BACKGROUND: AGR2 expression is associated with luminal breast cancer. Overexpression of AGR2 is a predictor of poor prognosis. Several studies have found correlations between AGR2 in disseminated tumor cells (DTCs) in breast cancer patients. OBJECTIVE: This study aims to determine the correlation between anterior Gradient2 (AGR2) expression with the incidence of distant metastases in luminal breast cancer. METHODS: This study was an observational study using a cross-sectional method and was conducted at Wahidin Sudirohusodo Hospital and the network. ELISA methods examine AGR2 expression from blood serum of breast cancer patients. To compare the AGR2 expression in metastatic patients and the non-metastatic patient was tested with Mann Whitney test. The correlation of AGR2 expression and metastasis was tested with the Rank Spearman test. RESULTS: The mean value of AGR2 antibody expression on ELISA in this study was 2.90 ± 1.82 ng/dl, and its cut-off point was 2.1 ng/dl. Based on this cut-off point value, 14 subjects (66.7%) had overexpression of AGR2 serum ELISA, and 7 subjects (33.3%) had not. The mean value AGR2 was significantly higher in metastatic than not metastatic, 3.77 versus 1.76 (p < 0.01). The Spearman rank test obtained a p-value for the 2 tail test of 0.003 (p < 0.05), which showed a significant correlation of both, while the correlation coefficient of 0.612 showed a strong positive correlation of AGR2 overexpression and metastasis. CONCLUSIONS: AGR2 expression is correlated with metastasis in Luminal breast cancer.

               Oxidative stress occurs as a result of disturbance in the balance between the production of reactive oxygen species (free radicals) and antioxidant defenses. Markers of oxidative stress were measured the markers of oxidative stress in breast cancer patients after diagnosis of breast cancer and compared these plasma blood levels controls This study was conducted to three markers of oxidative stress ;these are (SOD) enzyme ,malondialdehyde (MDA)and8-iso-prostaglandinF2α plasma of patients with breast cancer and compare with controls .In this study ;  the mean MDA (ng/ml) levels for the breast cancer patients and the controls were55.91±3.31 and40.61±3.76  respectively, while the SOD (pg/ml) levels were1530.37±80.4 and1851.4 9±93.65  respectively and the 8-iso-PGF2α (ng/ml ) levels were 40.16±3.31 and 30.16±2.34  difference of the mean were statistically significant (p value <0.05).

ObjectivePrognostic nutritional index (PNI), calculated as serum albumin (ALB) (g/L) + 5 × total lymphocyte count (109/L), is initially used to evaluate nutritional status in patients undergoing surgery and may evaluate the therapeutic effects and predict the survival of various solid tumors. The present study aimed to evaluate the potential prognostic significance of PNI in breast cancer patients receiving neoadjuvant chemotherapy (NACT).MethodsA total of 785 breast cancer patients treated with neoadjuvant chemotherapy were enrolled in this retrospective study. The optimal cutoff value of PNI by receiver operating characteristic curve stratified patients into a low-PNI group (&lt;51) and a high PNI group (≥51). The associations between breast cancer and clinicopathological variables by PNI were determined by chi-square test or Fisher’s exact test. Kaplan–Meier plots and log-rank test were used to evaluate the clinical outcomes of disease-free survival (DFS) and overall survival (OS). The prognostic value of PNI was analyzed by univariate and multivariate Cox proportional hazards regression models. The toxicity of NACT was accessed by the National Cancer Institute Common Toxicity Criteria (NCI-CTC).ResultsThe results indicated that PNI had prognostic significance by an optimal cutoff value of 51 on DFS and OS in univariate and multivariate Cox regression survival analyses. Breast cancer patients with a high PNI value had longer DFS and OS than those with a low PNI value [47.64 vs. 36.60 months, P &lt; 0.0001, hazard ratio (HR) = 0.264, 95%CI = 0.160–0.435; 73.61 vs. 64.97 months, P &lt; 0.0001, HR = 0.319, 95%CI = 0.207–0.491, respectively]. Furthermore, the results indicated that patients with high PNI had longer DFS and OS than those with low PNI in early stage and advanced breast cancer, especially in advanced breast cancer. The mean DFS and OS times for breast cancer patients with high PNI by the log-rank test were longer than in those with low PNI in different molecular subtypes. Moreover, the mean DFS and OS times in patients with high PNI by the log-rank test were longer than in those patients with low PNI without or with lymph vessel invasion. The common toxicities after neoadjuvant chemotherapy were hematologic and gastrointestinal reaction, and the PNI had no significance on the toxicities of all enrolled patients, except in anemia, leukopenia, and myelosuppression.ConclusionPretreatment PNI with the advantages of being convenient, noninvasive, and reproducible was a useful prognostic indicator for breast cancer patients receiving neoadjuvant chemotherapy and is a promising biomarker for breast cancer on treatment strategy decisions.

