(1) Background: To evaluate women’s satisfaction with the care received during childbirth undergoing labour and how this changes over time. (2) Methods: An observational, multicentre and longitudinal study in two public hospitals of Spain with a convenience sample of 248 women during the post-partum period. Satisfaction was evaluated using the Intrapartal-Specific Quality from the Patient’s Perspective questionnaire. Socio-demographic and clinical variables were considered. A bivariate analysis evaluated the relationship between satisfaction and the recorded variables to compare the hypotheses, and also to know if there were any significant differences between the scores obtained at the various evaluated time points (three days, 15 days, one year postpartum). The multivariate analysis by binary logistic regression helped complete the study about the variables related to the level of satisfaction; (3) Results: The mean scores for satisfaction at each time point were high, and they lowered with time. The bivariate analysis showed an association between satisfaction and the following variables: ‘mother’s place of birth’, ‘level of education’, ‘type of labour onset’, ‘type of birth’ and ‘type of perineal trauma’. For the multivariate analysis, women who gave birth vaginally, began labour spontaneously and had an epidural were more satisfied than those who did not; (4) Conclusions: Women seemed to obtain good levels of satisfaction with the care they received. Even so, based on continuously improved quality, we proposed some improvement measures, particularly regarding the physical environment and the information offered to patients. It is important to know at what time experience while giving birth should be evaluated as scores can lower with time as part of its normal evolution.

IntroductionProgress in reducing maternal and neonatal mortality, particularly in sub-Saharan Africa, is insufficient to achieve the Sustainable Developmental Goals by 2030. The first 24 hours following childbirth (immediate postnatal period), where the majority of morbidity and mortality occurs, is critical for mothers and babies. In Uganda,<50% of women reported receiving such care. This paper describes the coverage, changes over time and determinants of immediate postnatal care in Uganda after facility births between 2001 and 2016.MethodsWe analysed the 2006, 2011 and 2016 Ugandan Demographic and Health Surveys, including women 15–49 years with most recent live birth in a healthcare facility during the survey 5-year recall period. Immediate postnatal care coverage and changes over time were presented descriptively. Multivariable logistic regression was used to examine determinants of immediate postnatal care.ResultsData from 12 872 mothers were analysed. Between 2006 and 2016, births in healthcare facilities increased from 44.6% (95% CI: 41.9% to 47.3%) to 75.2% (95% CI: 73.4% to 77.0%) and coverage of immediate maternal postnatal care from 35.7% (95% CI 33.4% to 38.1%) to 65.0% (95% CI: 63.2% to 66.7%). The majority of first checks occurred between 1 and 4 hours post partum; the median time reduced from 4 hours to 1 hour. The most important factor associated with receipt of immediate postnatal care was women having a caesarean section birth adjusted OR (aOR) 2.93 (95% CI: 2.28 to 3.75). Other significant factors included exposure to mass media aOR 1.38 (95% CI: 1.15 to 1.65), baby being weighed at birth aOR 1.84 (95% CI: 1.58 to 2.14) and receipt of antenatal care with 4+Antenatal visits aOR 2.34 (95% CI: 1.50 to 3.64).ConclusionIn Uganda, a large gap in coverage remains and universal immediate postnatal care has not materialised through increasing facility-based births or longer length of stay. To ensure universal coverage of high-quality care during this critical time, we recommend that maternal and newborn services should be integrated and actively involve mothers and their partners.

Objective: We analyzed the changes in voice- and swallowing-related symptoms that occurred over time in patients who underwent thyroidectomy and identified any associated risk factors. Methods: One hundred and three patients who underwent thyroidectomy were enrolled. Results: The mean thyroidectomy voice-related questionnaire (TVQ) score before surgery was 12.41 ± 12.19; it significantly increased to 28.24 ± 18.01 ( P < .001) 1 month postoperatively, decreased to 24.02 ± 17.30 ( P = .014) and 20.66 ± 15.29 ( P = .023) 3 and 6 months postoperatively, respectively. It was continuously decreased to 18.83 ± 14.63 twelve months postoperatively. The temporal changes in TVQ scores between patients who underwent total thyroidectomy or lobectomy were significantly different. There was a statistically significant difference in the temporal changes in TVQ according to whether neck dissection was performed. The temporal changes in TVQ in patients with and without extrathyroidal extension were significantly different. Conclusions: Voice- and swallowing-related discomfort in patients who received thyroidectomy showed dynamic changes over time. There was a significant difference in the degree of change according to clinicopathological factors. Patients with these risk factors may benefit from appropriate patient education and various rehabilitation programs for symptom relief.

