The incidence of acute pancreatitis (AP) in children has significantly increased in the past two decades. However, it can be associated with severe morbidity and mortality. Therefore it should be considered in every child presented with unexplained severe abdominal pain. The etiology, clinical manifestation, and course of acute pancreatitis in children are often different than in adults. Therefore the early and effective diagnosis of acute pancreatitis in children is challenging. In this review, we will discuss the etiology, clinical features and laboratory testing for the disease and also the management outlines of acute pancreatitis in children.Northern International Medical College Journal Vol.8(2) January 2017: 235-238

Introduction:Acute Pancreatitis is a medical emergency, which is one of the most common conditions for hospital admission. Very few studies have yet investigated in Bangladesh. Objective of this study was to define demographic characteristics, clinical profile & underlying etiologies. Materials and Methods: This hospital based cross sectional descriptive study was performed in Sir Salimullah Medical College Mitford Hospital, Dhaka & North East Medical College, Sylhet, Bangladesh from January 2015 to December 2017. One hundred and five consecutive patients of acute pancreatitis (AP) were enrolled for this study. Clinical features and investigations were systematically recorded. Diagnosis of acute pancreatitis was made by the presence of the two of the three following criteria: i. abdominal pain consistent with the disease ii. serum amylase and /or lipase greater than three times from the upper limit of normal, and/or iii. characteristic findings of abdominal imaging. Results: Total of 105 patients took part in the study, of them 65 were male. The mean age was 42.76 ±15.88. Abdominal pain & vomiting was the most common mode of presentation. Gall stone and hypertriglyceridaemia were responsible of 20% of acute pancreatitis. Ascariasis also causes acute pancreatitis in two patients. Conclusion: Acute pancreatitis is a condition associated with high morbidity and mortality. Ascariasis also causes acute pancreatitis in endemic area. Patients usually respond conservative treatment but endoscopic treatment is effective in few cases. Surgery is rarely required. Medicine Today 2020 Vol.32(1): 21-24

Obstructive sleep apnea (OSA) is one of the most common causes of sleep disordered breathing in children. However, it is associated with significant morbidity, potentially impacting on long term neurocognitive and behavioural development, as well as cardiovascular outcomes and metabolic homeostasis. Diagnosis and treatment of this condition in children differs in many aspects from that in adults. Background: Pancreatitis is a serious disorder of pancreas. This study was designed to investigate the clinical features and outcomes of the hospitalized patients with acute pancreatitis (AP) with or without diabetic mellitus (DM). Methods: A total of 90 adult hospitalized patients with AP diagnosed in three different tertiary hospitals were evaluated for inclusion in prospective study on clinical, laboratory and outcome parameters. After exclusion according to exclusion criteria, 68 patients were enrolled finally, and out of 68 patients 34 were diabetic with AP from BIRDEM and 34 AP patients were non-diabetic from Dhaka Medical College and BSMMU. Results: The major clinical features, laboratory markers were significantly (p<0.05) higher whereas serum albumin levels was significantly (p<0.05) lower in diabetic AP group as compared to the non-diabetic AP group. DM was associated with severe form of AP compared to the non-diabetic group (p<0.05). Conclusions: The results of the present study suggest that DM increases the disease severity of AP. Therefore, clinicians should pay more attention in the treatment and management of acute pancreatitis if the patients have DM. Birdem Med J 2020; 10(3): 172-181

IgA vasculitis, also known as the Henoch–Schönlein purpura, is the most common systemic vasculitis in children. Inflammation most often involves small blood vessels of the skin, joints, kidneys and the gastrointestinal tract, but other organs may also be involved. Acute pancreatitis is a rare clinical manifestation of IgA vasculitis. We present a case of a 12-year-old girl with cutaneous and abdominal symptoms, who was diagnosed with acute pancreatitis during IgA vasculitis. In patients with IgA vasculitis who report abdominal pain acute pancreatitis should be taken into account and pancreatic enzymes measurement should be considered.

