Introduction: Regular physical activities may have effect on the course of chronic kidney disease (CKD). Here, we aimed to ascertain the changes of serum interleukin-17 (IL-17) following eight weeks of aerobic training in CKD patients. Methods: The CKD patients referred to Zahedan Edalat Clinic and Ali-Ibn Abi Talib hospital in Zahedan city (Iran) were enrolled. Sixty patients aged between 30 and 50 years old were chosen by a random method and assigned into the control and intervention groups (each group constituted 30 people). In this study, aerobic exercises were performed at 50%–80% of the maximal heart rate. Peripheral blood was obtained one day before the beginning of exercise and one day after the end of the intervention. Serum IL-17 level was quantified using a commercial specific ELISA kit. Results: The mean values of IL-17 in CKD patients before and after 8 weeks of aerobic exercise were 1.67 ± 0.403 pg/mL and 1.58 ± 0.170 pg/mL in the intervention group (P value= 0.039) whereas the mean values of IL-17 in the control group before and after the intervention were 1.31 ± 0.529 pg/mL and 1.35 ± 0.505 pg/mL (P value= 0.794).Conclusion: Eight weeks of aerobic training can significantly reduce serum IL-17, an inflammatory marker, in CKD patients.

The effects of guided imagery on fatigue in patients with chronic kidney disease undergoing hemodialysisBackground: Kidney and urinary tract diseases contribute 850,000 mortality rates placing them as the disease burden in the world. One of the general nursing problems suffered from chronic kidney disease patients who undergo hemodialysis is fatigue. The intervention that can be given to overcome the fatigue on the clients is a relaxation therapy strategy. A well-recommended therapy to relieve the fatigue is known as guided imagery.Purpose: Knowing the effect of guided imagery on fatigue in patients with chronic kidney disease undergoing hemodialysis.Methods: A quantitative with a quasi experiment technique and nonequivalent control group design. The population of this study included 67 patients registered with chronic kidney disease undergoing hemodialysis at the hospital. The purposive sampling technique took 32 people. The samples were distributed into two groups; 16 people in each group. The analysis was a thorough T-Test.Result: Finding that the mean of the fatigue levels of experiment and control groups were different. The mean score before the intervention was 0.352, while the score after the intervention was 2.000 (p-value 0.025).Conclusion: There was the effect of guided imagery on fatigue in patients with chronic kidney disease undergoing hemodialysis. Chronic kidney disease patients who undergo hemodialysis should independently and regularly do relaxation therapy because this therapy is easy to apply and proven effective to reduce fatigue.Keywords: Guided Imagery; Fatigue; Chronic Kidney Disease; HemodialysisPendahuluan: Penyakit ginjal dan saluran perkemihan berkontribusi menjadi beban penyakit di dunia dengan sekitar 850.000 kematian setiap tahun. Masalah keperawatan yang banyak dihadapi pasien gagal ginjal kronik yang menjalani hemodialisa umumnya adalah fatigue (kelemahan). Intervensi keperawatan untuk membantu klien dalam mengatasi masalah kelemahan diantaranya melalui strategi pemberian terapi relaksasi. Salah terapi relaksasi yang dipercaya dapat membantu mengatasi fatigue adalah guided imagery.Tujuan: Diketahuinya pengaruh guided imagery terhadap tingkat fatigue pasien gagal ginjal kronik yang menjalani hemodialisis.Metode: Penelitian kuantitatif, desain quasi eksperiment dengan rancangan nonequivalent control group design. Populasi pasien gagal ginjal kronik yang menjalani hemodialisa yang berjumlah 67 orang, besar sampel yang diambil sebanyak 32 orang yang terbagi dalam 2 kelompok, masing-masing kelompok sebanyak 16 orang, teknik pengambilan sampel menggunakan purposive sampling. Analisis menggunakan uji t-test.Hasil: Didapatkan selisih rata-rata tingkat fatigue antara kelompok intervensi dengan kelompok kontrol sebelum perlakuan adalah 0,352 dan setelah perlakuan 2,000 (p-value 0,025).Simpulan: Ada pengaruh guided imagery terhadap tingkat fatigue pasien gagal ginjal kronik yang menjalani hemodialisis. Bagi pasien gagal ginjal kronik yang menjalani hemodialisa hendaknya dapat mempergunakan terapi relaksasi secara mandiri dan teratur karena selain mudah dilakukan, terapi ini telah terbukti efektif dalam menurunkan tingkat kelelahan.

