Objective: to provide an overview of modern methods for assessing the function and strength of the pelvic floor muscles in women available for clinical practice. The main provisions. Foreign and domestic sources found in free access Internet databases were analyzed. Visual observation, palpation, perineometry, electromyography, ultrasound and magnetic resonance imaging measure various aspects of the function and strength of the pelvic floor muscles. Perineometry allows you to accurately measure the force of contractions of the pelvic floor muscles and is an easy and affordable way. Oxford vaginal palpation technologies, biofeedback devices, and ultrasound can be important clinical tools in the practice of a physician to quantify dysfunction of the pelvic floor. Conclusion. The obtained data on methods for assessing the function and strength of the pelvic floor muscles indicate the need for perineometry in clinical practice for the timely diagnosis of pelvic floor insufficiency, which will allow choosing the right rehabilitation method for women of different age groups.

The aim of this study was to assess the reliability of pelvic floor muscles evaluation via transabdominal ultrasonography in young nulliparous women and to present the methodology for quantitative assessment of the ultrasound image of the pelvic floor muscles visible as displacement of the posterior wall of the bladder, caused by action of the pelvic floor muscles. The study comprised 30 young, Caucasian, nulliparous women (age 22–27; 168.6 ± 5.1 cm; 57.1 ± 11.8 kg) without pelvic floor muscle dysfunctions. The intra-rater, test-retest and inter-rater reliability of pelvic floor muscles evaluation was performed using transabdominal ultrasound at rest and during voluntary contraction. The reliability was assessed at three points of the image (at the middle, on the right and left side). The reliability of the three-point measurement of the pelvic floor muscles transabdominal ultrasound is excellent in the case of intra-rater assessments, both at rest (ICC = 0.98–0.99) and during contraction (ICC = 0.97–0.98); moderate at rest (ICC = 0.54–0.62) and poor during contraction (ICC = 0.22–0.50) in the case of test–retest assessment; excellent at rest (ICC = 0.95–0.96), and good during contraction (ICC = 0.81–0.87) in the case of inter-rater assessment. Transabdominal ultrasound is a reliable method of pelvic floor muscle evaluation. The three-points of assessment used in our study allowed for broader and more comprehensive imaging of the pelvic floor muscle, e.g., for quantitative detection contractility imbalances between the left and right side Due to the fact that understanding mechanisms of pelvic floor muscle functioning is crucial in the therapy of pelvic floor dysfunctions, therefore, reliable, valid tests and instruments are important.

The relevance of this study is due to the high prevalence of stress urinary incontinence in women of different age groups and the search for effective conservative treatment methods. The aim of this study was to present scientific data based on modern evidence on the effectiveness of conservative treatment methods and their safety in the treatment of stress urinary incontinence in women. A review of the literature (original articles, systematic reviews) on the use of conservative therapy for the treatment of stress urinary incontinence in women was carried out. Depending on the severity of stress urinary incontinence in women, their age and concomitant diseases, the options for the proposed treatment may vary. For the conservative treatment of the disease, along with lifestyle modification and the use of hygiene products, it is possible to use training and electromyostimulation of the pelvic floor muscles, innovative laser technologies, and drug therapy. The first line therapy is to train ones pelvic floor muscles. The best effect of the training is observed in younger patients with mild to moderate severity of the disease using stationary biofeedback simulators under the supervision of medical staff. An alternative to this type of treatment is to conduct training of the pelvic floor muscles in the biofeedback mode with portable simulators remotely under the supervision of specially trained personnel. In connection with the coronavirus pandemic, this therapy is gaining wide popularity, as every patient should be offered acceptable and effective treatment options.

Background. In recent decades, various physiotherapeutic methods have been widely used in surgical practice in order to obtain an anti-inflammatory and bactericidal effects, accelerate reparative and regenerative processes, and improve microcirculation in the postoperative area. Aim: to develop and scientifically substantiate optimized complex programs, including general magnetotherapy, fractional microablative therapy with a CO2 laser, electrical myostimulation with the biological connection of the pelvic floor muscles and a special complex of physiotherapy exercises (exercise therapy) in patients of different age groups after reconstructive plastic surgery for rectocele. Methods. The article presents data on treatment of 160 women of childbearing, peri- and menopausal age with rectocele of IIIII degree. Patients after surgical treatment of rectocele, in the late postoperative period for the full formation used rehabilitation programs, including in various combinations of General magnetotherapy, electromyostimulation with biological connection of the pelvic floor muscles, procedures of intra-vascular fractional microablative therapy with CO2 laser and a special complex of therapeutic physical education. Results. As a result of the research, it was found that when combined with General magnetotherapy, fractional microablative CO2 laser therapy, electromyostimulation with biological connection of the pelvic floor muscles, and a special complex of physical therapy in patients after plastic surgery for rectocele, a pronounced trophostimulating effect is formed, to a greater extent in patients with peri- and of menopausal age, which is particularly important, as they have in the background of the age hypoestrogenia perimenopausal and postmenopausal women marked atrophy. Conclusions. Complex treatment that promotes proper formation of postoperative scar by improving microcirculation and thickening of the dermis and mucosa, endometrium, submucosa and connective tissue, as confirmed by the ultrasound scan.