Introduction: Breast cancer cases are increase and fast becoming the leading cause of oncologic mortality among women worldwide. Delay in management will worsen the prognosis of breast cancer patients. The poor prognosis is correlate with high grade of breast cancer malignancy. Histology can be used as an indicator to assess the grade of breast cancer malignancy. In addition, a high predictive value of lymphocyte-monocyte ratio (LMR) has been suggested to indicate favorable prognosis of various cancer, and used as parameters for assessing breast cancer prognosis. Method: This study is a quantitative research, using an observational approach. Laboratory examination results are collected from 43 breast cancer patients who will conduct treatment at Urip Sumoharjo Hospital from June to September 2019. Data are processed by the Spearman rank test using SPSS 22.0. Results: Correlation test results between lymphocyte-monocyte ratio with histological grading of breast cancer obtained a P value of 0.376. Conclusions: Lymphocytes to monocytes ratio (LMR) value cannot be utilize as a parameter for evaluating prognosis of breast cancer.

Abstract Objectives Nigeria has one of the highest morbidity rates from breast cancer; therefore, this work examined the proliferation rate of breast cancer and the hospital presentation time lag among women in Osun State, Nigeria. Methods This cross-sectional study was carried out in Obafemi Awolowo University Teaching Hospital Complex, Ile-Ife and Ladoke Akintola University Teaching Hospital, Oshogbo, Nigeria. The request cards and tissue blocks were sorted from the year 2014 to 2017. The time lag it took the patients to report at the hospitals was retrieved from the request cards. The breast tissue blocks were sectioned, stained with H&E, and graded. A representative tissue block was selected for each patient. Sections obtained from the blocks were stained with Mki67 antibody using a diaminobenzidine horseradish peroxidase technique. The cells were semiquantitatively scored as percentage of tumor cells stained brown. The mean ± SD was calculated. Results Out of 240 breast cancer patients sampled, 6 (2.5%), 97 (40.4), and 137 (57.1%), respectively, were grades 1, 2, and 3. The mean value of MKi67 was 23.90 ± 20.38, median 16.00. The mean value for grade 2 cancer was 15.31 ± 14.34; grade 3 cancer was 37.95 ± 22.47. Only 21.9% of breast cancer patients came to clinic within 3 months of lumps awareness in their breasts while 64.5% attended clinic after 6 months. Conclusions Breast cancer among the Osun State women was not very aggressive when comparing the Mki67 values obtained in this study with the values obtained in the developed world. Greater percentages of women with breast cancer waited too long before presenting themselves at the hospital. Thus, a high morbidity rate was recorded.

Introduction: Breast cancer is the most common malignant neoplasm that affects women.  One of the treatments for breast cancer is chemotherapy which is carried out according to a particular schedule with a specified length of therapy to increase the patient's recovery rate.  In addition to the physiological effects, chemotherapy also has a psychological impact on the patient.  Fulfillment of psychosocial needs is important to improve the quality of life of patients caused by changes in physical, social, cognitive, spiritual, emotional and role functions.  The purpose of this study was to determine the relationship between the cycle of chemoteraphy with psychosocial problems in breast cancer patients. Method: This study is a quantitative correlational study using 54 breast cancer patients as samples by purposive sampling.  Data collection was carried out during April 2020 to June 2020 in the chemotherapy room at Koja Hospital, North Jakarta. The Depression Anxiety Stress Scale (DASS 21) questionnaire was used to identify psychosocial problems in breast cancer patients.  Statistical analysis of the Spearman rank test was carried out to analyze the relationship between chemotherapy cycles and psychosocial problems. Results: The results showed that there was a relationship between chemotherapy cycles and psychosocial problems (p value 0.001) although it showed a weak relationship between those  two variables (r 0.257).  Psychosocial problems identified included anxiety (53%), depression (9.3%), low self-esteem (16.7%) and stress (20.4%). Conclusion: Nurses need to provide comprehensive nursing care for patients undergoing chemotherapy including identifying problems related to physical complaints and psychosocial needs so that nursing interventions can be given comprehensively to breast cancer patients undergoing chemotherapy.