The magnitude of chronic renal disease is enormous, as the prevalence of kidney failure is rising. Anaemia is a common complication of chronic kidney disease (CKD) that develops early in its course and becomes increasingly severe as the disease progresses. The aim is to evaluate the serum level of iron, Total Iron Binding Capacity (TIBC), transferrin saturation and ferritin in chronic kidney disease population in Zaria and control subjects. This study was conducted at ABUTH Zaria were 125 patients in various stages of CKD who presented at the nephrology clinic and equal number of apparently healthy age and sex matched controls were recruited. The mean (SD) age of patient and controls were 48 (14) years. These were made up of 53.6% males, and 46.4% females. Mean values of serum creatinine significantly higher in the patients (<0.0001). There was no significant difference in the mean values of iron (p=0.32) and TIBC (p=1.29) in both study groups. The patients had a significantly (p˂0.0001) higher mean value for ferritin and TSAT than the control group. There were higher serum creatinine and ferritin values in males than in females while higher serum TIBC, estimated creatinine clearance and iron were observed in females than males. Serum creatinine, ferritin and estimated creatinine clearance of male patients were found to be significantly higher with p-value of 0.002, 0.000 and 0.028 respectively than that of female patients. No significant differences were noted in serum levels of iron, TIBC and TSAT. Serum creatinine, ferritin and TSAT were found to be significantly elevated in CKD patients while serum Iron and TIBC were not.

Background: Depression can occur in patients with chronic kidney disease undergoing hemodialysis and can lead to decreased quality of life, and will have a two-fold risk for the occurrence of death and hospitalization. Objective: The purpose of this study was to identify the effects of art therapy on the level of depression patients with hemodialysis.Methods: This study used a quasi-experimental method with pretest posttest with control group design. Collecting data using questionnaires of Beck Depression Inventory (BDI-II) versions of Indonesia, which was done twice, before and after art therapy intervention. Data were analyzed using paired t-test and unpaired t-test.Results: The results showed that after given intervention of art therapy there were differences in the average scores of depression in the intervention group (t = 0.764; p-value = 0.000). Art therapy is a medium to expose and express the feelings, fears or perceived problem, so it can be used as an adaptive coping method in patients with chronic kidney disease undergoing hemodialysis. Conclusion: It is concluded that art therapy could reduce depression in patients with chronic kidney disease undergoing hemodialysis. Nurses can act as a facilitator to provide art-based therapy in order to improve the ability of psychological adaptation in patients with chronic kidney disease undergoing hemodialysis.

Young women often feel primary dysmenorrhoea because the hormonal cycles experienced are not stable, this can disrupt the concentration and activity of young women. The principle of back to nature is increasingly popular today, the side effects of chemical drugs can cause new problems, it is one of the driving force of the development of traditional medicine. Papaya leaves contain Vitamin E which can reduce dysmenorrhea. In addition, turmeric acids also contain curcumine and anthocyanins that inhibit cyclooxygenase, thereby reducing the occurrence of inflammation during uterine contractions. The aim of this research is to know the effectiveness of papaya leaf stew with acidic turmeric to primary dysmenorrhea. Type of Research is an experimental study with a Pretest-Posttest Control Group Design design. The sample is 30 people. The research was conducted on December 27, 2017 s.d February 24, 2018 at SMP Negeri 46 Palembang. Instrument to measure pain Numeric Rating Scale. The statistical test used by Wilcoxon and Mann Whitney U. Average score of pain before papaya leaves stem 5.40 ± 0.73 while the mean score of pain after given turmeric acid 5.33 ± 0.61 The mean score of pain after being given papaya leaves stew 3.60 ± 0.91 while the mean score of pain after given turmeric acid 4.06 ± 0.79. There was a difference of mean score of dysmenorrhea pain before and after given papaya leaf stem p value = 0.000. There is difference of mean score of dysmenorrhea pain before and after given turmeric acid p value = 0,002. There was no difference of mean score of dysmenorrhea pain before and after given papaya leaf sting and turmeric acid p value = 0,217. The decoction of papaya leaf and turmeric acid have the same effectiveness in reducing primary dysmenorrhea pain.

Abstract Background Executive functions (EFs) are critical to daily life and sensitive to our physiological functioning and emotional states. The number of people living with chronic kidney disease (CKD) on hemodialysis (HD) globally is increasing steadily. We aimed to determine the impact of a single session of HD on EFs in patients with CKD receiving maintenance HD (MHD). Methods This was a quasi-experimental study conducted at the department of psychiatry and dialysis unit of a tertiary hospital. Patients undergoing MHD underwent screening to rule out delirium, using the Confusion Assessment Method prior to EF testing. The tests of EF used were the Trail-Making Test—Part B (TMT-B) and Frontal Assessment Battery (FAB), both of which were administered before and after a session of HD. Statistical tests used were Wilcoxon matched pairs signed ranks test, paired t-test, single sample t-test, and correlation analyses. Results The mean time taken on TMT-B before HD was 195.36 seconds and after HD, 171.1 seconds; difference is significant (p = 0.0001). The mean FAB score was 13.19 before HD and 14.83 after HD; the difference is significant (p < 0.0001). Significant differences were observed on similarities (p = 0.003), lexical fluency (p = 0.02), and go–no go (p = 0.003) subtests of FAB. Mean TMT-B scores before and after HD differed significantly from that of a reference study (reference TMT-B 150.69 seconds), p = 0.0002 and 0.04, respectively. Conclusion We conclude that patients with CKD on MHD, in general, have worse executive cognitive functioning compared with healthy populations. A session of HD results in significant improvement in these functions.