At two different times, Time 1 and Time 2, 13 participants in Japan (8 Japanese and 5 Americans) were asked to spontaneously respond in English to this prompt: “Tell me about one of the most exciting or dangerous moments in your life.” The Japanese responded during their first and fourth years of college, which involved an interval of 42 months. The Americans were native speakers of English and responded earlier and later in their one year study abroad program in Japanese language and culture. Three questions addressed by this paper were the following: (a) What types of topics and narrative structures characterize these 26 stories? (b) What types of speaker-initiated repairs appear, and are the repairs the same or different at Time 1 and Time 2? (c) How are the repairs related to different listener (American, Japanese, Filipino, and Taiwanese) assessments of the intelligibility of the narratives?

Charcoal warming tradition is believed to help the recovery process in postpartum mothers. The purpose of this study was to determine the relationship between charcoal warming traditions and physical conditions of the house with the incidence of acute respiratory infections in postpartum mothers. The method of reseaech used the analytic case control approach. The population of this research was 50 mothers who had history of acute respiratory infections in Berampu village and the control sample was 50 people. The technique of the sample of this research is probability sampling.The Statistical analysis used bivariate analysis with chi square test and multivariate analysis with logistic regression test. The results of the bivariate analysis showed a relationship between the Charcoal Warming Tradition, p value = 0.002, between maternal parity, p value = 0.043, between the physical conditions of the house, p = 0.027, and the incidence of acute respiratory infections. The results of the multivariate analysis show that there is an influence between the Charcoal Warming Tradition (Exp B: 3,626), Mother Parity (Exp B: 2,517) and the Physical Condition of the House (Exp B: 2,354) on the incidence of acute respiratory infections in post- partum mothers in Berampu Village, District, Dairi Regency in 2020 The Suggestions for researchers Furthermore, this research is used as reference material regarding acute respiratory infections in order to carry out the proper management of acute respiratory infections. or Respondents to replace the warming tradition by using blankets. Abstract Research Question How concentrated is the total harm of offences with detected offenders (identified suspects) among the complete list of all detected offenders in a given year in an English police agency, and how consistent is the list of highest-harm “felonious few” offenders from one year to the next? Data Characteristics of 327,566 crimes and 39,545 unique offenders as recorded by Northamptonshire Police in 7 years from 2010 to 2016 provide the basis for this analysis. Methods Crime and offender records were matched to harm weightings derived from the Cambridge Crime Harm Index (Sherman et al. 2016a; Sherman et al., Policing, 10(3), 171–183, 2016b). Descriptive statistics summarize a concentration of harm identifying the felonious few, changes over time in membership of the “few”, offender typologies and tests for escalation of severity, frequency and intermittency across repeated offences. Findings Crime harm is much more concentrated among offenders than crime volume: 80% of crime harm that is identified to an offender is linked to a felonious few of just 7% of all detected offenders. While chronic repeat offenders are the majority contributors to harm totals of this group, those with the most general range of offence types contribute the most harm. Individual members of the felonious few rarely maintain that position year on year; over 95% of each year’s list is composed of individuals not present in previous years. Within individual crime histories, we observe a pattern of de-escalation in crime harm per offence over time. “One-time” offenders, those with just one crime record, typically made up a third of the felonious few in both number and harm contribution. Conclusions These findings demonstrate the potential to target a small number of repeat offenders for harm reduction strategies using a metric of total crime severity, not just volume, despite a substantial portion of crime harm caused by one-time offenders that may be largely unpredictable.