e12540 Background: Cardiovascular disease is one of the leading causes of deaths in breast cancer patients. Pre-existing cardiac conditions and cardiovascular risks associated with chemotherapy affect the overall mortality of these patients. Cardio protective medications like beta-blocker, ACEIs, ARBs, statin, and potassium-sparing diuretics have shown to increase the survival odds. In our study, we investigated the survival of breast cancer patients who were on cardio protective medications. Methods: Our retrospective cohort study included breast cancer patients from Simmons Cancer Institute, Springfield, Illinois from January 1st, 2014 to December 31st, 2017. The first step comprised of screening patients using ICD code of breast cancer. Next, a retrospective chart review led to selecting eligible patients with biopsy proven breast cancer diagnosis within the aforementioned dates. We excluded patients who were diagnosed outside the give time frame. Data on age, gender, medical comorbidities, medications, and survival status were also collected. Comparisons between categorical variables were compared with the Chi-Squared test. Survival curves were estimated using Kaplan-Meier methodology and analyzed with a log rank test. Predictors of survival are assessed with Cox proportional hazards regression analyses. Results: Screening using ICD codes resulted in 1618 patients. Of those, 478 had biopsy proven diagnosis within the dates mentioned. 301 out of 478 patients were on some kind of cardio protective medication (88 on beta-blockers, 73 on ACEIs, 41 on ARBs, 87 on statins, and 12 on potassium sparing diuretics). We looked into survival analysis in patients on different cardioprotective medications. ARBs and potassium sparing diuretics showed increased likelihood of survival as the hazard ratio (HR) was found to be 0.512 (95% Cl: 0.068-3.832, p-value:0.0919) and HR 0.390 (95% CI: 0.130-1.66, p-value:0.514) respectively. In contrast, other cardio protective medications showed decreased trends for survival. The HR of beta blocker was 1.734 (95% CI: 0.508-5.919, p-value: 0.379) and of ACEIs was 1.350 (95% CI: 0.395-4.613, p-value: 0.632). Similarly, statins had a hazard ratio of 1.534 (95% CI: 0.512- 4.594, p-value:0. 444). Conclusions: We found that the cardio protective medications showed no statistically significant difference in survival in breast cancer patients. Given the smaller sample size and single center study, further multi-center clinical trial is warranted to establish a stronger association.

This is regarded as a possible etilogical factor in production of breast carcinoma, which is the most cmmon cancer among the females both globally as well as in India. Oxidative stress also plays an important role in progression of cancer breast. This study was conducted to compare the levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in breast cancer patients and normal age matched female participants.Methods: This study was conducted in AVBR Hospital, Sawangi (Meghe) among 30 female breast cancer patients admitted in the surgery ward for treatment after being diagnosed with the disease, who attended the hospital from July 2016 to June 2017. Breast cancer patients of stage I and II were included in the study. Patients suffering from any other diseases apart from breast cancer like diabetes, hypertension, thyroid illnesses and other sort of cancers are excluded from the study. For each of the study participant, age and sex matched controls were selected who were mostly the relatives of patients attending the hospital and were devoid of any illnesses. Venous blood sample was collected from all the patients as well as the controls that were used for determination of the levels of SOD (superoxide dismutase) and MDA (malondialdehyde). Written informed consent was taken from all the study participants before drawing the blood samples.Results: Authors have measured the markers of oxidative stress in breast cancer patients after diagnosis of breast cancer and compared theses blood levels with those of the controls. The mean MDA (nmol/ml) levels for the breast cancer patients and the controls were 26.14 and 15.83 respectively, while the SOD (U/gm Hb) levels were 0.12 and 0.16 respectively and the difference of the mean were statistically significant (p value <0.01).Conclusions: There is presence of increased oxidative stress in breast cancer patients as compared to the healthy controls.

