Background: LL-37 is the only human antimicrobial peptide that belongs to the cathelicidins. The aim of the study was to evaluate the efficacy of LL-37 in the management of MRSA-infected surgical wounds in mice. Methods: A wound on the back of adult male BALB/c mice was made and inoculated with Staphylococcus aureus. Two control groups were formed (uninfected and not treated, C0; infected and not treated, C1) and six contaminated groups were treated, respectively, with: teicoplanin, LL-37, given topically and /or systemically. Histological examination of VEGF expression and micro-vessel density, and bacterial cultures of wound tissues, were performed. Results: Histological examination of wounds in the group treated with topical and intraperitoneal LL-37 showed increased re-epithelialization, formation of the granulation tissue, collagen organization, and angiogenesis. Conclusions: Based on the mode of action, LL-37 has a potential future role in the management of infected wounds.

ABSTRACT Quorum sensing is a mechanism through which a bacterial population receives input from neighboring cells and elicits an appropriate response to enable survival within the host. Inhibiting quorum sensing by RNAIII-inhibiting peptide (RIP) has been demonstrated as a very effective mode of prevention and therapy for device-associated staphylococcal infections and was tested here for healing of wounds that are otherwise resistant to conventional antibiotics. Wounds, established through the panniculus carnosus of BALB/c mice, were inoculated with 5 × 107 CFU of methicillin-resistant Staphylococcus aureus. Mice were treated with Allevyn, RIP-soaked Allevyn (containing 20 μg RIP), daily intraperitoneal teicoplanin (7 mg/kg of body weight), Allevyn and teicoplanin, and RIP-soaked Allevyn and daily intraperitoneal teicoplanin. The main outcome measures were quantitative bacterial culture and histological examination with assessment of microvessel density and of vascular endothelial growth factor (VEGF) expression in tissue sections. Treatment with RIP-soaked Allevyn together with teicoplanin injection greatly reduced the bacterial load to 13 CFU/g (control untreated animals had 108 CFU/g bacteria). All other treatments were also significantly effective but only reduced the bacterial load to about 103 CFU/ml. Histological examination indicated that only treatment with RIP-soaked Allevyn with teicoplanin injection restored epithelial, granulation, and collagen scores, as well as microvessel density and VEGF expression, to the levels found with uninfected mice. In conclusion, we observed that RIP may be useful for the management of infected wounds and that it could represent an exciting and future alternative to the conventional antibiotics, at present considered the gold-standard treatments for methicillin-resistant S. aureus infections.

ABSTRACTDalbavancin is an effective antibiotic widely used to treat skin infection. Our aim was to determinate the effects of dalbavancin administration on wound healing compared to vancomycin, and to elucidate if EGFR, MMP-1, MMP-9 and VEGF could be involved in its therapeutic mechanism.A mouse model of MRSA skin infection was established. Mice were treated daily with vancomycin (10mg/kg) and weekly with dalbavancin, at day 1 (20 mg/kg) and day 8 (10 mg/kg). After 14 days wounds were excised and bacterial counts were perfomed. Wound healing was assessed by histological and immunohistochemical staining, followed by protein extraction and immunoblotting. Our microbiological results confirmed that both dalbanvancin and vancomycin are effective in reducing the bacterial load in wounds. Dalbavancin had a strong effect compared with infected untreated animals and vancomycin treated group. The wounds treated with dalbavancin showed robust epidermal coverage with a reconstitution of the regular and keratinized epidermal lining and a well-organized granulation tissue with numerous blood vessels, although slightly less than in the uninfected group, while in vancomycin treated group the epithelium appeared in general still hypertrophic, the granulation tissue appears even less organized.We observed elevated EGFR and VEGF expression in both treated groups, although it was higher in dalbavancin treated mice. MMP-1 and MMP-9 were decreased in uninfected and in both treated tissue when compared with untreatd infected wounds.This study showed faster healing with dalbavancin treatment that might be associated with a higher EGFR and VEGF levels.

To determine the microbiology of wound infection following caesarean section and to evaluate the use of Gram stain for the predicton of subsequent microbiological culture results, 1319 surgical wounds were followed up. We did Gram stains and cultures on exudates from open wounds and on aspirates if the wounds had demonstrable fluid collection. Incidence of post-caesarean wound infection was 8.1%. Ninety-three [86.9%] of 107 infected wounds were culture positive, with Staphylococcus aureus the most frequently found organism [42%]. Organisms seen by Gram stain yielded a sensitivity of 96.6%, specificity of 88.9%, positive predictive value of 97.7% and negative predictive value of 84.2% when used to predict positive culture results for bacterial wound infection