Introduction: Gait speed is a simple and safe measure with strong predictive value for negative health outcomes in clinical practice, yet in-laboratory gait speed seems not representative for daily-life gait speed. This study aimed to investigate the interrelation between and robustness of in-laboratory and daily-life gait speed measures over 12 months in 61- to 70-year-old adults. Methods: Gait speed was assessed in laboratory through standardized stopwatch tests and in daily life by 7 days of trunk accelerometry in the PreventIT cohort, at baseline, and after 6 and 12 months. The interrelation was investigated using Pearson’s correlations between gait speed measures at each time point. For robustness, changes over time and variance components were assessed by ANOVA and measurement agreement over time by Bland-Altman analyses. Results: Included were 189 participants (median age 67 years [interquartile range: 64–68], 52.2% females). In-laboratory and daily-life gait speed measures showed low correlations (Pearson’s r = 0.045–0.455) at each time point. Moreover, both in-laboratory and daily-life gait speed measures appeared robust over time, with comparable and smaller within-subject than between-subject variance (range 0.001–0.095 m/s and 0.032–0.397 m/s, respectively) and minimal differences between measurements over time (Bland-Altman) with wide limits of agreement (standard deviation of mean difference range: 0.12–0.34 m/s). Discussion/Conclusion: In-laboratory and daily-life gait speed measures show robust assessments of gait speed over 12 months and are distinct constructs in this population of high-functioning adults. This suggests that (a combination of) both measures may have added value in predicting health outcomes.

Latar Belakang: Post partum dikategorikan sebagai sindroma gangguan mental yang ringan yang terjadi pada masa nifas. Namun sering tidak diperdulikan sehingga tidak terdiagnosis dan tidak ditindak lanjuti sebagaimana seharusnya. Padahal keadaan ini bisa menjadi serius dan bisa bertahan dua minggu sampai satu tahun serta akan berlanjut menjadi depresi dan psikosis post partum. Prevalensi kejadian post partum blues di Indonesia cukup tinggi dengan prevalensi 50-70%. Banyak faktor yang mempengaruhi kejadian post partum blues diantaranya usia, paritas, pekerjaan, pendidikan, pendapatan, dukungan suami dan keluarga dan status kehamilan.Tujuan: Tujuan penelitian ini adalah untuk mengetahui faktor determinan yang mempengaruhi kejadian post partum blues pada ibu nifas di wilayah kerja Puskesmas Ibuh Kota Payakumbuh tahun 2019. Jenis penelitian ini adalah deskriptif korelatif. Penelitian ini dilakukan di wilayah kerja Puskesmas Ibuh pada bulan Mei-Juli 2019. Populasi dari penelitian ini adalah ibu nifas hari ke 2-7.Metode: Teknik pengambilan sampel dengan cara consecutive sampling. Instrumen penelitian yang digunakan berupa kuesioner EPDS (Edinburgh Postnatal Depressions Scale) untuk mengukur kejadian post partum  blues pada ibu nifas. Teknik analisis data yang digunakan adalah chi square dan regresi logistic berganda.Hasil: Hasil analisis bivariat didapatkan tiga variabel independen yang berhubungan dengan variable dependen yaitu usia, pekerjaan dan status kehamilan. Hasil analisis multivariat didapatkan bahwa faktor determinan yang mempengaruhi kejadian post partum blues adalah status kehamilan dengan nilai OR 20,598.Simpulan: Upaya yang bisa dilakukan untuk mencegah kejadian post partum blues dengan pemberian pendidikan kesehatan mengenai perubahan psikologis dan persiapan menghadapi persalinan yang diberikan petugas kesehatan kepada ibu hamil.Kata Kunci:  ibu nifas, Post partum blues Background: Post partum is categorized as a mild mental disorder syndrome that occurs during the puerperium. However, it is often ignored so it is not diagnosed and not followed up as it should. Though this situation can be serious and can last two weeks to one year and will continue to be depression and post partum psychosis. The prevalence of post partum blues in Indonesia is quite high with a prevalence of 50-70%. Many factors affect the incidence of post partum blues including age, parity, work, education, income, husband and family support and pregnancy status. Objective: The purpose of this study was to determine the determinant factors that influence the incidence of post partum blues in postpartum mothers in the working area of Ibuh Payakumbuh City Health Center in 2019. This type of research is descriptive correlative. This research was conducted in the working area of Ibuh Public Health Center in May-July 2019. The population of this study was post-partum mothers day 2-7. The sampling technique is by consecutive sampling. Method: The research instrument used was a questionnaire EPDS (Edinburgh Postnatal Depressions Scale) to measure the incidence of post partum blues in postpartum mothers. Data analysis techniques used are chi square and multiple logistic regression. Results: The results of the bivariate analysis found three independent variables related to the dependent variable, namely age, occupation and pregnancy status. The results of multivariate analysis found that the determinant factor that influenced the incidence of post partum blues was pregnancy status with an OR value of 20.598. Conclusion: Efforts can be made to prevent the post partum blues incident by providing health education regarding psychological changes and preparation for childbirth provided by health workers to pregnant mothers. Keywords: Post partum blues, postpartum mothers