Background. The specific weight of genital prolapse among gynecological nosologies in Russia in the departments of operative gynecology ranges from 28 to 38.9% and in the structure of planned indications for surgical treatment ranks third after benign tumors and endometriosis. Aim. Development and implementation of new comprehensive rehabilitation programs for women with IIIII degree rectocele after surgical treatment. Methods. The article presents the treatment data for 100 women of different age groups with IIIII degree rectocele who, in the early postoperative period after plastic surgery for rectocell, used to improve the condition of the vaginal mucosa with general magnetotherapy, electromyostimulation with biological connection of the pelvic floor muscles, a special complex of physiotherapy exercises and intravaginal procedures of fractional microablative carbon dioxide laser therapy. The patients were divided into 2 groups comparable in terms of clinical and functional characteristics; within each group, depending on age, they were divided into 2 subgroups: women of childbearing age were included in subgroup A, and peri- and menopausal women were included in subgroup B. To assess the condition of the vaginal mucosa and maturity of the vaginal epithelium after surgery in patients included in the study, a pH-metry of the vaginal discharge was performed and the Vaginal Health Index was determined. Results. As a result of the studies, it was found that a more pronounced therapeutic effect was obtained with the use of the complex, which includes a course of general magnetotherapy from the first day after surgery, as well as a month after surgery, a course of electromyostimulation with biological connection of the pelvic floor muscles and a special complex of physiotherapy exercises and 2 intravaginal procedures of fractional microablative carbon dioxide laser therapy. Conclusion. The obtained results indicate a pronounced therapeutic effect of the complex, which includes a course of General magnetotherapy from the first day after surgery, a course of electromyostimulation with biological connection of pelvic floor muscles and a special complex of physical therapy, as well as 2 intravaginal procedures of fractionation microablative therapy with a carbon dioxide laser 1 month after surgery..

The study of law focuses, among other aspects, on important issues relating to equality, fairness and justice in as far as free access to information and knowledgeis concerned. The launching of the Open Access to Law Movement in 1992, the promulgation of the Durham Statement on Open Access to Legal Scholarshipin 2009, and the formation of national and regional Legal Information Institutes (LIIs) should serve as an indication of how well the legal world is committed to freely publishing and distributing legal information and knowledge through the Internet to legal practitioners, legal scholars and the public at large aroundthe world. In order to establish the amount of legal scholarly content which is accessible through open access publishing innovations and initiatives, this studyanalysed the contents of websites for selected open access resources on the Internet internationally and in South Africa. The results of the study showed that there has been a steady developing trend towards the adoption of open access for legal scholarly literature internationally, while in South Africa legal scholarly literature is under the control of commercial publishers. This should be an issue for the legal scholarship which, among its focus, is to impart knowledge about the right of access to information and knowledge.

Introduction. The use of classical methods for diagnosing muscle fatigue of physical workers, including dynamometry and electromyography, is often limited due to the complexity of the process, the inability to use them in production, and the subjectivity of the methodology. At the same time, such a method as myotonometry does not have these disadvantages, but the main area of its use at the moment is clinical practice. The aim of study was to determine the possibility of using myotonometry to assess muscle fatigue. Materials and methods. In the course of the study, the biomechanical characteristics of muscles that differ in their depth were evaluated using the "MyotonPro" device (Myoton AS, Estonia). We determined the tone and stiffness of the biceps of the shoulder, the soleus muscle, and the muscle that straightens the back at different periods of time under the influence of maximum load on them. Results. It was found that the studied parameters of these muscles immediately after performing a series of exercises "to failure" statistically significantly increased, and after 30 minutes - again decreased, but did not reach the initial level. Conclusions. The myotonometric study showed objectivity, reliability, repeatability and proved to be one of the most promising modern methods for assessing muscle fatigue, including for solving problems in the field of occupational health.

A forest owner can only commercialize non-wood products and services within a tightly restricted market niche. On account of free access being permitted to the forest it is impossible to deny to third parties the consumption of many non-wood products and services: everybody has the right to be in the forest for recreation. As a result many non-wood services cannot be commercialized by the forest owner, or not exclusively. What would seem unthinkable elsewhere on private property seems to be taken for granted in the forest: third parties may take products from the forest and even sell them without being the forest owners. For certain nonwood services or products, such as the installation of rope parks or for burial in the forest, the organizer must conclude an agreement with the forest owner or draw up a contract for servitude or benefit. In addition, for these activities a permit from the Forestry Department is necessary. On the other hand, for an itinerant school class or for the production of forest honey neither a binding regulation with the forest owner nor a permit from the Forestry service is necessary, provided that no constructions are erected in the forest. The only exclusive right which remains to the forest owner, besides the sale of his property, is the exploitation of his trees within the legal framework.

Nowadays there are certain difficulties in the early diagnosis of ulcerative colitis, proceeding with minimal intestinal symptoms. The etiology of the disease remains unclear to this day; there is no exact information about the prevalence of the disease due to the large number of latent forms and the low number of patients seeking medical help. This article presents the results of a retrospective analysis of the incidence of ulcerative colitis in the coloproctology department of the First Republic Clinical Hospital of Izhevsk. The study was conducted to assess the frequency of occurrence and determine the internal picture of ulcerative colitis. To achieve this goal, we selected 34 patients with ulcerative colitis. A statistical study was conducted on the following criteria: gender composition, age groups, forms of the disease, localization, complications, and main complaints. The length of hospital stay often was not more than 20 days. The literature on this pathology was also studied and presented in the form of a theoretical basis, which consisted of the determination and etiology of inflammatory bowel diseases. Due to the unknown etiology, insufficiently studied pathogenesis, difficult differential diagnosis with other intestinal diseases and insufficiently perfect treatment methods, there are a large number of unresolved problems in the field of ulcerative colitis. That is why, in this article we tried to reveal the problem of the features of the course and complex therapy of that disease in clinical practice.