Abstract Background Anti-vascular endothelial growth factor (VEGF) has been used as a therapeutic drug for the treatment of some human diseases. However, no systematic evidence is performed for assessing the role of VEGF in periodontitis. We carried out a comprehensive analysis to explore the role of VEGF in patients with periodontitis. Methods Multiple databases were searched for eligible studies. The pooled standardized mean difference (SMD) and odds ratio (OR) with the corresponding 95% confidence interval (CI) were applied to evaluate the effect sizes. Clinical data validation from microarray analysis was used. Pathway and process enrichment analysis were also investigated. Results Finally, 16 studies were included in this analysis. Overall, there was a significantly higher level of VEGF expression in periodontitis than in healthy control groups (OR = 16.64, 95% CI = 6.01–46.06, P < 0.001; SMD = 2.25, 95% CI = 1.25–3.24, P < 0.001). Subgroup analysis of ethnicity showed that VEGF expression was still correlated with periodontitis in the Asian and European populations. No correlation was observed between VEGF expression and age, gender, and pathological type. A large clinical sample data (427 periodontitis patients and 136 healthy controls) further validated that VEGF expression was higher in periodontitis than in healthy control groups (P = 0.023). VEGF was involved in many functions such as blood vessel development, response to growth factor, cell proliferation, and cell adhesion. Conclusions High levels of VEGF were credible implications for the development of periodontitis. Anti-VEGF therapy may be valuable for the treatment of periodontitis in clinical management.

In the studies described in the article, the presence of antimicrobial activity in photosensitizers in experiments on bacterial cultures was confirmed. In the experiments, photosensitizers of the second generation were used: «Photoditazine», the active substance of which is dimeglumin chloride E6, and «Dimegin», which includes a derivative of protoporphyrin IX-2,4-di(1-methoxyethyl)- deuteroporphyrin IX disodium salt. In the described research work, the minimum concentrations of the studied drugs were determined by the method of sequential tenfold dilutions to obtain a bactericidal effect on strains of sanitary-indicative microorganisms Enterococcus faecalis, Staphylococcus aureus, Salmonella enteritidis.

Abstract Antibiotic resistance is one of the greatest challenges to treat bacterial infections worldwide, leading to increase in medical expenses, prolonged hospital stay and increased mortality. The use of blue light has been suggested as an innovative alternative to overcome this problem. In this study we analyzed the antibacterial effect of blue light using low emission parameters on Staphylococcus aureus cultures. In vitro bacterial cultures were used in two experimental approaches. The first approach included single or fractionated blue light application provided by LED emitters (470 nm), with the following fluencies: 16.29, 27.16 and 54.32 J/cm2. For the second approach a power LED (470 nm) was used to deliver 54.32 J/cm2 fractionated in 3 applications. Our results demonstrated that bacterial cultures exposed to fractionated blue light radiation exhibited significantly smaller sizes colonies than the control group after 24 h incubation, however the affected bacteria were able to adapt and continue to proliferate after prolonged incubation time. We could conclude that the hypothetical clinical use of low fluencies of blue light as an antibacterial treatment is risky, since its action is not definitive and proves to be ineffective at least for the strain used in this study.

Abstract: A cesarean delivery increases the risk of wound infection it should be prevented by using antibiotics. This study was to determine the pattern of bacterial sensitivity in surgical wounds of cesarean section patients for selected antibiotics, i.e ceftriaxone, cefixime, sulbactam ampicillin, ciprofloxacin, clindamycin and gentamicin.This observational study was conducted at the Ulin Public Hospital in Banjarmasin from July to September 2019. Using a cross sectional approach Samples of bacteria were identidied from 36 that were pusposively sampled, i.e., Staphylococcus aureus, Staphylococcus epidermidis and Escherichia coli. Antibiotic sensitivity test showed that Staphylococcus aureus was sensitive against gentamicin (100%),whereas Staphylococcus epidermidis and Escherichia coli. ABSTRACTThe aim of this study was to evaluate the combination of daptomycin and fosfomycin in experimental chronic implant-associated osteomyelitis due to methicillin-resistantStaphylococcus aureus(MRSA). Infection was induced in the tibiae of rats by the insertion of a bacterial inoculum (1 to 5 × 108CFU/ml) of a clinical MRSA isolate and a titanium wire. Four weeks after infection, each animal was assigned to a treatment group: daptomycin monotherapy at 60 mg/kg of body weight once daily (n= 10), fosfomycin monotherapy at 40 mg/kg once daily (n= 10), or daptomycin and fosfomycin combined at 60 mg/kg and 40 mg/kg, respectively, once daily (n= 9). Ten animals were left untreated. After a 3-week treatment period, the animals were euthanized, and the infected tibiae and implants were processed for quantitative bacterial cultures. The bacterial cultures from bones were positive for MRSA in all animals in the untreated group, the daptomycin group, and the fosfomycin group, with median bacterial counts of 2.34 × 106CFU/g bone, 1.57 × 106CFU/g bone, and 3.48 × 102CFU/g bone, respectively. In the daptomycin-fosfomycin group, 6 out of 9 animals were positive for MRSA, with a median count of 7.92 CFU/g bone. Bacterial cultures derived from the titanium wires were negative in the fosfomycin- and daptomycin-fosfomycin-treated groups. Based on bacterial counts in bones, treatment with daptomycin-fosfomycin was statistically significantly superior to all that of the other groups (P≤ 0.003). Fosfomycin was superior to daptomycin and no treatment (P< 0.0001). No development of resistance was observed in any treatment arm. The combination of daptomycin and fosfomycin demonstrated synergism against MRSA in experimental implant-associated osteomyelitis.