ABSTRACT: Intermittent hemodialysis (IHD) is a form of renal replacement that is used in veterinary medicine for cases involving drug removal, electrolyte imbalance, acute kidney injury, and chronic kidney disease (CKD). The aim of the present study was to verify the efficacy of IHD in dogs with CKD staged at grade III and to evaluate the effect of IHD on quality of life. Twelve dogs with CKD at stage III met the inclusion criteria and were divided equally into two groups. The control group (n=6) received only clinical treatment and intravenous fluid therapy, and the hemodialysis group (n=6) received clinical and IHD treatments. Blood samples were collected before and after treatments in both groups. We evaluated complications and clinical parameters of IHD every 30 minutes. Hemodialysis decreased serum urea, creatinine, and phosphorus. Despite the evident removal of nitrogen compounds, dialysis treatment did not increase survival time in these patients. The results of this study do not support the early use of dialysis in dogs with chronic kidney disease stage III.

 Background: Chronic kidney disease (CKD) includes irreversible destruction of nephrons leading to progressive decline in glomerular filtration rate. A preferential defect in Homocysteine disposal could hypothetically occur in CKD and subsequently lead to hyperhomocysteinemia. Understanding the status of Homocysteine and other parameters in CKD is useful in the management of the disease. Objective of the study is to estimate serum Homocysteine in CKD patients and its association with renal function and serum albumin in patients with CKD.Methods: The study design involves hospital based observational comparative study. The study was conducted in Department of Biochemistry in association with Department of Nephrology of Mahatma Gandhi Medical College and Hospital, Jaipur between May 2017 to June 2018. 100 diagnosed patients of CKD, visiting the Outpatient Department of Nephrology were enrolled as cases for the study. Patients having cardiovascular disease, Chronic liver disease, Age more than 60 years and pregnant females were excluded from study. The control group consists of 100 age and sex matched healthy individuals.Results: The mean serum creatinine levels of case and control group were 7.50±3.74 mg% and 0.83±0.22 mg% respectively. The mean of serum homocysteine levels of subject group was 27.35±12.52 µmol/L while the mean serum homocysteine levels of control group was 11.06±3.52 µmol/L. The serum homocysteine levels were significantly higher in the CKD patient group. The serum level of albumin in CKD patients and control group were 2.86±0.86 g/dl and 4.10±0.58 g/dl respectively. A positive correlation was found between serum creatinine and serum homocysteine levels. A negative correlation between serum homocysteine and serum albumin was found.Conclusions: Findings of the present study exhibit that serum homocysteine levels are elevated in CKD in comparison to healthy controls and it is positively correlated with serum creatinine level.

Background: Foodborne illnesses linked to fresh produce has been on the rise in recent years. This increase in illnesses pose a massive burden to the healthcare system. One way to prevent this is through educating the public the importance of handwashing produce. There are many studies that have tested ways of washing and sanitizing fresh produce to reduce surface pathogens. However the average consumer mainly hand washes produce before consuming it. Thus this study intends to test the efficacy of hand washing by testing surface ATP on apples before and after they have been washed. Methods: Apples were purchased from the bulk section of a super market. ATP swabs were used to test the concentration of ATP on the surface of the apples. The values were recorded. All the apples were hand washed under running tap water for 15 seconds and the surface ATP concentration were obtained and recorded. The values were then compared to draw a conclusion. Results: The results show that there is statistically significant reduction in surface ATP values on apples after washing them (mean ATP value of 33.2) compared to before washing them (mean ATP value of 116.67). The p-value obtained was 0.00033 when α = 0.05. Conclusion: This study was able to conclude that there was a significant reduction in surface ATP values following handwashing of the apples. There was an observed 60% reduction in the mean values of ATP of before and after hand washing. In conclusion, hand washing apples do provide an adequate reduction of surface ATP values thus attaining surface cleanliness.