Background: Pancreatitis is quite common problem, it may present either as abdominal emergency with fulminant course or as an indolent process leading to long-term medical as well as surgical complications often leading to poor prognosis if not intervened timely.Methods: The present study conducted in the department of medicine, Dr. S. N. medical college, Jodhpur. Participants after understanding the study protocol and procedure, asked to give their written consent for the study. A cross sectional hospital based study in patients admitted in various medical wards of Dr. S. N. medical college, Jodhpur. 50 patients with acute and chronic pancreatitis were studied.Results: In the present study, most common cause of acute pancreatitis is biliary disease (50%) followed by alcoholism (37.5%) and in chronic pancreatitis is alcoholism (80%). Acute pancreatitis was more common in males (62.5%, 25 males) whereas chronic pancreatitis in males (80%, 8 male). Abdominal pain is the most common symptom (97.5%) followed by nausea-vomiting (92.5%) in acute pancreatitis. The history of previous abdominal pain in 100% of cases followed by epigastria pain in 90% of cases, in chronic pancreatitis. The amylase and lipase were elevated in 90% of cases. The amylase and lipase levels did not correlate with the severity. 66.6% of patients had severe pancreatitis with a positive predictive value of 66.6%. The ultrasonography imaging of pancreas was helpful in 70% and 100% in acute and chronic pancreatitis respectively. The CT scan was a better imaging modality as compared to ultrasonography in acute pancreatitis, where as it scored over ultrasonography imaging in chronic pancreatitis with complications.Conclusions: Relevant clinical history, ultrasonography and computed tomography scan of pancreas are helpful in diagnosis of pancreatitis. The computed tomography scan was a better imaging modality as compared to ultrasonography.

Irritable bowel syndrome (IBS) is a biopsychosocial disease consisting of a set of functional disorders not explained by organic changes in the intestine. A total of 117 patients and healthy people were examined. Patients were divided into 2 groups: the first group were patients with IBS, which were divided into 2 subgroups: IBSd (diarrhea) - 51 patients (20 men and 31 women), IBSc (constipation) - 66 patients (33 men and 33 women). More abdominal pain syndrome was noted in patients with a clinical form of IBS with a predominance of diarrhea. The ordinal assessment of the severity of SRC symptoms allows to objectify the severity of clinical manifestation of intestinal dysfunction in patients, identify the clinical features of the course of this pathology.

Nipah viral encephalitis is one of the fatal re-emerging infections especially in southeast Asia. After its outbreak in Malaysia and Singapore; repeated outbreaks occurred at western part of Bangladesh especially in Faridpur region. Besides, sporadic attacks appear to occur in the country throughout the year. Here two Nipah outbreaks in greater Faridpur district in 2003 and 2004 are described along with brief review on transmission of the virus. Where the history of illness among patients are very much in favour of man to man transmission. Moreover the death of an intern doctor from Nipah encephalitis who was involved in managing such patients in Faridpur Medical College Hospital strongly suggests man to man transmission of this virus. So, aim of this review article to make the health personnel and general people be aware about man to man transmission of virus, so that they can adapt personal protection equipment (PPE) for their protection against this deadly disease. This review of the Literacy and Education Bklletin1 of the 1961 Census is fourth in the series of review articles published in this journal2. The Bulletin under review forms a part of the interim report on the characteristics of the population of Pakistan. It gives information on the number of illiterate and literate persons by age and sex for rural and urban areas on division and district basis; illiterate and literate.population in selected cities and towns; and the educational levels attained by the literate population by age and sex for divisions and districts. Relevant statistical notes and statements precede the tables in the Bulletin. The objective of this review is to describe the meaningfulness and significance of literacy statistics. To this end, a distinction is made between formal and functional levels of literacy. Comparisons of the 1951 and 1961 census figures are undertaken to indicate the progress of literacy and education during the past decade with reference to the effect of intercensal rate of population growth on such progress. Certain questions regarding the reliability of data are raised, which emphasize the need for caution in the interpretation of literacy statistics.

: Nucleosides and their analogues have been in use for many years and have become essential for treating patients with viral infections. Many additional nucleoside drugs have been approved over the past decades. This strongly demonstrates how important these compounds are and the crucial role they play. Given that a significant amount of research and literature has been documented regarding nucleoside analogues, this review article mainly focuses the discussion on nucleosides and nucleoside analogous that have proven to play significant role or be emerging in the treatment of known viral infections. This covers the names, structures, applications, toxicity, and mode of action of relevant nucleoside analogues.
