. To describe the use of L-arginine as a medication. Materials and methods. Analysis of literature sources on this topic. Results and discussion. Nitric oxide (NO) is a universal mediator in the regulation of cellular functions and intercellular communication. Deficiency of this substance is a key element of endothelial dysfunction. In the human body, NO is produced from L-arginine and performs an extremely important function of vasodilation due to the relaxation of vascular smooth muscle cells. Other physiological functions of NO include the transmission of neural signals in the central and peripheral nervous system, nervous activity, histamine secretion by mast cells, intestinal peristalsis, erection, killer action against bacteria and cancer cells. Arginine deficiency occurs in hypertension, coronary heart disease, obliterating disease of peripherl arteries, primary pulmonary hypertension, obstetric and perinatal pathological conditions. Potential ways to eliminate NO deficiency include the administration of its precursor – L-arginine (Tivortin, “Yuria-Pharm”) or its donors (molsidomine, nitrates). Tivortin has antioxidant and detoxifying effects, reduces endothelial dysfunction, activates urea synthesis, promotes glucose utilization, increases blood insulin. Conclusions. 1. NO deficiency is a key element of endothelial dysfunction. 2. Physiological functions of NO are vasodilation, transmission of neural signals, intestinal motility, etc. 3. In the human body NO is formed from L-arginine. 4. Arginine deficiency occurs in hypertension, coronary heart disease, obliterating disease of the peripheral arteries, primary pulmonary hypertension, obstetric and perinatal pathological conditions. 5. L-arginine (Tivortin) prescription is one of the ways to eliminate NO deficiency. 6. Tivortin has antioxidant and detoxifying effects, reduces endothelial dysfunction, and promotes glucose utilization.

Cardiovascular disease has serious economic and social consequences that affect people, health systems and societies around the world. Ischemic heart disease is one of the main global causes of death of the population, and its growth is predicted in the coming years. This fact continues to be of serious concern to health, social and economic services. Cardiovascular disease remains the most common cause of death, and age-standardized rates are higher for men than for women. Cardiovascular comorbidity increases significantly with age.The combination of ischemic heart disease and comorbid pathology is noted in cancer incidence, bronchial asthma, non-alcoholic fatty liver disease, type 2 diabetes mellitus, chronic obstructive pulmonary disease, periodontal disease, etc. It is necessary to analyze the existing disease with past diseases, risk factors and predictors available in the patient. The presence of concomitant diseases quite often requires additional diagnostic methods and changes in the tactics of treating coronary heart disease due to the fact that some of them are a contraindication to the use of certain groups of drugs. Periodontal disease includes a wide range of inflammatory conditions that affect the supporting structures of the teeth, which can lead to tooth loss and contribute to systemic inflammation. Periodontal disease is associated with several systemic diseases, one of which is coronary artery disease. It is imperative that clinicians understand the link between periodontal disease and cardiovascular disease. Comprehensive treatment of periodontitis and restoration of a healthy periodontium can help reduce overall inflammation in the body and reduce the risks of coronary heart disease.

To maintain homeostasis of the internal environment of the body, mechanisms are needed that control the exchange of various components between blood and the internal environment of tissues and organs. One of these physiological structures is the blood-salivary barrier, which regulates the selective metabolism between blood and oral fluid. Changes in the composition of biological fluids (blood and saliva) can be used as markers for diagnosing pathological conditions of the body and the dynamics of therapeutic measures. Of great importance is the study of the blood-salivary barrier for the search for drugs that are selectively transferred from the blood to the saliva. Some drugs are excreted in saliva at a concentration higher than they are in the blood. However, more often the concentration of drugs in saliva is much lower than in blood serum. Purpose: to form a list of the main trends and scientific and practical directions on the func-tioning of the hematosalvarial barrier. Materials and methods: to analyze the topic of the clinical significance of the hematosalvarial barrier, periodicals and scientific journals of domestic and foreign literature were studied, which determined the difference in the levels of trace elements, hormones, immune complexes between the oral fluid and serum. Conclusion: Analysis of literature data shows that the function of the blood-salivary barrier acts as a nonspecific adaptive mechanism, while saliva reflects the state of the protective functions of the barrier at the organismal level. Analysis of the selectivity and permeability of the blood-salivary barrier, as well as determination of changes in the composition and proper-ties of blood and oral fluid, can optimize and facilitate the diagnosis and control of treatment of various diseases of the oral cavity and the body as a whole.

The prevalence of 14 selected modifiable coronary heart disease (CHD) risk factors was determined in randomly selected adolescent boys (n = 117) and girls (n = 93) from provincial Greece. Based on published criteria thresholds for CHD, 45% of boys and 50% of girls exhibited three or more risk factors with time spent on “vigorous” activities, low cardiorespiratory fitness and fatness being among the most frequent in both sexes. Stronger associations were found between cardiorespiratory fitness and time spent on “vigorous” rather than “moderate-to-vigorous” activities in both boys and girls. Regression analysis indicated that energy expenditure (P < .01) in boys and energy expenditure (P < .05) and energy intake (P < .01) in girls could alone explain about 60% of the body-fat related findings in either group. Broadly based primary prevention strategies aimed at children should concentrate on reducing the overall energy intake and increasing the time spent on “vigorous” activities if future Greek adult CHD mortality is to be reduced.