Abstract Background. The mental health effects of healthcare work during the COVID-19 pandemic have been substantial, but it is not known how long they will persist. This study aimed to determine if hospital workers’ burnout and psychological distress increased monotonically over one year, during which waves of case numbers and hospitalizations waxed and waned, or followed some other pattern.Methods. A prospective longitudinal survey was conducted at four time-points over one year in a cohort of 538 hospital workers and learners, which included validated measures of burnout (emotional exhaustion scale of Maslach Burnout Inventory) and psychological distress (K6). Repeated measures ANOVA tested changes over time and differences between subjects by occupational role and age. The direction and magnitude of changes over time were investigated by plotting rates of high scores (using cut-offs) at each time-point compared to case rates of COVID-19 in the city in which the study took place. Results. There were significant changes in emotional exhaustion over time (F = 4.6, p = .004) and significant effects of occupational role (F = 11.4, p < .001) and age (F = 12.3, p < .001). The rate of high burnout was highest in nurses, followed by other healthcare professionals, other clinical staff, and lowest in non-clinical staff. Peak rates of high burnout occurred at the second or third measurement point for each occupational group, with lower rates at the fourth measurement point. Similarly to the results for emotional exhaustion, rates of high psychological distress peaked at the winter 2021 or spring 2021 measurement point in each occupational group and were highest in nurses.Conclusions. Neither emotional exhaustion nor psychological distress was rising monotonically. Burnout and psychological distress were consistently related to occupational role and were highest in nurses. Although emotional exhaustion improved as the case rate of COVID-19 decreased, rates of high emotional exhaustion in nurses and other healthcare professionals remained higher than was typically measured in hospital-based healthcare workers prior to the pandemic. Ongoing monitoring of healthcare workers’ mental health is warranted. Organizational and individual interventions to support healthcare workers continue to be important.

Abstract. Summary: Background: We aimed at estimating the agreement between the Medicap® (photo-optical) and Radiometer® (electro-chemical) sensors during exercise transcutaneous oxygen pressure (tcpO2) tests. Our hypothesis was that although absolute starting values (tcpO2rest: mean over 2 minutes) might be different, tcpO2-changes over time and the minimal value of the decrease from rest of oxygen pressure (DROPmin) results at exercise shall be concordant between the two systems. Patients and methods: Forty seven patients with arterial claudication (65 + / - 7 years) performed a treadmill test with 5 probes each of the electro-chemical and photo-optical devices simultaneously, one of each system on the chest, on each buttock and on each calf. Results: Seventeen Medicap® probes disconnected during the tests. tcpO2rest and DROPmin values were higher with Medicap® than with Radiometer®, by 13.7 + / - 17.1 mm Hg and 3.4 + / - 11.7 mm Hg, respectively. Despite the differences in absolute starting values, changes over time were similar between the two systems. The concordance between the two systems was approximately 70 % for classification of test results from DROPmin. Conclusions: Photo-optical sensors are promising alternatives to electro-chemical sensors for exercise oximetry, provided that miniaturisation and weight reduction of the new sensors are possible.