Background: Hypertensive disorders complicate upto 5 to 10% of all pregnancies. Though the exact cause of pre-eclampsia is still undecided, maternal hyperhomocysteinemia has been implicated as a risk factor for pre eclampsia, placental abruption and other vascular diseases. The objectives of present study were to estimate the levels of serum homocysteine in antenatal patients and to study the above parameters in patients of pre-eclampsia.Methods: A prospective observational study was performed with 30 pre eclamptic patients and an equal number taken as control having comparable demographic characteristics. Level of homocysteine was measured by an enzymatic method, using Diazyme homocysteine 2 reagent enzymatic assay kit on Beckman coulter analyzer in all the patients. Obstetrics and neonatal outcomes were observed in all the patients. The statistical analysis was done using unpaired T test for determining level of significance.Results: Mean Serum homocysteine in the study group was 13.99±5.46 µmol/l and was 6.03±2.58 µmol/l in control group. This was statistically significant (p value 0.002). However the mean values of serum homocysteine did not correlate with severity of pre-eclampsia 14.32±6.72 µmol/l in mild pre-eclampsia and 13.60±3.77μmol/l in severe pre-eclampsia respectively (p value - 0.727).Conclusions: It appears that maternal serum homocysteine has a causal role in pathogenesis of pre eclampsia, however to recommend it as a routine test, larger studies are required.

Abstract Background and Aims Atherosclerosis and its associated cardiovascular diseases starting from the early stages of chronic kidney disease (CKD) are the most important cause of increased morbi-mortality in the CKD process. In studies performed in patients with end-stage renal disease (ESRD), it is observed that the calcification occured in the vascular structures was an important component of the atero-arteriolosclerosis process. The number of studies investigating the relationship between vascular calcification and the development of atherosclerosis and increased morbi-mortality in the process of CKD are quite small and limited to patients undergoing hemodialysis (HD) treatment for ESRD. We aimed to investigate the factors affecting the development of atherosclerosis and the role of calcification inhibitors fetuin-A, matrix-Gla protein (MGP), osteoprotegerin (OPG) in atherosclerosis progress. Method Our study was planned to investigate the relationship of serum OPG, MGP and fetuin-A levels with the development of atherosclerosis in the stage 2-3-4-5 chronic kidney patients who did not require dialysis treatment. Thirty-two (17 female, 15 male) healthy individuals and 92 (49 females, 43 males) CKD cases were included. The healthy control group did not have a history of regular use of medication for any reason, known acute or chronic disease. Chronic kidney disease group, with no acute disease, no history of known malignancy and cerebrovascular disease. The patients' GFR was also calculated with CKD-EPI Formula. The mean carotid artery intima media thickness was calculated by dividing the sum of right and left carotid artery intima media thickness. Statistical analysis was performed with IBM SPSS Statistics 20.0.0. Results The laboratory data of the healthy control group, stage 2 CKD group, stage 3 CKD group, stage 4 CKD group and stage 5 CKD groups were statistically compared with the healthy control group, between themselves and the whole CKD group, the results were given in Table-1. Chronic kidney disease group divided into two groups; carotid artery intima media thickness less than 0.750 millimeters (without subclinical atherosclerosis) and those above 0.750 millimeters (with subclinical atherosclerosis). The mean C-IMT, CRP, FETUIN-A, OPG and MGP of the two groups were compared statistically and the results are shown in Table-2. In chronic kidney patients, age (r = 0.493, p &lt;0.001), BMI (r = 0.337, p = 0.001), CRP (r = 0.301, p = 0.004), TG (r = 0.245, p = 0.019 ), urea (r = 0.228, p = 0.029), SBP (r = 0.212, p = 0.043), fasting blood sugar (r = 0.212, p = 0.043) have positive linear relationship, fetuin-A (r = -0.409, P = 0.001), OPG (r = -0.235, p = 0.024), GFR (r = -0.209, p = 0.046) have a negative linear relationship with CIMT. The multiple relationships between CIMT and other variables are given in Table-3. The mean CIMT (r =-0.417, p = 0.001), right CIMT (r = -0.412, p = 0.001), left CIMT (r = -0.410, p = 0.001), urea (r = -353, p = 0.007), CRP (r = -0.322, p = 0.014), UPE (r = -0.301, p = 0.022), creatinine (r = -0.277, p = 0.035), age (r = -0.262, p = 0.047) show a negative linear relationship with Fetuin-A. Multiple relationships between fetuin-A and other variables are given in Table-4. Conclusion Our study shows that; In particular, fetuin-A levels, which is a vascular calcification inhibitor, begin to decline from the early stages of CKD and is significantly lower in patients with atherosclerosis. This suggests that fetuin-A may be used as an early marker in CKD with increased cardiovascular mortality. On the other hand, contradictions related to the levels of OPG and MGP in CKD and its role in the development of atherosclerosis continue. The results in our study also support this situation. Reducing mortality and morbidity in CKD primarily depends on reducing the risk of cardiovascular events. Pre-recognition of these risks is important, so large-scale studies on vascular calcification inhibitors are needed.