AbstractSurvival of breast cancer patients has improved, and treatment-related changes regarding metabolic profile deterioration after neoadjuvant systemic treatment (NST) become important issues in cancer survivors. We sought to compare metabolic profile changes and the neutrophil-to-lymphocyte ratio (NLR) between patients undergoing neoadjuvant chemotherapy (NCT) and neoadjuvant endocrine therapy (NET) 3 years after the treatment. In a prospective, randomized, phase III trial which compared 24 weeks of NCT with adriamycin and cyclophosphamide followed by docetaxel and NET with goserelin and tamoxifen (NEST), 123 patients in the Asan Medical Center were retrospectively reviewed to evaluate metabolic changes, such as body mass index (BMI), blood pressure (BP), total cholesterol (TC), fasting glucose, and the NLR. The mean age of patients was 42 years. The changes in BMI, serum glucose, and TC during NST and after 3 years were significantly different between NCT and NET. The proportion of overweight + obese group and the mean BMI were significantly increased during NCT (26.6% to 37.5%, 22.84 kg/m2 to 23.87 kg/m2, p < 0.05), and these attributes found to have normalized at the 3-year follow-up. In the NET group, BMI changes were not observed (p > 0.05, all). There were no differences in changes over time among in the Hypertension group during NCT and NET (p = 0.96). The mean value of serum TC and fasting glucose significantly increased (< 0.05, both) during NCT and decreased 3 years after NCT (p < 0.05); however, no significant changes were observed in the NET group. The NLR was increased from 1.83 to 3.18 after NCT (p < 0.05) and decreased from 1.98 to 1.43 (p < 0.05) after NET. Compared with minimal metabolic effect of NET, NCT worsens metabolic profiles, which were recovered over 3 years. The NLR was increased after NCT but decreased after NET.

Purpose: The aim of this study is to identify differential metabolomic signatures in plasma samples of distinct subtypes of breast cancer patients that could be used in clinical practice as diagnostic biomarkers for these molecular phenotypes and to provide a more individualized and accurate therapeutic procedure. Methods: Untargeted LC-HRMS metabolomics approach in positive and negative electrospray ionization mode was used to analyze plasma samples from LA, LB, HER2+ and TN breast cancer patients and healthy controls in order to determine specific metabolomic profiles through univariate and multivariate statistical data analysis. Results: We tentatively identified altered metabolites displaying concentration variations among the four breast cancer molecular subtypes. We found a biomarker panel of 5 candidates in LA, 7 in LB, 5 in HER2 and 3 in TN that were able to discriminate each breast cancer subtype with a false discovery range corrected p-value < 0.05 and a fold-change cutoff value > 1.3. The model clinical value was evaluated with the AUROC, providing diagnostic capacities above 0.85. Conclusion: Our study identifies metabolic profiling differences in molecular phenotypes of breast cancer. This may represent a key step towards therapy improvement in personalized medicine and prioritization of tailored therapeutic intervention strategies.

Abstract Background The role of surgery for the primary tumor in breast cancer patients with bone metastases (BM) remains unclear. The purpose of this study was to determine the impact of surgery for the primary tumor in breast cancer patients with BM and to develop prognostic nomograms to predict the overall survival (OS) of breast cancer patients with BM. Methods A total of 3956 breast cancer patients with BM from the Surveillance, Epidemiology, and End Results database between 2010 and 2016 were included. Propensity score matching (PSM) was used to eliminate the bias between the surgery and non-surgery groups. The Kaplan-Meier analysis and the log-rank test were performed to compare the OS between two groups. Cox proportional risk regression models were used to identify independent prognostic factors. Two nomograms were constructed for predicting the OS of patients in the surgery and non-surgery groups, respectively. In addition, calibration curve, receiver operating characteristic (ROC) curve, and decision curve analysis (DCA) were used to evaluate the performance of nomograms. Result The survival analysis showed that the surgery of the primary tumor significantly improved the OS for breast cancer patients with BM. Based on independent prognostic factors, separate nomograms were constructed for the surgery and non-surgery groups. The calibration and ROC curves of these nomograms indicated that both two models have high predictive accuracy, with the area under the curve values ≥0.700 on both the training and validation cohorts. Moreover, DCA showed that nomograms have strong clinical utility. Based on the results of the X-tile analysis, all patients were classified in the low-risk-of-death subgroup had a better prognosis. Conclusion The surgery of the primary tumor may provide survival benefits for breast cancer patients with BM. Furthermore, these prognostic nomograms we constructed may be used as a tool to accurately assess the long-term prognosis of patients and help clinicians to develop individualized treatment strategies.
